Objective:To investigate the value of auditory evoked potential(BAEP)parameters of brainstem in assessing damage of auditory pathway of preterm infants after they occurred intracranial hemorrhage, and the correlation between the damage and long-term neurodevelopmental outcomes. Methods:This study adopted a prospective cohort study design.A total of 158 preterm infants admitted to the neonatal intensive care unit of Anhui Provincial Hospital between June 2021 and December 2023 were included.According to whether occurred intracranial hemorrhage, they were divided into observation group(n=78,with intracranial hemorrhage)and the control group(n=80,without intracranial hemorrhage). The standardized BAEP test were used to obtain a series of parameters including the Ⅲ~Ⅴ wave interval,Ⅴ wave incubation period, and amplitude. The Papile grading system was combined to assess the severity of hemorrhage. The Gesell Developmental Scale was adopted to assess neurodevelopmental levels when follow-up was conducted until the corrected age of 12 months. The multifactorial logistic regression analysis was used to screen and identify independent risk factors. Results:Compared to the indicators of control group, the observation group significantly prolonged Ⅲ~Ⅴ wave interval and Ⅴ wave incubation period, and decreased Ⅴ wave amplitude, and the differences of them between two groups were statistical significance(t=5.038,5.642,-5.476,P<0.05).With aggravating of bleeding degree, the Ⅲ~Ⅴ wave interval appeared gradient prolonging, and with the grading change, the Ⅴ wave amplitude decreased progressively, and the differences were significant(F=18.326,F=7.894,P<0.001).There was not significant difference in Ⅲ~Ⅴ wave interval among different grading(P>0.05).The correlation analysis revealed that there was a strong negative correlation between Ⅲ~Ⅴ wave interval and the Gesell total score, with the strongest correlation between that and the dimension of language communication.Ⅴ wave amplitude was positively correlated with the Gesell total score, which had the highest correlation with the dimension of language communication, and all correlation coefficients reached to the biggest significant level(r=-0.632,-0.634,0.549,P<0.05).Multifactorial model confirmed that prolonging Ⅲ~Ⅴwave interval, increasing Papile grading, and reducing Ⅴ wave amplitude were independent predictors of neurodevelopmental abnormalities(OR=3.214,2.441,0.133,P<0.001).Conclusion:In the BAEP parameters, the Ⅲ~Ⅴ wave interval and Ⅴ wave amplitude can be used as sensitive indicators for functional damage of brainstem conduction in preterm infants with intracranial hemorrhage, which dynamic changes show significant correlation with long-term neurodevelopmental outcomes.

Objective In view of the controversy over radiotherapy target volume for patients with limited-stage small cell lung cancer ( SCLC), a prospective randomized controlled trial was conducted to compare the impact of different radiotherapy target volumes on prognosis. Methods After 2 cycles of EP chemotherapy, patients without progressive disease were randomly assigned to receive thoracic radiotherapy (TRT) to either the post-or pre-chemotherapy primary tumour extent as study arm or control. Involved field radiotherapy (IFRT) to the entire metastatic lymph node regions was applied for both arms. TRT consisted of 45 Gy/30Fx/19 d administered concurrently with cycle 3 chemotherapy. Prophylactic cranial irradiation was administered to patients achieved complete or partial remission. Kaplan-Meier method was used for survival analysis. Results Between June 2002 and December 2017,159 and 150 patients were randomly assigned to study arm and control respectively. The 1-,2-,and 5-year local/regional control rates were 79. 4%,61. 5% and 60. 1% respectively in the study arm versus 79. 8%,66. 5%,and 57. 3% in the control arm (P=0. 73). The median OS time was 22. 1 months in the study arm (95%CI,18. 2-26. 0 months) and 26. 9 months (95%CI,23. 5-30. 3 months) in the control arm,the 1-,3-,5-,and 7-year OS rates were 81. 1%,31. 6%, 23. 9% and 22. 2% respectively in the study arm versus 85. 3%,36. 6%,26. 1% and 20. 0% in the control arm (P=0. 51).Grade 2-3 acute esophagitis was developed in 32. 9% and 43. 2% of patients respectively in study arm and control arm (P=0. 01),while grade 2-3 pulmonary fibrosis was observed in 2. 0% and 10. 9% of patients ( P= 0. 01 ) respectively. Conclusions For patients with limited-stage SCLC who received induction chemotherapy, thoracic radiotherapy can be limited to post-chemotherapy tumour extent and IFRT can be routinely applied.

Objective To investigate the effect of construct the Notch1 (NICD) eukaryotic expression vector on the proliferation and differentiation of rat bone marrow mesenchymal stem cells (BMSCs) in vitro. Methods Rat BMSCs were experimented as the object. NICD eukaryotic expression vector was constructed. pEGFP-N1-NICD expressing plasmids were used to transfect BMSCs. The study included control group (CON group), empty vector group (VEC group) and the trans-fection group (TRA group). After 48-hour transfection, BMSCs were observed for general morphology. The protein expres-sions of NSE, GFAP and Notch1 were detected by real-time PCR and Western blotting assay respectively. The apoptosis, cy-cle distribution and cell proliferation were evaluated by flow cytometry and MTT assay. Results The DNA sequencing con-firmed that the pEGFP-N1-NICD recombinant plasmid was successfully constructed, and both VEC group and TRA group expressed green fluorescence after 48-hour transfection. The relative expression levels of Notch1 and GFAP mRNA and pro-tein were significantly higher in TRA group than those in VEC group and CON group (P<0.05), and there was no significant difference between VEC group and CON group. After 48-hour transfection, the ratio of living cells was significantly lower in TRA group than that of CON group and VEC group, and the early apoptotic rate and late apoptotic rate were significantly higher in TRA group than those of CON group and VEC group (P<0.05). The late apoptotic rate was significantly higher in VEC group than that of CON group. The proportion of G1/G0 cells was significantly higher in TRA group than that of CON group and VEC group, but S and G2/M cells were significantly lower (P<0.05). The value of growth curve was gradually de-creased in TRA group than that of CON group and VEC group (P<0.05). Conclusion The high expression of NICD gene might induce apoptosis of BMSCs, inhibit the proliferation in part, and induce into glial-like cell differentiation.
