OBJECTIVE To investigate the effect of Huangqin decoction (HQD)-based self-assembled nanoparticles (SAN) co-loaded with terbinafine (TBF) (TBF-HQD-SAN NPs) on the transdermal absorption of TBF. METHODS High-speed centrifugation combined with dialysis was used to separate HQD-SAN, and TBF-HQD-SAN NPs were obtained by loading TBF using the ultrasound magnetic stirring method; the particle size distribution, Zeta potential and polydispersity index (PDI) of the nanoparticle were characterized, and the encapsulation efficiency (EE) and drug loading (DL) of TBF were determined; using in vitro and in vivo transdermal experiments, the differences in transdermal performance between TBF-HQD-SAN NPs and TBF raw materials, as well as TBF and HQD-SAN physical mixture (TBF-HQD-SAN PM), were compared and analyzed. RESULTS TBF- HQD-SAN NPs were spherical with a particle size of (177.60±2.57) nm, a PDI of 0.197 4±0.007 9, and a Zeta potential of (-14.63±0.85) mV. The EE and DL of TBF were (99.49±0.71)% and (3.22±0.10)% , respectively. In vitro transdermal experiments, compared with TBF raw materials, the steady-state permeation rate (Jss) and skin retention of TBF-HQD-SAN NPs increased by 3.34 times and 27.56 times, respectively (P<0.05); compared with TBF-HQD-SAN PM, its Jss and skinretention were increased by 2.04 times and 7.44 times, respectively (P<0.05). In vivo transdermal experiments 69号) showed that, the area under the drug-time curve and the maximum concentration of TBF-HQD-SAN NPs increased by 2.13 times and 2.06 times respectively compared to TBF raw materials, and increased by 1.59 times and 1.65 times respectively compared to TBF-HQD-SAN PM (P<0.05). CONCLUSIONS TBF-HQD-SAN NPs can significantly enhance the in vitro and in vivo transdermal absorption efficiency and skin retention of TBF.

Interleukin- (IL) 23 drives pathogenic differentiation of Th17 cells via the JAK2-STAT3 signaling pathway, upregulates IL-17A/IL-22 expression, and disrupts intestinal barrier integrity, thereby playing a pivotal role in the pathogenesis of Crohn's disease (CD). IL-23p19 inhibitors—exemplified by risankizumab, mirikizumab, and guselkumab—precisely block this pathway. Key phase 3 trials have demonstrated their efficacy in CD, showing significant clinical benefits in patients refractory to conventional therapies or biologics, with no new safety signals identified. The ultimate treatment goal for CD is deep healing (mucosal-transmural-biochemical composite remission) as defined by STRIDE II. However, current evidence exhibits critical limitations: absence of head-to-head drug comparisons, insufficient data on biologic-experienced subpopulations, and heterogeneous follow-up durations leading to uncertainties in long-term safety. Future studies require standardized head-to-head trials with enhanced subgroup analyses to optimize precision therapeutics.

Chinese Journal of Microbiology and Immunology 2025;45(6):461-466
Objective:To understand the clinical manifestations, laboratory characteristics, and treatment of Staphylococcus aureus ( Sa) infection in children, and explore the predictive value of lymphocyte count, neutrophil to lymphocyte ratio(NLR), and high-sensitivity C-reactive protein to albumin ratio(CAR) in patient with secondary bloodstream infection(BSI). Methods:A retrospective analysis was conducted on 72 children with Sa infection admitted to the Nanchong Hospital of Beijing Anzhen Hospital Affiliated to Capital Medical University from January 2020 to April 2024. Clinical data including basic information, drug resistance, antibiotic treatment were collected. Differences in laboratory test results and inflammatory indicators were analyzed. The influencing factors of Staphylococcus aureus bacteraemia(SAB) in pediatric patients were analyzed using univariate/multivariate Logistic regression and correlation analysis. The predictive value was analyzed by receiver operating characteristic curve (ROC) and area under curve (AUC). Results:Seventy-two patients were selected, of whom 60 (83.33%) had skin and soft tissue infections (SSTIs), mainly community-associated Staphylococcus aureus(CA-SA)(50 cases), including 16 community-associated methicillin-resistant Staphylococcus aureus (CA-MRSA) cases (32.00%, 16/50). The detection rate of MRSA was 37.50% (27/72). The resistance rates of Sa to penicillin, clindamycin, and erythromycin were 97.22%(70/72), 69.44%(50/72), and 69.44%(50/72), respectively. Compared with the SSTIs group, the levels of lymphocyte count in children with bone and joint infections were significantly lower, while NLR and CAR levels were higher ( P<0.05). Fourteen patients had secondary SAB. In the secondary SAB group, lymphocyte counts were lower, while inflammatory indicators (NLR, CAR), and D-dimer were higher. CAR and NLR are positively correlated with the occurrence of bloodstream infections ( P<0.05). CAR ( OR=4.866, 95% CI: 1.37-17.25) and NLR ( OR=1.293, 95% CI: 1.032-1.620) were independent risk factors for secondary bloodstream infection, and ROC curve results showed that the combination of the two indicators had a higher sensitivity (0.929) and AUC (0.909) than individual testing. After undergoing effective treatment, the levels of CAR and NLR in the children in the secondary SAB group were significantly reduced. Wound infection patients underwent debridement treatment in 38 cases (52.78%, 38/72), while antibiotic treatment still primarily used first- and second-generation cephalosporins. For patients with secondary bloodstream infection, vancomycin was the first choice. Conclusions:Children′s wound infections with Sa primarily involve skin and soft tissue infections, commonly seen in CA-SA. NLR and CAR are independent risk factors for children with secondary bloodstream infection, and their combined detection has certain value for early diagnosis.

High mobility group box 1 (HMGB1), when released extracellularly, plays a pivotal role in the development of spinal cord synapses and exacerbates autoimmune diseases within the central nervous system. In experimental autoimmune encephalomyelitis (EAE), a condition that models multiple sclerosis, the levels of extracellular HMGB1 and interleukin-33 (IL-33) have been found to be inversely correlated. However, the mechanism by which IL-33 deficiency enhances HMGB1 release during EAE remains elusive. Our study elucidates a potential signaling pathway whereby the absence of IL-33 leads to increased binding of P300/CBP-associated factor with HMGB1 in the nuclei of astrocytes, upregulating HMGB1 acetylation and promoting its release from astrocyte nuclei in the spinal cord of EAE mice. Conversely, the addition of IL-33 counteracts the TNF-α-induced increase in HMGB1 and acetylated HMGB1 levels in primary astrocytes. These findings underscore the potential of IL-33-associated signaling pathways as a therapeutic target for EAE treatment.

[Objective] To investigate the differences in metabolomics between apheresis platelets stored at 4℃ and at 22℃ with agitation, aiming to provide a theoretical basis for the cold storage of apheresis platelets. [Methods] Samples were collected at four time points (d1, d5, d10, d15) for platelets stored at 4℃ (experimental group) and two time points (d1, d5) for platelets stored at 22℃ with agitation (control group). Liquid chromatography-tandem mass spectrometry (LC-MS/MS) technology was used to detect changes in platelet metabolome levels under different storage conditions. Platelet functional activity was assessed by thromboelastography (TEG) for maximum amplitude (MA) values and flow cytometry for CD62P activation rates. [Results] Metabolites in the glycolytic pathway, key metabolites in the tricarboxylic acid cycle (citrate, α-ketoglutarate), metabolites in the purine metabolism pathway (adenine, inosine monophosphate, guanine, etc.) and amino acid metabolites significantly decreased by d5 in the control group, whereas they remained stable in the experimental group. The content of fatty acid metabolites, such as prostaglandin G2, 13(S)-HOTrE, and linoleic acid, significantly increased in the control group. Statistically significant differences in MA values were observed between the two groups at d1 and d5 (P<0.05). However, in the experimental group, as the storage time extended, the MA values at d10 and d15 showed no significant difference compared to the control group at d5 (P>0.05). The CD62P activation rate between the two groups was statistically significant (P<0.05). Additionally, the CD62P activation rate of platelets in the 22℃ group increased rapidly from d1, while it rose gradually in the 4 ℃ group. [Conclusion] Platelets stored at 4 ℃ exhibit more stable metabolic activity and slower functional deterioration, which is beneficial for extending the effective storage period of platelets.

Objective:To investigate the impact of deep learning image reconstruction (DLIR), adaptive statistical iterative reconstruction-veo (ASiR-V) and filtered back projection (FBP) on the histogram and wavelet features of portal venous phase abdominal CT in patients with hepatic tumor. Methods:The CT data of 68 patients with hepatic tumor who underwent enhanced CT scans were retrospectively collected. FBP, 30%ASiR-V, DLIR-L, DLIR-M and DLIR-H images were reconstructed. The images of portal venous phase were reconstructed with five algorithms, including FBP, ASIR-V at a level of 30% (ASiR-V 30%), DLIR at low (DLIR-L), medium (DLIR-M), and high (DLIR-H). Histogram and wavelet features were extracted from hepatic lesion, liver, spleen, kidney and erector spinae muscle, and compared using one-way analysis of variance or Kruskal-Wallis test. Two radiologists delineated the three-dimensional lesions independently and one of them repeated the delineation after one month. Intra-class correlation coefficients ( ICC) among five sets of images were calculated to evaluate the consistency of radiomics features of hepatic lesion. P<0.05 was considered to indicate statistical significance. Results:Most histogram and wavelet features extracted from hepatic lesion, liver, spleen, kidney and erector spinae muscle showed significant differences among five groups (all P<0.05). The number of features without significant differences decreased with the intensity of DLIR reconstruction increased. For histogram features, there were no significant differences of energy, mean, median, and total energy among five sets of images ( P>0.05). For wavelet features, there were no significant differences of mean and median among five sets of images ( P>0.05). The consistency of all histogram features was high except for the mean value of wavelet feature. The intra-and inter-observer ICC ranged from 0.756 to 1 and 0.767 to 1, respectively. Conclusion:Both 30%ASiR-V and DLIR at three levels algorithms had influence on the histogram and wavelet features of abdominal organs and hepatic tumors extracted from CT images in portal venous phase, and the effects expanded with the strengthening of levels. Median can be a reliable quantitative parameter for CT texture analysis of hepatic tumor.
