1.A Preliminary Discussion on the Method of Regulating Qi and Calming the Mind

Based on the theoretical framework of the core concepts of the Inner Canon of Yellow Emperor (《黄帝内经》), this paper systematically elucidates the connotations of the mind and qi in traditional Chinese medicine and their interrelationship, and proposes the method of regulating qi and calming the mind. It emphasizes that the mind represents the outward manifestation of life phenomena and their underlying laws, while qi encompasses both a material foundation and functional dynamics. Mind and qi are jointly expressed in life activities through observable manifestations. In clinical practice, image-based thinking should be adopted, integrating theories such as yin-yang, the five elements, and essence-qi, to construct a diagnostic and therapeutic model of "manifestation-mechanism-syndrome-treatment". Treatment should be carried out from four aspects which include regulating body and constitution, recti-fying qi movement, calming the mind changes, and stabilizing emotional states. Moreover, it is necessary to take into account the coordinated regulation of both body and mind, as well as the maintenance of dynamic balance, to select appropriate formulas and medications.

3.Prediction of Spontaneous Preterm Birth by Cervical Length at 16-24 Weeks in Twin Pregnancies

Objective:To explore the predictive value of cervical length(CL)measured at 16-24 weeks of ges-tation for spontaneous preterm birth in twin pregnancies.Methods:A retrospective study was conducted on 1001 twin pregnancy cases,including 569 cases in the spontaneous preterm birth group and 432 cases in the term birth group.Univariate and multivariate Logistic regression analyses were used to identify factors associated with spon-taneous preterm birth.The predictive value of CL for spontaneous preterm birth was analyzed by the receiver op-erating characteristic(ROC)curve.Results:At 16-24 weeks of pregnancy,the CL in the spontaneous preterm birth group(3.460±0.866 cm)was significantly shorter than that in the term birth group(3.711±0.589 cm),and the difference was statistically significant(P=0.005).There were statistically significant differences in the cervical fun-neling and its morphology between the term birth group and the spontaneous preterm birth group(P<0.05).Multivariate Logistic regression analysis showed that monochorionicity(aOR 0.161,95% CI 0.074-0.351) and CL at 16-24 weeks of pregnancy(aOR 0.655,95% CI 0.496-0.864) were influencing factors for spontaneous preterm birth(P <0.05).ROC curve analysis indicated that a CL cutoff of 3.045 cm at 16-24 weeks of pregnancy predicted spontaneous preterm birth before 34 weeks,with a sensitivity of 90.2% ,specificity of 64.9% ,and an area under the curve of 0.650.Conclusions:Measurement of CL at 16-24 weeks of pregnancy is a potential predictor of spontaneous preterm birth in twin pregnancies.In addition,when establishing a prediction model for spontaneous preterm birth,future research should also consider cervical funnel morphology.
4.Research on Lightweight Large Language Models for Ancient Traditional Chinese Medicine Texts Based on Lora Fine-Tuning

Objective To address the challenges of constructing large language models for traditional Chinese medicine(TCM)classics,which are complex and expensive to fine-tune,this study explores a lightweight fine-tuning method for such models,aiming to develop a question-answering model centered on TCM classics,particularly various editions of Shang Han Lun through the ages.Methods Dataset construction involved designing prompts to guide GPT-4 in generating Q&A pairs based on Shang Han Lun and integrating them with the ShenNong_TCM_Dataset and cMedQA2 datasets.Five general-purpose large models were selected for Lora fine-tuning.The best model was chosen through evaluation,and the performance of multiple quantized versions was validated.Results After fine-tuning,the BLEU,ROUGE-1,ROUGE-2,and ROUGE-L metrics for the Qwen-7B-Chat model improved by 17.61,19.63,14.3,and 21.4,respectively,compared to the base model.Conclusion The selected model in this study is capable of effectively understanding and utilizing professional terms and concepts from TCM classics,such as Shang Han Lun,to provide accurate answers to user queries.Compared to similar models,it requires lower fine-tuning costs and computational power,contributing to the dissemination of TCM knowledge and the development of intelligent systems.
5.Abnormalities of cerebellar-cerebral circuits and social impairment in ASD

Autism spectrum disorder(ASD) is a neurodevelopmental disorder, and social impairment was one of the core symptoms of ASD, which can seriously affects the social life of patients.The pathogenesis of social impairment in ASD is unclear and it may involves many brain abnormalities.The related theories and hypotheses are numerous and there is no unified conclusion. Studies have shown that the cerebellum has extensive connections with brain networks and is involved in the regulation of social cognition, but its role in ASD has not been fully emphasized.The structural and functional abnormalities of the cerebellar-cortex (CC) loop in ASD patients can lead to language communication disorders, empathy disorders, difficulties in interpreting social cues, abnormal social reward processing and emotional regulation disorders, which are closely related to ASD social impairment. Noninvasive brain stimulation of the superficial cerebellum can improve the abnormal CC circuit in ASD patients, and the cerebellum can be considered as a target for the treatment of social disorders in ASD in the future.Based on the clinical and basic researches on social impairment in ASD in recent years, this article reviews the relevant manifestations of disorders which cerebellar and CC circuit involved, aiming to promote the development of related research in the future.
6.Screening and biological characteristics of bacteriophage HN_Aba_01 against multidrug-resistant Acinetobacter baumannii

OBJECTIVE To analyze the biological characteristics and genomic features of highly lytic bacteriopha-ges isolated from sewage in tropical hospitals and provide references for the hospital-associated infection prevention and control of multidrug-resistant Acinetobacter baumannii(MDR-Ab).METHODS With MDR-Ab as the host bacterium,bacteriophages were isolated from sewage.Transmission electron microscopy was employed to observe their morphology,and determine the optimal multiplicity of infection(MOI),one-step growth curve and environ-mental stability.Whole-genome sequencing and bioinformatics analysis were conducted to annotate functional genes and construct a phylogenetic tree.RESULTS A virulent bacteriophage,HN_Aba_01,was isolated.Elec-tron microscopy revealed that it belonged to the Myoviridae family,with a head diameter of 50 nm and a tail length of 90 nm.This bacteriophage exhibited strong lytic activity,with an optimal MOI of 0.000 000 1,a latent period of 5 minutes and a lysis yield of 15 PFU/cell.It remained stable at temperatures ranging from 4 ℃ to 60 ℃and pH values from 3 to 10.Genomic analysis identified 85 ORFs,including lyase,perforin and depolymerase genes.It shared 98.12%identity with the bacteriophage AbP2 genome(reference genome)and was classified into the Obolenskvirus genus.CONCLUSIONS The bacteriophage isolated from tropical hospital sewage,with high lyt-ic activity and good environmental adaptability,can be used for the hospital-associated infection prevention and control of MDR-Ab.
7.Prediction of Spontaneous Preterm Birth by Cervical Length at 16-24 Weeks in Twin Pregnancies

9.A qualitative study on needle-related distress of arteriovenous fistula experiences in hemodialysis patients

Objective To explore the authentic experiences of needle-related distress during arteriovenous fistula cannulation in hemodialysis patients and provide a basis for developing targeted assessment tools and intervention plans in clinical practice.Methods This study adopted a descriptive phenomenological research method.Using purposive sampling,13 hemodialysis patients with arteriovenous fistula from the blood purification center of a tertiary hospital in Changzhou were recruited for semi-structured interviews between November 2024 and January 2025.Data were analyzed using Colaizzi's 7-step method.Results Totally 4 themes and 10 sub-themes were extracted,namely dynamic evolution of distressing experiences(cognitive gaps and unfamiliar environments triggering distress,familiarity with procedures and self-management alleviating distress,sudden medical events reawakening distress),embodied cognition of distressing experiences(somatic distress,cognitive distress,mind-body dualistic interaction),subjective interaction in distress perception(subjective perceptions of differences in puncture skills,positive regulation through good communication attitudes),coping strategies for distressing experiences(empowerment-based coping,adaptive compromise).Conclusion The needle-related distress associated with arteriovenous fistula puncture in hemodialysis patients is not a single physical feeling,but a complex experience involving physical and mental interaction,nurse-patient interaction and coping strategies.Clinically,a dynamic assessment and precise intervention system should be constructed to improve patients' puncture experience and quality of life.
10.Effect of PD-1/PD-L1 inhibitors on lipopolysaccharide-induced inflammation in mouse microglia

Objective:To investigate the effect of PD-1/PD-L1 inhibitor BMS-1 on LPS-induced inflammation in mouse microg-lia cells(BV-2 cells).Methods:Bv-2 cells were divided into Control group,LPS group and LPS+BMS-1 group.Bv-2 cells in Control group were cultured in DMEM medium for 78 hours,cells in LPS group were stimulated with 100 ng/ml LPS for 6 hours after 72 hours of normal culture,Bv-2 cells in LPS+BMS-1 group were treated with 50 nmol/ml BMS-1 for 72 hours and then stimulated with 100 ng/ml LPS for 6 hours.Expressions of PD-1 and iNOS mRNA in each group were detected by RT-qPCR,and expressions of PD-1 and iNOS protein in microglia were detected by Western blot.Flow cytometry was used to detect cell apoptosis in each group.Levels of inflamma-tory cytokines IL-1β,IL-6,TNF-α and IL-10 were detected by ELISA.Results:RT-qPCR and Western blot results showed that com-pared with Control group,LPS group had significantly increased expression of PD-1 and iNOS(P<0.05).Compared with LPS group,LPS+BMS-1 group had significantly decreased expression of PD-1(P<0.05)and significantly increased expression of iNOS(P<0.05).Flow cytometry showed that compared with Control group,LPS group had a significantly increased in apoptosis of microglia(P<0.000 1).Compared with LPS group,LPS+BMS-1 group had a significantly increased in apoptosis of microglia(P<0.000 1).ELISA results showed that compared with Control group,LPS group had no significantly increased in pro-inflammatory factors IL-1β and IL-6(P>0.05),while significantly increased in TNF-α(P<0.000 1)and anti-inflammatory factor IL-10(P<0.000 1).Pro-inflammatory cyto-kine IL-1β in LPS+BMS-1 group was significantly higher than that in LPS group(P=0.000 1),IL-6 and TNF-α were also significantly higher than those in LPS group(P<0.000 1),while anti-inflammatory cytokine IL-10 in LPS+BMS-1 group was significantly lower than that in LPS group(P<0.000 1).Conclusion:BMS-1 can promote LPS-induced inflammatory response or impede the recovery of inflammation,and increase apoptosis of microglia.PD-1/PD-L1 pathway may be a potential therapeutic target for neuroinflammation.
