Objective:To investigate the relationship between serum annexin A1 (ANXA1), macrophage inflammatory protein 1-α (MIP-1α), soluble triggering receptor expressed on myeloid cells-1 (sTREM-1) and pulmonary infection in elderly patients with coronary heart disease complicated with heart failure and their predictive value. Methods:A total of 197 elderly patients with coronary heart disease combined with heart failure admitted to the Second Hospital of Qinhuangdao from January 2021 to January 2023 were retrospectively selected, and divided into pulmonary infection group (36 cases) and non pulmonary infection group (161 cases) according to whether the patients had pulmonary infection during hospitalization. Serum ANXA1, MIP-1α and sTREM-1 levels were detected in two groups. Multivariate logistic regression model was used to analyze the influencing factors of lung infection in elderly patients with coronary heart disease combined with heart failure. The predictive value of serum ANXA1, MIP-1α and sTREM-1 levels on lung infection in elderly patients with coronary heart disease combined with heart failure was analyzed by receiver operating characteristic (ROC) curve. Results:The incidence of pulmonary infection in 197 elderly patients with coronary heart disease combined with heart failure was 18.27%(36/197). Compared with the non pulmonary infection group, the pulmonary infection group had higher levels of serum ANXA1, MIP-1 α, sTREM-1, C-reactive protein, procalcitonin, and higher proportion of New York Heart Association (NYHA) heart function grade Ⅳ and diabetes, and lower left ventricular ejection fraction (all P<0.05). Multivariate logistic regression analysis showed that NYHA cardiac function grade Ⅳ, diabetes mellitus and elevated levels of procalcitonin, ANXA1, MIP-1α and sTREM-1 were independent risk factors for pulmonary infection in elderly patients with coronary heart disease combined with heart failure (all P<0.05). ROC curve analysis showed that the area under the curve predicted by serum ANXA1, MIP-1α and sTREM-1 combined was 0.909, which was larger than that predicted by serum ANXA1, MIP-1α and sTREM-1 alone. Conclusions:Elevated levels of serum ANXA1, MIP-1α and sTREM-1 are independent risk factors for pulmonary infection in elderly patients with coronary heart disease combined with heart failure, and can be used as auxiliary predictors of pulmonary infection in elderly patients with coronary heart disease combined with heart failure.

The role of radiotherapy in activating antitumor immune responses has attracted growing attention. Proton beam therapy (PBT), owing to its unique physical and biological properties, holds great potential in cancer immunotherapy. PBT not only directly kills tumor cells but also induces immunogenic cell death, remodels the tumor microenvironment, and modulates immune cell functions. Moreover, PBT shows distinct advantages in inducing systemic immune responses and establishing immune memory. Recent studies have demonstrated that PBT offers unique benefits over conventional photon radiotherapy in activating antitumor immunity and exhibits marked synergistic effects when combined with immune checkpoint inhibitors (ICIs). This review systematically summarizes recent advances in understanding the role and mechanisms of PBT in tumor immune modulation, discusses the prospects of its combination with immunotherapy, and provides new insights and theoretical evidence for comprehensive cancer treatment.

Essential oils (EOs) are natural, volatile substances derived from aromatic plants. They exhibit multiple pharmacological effects, including antibacterial, anticancer, anti-inflammatory, and antioxidant properties, with broad application prospects in health care, food, and agriculture. However, the instability of volatile components, which are susceptible to deterioration under light, heat, and oxygen exposure, as well as limited water solubility, have significantly impeded the development and application of EOs. Porous nanoclays are natural clay minerals with a layered structure. They possess unique structural characteristics such as large pore size, regular distribution, and tunable particle size, which are extensively utilized in drug delivery, adsorption separation, reaction catalysis, and other fields. Natural-derived porous nanoclays have garnered considerable attention for the encapsulation and delivery of EOs. This review comprehensively summarizes the structure, types, and properties of natural-derived porous nanoclays, focusing on the structural characteristics of porous nanoclays such as montmorillonite, palygorskite, halloysite, kaolinite, vermiculite, and natural zeolite. It also examines research advances in their delivery of EOs and explores engineering strategies to enhance the delivery of EOs by natural-derived porous nanoclays. Finally, various applications of natural-derived porous nanoclays for EOs in antibacterial, food preservation, repellent, and insecticide aspects are presented, providing a reference for the development and application of EOs.

Chinese Journal of Digestive Endoscopy 2022;39(2):96-102
Objective:To evaluate the performance, efficacy and safety of a novel portable endoscopy system for upper gastrointestinal examination. Methods:A multicentered, open-label, randomized, non-inferiority controlled study was conducted in 3 clinical research centers from June 2019 to June 2020, and a total of 90 outpatients admitted to Department of Gastroenterology were randomly assigned to the trial group ( n=44) undergoing portable endoscopy and the control group ( n=46) undergoing Olympus endoscopy. The examination success rate, image quality, performance, overall operation satisfaction rate, biopsy success rate and adverse events of the two groups were compared. Results:The examination success rates of the trial group and the control group were 97.73% (43/44) and 100.00% (46/46) respectively with a difference of -2.27% (95% CI: -6.68%-2.13%), higher than the set non-inferiority margin of -10%. Rates of good and excellent image quality were 100% in both groups, and the difference of 0 was higher than the set non-inferiority margin of -10%. There was no significant difference in the rate of good and excellent performance of the operating system between the two groups [97.67% (42/43) VS 100.00% (46/46), P=0.483]. There was significant difference in the overall satisfactory rate of the operation between the two groups [86.05% (37/43) VS 100.00% (46/46), P=0.011]. A total of 9 cases underwent endoscopic biopsy, including 5 cases in the trial group and 4 cases in the control group. The biopsy channels in both groups were smooth and the biopsy were successfully completed. There was no significant difference in adverse event rate between the two groups [25.00% (11/44) VS 10.87%(5/46), χ2=3.07, P=0.080]. All adverse events disappeared in 48 hours, and no severe adverse events or device defect events occurred. Conclusion:The novel portable endoscopic system is comparable to Olympus endoscopic system in terms of the operating performance, the image quality and safety. Therefore, this system is safe and effective for upper gastrointestinal examination.

Objective:To investigate the protective effects of Jiannao Yishou capsule(JYC) on hippocampal in Alzheimer's dis-ease (AD) mice. Methods:Male mice were randomly divided into normal control group, model control group, positive control group (donepezil hydrochloride tablets,0.001 g·kg-1),JYC high-dose (3.744 g·kg-1) and low-dose (0.936 g·kg-1) group. Morris water maze was used to test the learning and memory ability of mice. The hippocampal capillary structure was observed by light micros-copy and electron microscopy and the level of hippocampal Aβ was measured by enzyme-linked immunoassay. Results:Compared with the normal control group, the model control group learning and memory significantly reduced, and capillary endothelial cell mitochon-drial degeneration, nuclear condensation, broadening of the perinuclear space, overlapping,Aβ in hippocampal of AD mice were sig-nificantly reduced (P<0.05). Compared with the model control group, the JYC high-dose group learning and memory of mice im-proved significantly (P<0.05),and hippocampal capillary endothelial cell morphology improved significantly, and Aβ in Hippocam-pal of AD mice were significantly reduced(P<0.05). Conclusion:JYC can maintain normal morphological structure of hippocampus capillaries of AD mice, decrease Aβ responses and Improve learning and memory. JYC has protective effects on hippocampal tissues in AD mice.
