Since the discovery of circulating hepatitis B virus (HBV) RNA in the peripheral blood of patients with chronic hepatitis B in 1996, a growing number of studies have focused on clarifying the biological characteristics and clinical application value of serum HBV RNA. This consensus mainly summarizes the research progress of serum HBV RNA existing profiles, quantitative detection methods, and current clinical applications. In order to better apply this indicator for the clinical management of patients with chronic HBV infection, recommendations on quantitative detection target regions, detection results, and clinical applications are put forward.

Objective To explore the efficacy of multilevel anterior cervical discectomy and fusion for aged patients with cervical spondylotic myelopathy. Methods 25 aged patients with cervical spondylotic myelopathy were treated by multilevel anterior cervical discectomy and fusion. Japanese Orthopedic Association (JOA) scores were evaluated,clinical results were graded from excellent to poor using Odoms criteria,and the sagittal alignment of fusion segements were measured using Cobb method. Bone fusion and complications was evaluated. Results The final follow up score according to Odoms criteria was excellent in 8 patients ,good in 14 patients,and fair in 3 patients ,with excellent and good rate of 88%. The JOA score at final follow up(7.8) was significantly higher than preoperative(12. 8),with average improvement rate of 54%. At final follow up,bone fusion rate was 100%. Conclusion Multilevel anterior discectomy and fusion for aged patients with cervical spondylotic myelopathy was effective.

Background Influence of weightlessness on visual function has been a hot topic in aerospace medicine field.Electroretinography (ERG) is valuable tool of evaluating visual function.Objective This study aims to observe the influence of head down tilt simulated weightlessness on ERG.MethodHead down tilt for 6° was adopted in 6 healthy volunteers.Flash ERG,including rod response,maxium response,cone response,oscillatory potentials (OPs) and 30 Hz flicker was recorded,and the latency and amplitude from each wave were analyzed before,two days and five days after trial.The record procedure followed the ISCEV standard for full field clinical electroretinography (2008 update).Oral informed consent was obtained from all the subjects prior to the trail.Results No significant differences were detected in the latencies and amplitudes of a,b waves of cone response and 30 Hz flicker among various time points(P＞0.05).The latencies were significantly prolonged in b wave of rod and a,b waves of maxim responses(P＜0.01),but no obvious change was found in amplitudes (P＞0.05).Both the latency and amplitude of all of the wavelets of OPs were considerably among the different time points(P＜0.05-0.01),and the ∑OPs was evidently different among the different time points(P＜0.05).Conclusion Head down tilt simulated weightlessness induce the abnormality of visual function in the early stage.

Objective To observe the influence of head-down tilt on visual evoked potential.Methods Six healthy volunteers were exposed to-6? head-down tilt.The visual evoked potential were measured before the test,the second day and the fifth day of the test under different space frequency.Results No significant difference was found in the latency of P100 before and after the test.The amplitude of P100 had significant change under high space frequency.But not the same happened on middle and low space frequency.Conclusion head-down tilt can induce the change of visual evoked potential under high space frequency,which means the weightlessness may influent the visual function.

BACKGROUND: Impaired glucose tolerance(IGT) is mainly related to genetic factors and environmental factors including excessive calorie intake and obesity. Insulin resistance(IR) plays a major role in the onset of IGT.OBJECTIVE: To investigate the relationship between erythrocyte insulinase activity(EIA) and IR in patients with IGT soas to provide theoretic basis for improving IR in patients with IGT by exercise.DESIGN: Observational and comparative study based on IGT patients as the subjects and adults with normal glucose tolerance as controls.SETTING: Department of endocrinology of a hospital affiliated to a military medical university.SUBJECTS: The study was conducted in the Department of Endocrinology of Nanfang Hospital, First Military Medical University of Chinese PLA, from January 2001 to April 2003. A total of 50 inpatients and outpatients with IGT, 26 males and 24 females aged(52 ± 7) years, were randomly selected. Inclusion criteria: those whose diagnosis met the WHO 1999 for criteria oral glucose tolerance test (OGTT) and whose heart, liver and kidney functions and blood test were within the normal range without taking any antidiabetics. Exclusion criteria: those who had liver and kidney diseases, infection, malignant tumor, coronary heart disease, cerebrovascular disorder and connective tissue disease. The patients with IGT were divided into 2 subgroups according to the presence of abnormal fasting plasma glucose (FPG).Subgroup A consisted of 20 patients (9 males and 11 females) with IGT accompanied with impaired FPG. Subgroup B was composed of 30 IGT patients (17 males and 13 females) with normal FPG. Twenty adults with normal glucose tolerance were set as control group(20 females and 10 males) with the age of(48 ± 12)years.METHODS: EIA was measured with the method of radioenzymatic assay in all subjects. Blood sugar, serum insulin and glycosylated hemoglobin were also measured, and homeostasis model analysis-insulin resistance (HOMA-IR) index was calculated for estimation of insulin sensitivity.MAIN OUTCOME MEASURES: The differences and correlation between EIA and HOMA-IR in IGT patients of each group.RESULTS: EIA, serum fasting insulin and HOMA-IR indexes of the patients with IGT were significantly higher than those of the controls ( P < 0.01 or P < 0.05) . EIA and HOMA-IR of the patients in subgroup A was significantly higher than those in subgroup B ( P < 0. 01 ). Linear regression analysis showed that EIA had significant positive correlation with serum fasting insulin, glycosylated hemoglobin and HOMA-IR indexes( r = 0.51, 0.44,0.49, P <0.01).CONCLUSION: The degradation rate of erythrocyte insulinase in patients with IGT tolerance is significantly higher than that of normal persons, and is closely related to the onset and development of insulin resistance.

Objective To sludy the relationship between advanced glycosylation end products (AGE) and protein kinase C (PKC), and their effects on ranal alteration in diabetic rats. Methods Insulin or anlinoguanidine was administered to diabetic rats. Blood glucose, hermoglobin A1c (HbA1c ), glomemlar tissue extracts AGE ( GTE AGE ), PKC, glomerular basement membrane thickness ( GBMT ) and udne protein/creatinine (Pr/Cr) ratio in diabetic rats were measured and analysed. Results LeveLs of blood glucose, HbA1C and AGE, PKC activity, the Pr/Or ratio and GBMT were all significantly increased ( p values all less than 0.01 ) in diabetic rats. Insulin could decrease the formation of kbAlc and AGE, and improve PKC activity. Aminoguanidine had no influecce on PKC activity (P>O.05) although it decreased the formation of AGE. Botah drugs could de4ay the increase of urine Pr/Cr ratio and GBMT ( P<0.05 or P< 0.01). Conclusions Chronic hyperglycemia may lead to an increase of PKC activity. HbAlc and AGE may not directly coritribute to alterations of PKC activity, but the increase of PKC activity could promote the action of AGE on GEM thickening. It is important to inhibit the formanion of AGE and reduce the PKC activity so as to pceveat or delay the development of diabetic nephropathy.

Objective To study plasma tumor necrosis factor ?(TNF ?) level and its relationship with insulin resistance in familial type 2 DM. Methods Plasma TNF ? were determined by RIA in 32 familial type 2 DM patients, their 37 non diabetic first degree relatives and 40 healthy control subjects. Results Plasma TNF ? was significantly higher in familial type 2 DM patients[(1.19?0.21)?g/L] than in healthy control subjects[(1.00?0.18)?g/L]( P 0.05). Regression analysis indicated that TNF ? was inversely related to ISI and was a significantly independent contributor to variations in ISI. 39% of the variance in ISI was explained by TC, body mass index(BMI)and TNF ?. Conclusion The finding of an association between high plasma levels of TNF ? and insulin resistance suggests that TNF ? may be involved in the pathophysiology of insulin resistance in familial type 2 DM patients.

The activity of erythrocyte membrane Na-pumping K~+-activated pera-nitro-phenylphosphatase (K~+-pNPPase) and red cell deformability (RCD) were stndied in 50adult patients with non-insulin-dependent diabetes mellitus (NIDDM). RCD was expressedas red blood cell filtration index (RBC-IF). In comparison with the healthy subject, theactivity of erythrocyte membrane Na-pumping K~+-pNPPase was significantly decreased,and RBC-IF was significantly increased. The above-mentioned changes were more severein NIDDM with retinopathy than those without. There was a very close negative corre-lation between Na-pumping K~+-pNPPase and RBC-IF. These results indicated that thedecrease of erythrocyte membrane Na-pumping K~+-pNPPase activity might be an importantfactor fot the decrease of RCD.

The relationship between plasma magnesium and plasma lipids was studied in 80 cases of Type II diabetes mellitus. It was found that plasma magnesium in diabetic patients was much lower than that in normal controls. Plasma magnesium was negatively correlated with plasma cholesterol, triglyceride, and LDL-C, and positively correlated with plasma HDL-C, HDL2, and HDL-C/LDL-C, suggesting that hypom-agnesemia was closely related with disturbance in lipid metabolism in diabetes mellitus. Hypomagnesemia seemed to worsen the disturbance of lipid metabolism, and might be one of the factors causing vascular complications in diabetes mellitus
