1.Efficacy of flipped classroom combined with 3D body anatomy software in teaching of ultrasound-guided transversus thoracic muscle plane block

Objective:To evaluate the efficacy of the flipped classroom combined with 3D body anatomy software in the teaching of ultrasound-guided transversus thoracic muscle plane block.Methods:In this randomized controlled trial, 100 second-year resident physicians from the Department of Anesthesiology and Perioperative Medicine at our hospital, male or female, aged 22-26 yr, who rotated during January 2023 to January 2025, were selected and divided into 2 groups ( n=50 each) using a table of random numbers: experimental group and control group. Experimental group employed the teaching model of flipped classroom combined with 3D body anatomy software, while control group used the traditional teaching model. The scores of theoretical assessment, accuracy rate of ultrasound image identification, scores of procedural skills, success rates of clinical procedure, teaching satisfaction, and success rates of clinical translation after 1 month follow-up were compared between two groups. Results:Compared with control group, the scores of theoretical assessment, accuracy rate of ultrasound image identification, scores of procedural skills, success rates of clinical procedure, teaching satisfaction, and success rates of clinical translation after 1 month follow-up were significantly increased in experimental group ( P<0.05). Conclusions:The combination of flipped classroom and 3D body anatomy software is more effective than the traditional teaching methods when used for teaching of ultrasound-guided transversus thoracic muscle plane block.
2.Research Progress on the Scientific Connotation of the Theory of Five Spirit-Organs in Traditional Chinese Medicine

The theory of Five Spirit-Organs in TCM originates from the"five Zang-organs storing spirit"in"Huangdi Neijing",constructing a regulatory system for mental activities centered on the unity of form and spirit,which includes the heart storing spirit,liver storing soul,spleen storing intention,lung storing corporeal soul,and kidney storing will.By connecting mental activities with the functions of the five Zang-organs through the symbolic thinking of the Five Elements,this theory provides a systematic framework for the qualitative and positional diagnosis of mental symptoms,and its research holds theoretical and clinical significance for breaking through the bottlenecks in the diagnosis and treatment of mental diseases in TCM.This review summarized the multi-dimensional research progress on the scientific connotation of the theory,finding that psychological studies have confirmed the correlation between dysfunction of the five Zang-organs and psychological abnormalities such as attention disorders.Neurobiological research has provided neuroscientific evidence for the"Zang-spirit-brain"mapping via imaging and neurotransmitter detection.And clinical practice has shown the unique advantage of"simultaneous regulation of form and spirit"in the management of mental diseases(anxiety,depression,etc.)and organic diseases complicated with mental symptoms based on the differentiation of five spirit-organs.Current research faces controversies and bottlenecks such as the integration of TCM and Western medicine theories,requiring future construction of a modern interpretive system through interdisciplinary technologies to promote the standardization and internationalization of the theory.

Bidirectional communication between the heart and brain has been well documented,and their respective electrophysiological activities are not independent but rather exhibit a measurable degree of coupling.The traditional Chinese medicine(TCM)doctrine of"heart storing the spirit"posits that the heart governs all vital activities and that the heart and brain are functionally interdependent and mutually reinforcing.This paper systematically reviews current methodological approaches for investigating cardio-electroencephalographic coupling,summarizes empirical findings that elucidate human physiological and pathological mechanisms through this coupling framework,and integrates TCM-based perspectives on heart-brain interactions.Finally,prospects for applying emerging cardio-electroencephalographic techniques is outline to advance research on the"heart storing the spirit"theory.

Objective To explore their inter-relationship,cognitive performance and cardio-electroencephalographic coupling in coronary heart disease(CHD)patients with heart-qi deficiency pattern were characterized.Methods Thirty CHD patients who met the diagnostic criteria for heart-qi deficiency were enrolled.Thirty healthy volunteers without heart-qi deficiency or CHD served as controls.Cognitive function was assessed with the repeatable battery for the assessment of neuropsychological status(RBANS).Simultaneous resting-state electrocardiogram and electroencephalogram were recorded,and the strength of heart-brain coupling was quantified by calculating the maximal information coefficient(MIC)between heart-rate variability(HRV)and EEG signals.Results Compared with controls,heart-qi deficiency CHD patients showed lower RBANS total scores and reduced performance in immediate memory,visuospatial/constructional ability,and attention.Significant between-group differences in HRV-EEG MIC values were observed at several channels(P<0.05).In patients,the MIC values for HRV-Beta(channel FC1)and HRV-Delta(channel F2)were positively correlated with RBANS total score(P<0.05).Conclusion CHD patients with heart-qi deficiency exhibit impaired cognition and altered cardio-electroencephalographic coupling.These findings suggest that heart-brain interactions may contribute to the pathophysiological mechanisms underlying cognitive decline in this population.

8.Identification of 4 rare Jr(a-) blood groups with anti-Jr antibody and follow-up analysis of newborns in subsequent delivery

Objective: To perform serological identification and molecular analysis of four samples with antibodies against high-frequency antigens, and to track the condition of newborns after delivery. Methods: Blood group serological tests were conducted using tube method, and unexpected antibody screening and identification were performed using polybrene and human globulin card. Gene haplotype analysis of blood group system was performed using PacBio third-generation sequencing. The DNA mutations were confirmed through Sanger sequencing. Results: Four samples showed normal blood types in common blood type systems. However, they were positive in all unexpected antibody screening and identification, with negative direct antiglobulin tests results. Third-generation sequencing revealed 3 cases of c.376C>T homozygous mutation and 1 case of c.421C>A homozygous mutation in the ABCG2 gene. Three pregnant women gave birth to four children, all of whom developed hyperbilirubinemia, accompanied by decreased red blood cell count and normal or low hemoglobin concentration. Conclusion: Four samples were obtained from individuals with the rare Jr(a-) blood group. Immunization during pregnancy led to the production of anti-Jr
antibody, which may contribute to hyperbilirubinemia in newborns.
9.Comprehensive Analysis of Oncogenic, Prognostic, and Immunological Roles of FANCD2 in Hepatocellular Carcinoma: A Potential Predictor for Survival and Immunotherapy.

OBJECTIVE:
Hepatocellular carcinoma (HCC) is sensitive to ferroptosis, a new form of programmed cell death that occurs in most tumor types. However, the mechanism through which ferroptosis modulates HCC remains unclear. This study aimed to investigate the oncogenic role and prognostic value of FANCD2 and provide novel insights into the prognostic assessment and prediction of immunotherapy.
METHODS:
Using clinicopathological parameters and bioinformatic techniques, we comprehensively examined the expression of FANCD2 macroscopically and microcosmically. We conducted univariate and multivariate Cox regression analyses to identify the prognostic value of FANCD2 in HCC and elucidated the detailed molecular mechanisms underlying the involvement of FANCD2 in oncogenesis by promoting iron-related death.
RESULTS:
FANCD2 was significantly upregulated in digestive system cancers with abundant immune infiltration. As an independent risk factor for HCC, a high FANCD2 expression level was associated with poor clinical outcomes and response to immune checkpoint blockade. Gene set enrichment analysis revealed that FANCD2 was mainly involved in the cell cycle and CYP450 metabolism.
CONCLUSION
To the best of our knowledge, this is the first study to comprehensively elucidate the oncogenic role of FANCD2. FANCD2 has a tumor-promoting aspect in the digestive system and acts as an independent risk factor in HCC; hence, it has recognized value for predicting tumor aggressiveness and prognosis and may be a potential biomarker for poor responsiveness to immunotherapy.
