1.Association between the transverse area of vertical spinal muscle at T 12 level and prognosis in elderly patients with acute coronary syndrome

Objective:To analyze the association between the transverse area of erector spine muscle at T 12 level and prognosis in elderly patients with acute coronary syndrome (ACS). Methods:This was a prospective cohort study. Clinical data of elderly patients with ACS admitted to the coronary care unit or the emergency intensive care unit in Beijing Bo'Ai Hospital from January 2019 to June 2022. Blood routine and biochemical indicators were detected and echocardiography was performed within 24 hours after admission, chest CT examination was completed and the cross-sectional areas of the left and right vertical spinalis muscles were calculated at T 12 level, thrombolysis in myocardial infarction (TIMI) was performed within 24 hours after admission. The occurrence of major adverse cardiovascular and cerebral events (MACCE) within 12 months was followed up. The differences of clinical data and laboratory indexes between the MACCE group and the non-MACCE group were compared. Multivariable logistic regression model was used to analyze the association between the cross-sectional area of vertical spinal muscle at T 12 level and the occurrence of MACCE within 12 months after ACS. Results:A total of 406 patients were enrolled in the study, the incidence of MACCE within 12 months was 22.2%. The cross-sectional area of erector spine muscle at T 12 level [(20.43±3.51) cm 2vs. (24.36±3.66) cm 2, (Z=4.412, P<0.001)] in the MACCE group was lower than that in the non-MACCE group. Logistic regression analysis showed that after adjusting for sex, age, body mass index (BMI), TIMI score and revascularization, the cross-sectional area of erector spine muscle at T 12 level ( OR = 0.883, 95% CI: 0.803-0.971, P=0.010) was independently associated with MACCE within 12 months after ACS. Conclusion:The transverse area of vertical spinal muscle at T 12 level can be used as an independent predictor of MACCE within 12 months in elderly patients with ACS.
2.Role of HBV DNA polymerase in mediating the immune escape of tumor cells in HBV-related hepatocellular carcinoma

ObjectiveTo determine whether HBV DNA polymerase is associated with T-cell failure and thus mediates the immune escape of HBV-related hepatocellular carcinoma (HCC) tumor cells, and to investigate the specific molecular mechanisms. MethodsLiver cancer cell lines Huh7 and HepG2 stably transfected with HBV DNA polymerase expression plasmid with Flag (Flag-HBV-P) and intercellular adhesion molecule-1 (ICAM1) were co-cultured with Jurkat cells, and MTT assay, qRT-PCR, and ELISA were used to measure Jurkat cell proliferation, activation (CD69 expression), and secretion of the cytokine IFN-γ. RNA-seq was used to screen for differentially expressed immune-associated molecules between stably transfected cell lines and control cells, and mRNA half-life and protein half-life assays were used to determine the specific levels of the immune-associated molecules that were affected by HBV DNA polymerase. Related websites were used to predict the transcription factors that may bind to the promoter region of this immune-associated molecule, Western blot was used to verify the effect of transcription factors on the immune-associated molecule, and rescue experiment was used to determine whether HBV DNA polymerase affects the expression level of the immune-associated molecule through this transcription factor. The independent-samples t test was used for comparison between two groups. ResultsThe experimental group had significant reductions in Jurkat cell proliferation, activation, and cytokine secretion compared with the control group (all P<0.01). Compared with the control group, the experimental group (Huh7 and HepG2 cell lines) had significant reductions in the mRNA and protein expression levels of ICAM1 (all P<0.01). Website prediction identified the ICAM1 promoter and preliminarily highlighted NFKB1, RELA, and STAT3. Compared with the control group, the experimental group (Huh7 and HepG2 cell lines) had a significant reduction in the protein expression level of p65 (all P<0.01). After p65 overexpression, there was a significant increase in the protein expression level of ICAM1, and after the expression of p65 was reduced, there was a significant reduction in the protein expression level of ICAM1 (all P<0.01). In the rescue experiment, there was no significant difference in the protein expression level of ICAM1 between the control group and the experimental group after p65 overexpression (all P>0.05). After the overexpression of ICAM1, there were no significant differences in the proliferation, activation, and cytokine secretion of Jurkat cells between the control group and the experimental group (Huh7 and HepG2 cell lines) (all P>0.05). ConclusionHBV DNA polymerase downregulates the level of ICAM1 to mediate HCC immune escape by inhibiting the expression of p65 in NF-κB.

4.Development of grip strength and its relationship with body composition in preschoolers

Objective:
To study the development of grip strength and its relationship with body weight, height and body composition in preschool children.
Methods:
Data was collected from 900 children aged 3-6 years in 4 preschools in Beijing during Sept. to Oct. of 2020. A questionnaire survey (birth weight, dietary and exercise), height, weight, bioelectrical impedance analysis of body composition and dominant hand grip strength tests were conducted. Partial correlational analysis, multivariate linear regression were used for association between grip strength and body composition.
Results:
The grip strength gradually increased with age, from (5.2±1.7) to (9.3±2.5)kg in boys ( F=57.93, P <0.01), and from (4.3±1.6) to (7.9±2.5)kg in girls ( F=48.77, P <0.01). Grip strength in boys were significant higher than that of girls ( t=6.42, P <0.01). The grip strength increased with weight status in boys ( F=5.35, P <0.01), with highest in obese group, followed by overweight and normal weight group. After adjusted for height, weight and birth weight, as well as maternal education, food consumption during the past week, grip strength in boys was positively correlated with fat free mass, upper limb muscle mass ( r=0.11, 0.09, P <0.05), and negatively correlated with body fat mass ( r=-0.18, P <0.05). Grip strength in girls was negatively correlated with body fat mass, and positively correlated with fat free mass ( r= -0.09, 0.09, P <0.05). Grip strength varied significantly by age, fat free mass and upper muscle mass in boys ( F=66.75, P < 0.01), and by age and fat free mass in girls ( F=150.81, P <0.01).
Conclusion
The grip strength can partially reflect changes in fat free mass for boys and girls, it also reflect changes in upper limb muscle mass for boys. Increases in fat free mass helps grip strength improvement.
5.Relationship between serum retinol binding protein, stromal cell derived factor-1 and renal function in patients with diabetic nephropathy

Objective:To investigate the relationship between serum retinol binding protein (RBP), stromal cell derived factor-1 (SDF-1) and renal function in patients with diabetic nephropathy (DKD).Methods:The patients with type 2 diabetes mellitus (T2DM) admitted to Yantai Affiliated Hospital of Binzhou Medical College from October 2017 to October 2020 were prospectively selected, 438 patients were divided into simple T2DM group ( n=276)and DKD group( n=162) according to the presence or absence of DKD, according to the ratio of urinary albinin/creatinine (UACR) were divided into normal( n=25), microalbuminuria ( n=75) and macroalbuminuria group ( n=62), according to the estimated glomerular filtration rate (eGFR) were divided into G1 stage ( n=28), G2 stage ( n=27), G3A + G3B stage ( n=35), G4 stage ( n=39)and G5 stages( n=33). The relationship between RBP, SDF-1 and renal function index UACR, serum uric acid (UA), blood urea nitrogen (BUN), β 2-microglobulin (β 2-MG) and serum creatinine (Scr) was analyzed. Measurement data of normal distribution were expressed as Mean± standard deviation ( Mean± SD). Independent sample t-test was used for comparison between two groups, and one-way analysis of variance was used for comparison between multiple groups.Chi-square test was used to compare the enumeration data between groups. Receiver operating characteristic curve (ROC) was used to analyze the discriminant value of RBP and SDF-1 for DKD. Pearson was used for correlation analysis among indicators. Multivariate linear regression analysis was used to analyze the influencing factors of RBP. Results:In the DKD group, the duration of diabetes was longer, the levels of RBP, UACR, UA, BUN, β 2-MG, Scr were high, SDF-1 and eGFR were lower, with statistically significant differences compared with the simple T2DM group( P<0.05).The areas under the curve of RBP and SDF-1 to distinguish DKD were 0.903 and 0.868, and the optimal cut-off values was 70.71 mg/L and 5.69 ng/mL. With the increase of urinary albumin and clinical stage, the levels of RBP, UACR, UA, BUN, β 2-MG, Scr increased gradually, while SDF-1 and eGFR decreased gradually, and the differences were statistically significant ( P<0.05).RBP was positively correlated with UACR, UA, BUN, β 2-MG and Scr in DKD patients ( r=0.764, 0.787, 0.693, 0.577, 0.801, P<0.000 1), and negatively correlated with EGFR ( r=-0.782, P<0.000 1). SDF-1 was negatively correlated with UACR, UA, BUN, β 2-MG and Scr ( r=-0.744, -0.794, -0.666, -0.605, -0.820, P<0.000 1), and positively correlated with EGFR ( r=0.767, P<0.000 1). The multiple linear regression equation was RBP=29.852+ 0.007UACR+ 0.101UA+ 0.497BUN+ 0.034Scr-0.083eGFR ( P<0.001). Conclusion:RBP and SDF-1 have certain discriminant value for DKD patients in T2DM population, and the degree of DKD renal function injury is positively correlated with RBP and negatively correlated with SDF-1, the increase of UACR, UA, BUN, Scr and the decrease of eGFR are risk factors for the increase of RBP.
6.Prenatal echocardiography in the diagnosis of fetal double aortic arch

To explore the way of prenatal echocardiography in the diagnosis of fetal double aortic arch . Methods T he data of fetuses diagnosed as double aortic arch in 6 prenatal centers in Hunan in echocardiograms performed at 20-36 weeks of gestation from 2013 to 2018 were reviewed . T he characteristics of echocardiographic with double aortic arch , and the associated malformations were observed ,the clinical outcome were analyzed . Results T he main echocardiographic features of the double aortic arch were three‐vessel‐tracheal view s ,which showed a bifurcation of the ascending aorta and a ring consisted of aortic right and left arch . From this retrospective analysis , 29 double aortic arches were identified ,which 8 cases ( 28% ) combined with cardiac defect and extracardiac abnormalities , 1 case with 22q11 deletion . Among them ,5 cases were confirmed by autopsy ,24 cases were diagnosed by computed tomography angiography ( 8 cases were confirmed by operation ) . Conclusions Systematic prenatal echocardiography in the diagnosis of fetal double aortic arch has significant clinical value in the cliagnose of double aortic arch ,w hether it is associated with other malformations and is important for assessing fetal prognosis .
8.Cross-sectional study of quality of life in liver transplant recipients

Objective To investigate the quality of life (QOL)in patients after liver transplantation (LT).Methods QOL of 326 liver transplant recipients was measured using the Medical Outcomes Study 36-item Short Form (SF-36)and the general information was collected at the same time.Results The mean scores of liver transplant recipients for SF-36 Mental Component Summary (MCS)and Physical Component Summary (PCS)were (49.47 ±7.66)and (49.58 ±9.68)respectively,which were statistically lower than national residents (P <0.05 ).Scores for all SF-36 dimensions in recipients in the first year after LT were statistically lower than those over 1 year after LT (P <0.05),while there was no significant difference between the scores of recipients who had LT among 1 -5 years and over 5 years (P >0.05).Recipients′PCS scores were significantly correlated to their gender and time after LT,while MCS scores were correlated to their age, gender and time after LT (P <0.05).Conclusions The QOL of liver transplant recipients is lower than normal people,especially in the first year after LT.The age,gender and time after LT are independent influence factors of QOL in liver transplant recipients.
9.Efficacy and safety of vitamin D3 in patients with diabetic nephropathy: a meta-analysis of randomized controlled trials.

BACKGROUNDSeveral studies found that vitamin D3 might alter glucose metabolism, protect kidney from injury and even proposed the mechanisms. But results from previous studies have been conflicting. The aim of this study was to evaluate the efficacy and safety of vitamin D3 in patients with diabetic nephropathy. The underlying mechanism of vitamin D3 decreasing proteinuria is also discussed.
METHODSWe conducted a search of English and Chinese articles using database of Pubmed, Embase, Sinomed, CNKI, Wanfang and clinical trial register centers, for randomized controlled trials of vitamin D3 in diabetic nephropathy patients. Two reviewers performed independently. Meta-analysis was used when studies were homogeneous enough.
RESULTSTwenty studies, including 1 497 patients with diabetic nephropathy, were involved in this systemic review. Vitamin D3-treated patients with diabetic nephropathy had a statistically significant reduction in 24-hour proteinuria (weighted mean difference -0.44, 95% CI -0.54 to -0.34, Z = 8.80, P < 0.000 01) and urine albumin/creatine ratio (standardized mean difference -0.29, 95% CI -0.48 to -0.10, Z = 2.96, P = 0.003). But vitamin D3 supplementation did not significantly reduce blood pressure and hemoglobin A1c compared with control group. The potential mechanisms about the renal protection of vitamin D3, including the inhibition of rennin-angiotensin system, the protection of kidney from inflammation, fibrosis and the structure change of kidney are discussed. In addition, vitamin D3 did not significantly increase the incidence of adverse effects, including total adverse effects, gastrointestinal adverse effects and fluctuation of blood pressure.
CONCLUSIONSVitamin D3 can ameliorate proteinuria and protect kidney from injury in patients with diabetic nephropathy. This renoprotective effect is independent of blood pressure and glucose reduction. And it does not increase any adverse effects than control, even in combination therapy with angiotensin converting enzyme inhibitors/angiotensin receptor blockers. But due to the limited randomized controlled trials of high quality, more clinical researches should be taken in the future.

10.Effects of Hedysarum Polybotys Saccharide on Lipid Metabolism and Expression of Stearoyl-CoA Desaturase-1 Gene in NAFLD Rats

Objective To investigate the effects of hedysarum polybotys saccharide (HPS) on lipid metabolism and the expression of stearoyl-CoA desaturase-1(SCD-1) gene in rats with non-alcoholic fatty liver disease (NAFLD). To discuss the interfering effects of HPS on NAFLD. Methods The SD rats were randomly divided into the blank control group and the experiment group. Rats in the experiment group were fed with lipid rich food for 8 weeks to establish model and were randomly divided into model group, drug positive group and HPS group. After 8 weeks of drug intervention, the level of ALT, AST, TC, TG, HDL-c and LDL-c were measured with automatic chemistry analyzer, and expression of SCD-1 gene was measured by semi-quantitative polymerase chain reaction.Results Compared with blank control group, serum ALT, AST and TC, TG, LDL-c of model group were higher (P<0.05,P<0.01), the level of HDL-c of model group and the expression of SCD-1 gene were lower (P<0.01). Compared with model group, HPS was useful to decrease serum ALT, AST, LDL-c, TC and TG (P<0.05,P<0.01), and increase the level of HDL-c (P<0.01) and the expression of SCD-1 gene (P<0.01).ConclusionHPS had a positive effect on regulating lipid metabolic disturbance of NAFLD rats and promoting the expression of regulatory gene SCD-1.
