1.Research on the performance evaluation system for clinical medicine advantageous disciplines in affiliated hospitals of Xinjiang universities

Objective:The level of discipline construction is an important indicator of a hospital′s comprehensive strength and core competitiveness.Establishing a scientific, reasonable, and effective performance evaluation system for the clinical medicine advantageous disciplines is of particular significance for university affiliated hospitals to strengthen discipline construction, improve governance systems, and promote high-quality development of hospitals.Methods:The Delphi method was used to determine the reliability of the indicator system by measuring the positivity coefficient, authority level, coefficient of variation, and coordination coefficient.Using Analytic Hierarchy Process, judgment matrix was established to determine the corresponding weights of elements, and calculate the total weight coefficients of each indicator, as well the consistency of the judgment matrix was checked.Results:The Cronbach′s coefficient of the questionnaire was 0.945, indicating high reliability.The effective recovery rates of the two rounds of expert consultation were 90.0%and 94.7%.Two rounds′ expert authority coefficients were 0.743 and 0.720.And the Kendall harmony coefficient was 0.196~0.731. The differences mentioned above were statistically significant(all P<0.05).Through two rounds consultation, a performance evaluation system for the clinical medicine advantageous disciplines was constructed, including 8 primary indicators and 24 secondary indicators.The weight coefficients of the primary indicators were 0.126, 0.208, 0.091, 0.162, 0.178, 0.118, 0.068, 0.049, and the total weight coefficients of each indicator were calculated, which showed good logical reliability. Conclusions:The performance evaluation system for the clinical medicine advantageous disciplines has strong scientific validity.It can provide evaluation tools for promoting the rational allocation and efficient utilization of hospital resources.
2.Study on the efficacy of Qi Shi Shen Shu Capsule combined with dapagliflozin in the treatment of type 2 diabetic nephropathy

Objective To investigate the clinical efficacy of Qi Shi Shen Shu Capsule combined with dapagliflozin in treating type 2 diabetic nephropathy.Methods 120 patients with type 2 diabetic nephropathy(qi-yin liangxu syndrome)admitted to the endocrinology department in the inpatient and outpatient depart-ments of endocrinology from May 2022 to December 2023 were randomly divided into control and observation groups(60 cases each).The control group received dapagliflozin plus placebo capsules,while the observation group received dapagliflozin plus Qi Shi Shen Shu Capsule for 6 months.The therapeutic effects were observed between the two groups.Parameters including serum creatinine(Scr),urinary albumin-to-creatinine ratio(uACR),24-hour urinary protein quantification(24hUP),glycated hemoglobin(HbA1c),and tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),and high sensitivity-C reactive protein(hs-CRP)were measured be-fore and after treatment.Results The total effective rate was higher(P<0.05)in the observation group(93.33%)than in the control group(78.33%).After treatment,both groups showed decreased levels of uACR,Scr,24hUP,HbA1c,TNF-α,IL-6,and hs-CRP compared with baseline,with lower levels in the obser-vation group(P<0.05).Conclusion Qi Shi Shen Shu Capsule combined with dapagliflozin can effectively re-duce uACR,Scr levels and serum inflammatory cytokines levels in patients with type 2 diabetic nephropathy.
3.Application of specialized scenario and workshop-based teaching method in occupational exposure training for newly recruited nurses

Objective To assess the efficacy of combining specialized scenarios with workshop-based teaching in training newly recruited nurses for occupational exposure.Methods This study was conducted with 200 nurses recruited between January 2020 and December 2021 as the control group and 200 nurses from January 2022 to December 2023 as the intervention group.The control group and the intervention group received routine training and workshop-based training,respectively.The efficacy was assessed through theoretical exams,skills assessments,occupational exposure incidence,and training satisfaction,measured u-sing the Simulation Design Scale(SDS).Results The intervention group demonstrated higher scores on theoretical knowledge and operational skills assessments compared to the control group.The occupational exposure were lower in the intervention group.Additionally,the intervention group scored higher on the SDS and reported greater satisfaction with the training(P<0.05).Conclusion The teaching method integrated with specialized scenarios and workshop-based teaching effectively enhances the theoretical knowledge and operational skills related to occupational exposure among newly recruited nurses.It reduces the inci-dence of occupational exposure,such as needlestick injuries,and improves training outcomes and nurse satisfaction,making it a valuable approach for clinical application.
4.Role of GRP75 and VDAC1 in heroin-induced arrhythmia

Objective To investigate the roles of glucose-regulated protein 75(GRP75)and voltage-dependent anion channel 1(VDAC1)in heroin-induced arrhythmia.Methods Based on transcriptomic data from primary myocardial cells treated with heroin and the GSE31821 dataset from the Gene Expression Omnibus(GEO),differentially expressed genes(DEGs)were identified using R software.Enrichment analysis of DEGs was performed.The protein-protein interaction(PPI)network was constructed using STRING database,and key genes were selected using Cytoscape.Molecular docking of key genes was performed using HDOCK,followed by in vivo valida-tion.A total of 60 SD rats were randomly divided into control,model,model+7 d,model+14 d,and model+21 d groups.A rat model of heroin addiction was established,and the expression levels of DEGs in myocardial tissue were assessed using immunohistochemical staining and quantitative real-time PCR.Results A total of 51 DEGs primarily enriched in muscle cell development,myofibrils,actin binding,arrhythmic right ventricular cardiomyopathy,calcium signaling,and apoptosis pathways were identified.Based on the PPI and Cytoscape analyses,GRP75 and VDAC1 were identified as key genes.Molecular docking indicated a strong interaction between GRP75 and VDAC1,which form a stable complex.The results of immunohistochemical staining and qRT-PCR showed that the expression levels of GRP75 and VDAC1 were significantly increased in the model group compared to those in the control group(P<0.05),and their expres-sion increased(P<0.05)with the intervention period of heroin in a time-dependent manner.Conclusion GRP75 and VDAC1 may con-tribute to heroin-induced arrhythmogenesis.
5.Role of SPP1 and MYD88 in diacetylmorphine-induced apoptosis in cardiomyocytes

Objective To explore the role of secreted phosphoprotein 1(SPP1)and myeloid differentiation primary response 88(MYD88)in morphine-induced cardiomyocyte apoptosis.Methods A morphine addiction model was established in Sprague-Dawley(SD)rats.Twelve SD rats were randomly assigned to the normal saline(NS)group or the morphine-dependent(DAM)group.Histopathological analysis was employed to observe and compare myocardial tissue morphology between the two groups.Terminal deoxynucleotidyl transferase-mediated dUTP nick-end labeling(TUNEL)staining was performed to assess the number of apoptotic cells in each group.The expression levels of SPP1 and MYD88 were evaluated using immunohistochemistry.Quantitative real-time poly merase chain reaction(RT-qPCR)and Western blot were used to detect the mRNA and protein expression of SPP1,MYD88,Bax,Bcl2,Caspase-3,and Caspase-9.Simultaneously,Western blot analysis was used to detected the expression of Cleaved Caspase-3 and Cleaved Caspase-9 proteins.In vitro,SPP1 expression was knocked down in primary neonatal rat cardiomyocytes(NRCMs),and cells were divided into three groups:control(CON),morphine treated(DA),and shSPP1#3+DA.Cell viability was assessed using the CCK-8 assay,and apoptosis rates were determined by flow cytometry.Results HE and TUNEL staining of myocardial tissues from morphine-addicted SD rats revealed that,compared with the NS group,myofibrils in the DAM group exhibited partial disruption and a significant increase in apoptotic cells(P<0.05).Western blot and RT-qPCR analyses demonstrated that,relative to the NS group,the mRNA and protein levels of SPP1,MYD88,Bax,Caspase-3,and Caspase-9 were significantly upregulated in the DAM group(P<0.05),whereas Bcl2 expression was significantly downregulated at both mRNA and protein levels(P<0.05),and the protein expression levels of Cleaved Caspase-3 and Cleaved Caspase-9 were also increased.with all differences being statistically significant.In NRCMs following morphine intervention,cell viability in the DA group was markedly reduced compared to the CON group(P<0.05),accompanied by a signifi-cant increase in apoptosis rate(P<0.05).Consistently,Western blot and RT-qPCR results showed elevated mRNA and protein expression of SPP1,MYD88,Bax,Caspase-3,and Caspase-9 in the DA group(P<0.05),along with decreased Bcl2 expression(P<0.05).The protein expression levels of Cleaved Caspase-3 and Cleaved Caspase-9 were elevated simultaneously.In contrast,the shSPP1#3+DA group exhibited opposing trends compared to the DA group,with statistically sig nificant differences(P<0.05).Conclusion SPP1 and MYD88 play critical roles in mediating morphine-induced cardiomyocyte apoptosis,and silencing SPP1 has been shown to significantly reduce the extent of cardiomyocyte apoptosis following morphine exposure.

10.Microneedle combined with photodynamic therapy in the treatment of oral leukopla-kia

Objective:To explore whether microneedle pretreatment can significantly improve the effi-cacy and safety of 5-aminolevulinic acid(ALA)-photodynamic therapy(PDT)in the treatment of oral leukoplakia.Methods:A non-randomized controlled clinical trial was conducted.Patients with clinical and pathological diagnosis of oral leukoplakia in the Department of Oral Mucosa,Peking University School and Hospital of Stomatology were divided into experimental group and control group.The control group was treated with conventional ALA-PDT,and the experimental group was pretreated with micro-needle buckling under superficial anesthesia with lidocaine before conventional ALA-PDT.The clinical mani-festations of the two groups were recorded,the lesion area was measured,the clinical efficacy was evalua-ted,the number of treatment sessions and treatment unit duration were analyzed,and the pain after treat-ment was evaluated by visual analogue scale.The above data of the two groups were statistically ana-lyzed.Results:A total of 11 patients were included in the experimental group and 19 patients were in-cluded in the control group.The complete remission rate of the experimental group and the control group was 45.5%and 36.8%,the partial remission rate was 54.5%and 57.9%,and the no remission rate was 0%and 5%,respectively.There was no significant difference in the treatment effect between the two groups.Meanwhile,the treatment unit duration of the experimental group and the control group were(9.05±5.74)min/cm and(21.38±15.44)min/cm2,respectively,and the number of treatment ses-sions were(2.36±0.67)times and(3.58±1.57)times,respectively.These differences between the two groups were statistically significant(t=-3.125,P<0.05;t=-2.932,P<0.05).Similarly,multiple linear regression analysis with 7 factors including age,dysplastic pathology,lesion classification,etc.,also confirmed that pretreatment could significantly shorten the treatment unit duration(P<0.05).In addition,there was no significant difference in pain score(visual analogue scale)between the two groups after treatment,and the microneedle puncture pretreatment did not increase the adverse reactions of ALA-PDT treatment.Conclusion:Microneedle pretreatment followed by conventional ALA-PDT shows a good clinical effect on oral leukoplakia,which can significantly shorten the clinical treatment time,re-duce the number of visits,and save medical costs.
