1.Clinical study of salvage second allogeneic hematopoietic stem cell transplantation in 17 cases

Objective To summarize and analyze the efficacy and influencing factors of second allogeneic hematopoietic stem cell transplantation (allo-HSCT) for acute leukemia relapsing after the first allo-HSCT. Methods Clinical data of 17 patients with acute leukemia who underwent second allo-HSCT at Peking University First Hospital from January 2005 to December 2024 were retrospectively analyzed. Results Among the 17 patients, 7 achieved long-term disease-free survival after second transplantation. The median progression-free survival after successful second transplantation was 7 months (range 8 days to 69 months). The relapse fatality was 24%, and the transplant-related fatality was 35%. Conclusions Second transplantation is an effective treatment for relapsed and refractory acute leukemia, but the relapse fatality and transplant-related fatality remain high. Patient age, time of relapse after the first transplantation and disease status before second transplantation are all factors that affect the efficacy of second transplantation. Younger age, late relapse and complete remission of disease before second transplantation are all beneficial for long-term disease-free survival after second transplantation.
2.Existentialist philosophical analysis of existential distress in advanced cancer patients

Existentialist philosophy focuses on human existence, explores subjectivity and uniqueness, and highlights the value of human existence. Advanced cancer patients generally suffer immense existential distress, each with their own modes of appearance and numerous possibilities. From the perspective of existentialist philosophy, targeting the group of advanced cancer patients, this paper clarified the analysis of “Dasein” (holistic phenomenon and modes of appearance). Through recognition (achieving to deepen the inner scope of the subject and object) and comprehension (seeking appropriate “self-knowledge”), it introduced the planning of variable possibilities. Subsequently, based on the patients’ pre-possessing, pre-seeing, and pre-grasping, explanatory activities were carried out to complete the “self-formation” in the planning. Ultimately, it facilitated the realization of existential meaning in the presupposition and proposed actionable clinical practices (meaning therapy and emotional touch therapy) to alleviate the existential distress of advanced cancer patients.
3.The impact of Swanson theory combined with hand touch on the existential distress and quality of life of patients with advanced lung cancer

Objective:To explore the effect of Swanson theory combined with hand touch on the existential distress of patients with advanced lung cancer, in order to provide reference for clinical humanistic care.Methods:A non synchronous controlled study was conducted using convenience sampling to select advanced lung cancer patients admitted to the Oncology Department of the First People's Hospital of Yongkang City, Zhejiang Province from October 2023 to March 2024 as the control group, and advanced lung cancer patients admitted from April to October 2024 as the experimental group. The control group received routine care, while the experimental group received Swanson theory combined with hand touch intervention on the basis of routine care. The Existential Distress Scale (EDS) and Functional Assessment of Cancer Therapy- General (FACT-G) scores of the two groups were compared before and 2 weeks after intervention.Results:During the study period, there were 3 dropouts in the experimental group and 2 dropouts in the control group. Finally, 37 cases were included in the experimental group, including 16 males and 21 females, with an age of (62.35 ± 12.13) years. There were 38 cases in the control group, including 25 males and 13 females, with an age of (65.68 ± 13.46) years. After 2 weeks of intervention, the EDS score of the experimental group patients was (6.65 ± 2.89) points, while that of the control group was (8.16 ± 2.27) points. The difference between the two groups was statistically significant ( t = 2.52, P<0.05). After intervention, the social/family status dimension, emotional status dimension, and total score of FACT-G in the experimental group were (15.35 ± 4.18), (19.27 ± 3.54), and (57.46 ± 14.69) points, respectively, which were better than the control group's (13.08 ± 3.77), (16.53 ± 3.88), and (50.58 ± 11.86) points. The difference between the two groups was statistically significant ( t = - 2.47, - 3.20, - 2.24, all P<0.05). Conclusions:The combination of Swanson theory and hand touch can effectively reduce the existential distress of patients with advanced lung cancer, improve their quality of life, and embody the concept of nursing humanistic care into simple and safe intervention measures, providing reference for clinical humanistic care.

The construction,renovation,and expansion of general hospitals involve numerous infection control issues,including zoning,flow line design,and air flow direction.Although relevant regulations have specified infection control require-ments for hospital architectural design,infection control professionals often struggle to provide planning and design recommenda-tions from a professional perspective due to the lack of integrated guidelines.Based on this,this article summarizes the standards and key issues that should be considered during the planning and design of critical departments in general hospitals,aiming to provide reference for infection control professionals.
5.Ileal injury secondary to percutaneous nephrolithotomy:a case report and literature review

Objective To investigate the clinical characteristics,diagnosis and treatment of ileal injury secondary to percutaneous nephrolithotomy percutaneous nephrolithotomy(PCNL).Methods The diagnosis and treatment of a patient were reviewed,and relevant literature were retrieved.Results The patient was a 41-year-old male,who underwent stage PCNL(initial percutaneous nephrostomy,followed by secondary PCNL)due to right ureteral calculi with severe hydronephrosis.On postoperative day 1,he developed abdominal distension and pain.Abdominal X-ray revealed subdiaphragmatic free gas,and CT showed pelvic and abdominal fluid and gas accumulation,suggesting peritonitis due to intestinal perforation.Emergency exploratory laparotomy identified a 3 mm×3 mm ileal perforation approximately 30 cm from the ileocecal valve,which was repaired surgically.The patient recovered well and was discharged after one week,with no discomfort reported during a 6-month follow-up.Conclusion The clinical features of ileal injury secondary to PCNL include early postoperative abdominal distension,pain and peritonitis.Diagnosis relies on clinical manifestations,abdominal X-ray and CT,with surgical exploration if necessary.Conservative management under vigilant observation can be cautiously adopted for localized injuries,while surgical repair is required for peritonitis or failed conservative therapy.
6.Combined physician-modified fenestration and inner branch techniques for aortic pathology with aberrant subclavian artery

Objective:To evaluate the early and mid-term efficacy of physician-modified fenestrated endovascular repair combined with inner branch techniques for aortic pathologies complicated by aberrant subclavian artery (ASA).Methods:A retrospective case series was conducted, including 24 patients with ASA-associated aortic pathologies who underwent thoracic endovascular aortic repair (TEVAR) with physician-modified fenestration and inner branch reconstruction at 7 centers in China from February 2021 to March 2025. The cohort comprised 18 males and 6 females, with an age of (54.4±11.7) years (range:37 to 80 years). Pathological diagnoses included aortic aneurysm in 7 patients (29.2%), aortic dissection in 11 (45.8%; 6 chronic, 4 subacute, 1 acute), and penetrating aortic ulcer in 6 (25.0%; 3 with concomitant intramural hematoma). Preoperative planning was performed using three-dimensional CT angiographic reconstruction, incorporating both the greater-curvature hemodynamic length and the centerline wall-adherent length. Fenestration sites were verified on three-dimensional printed models, and precise fenestrations were created at the covered stent-graft locations corresponding to the subclavian artery and ASA anatomy. Patients subsequently underwent TEVAR combined with supra-aortic revascularization as indicated, followed by completion ascending aortography to evaluate the sealing of the main stent-graft and the patency of fenestrated or branched stents. Perioperative outcomes, complications, and early-to mid-term clinical efficacy were analyzed.Results:All procedures were technically successful. Immediate angiography identified one case of minor type Ⅳ endoleak that resolved spontaneously on 3-month follow-up CT angiography, and one case of mild type Ⅱ endoleak that was left untreated with a stable false lumen during follow-up. One patient died on postoperative day 7 of an undetermined cause. The mean follow-up period was (23.1±11.3)months (range:3 to 37 months). During follow-up, one patient developed mild bilateral lower-limb weakness 1 month after surgery. Vascular occlusion and spinal cord infarction were excluded, and the symptoms were considered related to postoperative spinal hemodynamic changes; the weakness resolved after blood pressure adjustment without recurrence. No other complications, including upper limb ischemia, spinal cord ischemia, or posterior circulation ischemia, were observed. Throughout follow-up, all branch and main stents remained patent with good structural integrity, without migration or device-related complications.Conclusions:Physician-modified fenestration combined with inner branch techniques for ASA-associated aortic pathologies is technically feasible and yields satisfactory early and mid-term results. Long-term outcomes require further follow-up.
7.DiaSphere embolized microsphere TACE for treating primary hepatocellular carcinoma:A prospective multicenter randomized controlled study

Objective To observe the effectiveness and safety of DiaSphere embolized microsphere TACE for treating primary hepatocellular carcinoma(HCC).Methods Totally 188 patients with HCC were prospectively enrolled and randomly assigned to research group(n=93)and control group(n=95),who underwent TACE with DiaSphere embolized microspheres and Embosphere embolized microspheres,respectively.The incidence of TACE-related adverse events were recorded.The therapeutic efficacy 1 month after the first TACE,also 1 and 3 months after the last TACE,and liver functions 1 month after the first and last TACE were compared between groups.Results In research group,there were 69 cases underwent 1 time TACE,22 cases underwent 2 times and 2 cases underwent 3 times TACE,while in control group,there were 82 cases underwent 1 time and 13 cases underwent 2 times TACE,respectively.No statistical difference of the incidence of adverse events was found between groups(77.42%[72/93]vs.76.84%[73/95],P=1.000).One month after the first TACE,7 cases in research group and 11 cases in control group were lost to follow-up,respectively.One month after the last TACE,12 cases were lost to follow-up in both groups,and 3 months after the last TACE,28 cases were lost to follow-up in both groups.No significant difference of objective response rate nor disease control rate was found between groups at the above time points(all P>0.05).One month after the first and last TACE,liver function indicators were not different between groups(all P>0.05).Conclusion Both the short-term efficacy and safety of TACE with DiaSphere embolized microspheres for treating HCC were good.
8.Study on the Relevance of People at Risk of Obstructive Sleep Apnea and Tinnitus

Objective To investigate the relation between obstructive sleep apnea(OSA)and tinnitus.Meth-ods The data of subjects aged 20 years or above who participated in tinnitus and sleep questionnaire survey from the 2005-2008 and 2015-2018 National Health and Nutrition Surveys(NHANES)had been used,the people at risk of OSA were selected via the STOP-Bang questionnaire,and the association between the people at risk of OSA and tinnitus was determined by a Logistic regression model.Results Among the 5 183 participants,83.3%(4 320/5 183)and 16.7%(863/5 183)were classified into the non-tinnitus and tinnitus group,respectively.The tinnitus group had higher STOP-Bang scores(3.71±1.51)than those of the group without tinnitus(2.81±1.62)(P<0.001).The observed prevalence rates of tinnitus in the low-risk,middle-risk and high-risk group were 8.1%(175/1 973),20.5%(435/1 689),27.8%(253/658),respectively.The multivariate Logistic regression model showed that patients with middle-risk and high-risk of OSA had an increased risk of tinnitus compared with low-risk group,with the OR values and confidence intervals of 2.348(1.920~2.871)and 3.136(2.488~3.953),respec-tively(P<0.001).Conclusion People at risk of OSA are independently risk factors of tinnitus.Higher risk of OSA is associated with higher prevalence and higher risk of tinnitus.

Deep learning(DL)technology has strong ability for acquiring and learning information,which has been widely used in the field of stomatology imaging,such as segmentation of oral and surrounding structures,diagnosis of tooth and dental pulp diseases,perioperative evaluation of implant surgery,localization of craniomaxillofacial markers,evaluation of temporomandibular joint diseases and maxillary sinus diseases,etc.,contributing to accurate and efficient individualized diagnosis and treatment of oral diseases.The research progresses of DL in oral cone beam CT were reviewed in this article.
