Objective To investigate the epidemiological characteristics of cross-county imported dengue fever cases within Yunnan province in 2023, so as to provide insights into formulation of preventive and control measures for intra-provincial spread of dengue fever. Methods All data pertaining cross-county imported dengue fever cases within Yunnan Province in 2023 were collected, and the temporal, regional and population distributions of the cases were descriptively analyzed. Results A total of 1 664 intra-provincial cross-county imported dengue fever cases were reported in 95 counties (cities, districts) cross 16 profectures (cities) in Yunnan Province in 2023, accounting for 12.34% of total cases in the province. Cross-county imported dengue fever cases were predominantly reported during the period between August and October (1 516 cases, 91.11% of total cases), and peaked in September (659 cases), with a single-day peak on October 8 (36 cases). During the period from September 4 to 10, five counties (cities) with local dengue fever epidemics, including Jinghong City of Xishuangbanna Dai Autonomous Prefecture, Gengma Dai and Wa Autonomous County of Lincang City, Ruili City of Dehong Dai and Jingpo Autonomous Prefecture, Mengla Coun ty of Xishuangbanna Dai Autonomous Prefecture, and Zhenkang County of Lincang City, exported 165 cross-county imported dengue fever cases to the rest of the province. Among the 1 644 intra-provincial cross-county imported dengue fever cases, the male to female ratio was 1.40∶1.00, and 1 329 cases were at ages of 15 to 55 years (79.87%), with farmers as the predominant occupation (886 cases, 53.25%). The top 5 counties (cities/districts) reporting the highest number of intra-provincial cross-county imported dengue fever cases included Simao District (266 cases) and Lancang Lahu Autonomous County (118 cases) of Pu’er City, Mengla County (91 cases) and Menghai County (91 cases) of Xishuangbanna Dai Autonomous Prefecture, and Mangshi City (73 cases) of Dehong Dai and Jingpo Autonomous Prefecture, which accounting for 38.40% of total imported cases. These intra-provincial cross-county imported dengue fever cases originated from 7 counties (cities/districts) in 4 prefectures (cities), including 1 261 cases (76.70%) from Jinghong City of Xishuangbanna Dai Autonomous Prefecture, 224 cases (13.63%) from Ruili City of Dehong Dai and Jingpo Autonomous Prefecture, 103 cases (6.27%) from Gengma Dai and Wa Autonomous County of Lincang City, 31 cases (1.89%) from Mengla County of Xishuangbanna Dai Autonomous Prefecture, 30 cases (1.82%) from Zhenkang County of Lincang City, 10 cases (0.61%) from Cangyuan Wa Autonomous County of Lincang City, and 5 cases (0.30%) from Mohan-Boten Economic Cooperation Zone of Kunming City. In addition, local dengue fever epidemics following intra-provincial cross-county importation of dengue fevers cases in Simao District, Jinggu Dai and Yi Autonomous County, Mangshi City, Longchuan County, and Cangyuan Wa Autonomous County. Conclusions Farmers and students are high-risk populations for intra-provincial cross-county imported dengue fever cases in Yunnan Province, and health education pertaining personal protection against dengue fever should be strengthened among these high-risk populations by governments at all levels. There is a high risk of local out-break of dengue fever following continuous introduction of intra-provincial cross-county imported cases. Standardized management of intra-provincial cross-county imported dengue fever cases should be reinforced to reduce the risk of local epidemics.

ObjectiveTo investigate the mechanisms by which the combination of berberine (BBR) and baicalin (BAI) ameliorates type 2 diabetes mellitus (T2DM) due to internal accumulation of dampness-heat from the perspectives of gut microbiota and metabolomics. MethodsAntibiotics were used to induce pseudo-sterile mice. Thirty pseudo-sterile mice were randomized into a normal fecal microbiota transplantation group (n=10) and a T2DM (syndrome of internal accumulation of dampness-heat) fecal microbiota transplantation group (n=20). The mice were then administrated with suspensions of fecal microbiota from healthy volunteers and a patient with T2DM due to internal accumulation of dampness-heat by gavage, respectively. Each mouse received 200 µL suspension every other day for a total of 15 times to reshape the gut microbiota. The T2DM model mice were then assigned into a model group (n=8) and a BBR-BAI group (n=11). BBR was administrated at a dose of 200 mg·kg-1, and BAI was administrated in a ratio of BBR-BAI 10∶1 based on preliminary research findings. The administration lasted for 8 consecutive weeks. Fasting blood glucose (FBG), glycated hemoglobin (HbA1c), insulin (INS), triglycerides (TG), total cholesterol (CHOL), low-density lipoprotein cholesterol (LDL-C), high-density lipoprotein cholesterol (HDL-C), aspartate aminotransferase (AST), and alanine aminotransferase (ALT) levels were measured to evaluate the effects of the BBR-BAI combination on glucose and lipid metabolism and liver function in T2DM mice. Hematoxylin-eosin staining was employed to observe pathological changes in the colon tissue. The expression of claudin-1, zonula occludens-1 (ZO-1), and occludin in the colon tissue was determined by Western blot. Real-time quantitative polymerase chain reaction(Real-time PCR) was employed to assess the levels of tumor necrosis factor-α (TNF-α), interleukin-1β (IL-1β), and interleukin-6 (IL-6) in the colon tissue. The fecal microbiota composition and differential metabolites were analyzed by 16S rRNA sequencing and ultra-high performance liquid chromatography-quadrupole-time of flight tandem mass spectrometry (UPLC-Q-TOF-MS), respectively. ResultsThe BBR-BAI combination lowered the FBG, HbA1c, and INS levels (P<0.05, P<0.01) and alleviated insulin resistance (P<0.01) in T2DM mice. Additionally, BBR-BAI elevated the levels of ZO-1, occludin, and claudin-1 (P<0.05, P<0.01) and down-regulated the expression levels of TNF-α, IL-1β, and IL-6 in the colon (P<0.05, P<0.01). The results of 16S rRNA sequencing showed that BBR-BAI increased the relative abundance of Ligilactobacillus, Phascolarctobacterium, and Akkermansia (P<0.05), while significantly decreasing the relative abundance of Alistipes, Odoribacter, and Colidextribacter (P<0.05). UPLC-Q-TOF-MS identified 28 differential metabolites, which were primarily involved in arachidonic acid metabolism and α-linolenic acid metabolism. ConclusionBBR-BAI can ameliorate T2DM due to internal accumulation of dampness-heat by modulating the relative abundance of various bacterial genera in the gut microbiota and the expression of fecal metabolites.

Yunnan Province borders with Myanmar, Vietnam, and Laos, the China-Myanmar border area is the key area for prevention of re-establishment from imported malaria after the disease was eliminated in China. Since the malaria elimination action plan was launched in Yunnan Province in 2011, 129 counties (cities, districts) were classified into three categories according to malaria incidence and transmission risk, and different technical strategies and measures were implemented with adaptations to local circumstances. A total of 68 malaria consultation service stations were established on the Chinese side of the China-Myanmar border and 80 malaria prevention and control stations were established on the Myanmar side by Yunnan Province in 2014. Then, the “Three Lines of Defense” strategy was implemented for malaria elimination in the China-Myanmar border area in Yunnan Province during the period from 2015 to 2018, and this strategy was further refined and adjusted to the “3 + 1” strategy for prevention of re-establishment from imported malaria in 2019. Through decades of multifaceted efforts, the malaria elimination goal was achieved in Yunnan Province in June 2021. However, the number of imported malaria cases appeared a tendency towards a rise in Yunnan Province in 2023 and 2024, due to changes in the situation in Myanmar and the gradual resumption of international travel and border crossings following the adjustment of the COVID-19 prevention and control policy in China. The joint malaria prevention and control cooperation between China and Myanmar was initiated with the pilot project for joint malaria prevention and control in the China-Myanmar border area in 2005, and this project was progressed into the joint malaria and dengue fever prevention and control project in parts of the Greater Mekong Subregion border areas in 2010. The threat of overseas malaria epidemics to border areas in Yunnan Province was effectively reduced through implementation of coordination meetings with Myanmar health departments, establishment of efficient information exchange mechanisms, establishment of overseas surveillance sentinel sites, technical training, provision of material supports, joint propagation activities and joint responses to malaria epidemics. This project was incorporated into the Five-Year Plan of Action on Lancang-Mekong Cooperation (2018—2022) in China in 2018, with 5 liaison offices and 20 liaison workstations established in Myanmar, Laos, Vietnam, Cambodia, and Thailand, and 21 cross-border malaria surveillance sites assigned in border areas of Myanmar, Laos and Vietnam, and a long-term malaria prevention and control cooperation mechanisms was established through meetings, training, propagation, and joint investigations. Currently, Yunnan Province is poised to engage in more extensive and in-depth cooperation with neighboring countries, including malaria diagnosis and treatment techniques, drug and vaccine research and development, talent cultivation, information sharing, cross-border human health services, and health promotion, under the guidance of the Five-Year Plan of Action on Lancang-Mekong Cooperation (2023—2027).

Chinese Journal of Ultrasonography 2025;34(1):49-51
Right atrial diverticulum is a rare congenital cardiac malformation with a very low incidence and few reports in the literature. It is a sac-like structure originating from the free wall of the right atrium or the right auricle, and the wall of the diverticulum is mostly composed of normal myocardial tissue. Patients with diverticulumare asymptomatic in the early stage of the disease, and it is mostly detected by imaging examinations. The patient was a 96-year-old male admitted to the Second Hospital of Hebei Medical University with intermittent chest tightness, shortness of breath, and dizziness for more than half a month. Echocardiography and contrast echocardiography were performed 1 d after the patient was admitted to the hospital, suggesting the diagnasis of right atrial diverticulum with thrombosis, and coronary CT angiography was performed 4 d after the patient was admitted to the hospital, which considered right atrial diverticulum, with an internal filling defect, and suspected thrombus formation. The patient was older and did not receive further diagnosis and treatment, and cardiac ultrasound was repeated 1 month later, with no significant change in the size and morphology of the mass within the right atrial diverticulum. By analyzing in detail a case with right atrial diverticulum initially misdiagnosed as pericardial effusion, this article aimed to raise cardiac sonographers' awareness of the diagnosis of right atrial diverticulum and to explore the key role of cardiac acoustic contrast technology in cardiac ultrasound diagnosis.

Objective:To utilize the left ventricular myocardial work(LVMW)technique for early identification of myocardial damage in patients with fabry disease(FD).Methods:In an observational cross-sectional study,35 patients with FD who visited the Second Hospital of Hebei Medical University from February 2023 to April 2024 were included. They were categorized into two groups based on left ventricular mass index(LVMI):the non-left ventricular hypertrophy group(LVH-FD group,14 cases)and the left ventricular hypertrophy group(LVH+FD group,21 cases). Further gender-stratified analysis was performed on parameters related to left ventricular global longitudinal strain(LVGLS)and LVMW. Additionally, for the FD group with normal LVGLS,a comparison of their LVMW-related parameters was made with a control group. A control group consisting of 28 healthy subjects from the same period was selected for comparison. The LVGLS,global work index(LVGWI),global constructive work(LVGCW),global wasted work(LVGWW),and global work efficiency(LVGWE)of the three groups were analyzed using two-dimensional speckle tracking and non-invasive myocardial work techniques, and intergroup comparisons of these parameters were performed. Pearson's linear correlation was applied to analyse the correlation between LVGWI,LVGWE and LVMI.Results:① According to the LVMI grouping results, compared with the control group, the LVH-FD group showed decreased LVGWI( P<0.05),while the LVH+FD group exhibited reductions in LVGLS,LVGWI,and LVGWE(all P<0.05). Compared to the LVH-FD group, the LVH+FD group demonstrated lower LVGLS,LVGWI,and LVGWE(all P<0.05). ② Gender subgroup analysis revealed that in the female subgroup,no statistically significant differences were observed in LVGLS,LVGWI,and LVGWE between the female control group and the female LVH-FD group(all P>0.05). However, the female LVH+ FD group showed decreased LVGLS,LVGWI,and LVGWE compared to both the female control group(all P<0.05)and the female LVH-FD group(all P<0.05). In the male subgroup, compared to the male control group, the male LVH-FD group had reduced LVGWI( P<0.05)but no significant differences in LVGLS or LVGWE(both P>0.05). The male LVH+ FD group displayed lower LVGLS,LVGWI,and LVGWE compared to both the male control group(all P<0.05)and the male LVH- FD group(all P<0.05). ③Compared with the control group, the LVGLS-normal FD group had a reduced LVGWI( P<0.05),while there were no statistically significant differences in LVGLS,LVGWE,LVGCW,and LVGWW between these two groups(all P>0.05). ④LVGWI and LVGWE were negatively correlated with LVMI( r=-0.617,-0.707;both P<0.001). Conclusions:LVMW can detect early cardiac dysfunction in patients with FD and is of guiding value for their clinical management.

Objective:To develop an artificial intelligence system using fully convolutional neural networks(FCN)to assist echocardiographers in the quantitative assessment of mitral regurgitation(MR)severity. Methods:From August 2021 to June 2024,echocardiographic images of 441 patients with MR were prospectively collected from Renmin Hospital of Wuhan University and the Central Hospital of Wuhan. After screening,a total of 269 patients(4 917 frames)were included in the study. Of these,3 644 frames(128 patients)of apical four-chamber color Doppler MR flow convergence images from Renmin Hospital of Wuhan University were selected as the training/validation set, while images from 121 patients(813 frames)were used as the internal test set. Additionally, images from 20 patients(460 frames)from the Central Hospital of Wuhan were selected as the external test set. The FCN algorithm was employed to capture features and segment the MR color region on the left atrial side, simultaneously outputting the regurgitant radius(r)for the calculation of the effective regurgitant orifice area and regurgitant volume. The severity of MR was then classified according to the 2017 guidelines of the American Society of Echocardiography. The segmentation and classification performance of the model was evaluated, and the measurement results of the AI system was compared with that of both senior and junior physicians. Results:In the internal test set,the accuracy of r identification for cases classified as Grade Ⅰ to Ⅳ was 0.48,0.81,0.86,and 0.87,respectively. In the external test set,the accuracy of r identification for cases classified as Grade Ⅰ to Ⅳ was 0.60,0.77,0.64,and 0.77,respectively. The average accuracy of MR classification in the internal and external test sets was 0.91 and 0.88,respectively.Conclusions:The FCN model is capable of segmenting the left atrial side regurgitant areas in apical four-chamber heart color Doppler images, aiding physicians in obtaining quantitative assessment parameters for MR,and assisting junior physicians in accurately assessing the severity of MR.
