1.Construction and validation of a machine learning-based risk assessment model for primary ovarian cancer

Objective To explore the clinical value of a machine learning model constructed based on computed tomography (CT) imaging features and serological markers for predicting the risk of primary ovarian cancer. Methods We retrospectively collected the baseline data, imaging features, and laboratory indicators of 490 patients with ovarian lesions who visited the Department of Gynecology and Obstetrics at Nanjing Pukou People’s Hospital between March 2021 and January 2025, all confirmed by histopathological examination. The patients were randomly divided into a training set and a validation set in a 7∶3 ratio. Based on the pathological results, patients in each dataset were categorized into a benign lesion group and a primary ovarian cancer group for comparison. All data from the training set patients were included in LASSO regression to screen for optimal indicators. Machine learning algorithms were established based on eight classifiers to identify the best model. Data were then incorporated into this model to screen for independent predictors. The prediction model was subsequently evaluated, visualized, and internally validated using the validation set. Results LASSO regression selected 12 variables. Among the various machine learning models, the logistic regression model showed the best performance, with the highest area under the receiver operating characteristic curve (AUC) and F1 score). Binary logistic regression model analysis indicated that venous phase CT value, CT perfusion collateral index score, CA199 level, HE4 level, tumor margin features, and lymphocyte-to-monocyte ratio were all influencing factors for primary ovarian cancer (P < 0.05). The AUC of this prediction model was 0.89, while the AUC in the validation set was 0.80, indicating favorable clinical benefit. Conclusion The machine learning model based on the integration of CT imaging features and laboratory indicators demonstrates high performance in the prediction of primary ovarian cancer risk.
2.Study on site-specific biotinylation of goat IgG mediated by photoactivatable protein G-Avitag fusion protein

Objective:To construct three photosensitive fusion proteins between protein G C3 domain and Avitag for site-spe-cific biotinylation of goat IgG.Methods:DNA sequences of protein G C3 domain with amber codon mutation and C-terminal containing 1,2,3 Avitag were obtained by chemical synthesis,respectively,and sub-cloned into pTXB1 plasmid to construct three fusion pro-tein expression vectors.Three plasmids and pEVOL-pBpF plasmid were co-transformed into competent bacterium E.coli BL21,respec-tively.Target fusion proteins CBpa-Avitag,CBpa-(Avitag)2 and CBpa-(Avitag)3 were prepared using genetic codon extension technology,and were purified by His Trap Chelating HP column,then biotinylated by Avitag/BirA system.Photo-conjugation efficiency,crosslink site and antigen-binding activity of photo-conjugates were investigated by SDS-PAGE,control experiment,and competitive ELISA.Results:Three expression vectors were successfully constructed,and three designed photoactivatable fusion proteins CBpa-Avitag,CBpa-(Avitag)2 and CBpa-(Avitag)3 were successfully expressed with molecular weights consistent with their theoretical values.Heavy-chain conjugation efficiencies of biotinylated fusion protein with goat IgG were 85%with Fc-specific conjugation,and conjugates maintained its antigen-binding activity after photo-conjugation.Conclusion:CBpa-Avitag,CBpa-(Avitag)2 and CBpa-(Avitag)3 fusion proteins are successfully prepared,and mediates by those photosensitive fusion proteins,biotins molecules are conjugated to Fc fragment of goat IgG,results site-specific biotinylation of goat IgG.

Objective:To assess the efficacy and safety profile of antaitasvir phosphate combined with yiqibuvir in the treatment of chronic hepatitis C (CHC) of various genotypes, without cirrhosis or with compensated cirrhosis.Methods:394 cases with CHC from 22 centers were collected from October 2021 to April 2023. They were randomly assigned to receive either the experimental drugs (antaitasvir phosphate 100 mg+yiqibuvir 600 mg) or placebo treatment in a 3∶1 ratio. The patients were administered drugs once a day for 12 consecutive weeks, and then followed up for 24 weeks after treatment cessation. All subjects were unblinded at the four-week follow-up following drug discontinuation, with the experimental drug group continuing to complete subsequent post-discontinuation follow-up. The placebo group was switched to receive the experimental drugs for a repeated 12-week treatment period and followed up for another 24 weeks after discontinuation of the drug (placebo delayed treatment phase).The sustained virologic response rate (SVR12) was observed for subjects in the double-blind phase and the placebo delayed-treatment phase at 12 weeks after treatment cessation.Virological resistance analysis was performed on subjects who failed treatment. Central venous pressure(CVP)is a commonly used hemodynamic index in clinical practice,and the accuracy of its measurement results is influenced by the measurement technical specifications.Based on research evidence and clinical practice,this article interprets the main content of the group standard of CVP measurement technology,and proposes suggestions for its use.It aims to guide clinical nursing staff to correctly understand the principles and methods of CVP measurement,and promote the standardization of nursing practice and the accuracy of measurement results.
5.Clinical efficacy of propofol combined with remifentanil anesthesia on plasma adenoidectomy in children

Objective:To investigate the application effect of propofol combined with remifentanil for anesthesia in children undergoing plasma adenoidectomy.Methods:Clinical data of 103 children who underwent plasma adenoidectomy in our hospital from Apr 2023 to Apr 2024 were retrospectively analyzed.Children were divided into two groups according to different anesthesia schemes.Children who received propofol+ketamine anesthesia were enrolled in the control group(n=52)and children who received propofol+remifentanil anesthesia were enrolled in the study group(n=51).Hemodynamic parameters were compared between the two groups at four time points:before anesthesia(T0),immediately after extubation(T1),5 minutes after extubation(T2),and 10 minutes after extubation(T3).The pain scores(CHIPPS)of the two groups were compared at 10,20,and 30 min after extubation.Post-anesthesia evaluation of discomfort(PAED)and incidence of restlessness during recovery in children were compared.The indicators of stress response(cortisol and epinephrine)were measured preoperatively and 1 day postoperatively.Total perioperative adverse events were analyzed.Results:The recovery times for spontaneous breathing,eye-opening,and extubation in the study group were shorter than those in the control group(P<0.05).The heart rate and mean arterial pressure level in the study group at T1-T3 were lower than those in the control group(P<0.05).The CHIPPS scores at 10,20,and 30 min after extubation and PAED scores during the recovery period in the study group were lower than those in the control group(P<0.05).The incidence of restlessness during the recovery period in the study group was lower than that in the control group(P<0.05).The levels of cortisol and epinephrine in the study group were lower than those in the control group on day 1 after surgery(P<0.05).There was no significant difference in the overall incidence of adverse reactions between the two groups(P>0.05).Conclusion:The use of propofol combined with remifentanil in plasma adenoidectomy can effectively shorten the recovery time of anesthesia in children,enhance analgesic effects,reduce blood circulation fluctuations and stress reactions,and reduce the incidence of restlessness,with reliable safety.

10.Changes in plantar pressure of subjects with different foot positions during walking and jogging after Kinesio taping

BACKGROUND:Kinesio taping is often used for the treatment of various sports injuries.The methods of foot and ankle sports taping are complex and diverse.Among them,Fascia taping is applicable to a wider range of people and can be used for different foot posture types,but it still lacks of practical verification,and its specific biomechanical role is not clear. OBJECTIVE:To observe the changes in plantar pressure characteristics of subjects with different foot positions during walking and jogging after Fascia taping. METHODS:Thirty-seven young healthy subjects were recruited from the Yantai campus of Binzhou Medical University to conduct the test.They were scored according to the foot posture index-six items version,and were divided into the supination foot group,the neutral foot group,and the pronation foot group.The static foot morphological indexes(including navicular drop,arch height index,arch height flexibility-longitudinal arch,and arch height flexibility-transverse arch)and the pressure-time integral of each foot zone during walking and jogging were collected and calculated respectively before and after Kinesio taping.The specific biomechanical mechanism of Fascia taping was analyzed. RESULTS AND CONCLUSION:(1)General data:There was no statistical difference among the three groups of subjects in general data,such as gender,height,and body mass index(P>0.05).Before taping,there was a significant difference in the foot morphological indexes and the areas of the outer front foot,midfoot,and hindfoot between different foot posture groups(P<0.01).(2)Static foot morphological indexes:After taping,there was no statistically significant difference between the groups in navicular drop,arch height flexibility-longitudinal arch,and arch height flexibility-transverse arch(P>0.05),while there was still a significant difference between the groups in the arch height index(P<0.05).In the supination foot group,the arch height index increased slightly,but there was no significant difference before and after taping(P>0.05).In the pronation foot group,the navicular drop and arch height flexibility-longitudinal arch was significantly reduced,and the arch height index was increased.There was a significant difference before and after taping(P<0.05).(3)The index of plantar pressure during walking:After taping,there was no significant difference between the three groups in the area of lateral forefoot and medial midfoot(P>0.05).In the pronation foot group,the lateral load of the forefoot increased after taping(P<0.05).In the supination position group,the load of the lateral forefoot and midfoot regions increased significantly(P<0.05),while the difference in the rear foot region was not significant(P>0.05).(4)The index of plantar pressure during jogging:After taping,there was no statistically significant difference between groups in the lateral forefoot(P>0.05).In the pronation foot group,the load of the medial forefoot increased significantly(P<0.05).In the supination position group,the load of the lateral forefoot,the middle foot and the rear foot region increased significantly(P<0.05).(5)The results showed that the Fascia taping was suitable for different foot postures.It could not only correct the static foot structure of subjects with different foot postures,but also regulate the abnormal plantar pressure distribution during the dynamic activities of walking and jogging,and the load of the midfoot,forefoot,and hindfoot in the supination and pronation posture tended to normal foot posture load level.
