1.Effect of Changji'an Formula (肠激安方) on the miR-29b-3p/TRAF3/NF-κB/MLCK Axis in Colonic Tissues in Diarrhea-predominant Irritable Bowel Syndrome Model Rat with Liver Depression and Spleen Deficiency Syndrome

ObjectiveTo explore the potential mechanism of Changji'an Formula (肠激安方) on intestinal permeability for rats with diarrhea-predominant irritable bowel syndrome (IBS-D) of liver depression and spleen deficiency syndrome by the microRNA-29b-3p (miR-29b-3p)/tumor necrosis factor receptor-associated factor 3 (TRAF3)/nuclear factor-κB (NF-κB)/myosin light chain kinase (MLCK) axis. MethodsTwenty-four 1-day-old male Sprague-Dawley (SD) suckling rats were selected, and the IBS-D rat model of liver depression and spleen deficiency syndrome was established via a three-factor method,i.e. maternal separation plus acetic acid stimulation and restraint stress, for 6 consecutive weeks. After successful modeling, the rats were randomly divided into a model group, pinaverium bromide group, low-dose and high-dose Changji'an Formula groups, with 6 rats in each group. Another 6 age-matched non-modeled SD rats were included as the control group. The low-dose and high-dose Changji'an Formula groups were given intragastric administration of Changji'an Formula solution at doses of 16.74 g/(kg·d) and 33.48 g/(kg·d), respectively; the pinaverium bromide group received intragastric administration of pinaverium bromide tablets at 0.018 g/(kg·d); and the control group was given distilled water at 10 ml/(kg·d) via intragastric gavage. The intervention was conducted once daily for 14 consecutive days. After the gavage treatment, the fecal water content of rats in each group was measured. Enzyme-linked immunosorbent assay (ELISA) was used to detect the serum levels of intestinal permeability indicators, including D-lactic acid (D-LA), diamine oxidase (DAO), and lipopolysaccharide (LPS). Quantitative real-time polymerase chain reaction (qRT-PCR) was conducted to determine the mRNA expression levels of miR-29b-3p, TRAF3, tumor necrosis factor-α (TNF-α), p65, p50, and MLCK in colonic tissues. Western Blot analysis was employed to detect the protein expression levels of TRAF3, TNF-α, p65, phosphorylated p65 (p-p65), MLCK, myosin light chain (MLC), phosphorylated MLC (p-MLC), and tight junction proteins including junctional adhesion molecule-A (JAM-A), Occludin, and Claudin-1 in colonic tissues. ResultsCompared with the control group, the model group exhibited significantly increased fecal water content and serum levels of D-LA, DAO, and LPS, along with decreased protein expression levels of JAM-A, Occludin, and Claudin-1 in colonic tissues (P<0.05 or P<0.01). Additionally, in the model group, the mRNA expression levels of miR-29b-3p, TNF-α, p65, p50, and MLCK in colonic tissues were up-regulated, while the mRNA and protein expression levels of TRAF3 were down-regulated; the protein levels of TNF-α and MLCK, as well as the ratios of p-p65/p65 and p-MLC/MLC, significantly elevated (P<0.01). Compared with the model group, all treatment groups showed reduced fecal water content and serum levels of D-LA, DAO, and LPS, along with down-regulated mRNA expression levels of miR-29b-3p, TNF-α, p65, p50, and MLCK, and up-regulated TRAF3 mRNA expression in colonic tissues. Moreover, the pinaverium bromide group and high-dose Changji'an Formula group presented increased protein levels of Occludin, Claudin-1, and TRAF3, as well as decreased protein levels of TNF-α and MLCK, and reduced ratios of p-p65/p65 and p-MLC/MLC in colonic tissues (P<0.05 or P<0.01). Compared with the low-dose Changji'an Formula group, the high-dose group had lower fecal water content and serum levels of DAO and LPS (P<0.01). In comparison with the pinaverium bromide group, the high-dose Changji'an Formula group showed a significant decrease in serum DAO level (P<0.01). ConclusionsChangji'an Formula can reduce intestinal permeability and restore intestinal barrier function in IBS-D rats of liver depression and spleen deficiency syndrome by regulating the miR-29b-3p/TRAF3/NF-κB/MLCK axis.
2.Epidemiological analysis and incidence trend prediction of mumps in Nantong City in 2016 - 2023

Objective To analyze the epidemiological characteristics and incidence trend of mumps in Nantong City from 2016 to 2023, and to provide a basis for scientific prevention and control. Methods Descriptive statistical analysis was conducted on monthly mumps incidence data in Nantong City from 2016 to 2023, and a SARIMA model was constructed to predict the incidence trend. Results A total of 3 928 mumps cases were reported in Nantong City from 2016 to 2023, with an average annual incidence rate of 6.36/100 000. Cases predominantly occurred during April–July and September–December, showing seasonal peaks. The male-to-female ratio was 1.43:1. The cases were concentrated in individuals aged ≤15 years (85.57%), primarily students, children in childcare settings, and scattered children (88.31%). The optimal model constructed by the SPSS Expert Modeler was SARIMA(1,0,0)(0,1,1)12, predicting an epidemic peak of mumps in Nantong City from September to November in the second half of 2024. Conclusion From 2016 to 2023, mumps incidence in Nantong City has shown an overall downward trend with a seasonal bimodal distribution. Males had higher incidence rates than females, and cases were predominantly students, kindergartens and scattered children, highlighting the need to strengthen mumps prevention in schools and childcare institutions. The SARIMA model is suitable for short to medium term prediction of mumps in Nantong City. A minor peak is predicted to occur in the second half of 2024 (September–November). It is necessary to strengthen the epidemic surveillance of mumps and vaccination of age-eligible children.
3.Influence of occupational hazard factors on incidence of hypertension in calcium carbide plant: Historical cohort study

Background The high work intensity and possible subsequently increased susceptibility to occupational hazards of calcium carbide plants may lead to hypertension in workers, but there are few studies on the relationship between occupational hazard exposure and hypertension in workers involving the production process of calcium carbide. Objective To explore the influence of occupational hazards on the incidence of hypertension in calcium carbide plants. Methods Using historical cohort design, the employees of a calcium carbide factory in the western part of Inner Mongolia Autonomous Region were selected as research subjects. According to the pre-determined inclusion and exclusion criteria, the study population comprised an exposure group of 377 employees (including furnace workers, inspection workers, and maintenance workers) exposed to dust, noise & carbon monoxide, and a control group of 388 employees (including central control workers, electricians, and administrative personnel) without above-mentioned exposure. The total sample size was 765 participants. The follow-up period was from April 2011 to October 2022, and the study endpoint was defined as the conclusion of the follow-up period or diagnosed hypertension in annual occupational health examination. Information on general demographic characteristics, living habits, and work status was collected from all study subjects. Cox proportional hazards regression model was used to analyze the association between occupational hazard exposure and the risk of hypertension among the calcium carbide plant employees. Results The average age, mean systolic and diastolic blood pressure, proportion of males, smoking rate, and alcohol consumption rate in the exposure group were higher than those in the control group (P<0.05). Compared to baseline, both systolic and diastolic blood pressure levels increased in the exposure group and the control group at the end of the follow-up (P<0.05). At the end of the follow-up, the average differences between systolic/ diastolic blood pressure and baseline values in the exposure group were higher than those in the control group (P<0.05). Objective To investigate the effect of lifestyle on cognitive decline in rural people aged 40 years and older in Xi'an.Methods This was a prospective cohort study.People aged 40 years and older in two villages in Huyi District were selected as the study population.They completed the baseline survey from October 2014 to March 2015 as well as two follow-up visits in 2016 and 2018,respectively.A comprehensive score of lifestyle was calculated based on factors including smoking,drinking,exercise,and diet collected at the baseline.The Mini-Mental State Examination(MMSE)was used to evaluate global cognitive function at both baseline and follow-up;a≥4-point decrease in MMSE score from the baseline was defined as marked cognitive decline.Multivariable Logistic regression,propensity score correction,and propensity score matching were used to investigate the relationship between lifestyle and cognitive decline.Results A total of 1 348 participants were ultimately enrolled and 56(4.2%)of them met the criteria for marked cognitive decline(△MMSE≥4-points).Among them,386(28.6%)people had smoking history,184(13.6%)were drinkers,214(15.9%)lacked physical activity,and 400(29.7%)ate a diet high in oil and salt.Generally,304(22.6%)met the definition of the unhealthy lifestyle(comprehensive score<6),which means more than one of the four subscales was unhealthy or more than two were relatively unhealthy.Multivariable Logistic regression analysis showed that unhealthy lifestyle was positively associated with marked cognitive decline(OR=2.838,95%CI:1.302-5.525,P=0.005).Propensity-score adjusted model yielded very similar results(OR=2.786,95%CI:1.371-5.661,P=0.005).Propensity score matching was performed to further balance the differences in covariates between the two groups.Multivariate Logistic regression analysis conducted in the matched population revealed that the risk of marked cognitive decline was still higher in those with unhealthy lifestyle(OR=3.994,95%CI:1.582-12.176,P=0.006).Conclusion Unhealthy lifestyle is associated with an increased risk of cognitive decline in cognitively normal people aged 40 years and older.
5.Impact of ultrasound guided interscalene versus axillary brachial plexus block on patency rate of arteriove-nous fistula

Objective To compare the safety and efficacy of various brachial plexus block techniques using local anesthesia(LA)in patients undergoing their first radiocephalic arteriovenous fistula(RCAVF)surgery.Methods Patients were randomly allocated into three groups:LA,interscalene brachial plexus block(ISBPB),and axillary brachial plexus block(ABPB).Ultrasound was utilized to evaluate the pre-and post-anesthesia changes in vessel diameter and blood flow.Postoperative follow-up assessments were performed at three days and three months to determine fistula patency.Results Immediate patency rates were 92.52%(LA),96.26%(ISBPB),and 95.33%(ABPB),with no statistically significant differences among the groups(χ2=1.615,P=0.446).However,at three months,primary patency rates differed significantly among the groups(χ2=22.691,P<0.001).Specifically,the ISBPB group(83.18%)exhibited significantly higher patency compared to the LA group(57.01%)(χ2=17.477,P<0.001).Similarly,the ABPB group(80.37%)demonstrated better patency than the LA group(χ2=13.580,P<0.001).Regarding respiratory complications,they were more prevalent in the ISBPB group(15.89%)compared to the LA group(2.80%)(χ2=9.761,P=0.002)and the ABPB group(0.93%)(χ2=14.377,P<0.001).No significant difference was observed between the LA and ABPB groups in terms of respiratory complications(χ2=1.019,P=0.313).Conclusions Both ISBPB and ABPB demonstrated superior primary patency compared to LA.Nevertheless,ABPB exerted a lesser impact on respiratory function and provided a more comfortable surgical experience for ESRD patients.
6.Emodin promotes autophagy to improve myocardial injury in septic model mice

BACKGROUND:Emodin has a variety of pharmacological activities such as anti-inflammatory,anti-viral and anti-oxidative stress,and also has a certain protective effect on sepsis-induced myocardial injury,but its mechanism of action is still unclear.OBJECTIVE:To investigate whether emodin can improve myocardial injury in septic mice by promoting autophagy.METHODS:Thirty-two male Kunming mice were divided into sham operation group(n=4),sham operation+emodin group(n=4),model group(n=8),model+emodin group(n=8),and emodin+3-methyladenine group(n=8).The myocardial injury model of septic mice was constructed by cecal ligation and puncture.3-methyladenine(10 mg/kg)was injected intraperitoneally 1 hour before modeling.Emodin(20 mg/kg)was injected intraperitoneally 30 minutes before modeling,and the other groups were injected with the same amount of normal saline at the same time point.Blood and myocardial samples were collected from all mice 24 hours after surgery.ELISA was used to detect the levels of brain natriuretic peptide and cardiac troponin Ⅰ in serum.Western blot assay was used to detect the protein expression of LC3B,Beclin-1,and p62 in myocardial tissue.Hematoxylin-eosin staining was used to observe the pathological changes in myocardial tissue.Ultrasound was used to evaluate the cardiac function of mice.RESULTS AND CONCLUSION:(1)Compared with the sham operation group,there was no significant difference in the levels of serum brain natriuretic peptide,cardiac troponin Ⅰ,and the protein expression of myocardial autophagy proteins LC3Ⅱ/LC3Ⅰ and p62 in the sham operation+emodin group(P>0.05).(2)Compared with the sham operation+emodin group,the levels of serum brain natriuretic peptide and cardiac troponin I were significantly increased in the model group(P<0.05).Compared with the model group,the levels of serum brain natriuretic peptide and cardiac troponin I were decreased in the model+emodin group(P<0.05).(3)Compared with the model group,the expression of LC3Ⅱ/LC3Ⅰ and Beclin-1 protein was increased and the expression of p62 protein was decreased in the myocardial tissue of the model+emodin group(P<0.05).Compared with the model+emodin group,the expression of LC3Ⅱ/LC3Ⅰ and Beclin-1 protein decreased and the expression of p62 protein increased in the emodin+3-methyladenine group(P<0.05).(4)The myocardial fibers in the sham operation group were normal,the myocardial fibers in the model group were disordered with a large number of inflammatory cell infiltration,the myocardial fibers in the model+emodin group were slightly disordered,and some vacuolar changes were observed.The myocardial fibers were disordered,and more inflammatory cell infiltration was observed in the emodin+3-methyladenine group.(5)Compared with the sham operation group,the left ventricular short axis shortening rate and left ventricular ejection fraction were decreased in the model group(P<0.05).Compared with the model group,the left ventricular short axis shortening rate and left ventricular ejection fraction were increased in the model+emodin group(P<0.05).Compared with the model+emodin group,the left ventricular ejection fraction of emodin+3-methyladenine group was decreased(P<0.05),and the left ventricular short axis shortening rate was reduced but not statistically significant(P>0.05).(6)The above results indicate that emodin pretreatment can improve myocardial injury and myocardial dysfunction in septic mice by promoting autophagy.
7.Does repeated low-level red light therapy affect cone density in the macula of myopic children?

The increasing prevalence of myopia has become one of the major global public health issues.Current myopia control measures, such as orthokeratology lenses, defocus spectacles, soft defocus contact lenses, low-concentration atropine eye drops all have limitations.Repeated low-level red light (RLRL) therapy emerging in recent years has become one of the treatments for childhood myopia.Multiple clinical studies have shown that RLRL irradiation can inhibit the rapid progression of myopia in children and adolescents.Recently, a research report titled Cone Density Changes After Repeated Low- Level Red Light Treatment in Children With Myopia, which has sparked widespread discussion among parents of myopic children.The main purpose of this article is to conduct a detailed analysis of the research methods and results, and to explore whether the conclusion can be drawn that RLRL affects the density of macular cone cells in myopic children.
8.Analysis of myopia progression and risk factors among Chinese young medical college students: a two-year longitudinal study

Objective:To investigate the myopia progression in Chinese young medical college students and explore the associated risk factors.Methods:A cohort study was conducted.Among 1 068 freshmen aged 16 to 22 years receiving health checkups at a medical university in Tianjin, 979 myopes were ultimately included in the baseline assessment and 812 participated in the follow-up assessment after two years.The anterior segment examination with a slit lamp, non-cycloplegic autorefraction with an autorefractor and axial length (AL) measurements with Lenstar 900 were performed on participants at baseline and during the two-year follow-up.Myopia progression was defined as a spherical equivalent (SE) change of ≤-0.50 D/year or an AL increase of ≥0.20 mm/year.Multivariate regression analysis with the generalized estimating equation model was employed to identify risk factors associated with myopia progression.This study adhered to the Declaration of Helsinki.The study protocol was approved by the Ethics Committee of Tianjin Medical University Eye Hospital (No.2021KY-16).Informed consent was obtained from each subject.Results:During the two-year follow-up, 92.5%(751/812) of the participants had stable SE and 96.1%(764/795) had stable AL.However, 7.5%(61/812) showed SE progression and 3.9%(31/795) exhibited AL growth, demonstrating a tendency of myopia progression.Multivariate linear regression analysis revealed that females ( β=0.064, 95% CI: 0.002~0.126; P=0.042) and low to moderate myopia at baseline ( β=0.083, 95% CI: 0.005~0.161; P=0.037) were significantly associated with AL growth compared to high myopia at baseline. Conclusions:More than 92% of young college students have stable myopia.In addition to high myopia, there is still a need for better follow-up and management of females and those with low to moderate myopia to control the high prevalence of high myopia.
9.Evaluation of the effect of spectacle lens wear on exophthalmometry values in myopia using propensity score matching

Objective:To investigate whether spectacle lens wear affects the exophthalmometry values (EVs) on myopia and explore the risk factors for EVs in myopia.Methods:A cross-sectional study was conducted.A total of 935 university freshmen (935 eyes) who received eye examinations were enrolled from September to December 2019.Anterior segments were examined by slit lamp microscopy.EVs were measured with a Hertel exophthalmometer.Non-cycloplegia auto-refraction, lensometer test, visual acuity test and subjective refraction were performed on all subjects.Ocular biometric parameters including axial length (AL) and average corneal radius (CR) were obtained by Lenstar 900.A 1∶1 propensity score matching (PSM) was performed between the spectacle group and the non-spectacle group to compare the differences in EVs and visual acuity.Linear regression was used to analyze the effect of different factors on the EVs in all myopic students.This study adhered to the Declaration of Helsinki.The study protocol was approved by the Ethics Committee of Tianjin Medical University Eye Hospital (No.2021KY-16).Written informed consent was obtained from each subject.Results:The mean EVs of students was (14.03±1.87)mm.There were significant differences in the distribution of myopia severity between the spectacle group and non-spectacle group before matching ( χ2=345.800, P<0.001), and after PSM, the baseline characteristics of the two groups were well balanced with good comparability.After PSM, there was no significant difference in EVs between the spectacle group and non-spectacle group ([13.93±1.87]mm vs.[13.66±1.85]mm; t=1.140, P=0.25), and the spectacle group had better visual acuity of 1.0(0.8, 1.0) than 0.4(0.2, 0.8) in non-spectacle group, with a statistically significant difference ( Z=-8.450, P<0.001).Multivariate linear regression showed that EVs increased by 0.06 mm for every 1 D increase in spherical equivalent towards myopia ( β=-0.06, 95% CI: -0.11--0.01, P=0.03), and EVs increased by 0.17 mm for every 1 mm increase in AL ( β=0.17, 95% CI: 0.06-0.28, P<0.01).Average CR did not influence EVs significantly ( β=0.07, 95% CI: -0.10-0.24, P=0.43). Conclusions:Wearing spectacles may not affect the EVs and not wearing spectacle may affect visual acuity in myopic patients.The higher the degree of myopia, the longer the AL, the higher the EVs may be.
10.A three-year cohort study comparing myopia progression between children with axial and refractive myopia

Objective:To compare the changes in spherical equivalent (SE) and axial length (AL) between children with axial myopia and refractive myopia.Methods:A prospective cohort study was conducted.A total of 1 738 students from grades 1 to 6 were recruited from two consistent 9-year schools in the Binhai New Area of Tianjin using cluster random sampling.Visual acuity, refractive status, and ocular biological parameters were measured from February to May in 2018 and 2021.Participants were categorized into subgroups as follows: low, moderate, and high myopia based on SE; longer AL group and shorter AL groups based on AL; and steeper cornea and flatter cornea groups based on corneal curvature radius (CCR). Myopic children were further classified into the following groups: axial myopia (longer AL and flatter cornea), refractive myopia (shorter AL and steeper cornea), mixed myopia (longer AL and steeper cornea), and non-axial non-refractive myopia (shorter AL and flatter cornea). Changes in SE (ΔSE) and AL (ΔAL) at the end of the follow-up period were compared among the different classification groups.This study adhered to the Declaration of Helsinki and was approved by the Ethics Committee of Tianjin Medical University Eye Hospital (No.2020KY-39). Written informed consent was obtained from the guardians of all participants.Results:The ΔSE in the longer AL group was (-1.57±1.52)D, which was significantly greater than (-1.17±1.47)D in the shorter AL group ( t=3.99, P<0.01). The ΔAL in the steeper cornea group was (0.92±0.50)mm, which was significantly greater than (0.86±0.54)mm in the flatter cornea group ( t=-2.12, P=0.04). Among children aged 10-12 years, males, and the low myopia, SE progression was faster in those with longer AL compared to shorter AL, with statistically significant differences ( t=2.66, 3.31, 3.90; all P<0.05). In children aged 10-12 years, AL growth was faster in the longer AL group than in the shorter AL group, with a statistically significant difference ( t=-1.29, P=0.04). Among females and the low myopia, AL growth was faster in those with steeper corneas than in those with flatter corneas, with statistically significant differences ( t=-3.22, -2.43; both P<0.05). Refractive myopia had a smaller ΔSE than axial myopia and the difference was statistically significant ( P<0.05). Within the low myopia, SE progression was greater in axial myopia than in refractive myopia, with a statistically significant difference ( P<0.05). Conclusions:Among myopic children, those with longer axial lengths exhibit faster SE progression, while those with steeper corneas show faster axial elongation.Among children with low myopia, axial myopia is associated with a greater risk of SE progression than refractive myopia.
