1.Analysis of new and serious adverse drug reactions from 2020 to 2024

Objective To investigate the clinical characteristics of new and serious adverse drug reactions(ADRs)and provide a basis for safe medication use.Methods A retrospective analysis was conducted on 279 cases of new and serious ADRs reported at Guangzhou Chest Hospital from 2020 to 2024.Data on the type of ADR report,organs/systems involved,patient age and gender,categories of drugs involved,and dosage forms were collected for characteristic and pattern analysis.Results The majority of cases were serious ADRs(63.8%),with a higher proportion in males(57.3%,male-to-female ratio=1.34∶1).Patients aged 45 years and above accounted for 63.1%of cases.The ADRs involved 96 drugs from 12 categories,with anti-infec-tive drugs accounting for 75.0%.Oral administration(65.1%)and intravenous infusion(32.6%)were the main routes of ad-ministration.Digestive system damage was the most common(39.1%),followed by skin and appendage damage(12.3%),he-matologic system damage(10.5%),and nervous system damage(9.6%).Conclusion The occurrence of ADRs is associated with multiple factors such as age and route of administration.There is a need to strengthen multidisciplinary collaboration,en-hance medication monitoring and ADR surveillance in clinical practice to ensure medication safety.

3.Application of subject-achieving quality control circle in the popular science intervention of rational medication use for patients with newly diagnosed tuberculosis

Objective To investigate the application value of the subject-achieving quality control circle theory in enhan-cing medication adherence among newly diagnosed tuberculosis patients through via popular science intervention.Methods From September 2023 to April 2024,100 patients with newly diagnosed tuberculosis treated at the outpatient department were ran-domly divided into control and intervention groups,each comprising 50 cases.The control group received only routine medication education;whereas the intervention group,in addition to this,implemented popular science intervention measures based on the quality control circle theory.The medication adherence,full course completion rate,non-disease interruption rate,and self-re-ported adverse reaction rate were compared.Results After treatment,the intervention group showed higher medication adher-ence(6.76±1.02 vs.6.15±1.36),better adherence rates(78%vs.62%),and higher cure rates(76%vs.42%)than the control group,all with statistical significance.The non-disease interruption rate was also significantly lower in the intervention group(2%vs.14%).Adverse reaction reports were 6%in control and 18%in intervention group.Conclusion The subject-achieving quality control circle effectively improves medication adherence in newly diagnosed tuberculosis patients.

5.Application of PDCA cycle method in internal error control management of intravenous drug dispen-sing center

Objective This study aims to explore the application effectiveness of the PDCA cycle method in preventing and reducing internal errors in the intravenous drug dispensing center(PIVAS).Methods Internal error data from our hospital's PIVAS in 2020 and 2021 were collected.The data from 2020 represented the pre-implementation of the PDCA cycle,while the data from 2021 represented the post-implementation period.The changes in internal errors and error rates before and after imple-mentation were compared.Results After implementing the PDCA cycle management measures,the annual error rate decreased from 0.887%o in 2020 to 0.681‰ in 2021,a decrease of 23.22%.Conclusion The PDCA cycle method can effectively reduce the occurrence of internal errors,improve work accuracy,and ensure the safety of clinical drug use in PIVAS.

7.Effect of tiopronin on renal function during antituberculosis therapy

Objective To investigate the effect of tiopronin on renal function during anti-tuberculosis liver protection therapy.Methods Clinical data of patients with initially treated sensitive tuberculosis treated in our hos-pital from September 2019 to September 2022 and whose anti-tuberculosis regimen was only isoniazid,rifampicin,pyrazinamide and ethambutol were retrospectively analyzed.The patients were divided into study group and control group according to whether tiopronin was used.The baseline data,blood creatinine(Scr),blood urea nitrogen(BUN),urine protein,creatinine clearance,drug combination and related adverse reactions of the two groups were compared.Results Patients obtained based on inclusion and exclusion criteria were divided into a study group(n=102)(antitubercular drugs+tiopronin)and a control group(n=105)(antitubercular drugs+glutathione).There were no statistically significant differences(P>0.05)in ALT,AST,DBIL,and TBIL levels between the two groups before treatment,at Middle and late treatment.At the later stage of treatment,serum Scr,BUN,creatinine clearance and urinary protein showed statistical differences between the study group and the control group(P<0.05).The abnormal rate of indicators and the incidence of adverse reactions in the study group were higher than those in the control group at the later stage of treatment(P<0.05).Conclusion For patients undergoing tuberculosis treatment,the efficacy of tiopronin and glutathione in protecting the liver is comparable.However,in terms of renal function,long-term use of tiopronin is associated with more pronounced damage.Due to the relatively low cost of tiopronin,for families with heavy economic burdens,short-term use of the drug can ensure safety,while long-term use requires close monitoring of renal function changes and timely adjustments to medication.
8.Analysis of adverse events related to linezolid in tuberculosis treatment based on the US FAERS database

Objective To analyze the adverse events(AEs)associated with linezolid in tuberculosis treatment reported in the FDA Adverse Event Reporting System(FAERS)database,assess its safety profile,and provide scientific evidence for ra-tional clinical use.Methods Reports of adverse events related to linezolid in tuberculosis treatment from Q1 2016 to Q1 2024 were extracted.The data were cleaned and pre-processed,and signal detection was performed using the Reporting Odds Ratio(ROR)and Information Component(IC)methods.Results A total of 1,135 linezolid-related AEs were identified,with the most common types being hospitalization(18.46%)and death(10.99%).The Results indicated that linezolid AEs primarily involved disorders of the blood and lymphatic system,as well as liver and biliary system diseases.Common adverse reactions in-cluded anemia,peripheral neuropathy,and optic neuropathy.Additionally,some adverse reaction signals not mentioned in the prescribing information were identified,such as polyneuropathy(ROR=75.25,IC025=5.8),toxic optic neuropathy(ROR=776.95,IC025=8.86),and hearing loss(ROR=14.66,IC025=3.26).Conclusion This study suggests that linezolid may be associated with some serious adverse reactions in tuberculosis treatment.It is recommended that patients on long-term linezolid therapy undergo regular neurological,ophthalmological,and auditory assessments to provide a more adequate basis for rational clinical use.
9.Hormone therapy for tuberculous meningitis patient with history of hormone allergy

Objective The aim is to provide drug regimen for patients who have hormone allergy history but need hor-mone therapy.Methods Pharmacists analyzed the sensitization mechanism of methylprednone in combination with insturction and literature,assisted clinicians to formulate oral hormone therapy for tuberculosis meningitis with history of menthylprednisolone and verified the effectiveness and safety of the drug regimen.Results Methylprednisolone injection contains trace amount of milk protein and should not be used in patients with definite allergy to cow's milk.After changing to oral hormone therapy,the patient did not appear allergic again,and the condition was stable.Conclusion Gucocorticoids can also induce allergic reactions,so patients'previous allergic history should be paid attention in the process of medical history collection.Meanwhile,pharmacists can also give full play to their pharmaceutical expertise to assist clinicians in formulating individualized drug administration plans to ensure the effectiveness and safety of patients.
10.Clinical analysis of pulmonary tuberculosis complicated with Nocardia farcinica

Objective To analyze the clinical characteristics of pulmonary tuberculosis patients compli-cated with infection of Nocardia farcinica aiming to improve the diagnosis and treatment the disease.Methods The clinical data of 22 cases of pulmonary tuberculosis complicated with Nocardia farcinica infection admitted to Guang-zhou Chest Hospital from June 2020 to December 2023 were collected and the clinical manifestations,imaging,laboratory tests,treatment process,and disease outcomes were retrospectively analyzed.Results Among the 22 patients,there were 13 males and 9 females,aged 20~86 years,with a median age of 52 years.Common clinical manifestations included cough(22/22),sputum(21/22),and underlying diseases(13/22).One case was positive for cerebrospinal fluid culture,and 21 cases were positive for sputum culture.The culture period was 5~26 days,with a median culture period of 18 days.The imaging manifestations were mainly plaques,plaques and cavities,and the lesions were spread in both lungs(17/22)and cavities(11/22).After anti-tuberculosis treatment,the absorption of lung lesions in some patients was poor,and the absorption of the lesions was improved after anti-nocardia treat-ment.8 cases were cured,13 cases were improved and 1 case died.Conclusion The clinical symptoms of pa-tients with pulmonary tuberculosis complicated with Nocardia farcinica were atypical,the culture period of Nocardia was long,and the imaging manifestations were similar to pulmonary tuberculosis,which is prone to misdiagnosis and missed diagnosis.For patients with poor response to anti-tuberculosis treatment and slow lesion absorption,the possi-bility of a concurrent Nocardia infection should be considered.
