1.Construction of doctor-patient intersubjectivity in traditional Chinese medicine diagnosis and treatment dialogues

The doctor-patient intersubjectivity is a special application of intersubjectivity within the medical field, representing the optimal developmental direction for doctor-patient relationships. Its construction fundamentally depends on communication and dialogue between both parties. Discourse plays an indispensable role in promoting communication, establishing relationships, and developing intersubjectivity. Traditional Chinese medicine (TCM) diagnosis and treatment dialogues possess distinct Chinese characteristics. The philosophical concepts, specific content, and inherent features they emphasize align with the requirements for constructing intersubjectivity. By focusing on and analyzing the entire process of TCM diagnosis and treatment dialogues, this paper sought to identify methods and pathways for constructing modern doctor-patient intersubjectivity, thereby exploring the significance of TCM diagnosis and treatment dialogues for the development of contemporary and international doctor-patient intersubjectivity.
2.Case report: cerebral and pulmonary fat embolism after facial autologous fat injection

Cerebral fat embolism (CFE) combined with pulmonary fat embolism (PFE) is an extremely rare and serious complication after fat injection. In order to improve the understanding of the mechanism of this complication, this article reported the diagnosis, treatment and pathological results of a patient with CFE and PFE after facial autologous fat injection. A 34-year-old female underwent an injection of autologous fat in her right cheek at another institution in March 2021 and developed into a coma. 1.5 hours later, she was taken to the Emergency Department of the Second Affiliated Hospital of Hainan Medical University. CT revealed a massive infarction in the right cerebral hemisphere and bilateral acute pneumonia. The patient was treated with dehydration, anti-infection, anticoagulation and microcirculation improvement in the intensive care unit. But the patient died due to the critical condition. Postmortem autopsy and histopathological examination, revealed ischemic stroke with hemorrhagic transformation, and pulmonary embolism with acute pneumonia. It was speculated that the fat embolus passed through the damaged main trunk or branch of the right facial artery, and then retrograde to the right external carotid artery, common carotid artery and internal carotid artery to cause middle cerebral artery occlusion. At the same time, the embolus returned to the right atrium along with the accompanying vein of the above artery, and reached the pulmonary vascular bed through the pulmonary circulation, resulting in pulmonary embolism. Plastic surgeons should have extensive knowledge with the anatomical structure of facial blood vessels, master the injection techniques, and improve the awareness of fat embolism when performing facial fat transplantation.

4.Jiuqiang Naoliqing affords protection to vital organs of spontaneous hypertension rats

ObjectiveTo investigate the Jiuqiang Naoliqing's (JNQ) histological influence on hearts, brains and kidneys of spontaneous hypertension rats (SHR). MethodsThe rats were randomly divided into four groups: Wistar control group, SHR group, higher dose JNQ treated SHR group(0.530 g/kg) and lower dose JNQ treated SHR group(0.265 g/kg). The treatment lasted five weeks, and the rats' blood pressure were monitored through tail pulse. After the perfusion procedure, rats' hearts, brains and kidneys were rapidly removed in low temperature condition and stored in 10% formalin solution of 4 ℃.Then routine sections were obtained and the slides were stained with HE.ResultsBefore treatment, the blood pressure of SHR groups were distinctly higher than that of the control group(P<0.01), nevertheless, no obvious blood pressure downgrade were observed after three week and five week treatment. Histopathologic study showed: in SHR group,heart with hypertrophic cardiac muscle, proliferative arterial wall and strictured lumina; Cortex with angiostenosis, proliferative vascular wall and large perivascular space; Glomerulus atrophy with hyaline degeneration. These pathologic changes got respective alleviation after five week treatment of JNQ, particularly in higher dose group.ConclusionSHR who got five week treatment of JNQ didn't gain obvious blood pressure downgrade. But the treatment did good to their vital organs and can obviously alleviate hearts, brains and kidneys' pathologic changes.
