1.Discussion on the Treatment of Insomnia from Liver Based on the Theory "Liver Governs Wei Qi (Defensive Qi)"

Psychological factors have become significant contributors to the onset and progression of insomnia. This article explored the treatment of insomnia from the perspective of “liver governs wei qi (defensive qi)”. The concept of “liver governs wei qi (defensive qi)” is summarized in three aspects, firstly, the liver assists the spleen and stomach in transformation and transportation, governing the generation of wei qi; secondly, the liver aids lung qi diffusion and dispersion, governing the distribution of wei qi; thirdly, the liver regulates circadian rhythms, governing the circulation of wei qi. It is proposed that the clinical treatment of insomnia should focus on the following methods: for regulating the liver to harmonize the five viscera, and facilitate the circulation of wei qi, medicinals entering the liver channel include Chaihu (Bupleuri radix), Baishao (Paeoniae Radix Alba), Zhizi (Gardeniae Fructus), and Suanzaoren (Ziziphi Spinosae Semen) could be commonly used; for nourishing the liver, the treatment should align with the day-night rhythm, and herbs such as Baihe (Lilium), Hehuan (Albizia julibrissin), and Yejiaoteng (Polygoni multiflori caulis) are commonly used; for soothing the liver and address both mental and physical health to calm wei qi, treatment should advocate verbal counseling, psychological regulation, and health education. Ultimately, this treatment approach can free liver qi to flow, soothe qi movement, restore the motion of wei qi, regulate during day and night, balance yin and yang, and resolve insomnia effectively.
2.Study on the Characteristics of TCM Syndromes of Insomnia Based on Multiple Data Analysis Methods

Objective To explore the distribution of common TCM syndromes and symptoms of insomnia;To prepare for the construction of the theoretical framework and item pool of syndrome diagnosis and efficacy evaluation scale.Methods TCM guideline standards of insomnia,textbooks and journals over the years were retrieved,the information of TCM syndromes,syndrome elements and symptoms was extracted,the guideline textbook and journal database were established,and descriptive statistics,association rules,systematic clustering,factor analysis,potential categories and implicit structure analysis were carried out.Results Totally 116 guide standards and textbooks over the years were included,and 454 articles of journals were included.The high-frequency symptoms accounted for≥3%of the guide textbooks and journal databases were 87 and 79 categories,respectively,and the cumulative proportion was 87.48%and 87.75%,respectively.According to the analysis results,five common TCM syndromes and their characteristic symptom classification of insomnia were finally deduced.According to the frequency/person time distribution,they were heart and spleen deficiency syndrome,yin deficiency and fire hyperactivity syndrome,liver fire disturbing heart syndrome,phlegm heat disturbing heart syndrome,heart and gallbladder qi deficiency syndrome.Conclusion There are five common TCM syndromes of insomnia,and the characteristic symptoms of each TCM syndrome provide a reference source for the theoretical framework of syndrome diagnosis and efficacy evaluation scale and the establishment of item pool.
3.A case of hepatoblastoma easily misdiagnosed as hemangioma

This paper reports a case of hepatoblastoma that is inclined to misdiagnosis and missed diagnosis. The patient, a 6-month-old male, was admitted to Dermatology Department of Children’s Hospital of Nanjing Medical University and Beijing Children’s Hospital successively, because of a erythema on the neck for 6 months and a mass in the neck for 3 months. Abdominal ultrasonography and CT indicated the possibility of hepatic hemangioma, but further examination of alpha fetoprote and other indicators suggested hepatic malignant tumor. Therefore, surgery was performed to remove the liver mass, and postoperative pathology revealed hepatoblastoma. Postoperative chemotherapy was combined, and follow-up was conducted six months after the end of chemotherapy, with no recurrence of the disease. This article summarized the diagnosis and treatment process of this case and combined with literature review to provide experience for clinicians in the diagnosis and treatment of relevant cases.

BACKGROUND:Psoralen has a strong anti-osteoporotic activity and may have a restorative effect on chemotherapy-induced osteoporosis. OBJECTIVE:To explore the restorative effect of psoralen on the osteogenic differentiation of bone marrow mesenchymal stem cells in mice inhibited by cyclophosphamide and its mechanism. METHODS:C57BL/6 mouse bone marrow mesenchymal stem cells were isolated and cultured.Effect of psoralen on viability of bone marrow mesenchymal stem cells was detected by MTT assay.Osteogenic induction combined with alkaline phosphatase staining was used to determine the optimal dose of psoralen to restore the osteogenic differentiation of bone marrow mesenchymal stem cells inhibited by cyclophosphamide.The mRNA expression levels of Runx2,alkaline phosphatase,Osteocalcin,osteoprotegerin,and Wnt/β-catenin signaling pathway-related genes Wnt1,Wnt4,Wnt10b,β-catenin,and c-MYC were measured by RT-qPCR at different time points under the intervention with psoralen.The protein expression of osteogenic specific transcription factor Runx2 and Wnt/β-catenin signaling pathway related genes Active β-catenin,DKK1,c-MYC,and Cyclin D1 was determined by western blot assay at different time points under the intervention with psoralen. RESULTS AND CONCLUSION:(1)There was no significant effect of different concentrations of psoralen on the viability of bone marrow mesenchymal stem cells.The best recovery of the inhibition of osteogenic differentiation of bone marrow mesenchymal stem cells caused by cyclophosphamide was under the intervention of psoralen at a concentration of 200 μmol/L.(2)Psoralen reversed the reduction in osteogenic differentiation marker genes Runx2,alkaline phosphatase,Osteocalcin and osteoprotegerin mRNA expression and Runx2 protein expression in bone marrow mesenchymal stem cells caused by cyclophosphamide conditioned medium.(3)Psoralen reversed the decrease in Wnt/β-catenin pathway-related genes Wnt4,β-catenin,c-MYC mRNA and Active β-catenin,c-MYC,and Cyclin D1 protein expression and the increase in DKK1 protein expression in bone marrow mesenchymal stem cells caused by cyclophosphamide conditioned medium.(4)The results showed that cyclophosphamide inhibited osteogenic differentiation of bone marrow mesenchymal stem cells in mice,and psoralen had a restorative effect on it.The best intervention effect was achieved at a concentration of 200 μmol/L psoralen,and this protective effect might be related to the activation of Wnt4/β-catenin signaling pathway by psoralen.
6.Sinicization and reliability and validity test of the Obsessive and Compulsive Behaviors Scale of Mothers in Postpartum Period Regarding Baby Care

Objective:To sinicize the Obsessive and Compulsive Behaviors Scale of Mothers in Postpartum Period Regarding Baby Care (OCBS-MPPRBC) to test its reliability and validity, and to provide an evaluation tool for evaluating the degree of compulsive behaviors of postpartum mothers regarding baby care.Methods:Following the principles of Brislin translation, back translation and cross-cultural adaptation, the Chinese version of OCBS-MPPRBC was formed. A convenient sampling method was used to investigate 360 women in Mianyang Central Hospital from November 2023 to February 2024, and to test the reliability and validity of the Chinese version OCBS-MPPRBC. The critical ratio method and the total correlation analysis method were used for item analysis, the reliability test was based on Cronbach α coefficient and half-reliability method, and the content validity index, exploratory factor analysis, confirmatory factor analysis and aggregate validity were used for validity test. Results:A total of 352 women aged 21-41 (31.55 ± 3.17) years who met the exclusion criteria were included. The Chinese version of OCBS-MPPRBC contained 9 items, and 2 common factors were extracted by exploratory factor analysis, and the cumulative total variance contribution rate was 65.523%. The content validity index of the scale level was 0.89, the content validity index of each item level was 0.86-1.00, and the Cronbach α coefficient of the total scale was 0.844, and the half-folded reliability was 0.759. The results of confirmatory factor analysis showed that the fitting index of the Chinese version of the OCBS-MPPRBC model was better. Conclusions:The Chinese version of the OCBS-MPPRBC scale has good reliability and validity, and has good operability, which can be used to evaluate the degree of compulsive behavior of postpartum mothers about baby care in China.
7.Summary of the best evidence for non-surgical treatment and management of patients with pelvic organ prolapse

Objective:To retrieve and summarize the evidence regarding the non-surgical treatment and management of patients with pelvic organ prolapse (POP) .Methods:According to the "6S" evidence pyramid model, clinical guidelines, expert consensus, systematic reviews, clinical decisions, and clinical best practices related to the non-surgical treatment and management of patients with POP were systematically searched in databases and websites such as UpToDate, National Institute for Health and Clinical Excellence, Australia Joanna Briggs Institute (JBI) Evidence-Based Health Care Center Database, Guidelines International Network, National Guideline Clearinghouse, PubMed, China National Knowledge Infrastructure. The search period was from the establishment of the database to January 31, 2024. Two investigators independently conducted literature quality assessment and extracted relevant information according to the Appraisal of Guidelines for Research and EvaluationⅡ and the Australia JBI Evidence-Based Health Care Center evaluation tools. The Australia JBI quality level of evidence and grade of recommendation (2014 version) were used for evidence integration and grading.Results:A total of 15 papers were finally included, including three clinical decisions, eight guidelines, two best practices, one expert consensus, and one systematic review. A total of 39 pieces of best evidence were summarized in nine areas of examination and assessment, treatment plan, lifestyle, expectant therapy, functional exercise, physical therapy, pessary, drug therapy, and follow-up observation.Conclusions:This study summarizes the best evidence on the non-surgical treatment and management of POP patients, which is scientific and comprehensive, and provides an evidence-based basis for medical and nursing staff to develop non-surgical treatment and management protocols for POP patients and self-management of POP patients.

Tuberculosis is a zoonotic disease posing a substantial public health threat.Immunological diagnosis and vaccine im-munization are both necessary to control tuberculosis prevalence.However,the identical antigenic components in diagnostic reagents and vaccines hinder the use of animal vaccines and limit the specificity of clinical diagnosis in humans.Differentiating infected from vaccinated animals can overcome these problems.This article reviews the progress in differential diagnosis research from three as-pects:the diagnostic effects of antigens,methods for discovering new antigens,and screening of new host immune markers,to provide a theoretical basis for future research.
