Objective This study mainly focuses on the relationship between the serum glycoprotein non-metastatic melanoma protein B(GPNMB)concentration and the degree of neurological damage and prognosis in patients with acute ischemic stroke(AIS),and screens potential biomarkers to provide a reference for clinical diagnosis and treatment.Methods A total of 105 AIS patients hospitalized in the Department of Neurology of the Hospital 2 of Nantong University from June 2023 to March 2024 were selected as the sample group.In this study,the patients were divided into mild group(n=42)and moderate to severe group(n=63)according to the National Institutes of Health Strobe Scale(NIHSS)score within 24 hours of admission.The Modified Rankin Scale(mRS)was used to evaluate the functional recovery 3 months after discharge.The samples were subdivided into good prognosis group(n=34)and poor prognosis group(n=71).The serum GPNMB protein level was detected by ELISA,and the correlation between serum GPNMB protein level and NIHSS and mRS scores was analyzed.The binary Logistic regression model was used to evaluate the predictive value and prognostic evaluation value of serum GPNMB protein level for AIS neurological function damage.Results The serum GPNMB protein concentration in patients with moderate to severe neurological impairment and poor prognosis was significantly lower than that in patients with mild and good prognosis(P<0.05).The serum GPNMB protein level was significantly negatively correlated with the NIHSS score(r=-0.196,P<0.05)and the mRS score(r=-0.334,P<0.05).Multivariate regression analysis showed that GPNMB was still a key independent risk factor for AIS(P<0.05).The evaluation results obtained based on the receiver operating characteristic curve(ROC)showed that the serum GPNMB protein level had diagnostic value in predicting neurological impairment and poor prognosis(sensitivity reached 55.6%,specificity was 81.8%,and the overall accuracy was 63.81%,P<0.05).Conclusion There is a significant positive correlation between the decrease in serum GPNMB protein concentration and the degree of neurological damage in AIS patients,and it is likely to become an important biological indicator for measuring the severity of the disease and long-term prognosis.

OBJECTIVE To construct a prediction model for the efficacy of serotonin-norepinephrine reuptake inhibitor(SNRI)in inpatients with moderate and severe depression by using a machine learning method.METHODS The case records of inpatients with moderate and severe depression treated with SNRI antidepressants were collected from a third-grade class-A hospital in Xinjiang from January 2022 to October 2024;those patients were divided into effective group and ineffective group based on the Hamilton depression scale-24 score reduction rate.After screening the characteristic variables related to the therapeutic efficacy of SNRI drugs through LASSO regression,five prediction models including support vector machine,k-nearest neighbor,random forest,lightweight gradient boosting machine and extreme gradient boosting were constructed using the training set.Bayesian optimization was used to adjust the hyperparameters of these models.The performance of the models was evaluated in the validation set to select the optimal model.The Shapley additive explanations method was used to perform explainable analysis on the best model.RESULTS The medical records from 355 hospitalized patients with moderate and severe depression were collected,comprising 285 cases in the effective group and 70 cases in the ineffective group,resulting in an overall therapeutic response rate of 80.28%.After feature variable screening,five characteristic variables for therapeutic efficacy were obtained,including Hamilton anxiety scale,blood urea nitrogen,combination of anti-anxiety drugs,drinking history,and first onset of the disease.Compared with other models,the random forest model performed the best.The area under the receiver operating characteristic curve was 0.85,the area under the precision-recall curve was 0.87,the accuracy was 0.74,and the recall rate value was 0.75.CONCLUSIONS The random forest model constructed based on five characteristic variables demonstrates potential for predicting the therapeutic efficacy of SNRI antidepressants in hospitalized patients with moderate and severe depression.

BACKGROUND:The adjustment of force lines in open wedge high tibial osteotomy can affect the retroversion of the knee joint and the height of the patella.In addition,the adjustment of force lines in open wedge high tibial osteotomy is also different according to different varus deformity.At present,the difference of weight bearing line adjustment is still controversial for the correlation index of knee joint and the function of knee joint after surgery.OBJECTIVE:To investigate the effect of line adjustment area on postoperative knee function and joint kinematics during open wedge high tibial osteotomy.METHODS:Totally 76 patients with knee osteoarthritis who received open wedge high tibial osteotomy in Binzhou People's Hospital Affiliated to Shandong First Medical University from March 2022 to March 2023 were selected,including 33 males and 43 females,all on one knee.The lateral border of the tibial plateau was considered as 100％.The position of the central intercondylar ridge of the knee joint was 0％.The weight bearing line of the affected limb was divided into two groups according to the difference of the lateral plateau area of the affected limb:the area of the force line passing through the lateral plateau of the tibial plateau(0-29％)was classified as group A(n=39),and the area of the lateral plateau of the tibial plateau was classified as group B(n=37)by 30％to 40％.The changes of tibiofemoral angle,medial proximal tibial angle,posterior inclination of tibia,and patellar height before and after operation were analyzed.The symptoms and function of the knee were evaluated by the Numeric Rating Scale score and the Hospital for Special Surgery knee score.RESULTS AND CONCLUSION:(1)At 1 year after surgery,the posterior inclination of tibia and the medial angle of proximal tibia were increased in both groups(P＜0.05),and the tibiofemoral angle was reduced and patellar height was decreased in both groups(P＜0.05).The posterior inclination of tibia,medial proximal tibial angle,tibiofemoral angle,and patellar height were significantly different between the two groups at 1 year after surgery(P＜0.05).(2)The Hospital for Special Surgery knee scores of the two groups of patients at all times after surgery were better than those before surgery,and showed a gradually increasing trend(P＜0.05).There was no significant difference in the Hospital for Special Surgery knee scores 1 week and 3 months after surgery between the two groups(P＞0.05).Hospital for Special Surgery knee score of group B at 6 and 12 months after surgery was better than that of group A(P＜0.05).(3)The Numeric Rating Scale score after surgery showed a gradual downward trend(P＜0.05).No significant difference was detected between the two groups(P＞0.05).(4)The results show that the knee function and symptoms were significantly improved in the early and middle period after open wedge high tibial osteotomy,and the knee joint related parameters changed with the outward shift of weight bearing line.At 1-year follow-up,the functional recovery of knee joint in group B was better than that in group A.Long-term knee function and clinical differences after surgery need further follow-up study.

OBJECTIVE To systematically evaluate risk prediction models for chemotherapy-induced myelosuppression in breast cancer， and provide a scientific reference for clinical healthcare workers in selecting or developing effective predictive models. METHODS A systematic search was conducted for studies on predictive models of the risk of chemotherapy-induced myelosuppression in breast cancer across the CNKI， VIP， Wanfang， PubMed， Web of Science， Cochrane Library， Embase， and Scopus databases， with a time frame of the establishment of the database to May 7， 2024. Literature was independently screened by 2 investigators， data were extracted according to critical appraisal and data extraction for systematic reviews of predictive model studies， and the risk of bias evaluation tool for predictive model studies was used to analyze the risk of bias and applicability of the included studies. RESULTS There were totally 7 studies， comprising 12 models. Among them， 11 models indicated an area under the subject operating characteristic curve of 0.600-0.908； 2 models indicated calibration. The common predictor variables of the included models were age， pre-chemotherapy neutrophil count， pre-chemotherapy lymphocyte count， and pre-chemotherapy albumin. The overall risk of bias of the 7 studies was high， which was mainly attributed to the flaws in the study design， insufficient sample sizes， inappropriate treatment of variables， non-reporting of missing data， and the lack of indicators for the assessment of the models， but the applicability was good. CONCLUSIONS The predictive performance of risk predictive models for chemotherapy-induced myelosuppression in breast cancer remains to be further enhanced， and the overall risk of model bias is high. Future studies should follow the specifications of model development and reporting， then combine machine learning algorithms to develop risk predictive models with good predictive performance， high stability， and low risk of bias， so as to provide a decision-making basis for the clinic.

Objective To explore the feasibility and safety of ultrasound-guided percutaneous microwave ablation for unresectable pancreatic cancer.Methods Totally 84 patients who underwent ultrasound-guided percutaneous microwave ablation for unresectable pancreatic cancer were enrolled,and the technical success rate,complete ablation rate,complication rate,pain relief rate and survival time,etc.were observed.Results The median age of 84 cases was 61.5 years.Totally 86 tumors,including 44.19％(38/86)at the head/neck and 55.81％(48/86)at the body/tail of pancreas were detected,and a total of 85 ablation sessions were performed with the median ablation energy applied per tumor of 9.90(1.08,21.60)kJ and the complete ablation rate of 42.86％(36/84).The technical success rate was 100％(85/85).Thirty-nine complication events occurred in 25 cases,no ablation-related death.Among 34 patients underwent ablation mainly for pain symptoms,the pain score decreased from(6.22±1.12)points before treatment to(1.94±1.64)points after treatment(P＜0.001).During 6.8(3.3,12.9)months' follow-up,the mean survival time was(8.5±6.7)months,and all 47 patients died due to tumor progression.Conclusion Ultrasound-guided percutaneous microwave ablation was safe and feasible for unresectable pancreatic cancer.

RNA-based gene therapy has been widely used for various diseases, and extensive studies have proved that suitable delivery routes greatly help the development of RNA therapeutics. Identifying a safe and effective delivery system is key to realizing RNA therapeutics' clinical translation. Inhalation is a non-invasive pulmonary delivery modality that can enhance the retention of therapeutic agents in the lungs with negligible toxicity, thereby improving patient compliance. Inhaled RNA therapeutics are increasingly becoming an area of focus for researchers; however, only several clinical trials have explored inhaled delivery of RNA for pulmonary diseases. This review presents an overview of recent advances in inhaled delivery systems for RNA therapeutics, including viral and nonviral systems, highlighting state of the art regarding inhalation in the messenger RNA (mRNA) field. We also summarize the applications of mRNA inhalants in infectious and other lung diseases. Simultaneously, the research progresses on small interfering RNAs (siRNAs), antisense oligonucleotides (ASOs), and different types of RNA are also discussed to provide new strategies for developing RNA inhalation therapy. Finally, we clarify the challenges inhaled RNA-based therapeutics face before their widespread adoption and provide insights to help advance this exciting field to the bedside.

Lophatheri Herba (Danzhuye in Chinese) is derived from the dried stems and leaves of Lophatherum gracile and has a long history of use as a medicinal and food source. Flavonoids and phenolic acids are the main active ingredients in Lophatheri Herba, which produce diuretic, anti-inflammatory, and antipyretic effects. Flavonoid glycosides and hydroxybenzoic acids are respectively the main structure in 44 flavonoids and 16 phenolic acids obtained from Lophatheri Herba. Modern pharmacological studies have found that the main chemical constituents of Lophatheri Herba play important roles in anti-inflammatory, cardioprotective, hepatoprotective and hypoglycaemic effects. Studies have demonstrated that flavonoid monomers, for example, luteolin, isoorientin, luteolin-7-O-β-D-glucoside and apigenin are more effective in exerting the above pharmacological effects. In addition, Lophatheri Herba is used in different food products as the main ingredient or as an accessory. This review describes Lophatheri Herba in terms of its chemical composition, pharmacological effects and efficacy, food development and applications, and clinical utility, and discusses the problems facing its use. This study provides valuable ideas and a scientific basis for the future development and use of L. gracile.

Cervical spinal cord injury without fracture-dislocation (CSCIWFD) is referred to as a special type of cervical spinal cord injury characterized by traumatic spinal cord dysfunction and no significant bony structural abnormalities on imagines. Duo to the high risk of missed diagnosis during the initial consultation, CSCIWFD may lead to progressive neurological deterioration or even complete paralysis, severely impacting patients′ prognosis. Currently, there are no established consensuses over the diagnosis and treatment of CSCIWFD, such as the lack of evidence-based standards for indications of non-surgical treatment and risk of secondary neurological injury, as well as debates over the optimal timing for surgical intervention and indications for different surgical approaches. To address these issues, the Spine Trauma Group of the Orthopedic Branch of the Chinese Medical Doctor Association organized experts in the relevant fields to formulate Diagnosis and treatment guideline for acute cervical spinal cord injury without fracture- dislocation in adults ( version 2025) . Based on evidence-based medicine and the principles of scientific rigor and clinical applicability, the guidelines proposed 11 recommendations covering terminology, diagnosis, evaluation treatment, and rehabilitation, etc., aiming to standardize the management of CSCIWFD.
