1.Systematic review of risk predictive models for chemotherapy-induced myelosuppression in breast cancer

OBJECTIVE To systematically evaluate risk prediction models for chemotherapy-induced myelosuppression in breast cancer, and provide a scientific reference for clinical healthcare workers in selecting or developing effective predictive models. METHODS A systematic search was conducted for studies on predictive models of the risk of chemotherapy-induced myelosuppression in breast cancer across the CNKI, VIP, Wanfang, PubMed, Web of Science, Cochrane Library, Embase, and Scopus databases, with a time frame of the establishment of the database to May 7, 2024. Literature was independently screened by 2 investigators, data were extracted according to critical appraisal and data extraction for systematic reviews of predictive model studies, and the risk of bias evaluation tool for predictive model studies was used to analyze the risk of bias and applicability of the included studies. RESULTS There were totally 7 studies, comprising 12 models. Among them, 11 models indicated an area under the subject operating characteristic curve of 0.600-0.908; 2 models indicated calibration. The common predictor variables of the included models were age, pre-chemotherapy neutrophil count, pre-chemotherapy lymphocyte count, and pre-chemotherapy albumin. The overall risk of bias of the 7 studies was high, which was mainly attributed to the flaws in the study design, insufficient sample sizes, inappropriate treatment of variables, non-reporting of missing data, and the lack of indicators for the assessment of the models, but the applicability was good. CONCLUSIONS The predictive performance of risk predictive models for chemotherapy-induced myelosuppression in breast cancer remains to be further enhanced, and the overall risk of model bias is high. Future studies should follow the specifications of model development and reporting, then combine machine learning algorithms to develop risk predictive models with good predictive performance, high stability, and low risk of bias, so as to provide a decision-making basis for the clinic.

Cervical spinal cord injury without fracture-dislocation (CSCIWFD) is referred to as a special type of cervical spinal cord injury characterized by traumatic spinal cord dysfunction and no significant bony structural abnormalities on imagines. Duo to the high risk of missed diagnosis during the initial consultation, CSCIWFD may lead to progressive neurological deterioration or even complete paralysis, severely impacting patients′ prognosis. Currently, there are no established consensuses over the diagnosis and treatment of CSCIWFD, such as the lack of evidence-based standards for indications of non-surgical treatment and risk of secondary neurological injury, as well as debates over the optimal timing for surgical intervention and indications for different surgical approaches. To address these issues, the Spine Trauma Group of the Orthopedic Branch of the Chinese Medical Doctor Association organized experts in the relevant fields to formulate Diagnosis and treatment guideline for acute cervical spinal cord injury without fracture- dislocation in adults ( version 2025) . Based on evidence-based medicine and the principles of scientific rigor and clinical applicability, the guidelines proposed 11 recommendations covering terminology, diagnosis, evaluation treatment, and rehabilitation, etc., aiming to standardize the management of CSCIWFD.

Vertebral refracture following percutaneous vertebral augmentation (PVA) is commonly seen in elderly patients with osteoporotic thoracolumbar compression fractures (OTLCF). It can lead to recurrent pain, loss of vertebral height, progression of kyphosis, and even neurological dysfunction, significantly impairing patients′ quality of life. Current diagnosis and treatment face multiple challenges, including high misdiagnosis rate, difficulty in choosing between surgical and non-surgical treatment options, lack of standardized surgical protocols, interference from intralesional bone cement during procedures, inadequate stability of internal fixation in osteoporotic bone, and suboptimal compliance of anti-osteoporotic therapy. Establishing a standardized diagnostic and therapeutic framework is urgently needed. To standardize the management process and improve outcomes for vertebral refractures after PVA in elderly OTLCF patients, Spinal Trauma Group of the Orthopedic Branch of Chinese Medical Doctor Association organized experts in the field to develop Guideline for the diagnosis and treatment of vertebral refracture after percutaneous vertebral augmentation in elderly patients with osteoporotic thoracolumbar compression fractures ( version 2025), based on current literature and clinical experience, and adhering to principles of scientific rigor and clinical applicability. A total of 11 recommendations were proposed, encompassing diagnosis, treatment, and rehabilitation of vertebral refracture after PVA in elderly patients with OTLCF, aiming to provide a foundation for a standardized management.
4.Effect of line adjustment area of lower limbs on knee joint function and kinematics after high tibial osteotomy

BACKGROUND:The adjustment of force lines in open wedge high tibial osteotomy can affect the retroversion of the knee joint and the height of the patella.In addition,the adjustment of force lines in open wedge high tibial osteotomy is also different according to different varus deformity.At present,the difference of weight bearing line adjustment is still controversial for the correlation index of knee joint and the function of knee joint after surgery.OBJECTIVE:To investigate the effect of line adjustment area on postoperative knee function and joint kinematics during open wedge high tibial osteotomy.METHODS:Totally 76 patients with knee osteoarthritis who received open wedge high tibial osteotomy in Binzhou People's Hospital Affiliated to Shandong First Medical University from March 2022 to March 2023 were selected,including 33 males and 43 females,all on one knee.The lateral border of the tibial plateau was considered as 100%.The position of the central intercondylar ridge of the knee joint was 0%.The weight bearing line of the affected limb was divided into two groups according to the difference of the lateral plateau area of the affected limb:the area of the force line passing through the lateral plateau of the tibial plateau(0-29%)was classified as group A(n=39),and the area of the lateral plateau of the tibial plateau was classified as group B(n=37)by 30%to 40%.The changes of tibiofemoral angle,medial proximal tibial angle,posterior inclination of tibia,and patellar height before and after operation were analyzed.The symptoms and function of the knee were evaluated by the Numeric Rating Scale score and the Hospital for Special Surgery knee score.RESULTS AND CONCLUSION:(1)At 1 year after surgery,the posterior inclination of tibia and the medial angle of proximal tibia were increased in both groups(P<0.05),and the tibiofemoral angle was reduced and patellar height was decreased in both groups(P<0.05).The posterior inclination of tibia,medial proximal tibial angle,tibiofemoral angle,and patellar height were significantly different between the two groups at 1 year after surgery(P<0.05).(2)The Hospital for Special Surgery knee scores of the two groups of patients at all times after surgery were better than those before surgery,and showed a gradually increasing trend(P<0.05).There was no significant difference in the Hospital for Special Surgery knee scores 1 week and 3 months after surgery between the two groups(P>0.05).Hospital for Special Surgery knee score of group B at 6 and 12 months after surgery was better than that of group A(P<0.05).(3)The Numeric Rating Scale score after surgery showed a gradual downward trend(P<0.05).No significant difference was detected between the two groups(P>0.05).(4)The results show that the knee function and symptoms were significantly improved in the early and middle period after open wedge high tibial osteotomy,and the knee joint related parameters changed with the outward shift of weight bearing line.At 1-year follow-up,the functional recovery of knee joint in group B was better than that in group A.Long-term knee function and clinical differences after surgery need further follow-up study.
5.Research progress on risk factors and risk prediction models for exposure keratopathy in ICU patients

This paper reviews risk factors for exposure keratopathy in ICU patients and summarizes risk prediction models for exposure keratopathy based on study subjects, design, model construction and validation methods, and predictive performance. This paper also analyzes the strengths and limitations of existing models, so as to provide reference for healthcare professionals to identify high-risk patients with exposure keratopathy early and implement personalized prevention and management measures.
6.Relationship between triglyceride-glucose index and acute ischemic stroke with anterior circulation large vessel occlusion

Objective To explore the relationship between triglyceride-glucose index(TyG)and acute ischemic stroke with large vessel occlusion(AIS-LVO)of anterior circulation.Methods A retrospective study was conducted on patients with anterior circulation AIS-LVO who underwent emergency endovascular thrombectomy at Neurovascular Center of The First Affiliated Hospital of Naval Medical University from Jan.2018 to Dec.2019.According to modified Rankin scale(mRS)score 90 d after operation,the patients were assigned to favorable outcome group(mRS score 0-2)or unfavorable outcome group(mRS score 3-6),and the TyG was compared.According to the median of TyG,the patients were assigned to low-TyG group(TyG<8.57)or high-TyG group(TyG ≥8.57),and the clinical data,laboratory indexes,and imaging characteristics were compared.Receiver operating characteristic curve was used to evaluate the predictive value of TyG for poor prognosis.Results A total of 135 patients were enrolled,with 72 in the favorable outcome group and 63 in the unfavorable outcome group.The TyG of the unfavorable outcome group was significantly higher than that of the favorable outcome group(8.82+0.63 vs 8.43+0.60,P<0.001).There were 67 patients in the low-TyG group and 68 in the high-TyG group.Compared with the low-TyG group,the proportion of patients with hyperlipidemia history(P=0.003),systolic blood pressure at admission(P=0.018),fasting blood glucose level(P<0.001),and triglyceride level(P<0.001)were significantly higher in the high-TyG group,the infarct core volume was significantly larger(P=0.025),the high density lipoprotein-cholesterol level was significantly lower(P=0.013),and the mRS score 90 d after operation was significantly higher(3[1,5]vs 1[0,5],P=0.049).The TyG had certain predictive value for poor prognosis in anterior circulation AIS-LVO patients(area under curve value=0.662,95%confidence interval 0.571-0.753).Conclusion TyG is elevated in anterior circulation AIS-LVO patients with poor prognosis,and may be a potential prognostic indicator for anterior circulation AIS-LVO patients.
7.Efficacy observation of endovascular treatment for acute large-vessel occlusion ischemic stroke over 24 h after onset

Objective To investigate the safety and effectiveness of endovascular treatment in patients with acute ischemic stroke due to large vessel occlusion(AIS-LVO)over 24 h after onset.Methods The clinical data of AIS-LVO patients who received endovascular treatment in Neurovascular Center of The First Affiliated Hospital of Naval Medical University from Jan.2018 to Dec.2022 were retrospectively analyzed,including baseline characteristics,imaging findings,treatment,degree of vascular recanalization(modified thrombolysis in cerebral infarction grade 2b and 3 for successful recanalization)and prognosis.Results A total of 57 patients were included,including 42 males and 15 females,aged from 30 to 84 years old.The most common risk factors were hypertension(39 cases,68.4%),followed by smoking history(24 cases,42.1%),diabetes mellitus(17 cases,29.8%),previous stroke history(16 cases,28.1%),and atrial fibrillation(9 cases,15.8%).Before treatment,the National Institutes of Health stroke scale score was 12.84±7.04,and the Alberta Stroke Program early computed tomography score was 9.00(7.00,10.00).Vascular occlusion sites included middle cerebral artery occlusion in 27(47.4%)cases,internal carotid artery occlusion in 24(42.1%)cases,and tandem lesions in 6(10.5%)cases.The time from onset to femoral artery puncture was 38.30(28.17,53.71)h,and the time from femoral artery puncture to vascular recanalization was 52.00(38.50,92.50)min.General anesthesia was the main anesthesia method,accounting for 64.9%(37/57).The etiological types of stroke were mainly large artery atherosclerosis(38 cases,66.7%),cardiogenic embolism(9 cases,15.8%),unknown causes(6 cases,10.5%),and other clear causes(4 cases,7.0%).Mechanical thrombectomy was the first choice in 41(71.9%)cases,balloon dilatation/stenting was used in 35(61.4%)cases,of which 15(26.3%)cases were the first choice.Finally,53(93.0%)cases were recanalized successfully.In terms of complications,1(1.8%)case had symptomatic intracranial hemorrhage.The 90-d prognosis rate was 59.6%(34/57),and 3(5.3%)cases died.Conclusion Endovascular treatment for AIS-LVO patients over 24 h after onset has high recanalization rate and good safety,but it still needs to be further verified by randomized controlled trials.
8.Intervention of Sinomenine in Rat Osteoarthritis by Modulating FABP4

ObjectiveThis study aims to observe the effect of sinomenine (SIN) on fatty acid binding protein 4 (FABP4) in synovial tissue of rats with osteoarthritis (OA) and investigate the therapeutic mechanism of SIN on OA, further providing new ideas for the management of osteoarthritis by traditional Chinese medicine (TCM). MethodsAn OA rat model was constructed and randomly divided into a control group, an OA group, an OA + SIN-L group (50 mg·kg-1), an OA + SIN-M (100 mg·kg-1), an OA + SIN-H (200 mg·kg-1), and an OA + prednisone (PDN) group (5 mg·kg-1). Following surgical modeling for three weeks, an appropriate medication was administered for four weeks. During modeling and administration, a hot plate test was performed to detect the pain and swelling of the knee joints of the rats. The periarticular tissue was collected for arthropathological observation at the end of drug administration. The expression of cleaved Caspase-3, B-cell lymphoma-2 (Bcl-2), Bcl-2 associated X protein (Bax), and FABP4 in the synovial tissue of rats was detected by Western blot and real-time quantitative polymerase chain reaction (Real-time PCR), and the expression and distribution of FABP4 protein in the synovial membrane were detected by immunofluorescence. ResultsCompared with those in the control group, the levels of inflammatory factors and FABP4 in the serum of rats in the OA group were significantly increased (P<0.05, P<0.01), and joint swelling was significantly elevated (P<0.01). The thermal pain threshold was significantly reduced (P<0.01), and the expression of FABP4 protein and the fluorescence intensity were significantly increased (P<0.01). The synovial tissue exhibited significantly increased inflammatory infiltration, proliferated fibroblasts, and an elevated apoptotic index (P<0.05, P<0.01). Compared with those in the OA group, the serum lipid metabolism indexes of rats in the SIN administration group gradually returned to normal (P<0.05, P<0.01), while the levels of inflammatory factors and FABP4 in the serum of rats in the SIN-administered group were significantly reduced (P<0.05, P<0.01), and joint swelling was significantly decreased (P<0.05, P<0.01). The thermal pain threshold was significantly elevated (P<0.05, P<0.01), and the expression of FABP4 protein and fluorescence intensity in the synovial tissue were significantly decreased (P<0.05, P<0.01). The synovial tissue displayed significantly reduced inflammatory infiltration and a decreased apoptotic index (P<0.05, P<0.01). ConclusionThe therapeutic effect of SIN on OA may be related to the down-regulation of FABP4 expression, reduction of apoptosis, and inhibition of inflammatory factor expression.

Nonunion of osteoporotic vertebral fractures (OVF), predominantly affecting the elderly, can lead to intractable pain, vertebral collapse, progressive kyphotic deformity, and neurological impairment, significantly compromising patients′ quality of life. There exists considerable debate on diagnosis and management of OVF, encompassing key issues such as clinical diagnosis and staging criteria for nonunion, surgical indications and procedure selection, and postoperative rehabilitation planning. Currently, there lacks standardized clinical guideline and expert consensus on the diagnosis and management of OVF nonunion in China. To address this gap, Minimally Invasive Surgery Group of Chinese Orthopedic Association, Osteoporosis Committee of Chinese Association of Orthopedic Surgeons, Prevention and Rehabilitation Committee for Osteoporosis of Chinese Association of Rehabilitation Medicine and Minimally Invasive Orthopedic Surgery Branch of China Association for Geriatric Care jointly organized domestic experts in spinal surgery, endocrinology, and rehabilitation to formulate the Clinical guideline for the diagnosis and treatment for nonunion of osteoporotic vertebral fractures ( version 2025), based on existing literature and clinical experience and adhering to principles of scientific rigor and practicality. The guideline provided 13 evidence-based recommendations encompassing diagnosis and treatment of OVF nonunion, aiming to standardize its clinical management.
10.Imaging guided percutaneous microwave ablation for unresectable pancreatic cancer:A multicenter retrospective study

Objective To explore the feasibility and safety of ultrasound-guided percutaneous microwave ablation for unresectable pancreatic cancer.Methods Totally 84 patients who underwent ultrasound-guided percutaneous microwave ablation for unresectable pancreatic cancer were enrolled,and the technical success rate,complete ablation rate,complication rate,pain relief rate and survival time,etc.were observed.Results The median age of 84 cases was 61.5 years.Totally 86 tumors,including 44.19%(38/86)at the head/neck and 55.81%(48/86)at the body/tail of pancreas were detected,and a total of 85 ablation sessions were performed with the median ablation energy applied per tumor of 9.90(1.08,21.60)kJ and the complete ablation rate of 42.86%(36/84).The technical success rate was 100%(85/85).Thirty-nine complication events occurred in 25 cases,no ablation-related death.Among 34 patients underwent ablation mainly for pain symptoms,the pain score decreased from(6.22±1.12)points before treatment to(1.94±1.64)points after treatment(P<0.001).During 6.8(3.3,12.9)months' follow-up,the mean survival time was(8.5±6.7)months,and all 47 patients died due to tumor progression.Conclusion Ultrasound-guided percutaneous microwave ablation was safe and feasible for unresectable pancreatic cancer.
