1.Endoscopic minimally invasive release surgery versus ultrasound-guided percutaneous needle knife release for carpal tunnel syndrome:a randomized controlled trial

Objective To explore the efficacy of endoscopic minimally invasive release surgery and ultrasound-guided percutaneous needle knife release in treating carpal tunnel syndrome(CTS).Methods A total of 96 patients with CTS,who received treatment at Yixing People's Hospital and Wuxi Ninth Hospital Orthopedic Hospital from January 2021 to December 2024,were selected for this study.Using a random number table method,the 96 patients were divided into an endoscopic group and an ultrasound group,with 48 patients in each group.The patients of endoscopic group received endoscopic minimally invasive release surgery,while the patients of ultrasound group received ultrasound-guided needle knife treatment.The surgical indicators,efficacy,wrist function,median nerve electrophysiological indicators,anatomical indicators,efficacy,and safety were compared between the two groups.Results In the ultrasound group,the surgical duration and postoperative recovery time were shorter than those in the endoscopic group(P<0.05),and the surgical cost was lower than that in the endoscopic group(P<0.05).The postoperative Symptom Severity Score(SSS)and Functional Status Scale(FSS)score of both groups were decreased when compared with their preoperative values(P<0.05),and the SSS and FSS of the ultrasound group were lower than those of the endoscopic group(P<0.05).After treatment,the resting pain score and activity pain score of both groups were decreased when compared with their preoperative values(P<0.05),and the resting pain score and activity pain score in the ultrasound group were lower than those in the endoscopic group(P<0.05).After treatment,both groups showed a reduction in median nerve flattening ratio(FR)and transverse carpal ligament(TCL)thickness when compared with their preoperative values(P<0.05),and the reduction degree in the ultrasound group was greater than that in the endoscopic group(P<0.05).After treatment,both groups showed an increase in sensory nerve conduction velocity(SNCV),sensory nerve action potential amplitude(SNAP),and motor nerve action potential amplitude(CMAP)when compared with their preoperative values(P<0.05),which in the ultrasound group showed a greater improvement than those in the endoscopic group(P<0.05).The postoperative distal motor latency(DML)was decreased in both groups(P<0.05),and the DML in the ultrasound group was lower than that in the endoscopic group(P<0.05).The therapeutic efficacy in the ultrasound group was higher than that in the endoscopic group(P<0.05).Conclusion Compared to endoscopic minimally invasive release surgery,ultrasound-guided needle knife treatment is more effective in promoting the recovery of wrist joint function in patients with CTS.It can effectively decrease the pain severity of patients,repair anatomical injuries in the wrist,promote the recovery of nerve electrophysiological indicators of the median nerve,with good safety and lower medical costs.

Dyslipidemia is one of the common metabolic disorders in type 2 diabetes mellitus,often coexisting with insulin resistance and central obesity to form metabolic syndrome,significantly increasing the risk of macrovascular and microvascular complications in diabetic patients.Non-high-density lipoprotein cholesterol(non-HDL-C)refers to the sum of cholesterol contained in other lipopro-teins except high-density lipoprotein cholesterol(HDL-C).In recent years,the importance of non-HDL-C in the management of diabetic complications has garnered increasing attention,and it has been established as a major lipid-lowering target along with low-density lipoprotein cholesterol(LDL-C)in the latest expert consensus on lipid management for Chinese diabetic patients.Clinical studies have demonstrated that non-HDL-C is closely associated with residual cardiovascular risk and recurrent stroke risk in diabetic patients,as well as carotid plaque vulnerability and the development of peripher-al arterial disease.Additionally,non-HDL-C is also closely related to diabetic nephropathy and reti-nopathy in type 2 diabetes mellitus,and achieving long-term control of non-HDL-C levels can reduce the risk of chronic kidney disease.This article summarized the relationship between non-HDL-C and complications of type 2 diabetes mellitus and elucidated its significance as a future clinical therapeutic target.
3.PAI-1 genetic polymorphisms influence septic patients' outcomes by regulating neutrophil activity.

BACKGROUND:
Plasminogen activator inhibitor-1 (PAI-1) plays an important role in the pathophysiology of sepsis, but the exact mechanism remains debatable. In this study, we investigated the associations among the serum levels of PAI-1, the incidence of 4G/5G promoter PAI-1 gene polymorphisms, immunological indicators, and clinical outcomes in septic patients.
METHODS:
A total of 181 patients aged 18-80 years with sepsis between November 2016 and August 2018 in the intensive care unit in the Xinhua Hospital were recruited in this retrospective study, with 28-day mortality as the primary outcome. The initial serum level of PAI-1 and the presence of rs1799768 single nucleotide polymorphisms (SNPs) were examined. Univariate logistic regression and multivariate analyses were performed to determine the factors associated with different genotypes of PAI-1, serum level of PAI-1, and 28-day mortality.
RESULTS:
The logistic analysis suggested that a high serum level of PAI-1 was associated with the rs1799768 SNP of PAI-1 (4G/4G and 4G/5G) (Odds ratio [OR]: 2.49; 95% confidence interval [CI]: 1.09, 5.68). Furthermore, a high serum level of PAI-1 strongly influenced 28-day mortality (OR 3.36; 95% CI 1.51, 7.49). The expression and activation of neutrophils (OR 0.96; 95% CI 0.93, 0.99), as well as the changes in the expression patterns of cytokines and chemokine-associated neutrophils (OR: 1.00; 95% CI: 1.00, 1.00), were both regulated by the genotype of PAI-1.
CONCLUSIONS
Genetic polymorphisms of PAI-1 can influence the serum levels of PAI-1, which might contribute to mortality by affecting neutrophil activity. Thus, patients with severe sepsis might clinically benefit from enhanced neutrophil clearance and the resolution of inflammation via the regulation of PAI-1 expression and activity.

4.Predictive value of MB isoenzyme of creatine kinase and poisoning severity score in the prognosis of patients with wasp sting

Objective:To explore the predictive value of MB isoenzyme of creatine kinase (CK-MB) and poisoning severity score (PSS) in the clinical prognosis of patients with wasp sting.Methods:A retrospective study was conducted. The clinical data of patients who were stung by wasps admitted to emergency department of the First Affiliated Hospital of Henan University of Science and Technology from July 2017 to November 2019 were collected. The 24-hour acute physiology and chronic health evaluation Ⅱ (APACHEⅡ), CK-MB and PSS scores of the patients were collected after admission, and 28-day outcome was recorded. Spearman correlation analysis method was used to analyze the correlation between CK-MB and PSS score. Logistic regression model was used to construct joint predictors, and the receiver operating characteristic (ROC) curve was drawn to evaluate the predictive value of various indicators for 28-day prognosis of patients with wasp stings.Results:Finally 90 patients were included in the analysis. There were 67 patients survived at 28 days, and 23 dead with the 28-day mortality of 25.6%. APACHEⅡ score, CK-MB and PSS score in the death group were significantly higher than those in the survival group [APACHEⅡscore: 19.7±2.7 vs. 13.7±2.3, CK-MB (U/L): 183 (151, 243) vs. 36 (21, 75), PSS score: 17.7±2.6 vs. 9.3±4.5, all P < 0.01]. The correlation analysis showed that CK-MB and PSS score were positively correlated ( r = 0.843, P < 0.01). Logistic regression model fitted CK-MB and PSS score, and Hosmer-Lemeshow test showed that the model fitted well. ROC curve analysis showed that the area under ROC curve (AUC) of CK-MB for predicting 28-day outcome was 0.957, the sensitivity was 91.3%, and the specificity was 88.1%; the AUC of PSS score was 0.908, the sensitivity was 91.3%, and the specificity was 90.8%. The AUC of CK-MB combined with PSS score was 0.964, the sensitivity was 100%, and the specificity was 79.4%, indicating that CK-MB combined with PSS score had higher predictive value and higher sensitivity for 28-day prognosis of patients with wasp sting. Conclusions:High CK-MB level and high PSS score in early stage of wasp sting injury indicate poor prognosis. Both CK-MB and PSS score can be used as predictors for predicting the prognosis of patients with wasp stings. In addition, CK-MB combined with PSS score have greater predictive value .
5.Intracanial thrombus suction combined with visible thrombectomy stent for the treatment of acute intracranial main artery infarction: preliminary results of 5 cases

Objective To discuss the clinical effect of Penumbra aspiration system combined with Trevo thrombectomy device in treating acute intracranial main artery infarction.Methods The clinical data of a total of 5 patients with acute intracranial main artery infarction,who were treated with ACETM thrombectomy device combined with Trevo visible thrombectomy stent,were retrospectively analyzed.The lesions included anterior circulation infarction (n=2) and posterior circulation infarction (n=3).The mean age of patients was (60.4±11.6) years old.The operation time spent for thrombectomy,the recanalization time of occluded vessels,and the recovery of neural function were assessed.Results The puncture-recanalization intervals in the 5 patients were 29 min,32 min,35 min,33 min and 30 min respectively,with a mean time of (31.8±2.4) min.After intraoperative cerebral infarction thrombolysis treatment,the cerebral blood flow grading reached level 3.NIHSS score was remarkably improved from preoperative (11.0±7.4) points to 24 hpostoperative(4.2±1.1) points,to 7 days-postoperative(1.8±1.3) points,and to 30 days-postoperative (0.9±0.6) points (P<0.05).Modified Rankin scale (mRS) score was 0-2 points in all patients.No intracranial hemorrhage transformation occurred.Conclusion In treating acute intracranial main artery infarction,ACETM thrombectomy device combined with Trevo visible thrombectomy stent can achieve the best recanalization effect with less times of thrombectomy procedure and short operation time.The clinical prognosis is satisfactory.
6.Intertrochanteric femoral fractures:a comparison between DHS and PFNA in efficacy

Objective To compare DHS(dynamic hip screw )with PFNA(proximal femoral nail antirotation) treatment for intertrochanteric fractures and to analyze their postoperative complications .Methods A total of 107 patients with inter-trochanteric fractures were treated with DHS and PFNA fixation respectively .The type of fracture , postoperative complica-tions and overall efficacy were compared .Results After the follow-up of 28(24 to 36) months.DHS group had a higher overall incidence of postoperative complications than the PFNA group and the difference was statistically significant ( P<0.05).Conclusion In order to reduce the intertrochanteric fracture fixation complications , the type of fracture, osteoporo-sis and biomechanical fixation characteristics need to be considered when selecting surgical procedures .Postoperative func-tional rehabilitation is to be improved .The curative effect of PFNA is better than that of DHS .
7.Hemiarthroplasty for the elderly with intertrochanteric femoral fractures:an observation

Objective To investigate the effect of femoral head replacement for the treatment of intertrochanteric frac -tures among elderly patients .Method There were 38 cases that received intertrochanteric fracture hemiarthroplasty and 35 patients were treated with open reduction and dynamic hip screw ( DHS) plate internal fixation .After follow-up, their opera-tion duration,blood loss, postoperative drainage flow , postoperative ambulation time , hospitalization time were compared according to the Harris score to assess the effect of postoperative follow-up.Results After the mean follow-up time of 18 months (12 to 36 months),the hemiarthroplasty group was superior to the open reduction and DHS plate fixation group ,as shown by the earlier postoperative ambulation time , fewer complications , and better fracture reduction , with an excellent rate of 94 .7%.Conclusion For patients with intertrochanteric fractures , hemiarthroplasty surgery is effective and can re-duce the incidence of postoperative complications .
8.Effect of Biological Fertilizers on the Yield and Quality of Amomum villosum

[Objective] To observe the effect of biological fertilizers on the yield and quality of Amomum villosum (AV). [Methods] Field experiment method was used to observe the effect of different kinds and different amount of biological fertilizers on AV. Biological fertilizers were: A (mainly composed of fermented chicken manure), B (mainly composed of fermented filter of sugarcane) and C (mixed with fertilizer A and biological fertilizer at certain proportion). The fertilizer amount was 150, 112 and 75 g****m-2 per time. Content of bornyl acetate in AV was detected by gas chromatography. [Results] The three kinds of biological fertilizers increased the yield of AV but had no obvious effect on its quality; high- and moderate-amount fertilizer increased the yield of AV, in particular fertilizer A (112 g?m-2 per time). The three kinds of fertilizers had no effect on the content of bornyl acetate in AV. [Conclusion] Reasonable application of fertilizers is one of the important ways to raise the yield of AV.
