1.Analysis of the contamination characteristics and pathogenic potential of Listeria monocytogenes in pre-packaged and refrigerated ready-to-eat cooked meat products in Chengdu City

Objective:To analyze the contamination characteristics and pathogenic potential of Listeria monocytogenes (LM) in pre-packaged and refrigerated ready-to-eat cooked meat products in Chengdu City. Methods:From September 2022 to May 2023, pre-packaged and refrigerated ready-to-eat cooked meat products were collected from six districts and counties in Chengdu City. Qualitative and quantitative determination of LM was performed on these samples. Whole genome sequencing was carried out on the isolated strains. Different ST strains were selected for cell adhesion and invasion experiments. The results were expressed as adhesion rate and invasion rate. One-way ANOVA was used for comparison between groups, and the Dunnett t-test was used for pairwise comparison. Results:A total of 145 samples were collected, and LM was detected and isolated in 29 samples. The total detection rate was 20.00%, and the detection rate of braised pork was the highest (68.18%). The contamination level of LM in 9 samples (31.03%) was greater than 100.00 MPN/g. The 29 strains of Listeria monocytogenes belonged to 7 STs, including ST 3 (27.59%), ST 8 (17.24%) and ST 87 (13.79%). The strain of ST 87 carried Listeria pathogenicity islands 4 (LIPI-4), which was a highly virulent strain. The medium and high virulence strains accounted for 79.31%. Some moderately virulent and highly virulent strains of ST 8 and ST 87 were closely related to clinical patient strains. Some LM isolates of the same ST type had little SNP differences (1-6) in the same manufacturer at different stages. In vitro cell experiments showed that the highly virulent strain ST 87 had the strongest adhesion and invasion ability towards Caco-2 cells. Conclusion:The pre-packaged and refrigerated cooked meat products have a high contamination rate. Some samples have high contamination levels and carry medium and high virulence strains, posing potential health risks to human beings. LM residues continue to persist in some manufacturers′ production and processing stages.
2.Intelligent robot-assisted minimally invasive reduction system for fresh subtrochanteric fractures of the femur

Objective:To evaluate the feasibility and efficacy of our self-designed intelligent robot-assisted reduction system for fresh subtrochanteric fractures of the femur.Methods:A retrospective cohort study was conducted to include 10 patients with fresh subtrochanteric fracture of the femur who had been admitted to Department of Orthopedic Trauma, Beijing Jishuitan Hospital from January 2024 to July 2024. There were 7 males and 3 females with an age of (45.0±14.3) years and an interval from injury to surgery of (7.9±3.7) d. All the patients were treated by minimally invasive reduction which was assisted by our self-designed intelligent robot, and internal fixation with intramedullary nails. The operation duration, intraoperative reduction duration, intraoperative blood loss, and intraoperative fluoroscopy frequency were recorded. The reduction effect was evaluated by calculating the differences between preoperative planning and postoperative CT reconstruction (i.e., the differences in femoral shaft length and in rotation angle). The hip functional recovery was assessed by Harris hip function Scoring.Results:The mean operation time was 200.0 (161.3, 217.5) min, the reduction time (83.0±35.5) min, the intraoperative blood loss (290.0±110.1) mL, and the intraoperative fluoroscopy 18.5 (9.0, 19.3) times. In all patients, the difference in femoral shaft length was (2.4±1.4) mm, and the difference in rotation angle 5.1°±3.0°. All patients were followed up for (8.2±2.0)months. All the fractures got united at the last follow-up. Their Harris hip function score was (83.3±4.1) points.Conclusion:Our self-designed intelligent robot-assisted reduction system is feasible and effective in the surgery of fresh subtrochanteric fracture of the femur, because the robot system can complete the autonomous planning of reduction approaches before surgery and assist fracture reduction under real-time monitoring with three-dimensional images, leading to fine outcomes.
3.Operative tutorial on closed reduction and cast immobilization for distal radius fractures

This tutorial addresses the current lack of standardized protocols for closed reduction and cast immobilization for distal radius fractures in China, along with a high incidence of the complications of these fractures. Based on the 2024 Evidence-Based Guidelines for Diagnosis and Treatment of Adult Distal Radius Fractures, it establishes a standardized operational procedure. Using the classic Colles fracture as an example, it provides a comprehensive and step-by-step explanations of the closed reduction and cast immobilization techniques, including detailed descriptions and schematic illustrations covering patient positioning, measurement, reduction maneuvers, cast fabrication, cast application, molding, and assessment.

This review aimed to provide a comprehensive analysis of the etiology, epidemiology, pathology, and conventional treatment of heterotopic ossification (HO), especially emerging potential therapies. HO is the process of ectopic bone formation at non-skeletal sites. HO can be subdivided into two major forms, acquired and hereditary, with acquired HO predominating. Hereditary HO is a rare and life-threatening genetic disorder, but both acquired and hereditary form can cause severe complications, such as peripheral nerve entrapment, pressure ulcers, and disability if joint ankylosis develops, which heavily contributes to a reduced quality of life. Modalities have been proposed to treat HO, but none have emerged as the gold standard. Surgical excision remains the only effective modality; however, the optimal timing is controversial and may cause HO recurrence. Recently, potential therapeutic strategies have emerged that focus on the signaling pathways involved in HO, and small molecule inhibitors have been shown to be promising. Moreover, additional specific targets, such as small interfering RNAs (siRNAs) and non-coding RNAs, could be used to effectively block HO or develop combinatorial therapies for HO.

Objective To conduct a quantitative risk assessment of Listeria monocytogenes(LM)in prepackaged,non-vacuum sealed,short shelf-life,ready-to-eat meat products in Chengdu using the@Risk software.Methods Based on monitoring data of LM contamination in pre-packaged,non-vacuum sealed,refrigerated ready-to-eat meat products in Chengdu obtained from a previous study,a risk assessment model was established.The risk of LM infection caused by consuming cooked meat products in different groups of people was quantitatively assessed.In addition,the growth of LM in cooked meat products,from retail to consumption,was also taken into consideration in the assessment.Results In Chengdu,the numbers of potential cases of listeriosis caused by consumption of prepackaged,non-vacuum sealed,refrigerated ready-to-eat meat products were 0.01(95%confidence interval[CI]:0-1.71 × 10-2)per year per million in healthy individuals aged 5-<65 years old,0.22(95%CI:0-2.67 × 10-1)in healthy individuals aged 65 and above,and 2.88(95%CI:3.85 × 10-8-4.35)in pregnant women.According to the results of the sensitivity analysis,the initial pollution level of LM in the retail stage was the most important factor affecting the prevalence(R=0.25),followed by retail temperature(R=0.08),retail time(R=0.07),and amount of consumption per meal(R=0.07).Conclusions For pre-packaged,non-vacuum sealed,cooked meat products,the most important measure to reduce the prevalence of listeriosis is to control the initial contamination level,which requires food processing plants to regularly clean and strictly disinfect the processing environment and equipment to minimize LM contamination at the source.Retail delicatessens should strictly maintain a storage temperature below 5.0℃ and strictly adhere to product shelf-life recommendations.As for consumers,they should consume these meat products as soon as possible after purchase or store them under refrigerated conditions and shorten the storage time.Pregnant women should thoroughly heat the meat products before eating to reduce the risk of listeriosis.

9.Operative treatment of geriatric pelvic fractures

Objective:To explore the clinical efficacy of operative treatment of geriatric pelvic fractures.Methods:A retrospective study was conducted to analyze the clinical data of 55 geriatric patients with pelvic fracture who had been operatively treated at Department of Orthopaedic Trauma, Beijing Jishuitan Hospital from January 2020 to October 2023. There were 13 males and 42 females with an age of 72.0 (68.0, 83.3) years. By the AO/OTA classification, there were 47 cases of type B and 8 cases of type C; by the classification for fragility fractures of pelvis, there were 5 cases of type Ⅱ, 44 cases of type Ⅲ, and 6 cases of type Ⅳ. The patients received operative treatment 4.0 (3.0, 6.3) days after injury. Of the 10 patients treated by open reduction and closed reduction, the posterior ring was fixated by a plate in 7 and by screws in 3, and the anterior ring was fixated by either a plate, an external fixator or a minimally invasive anterior pelvic ring internal fixator except in the 2 cases receiving no fixation. Of the other 45 patients treated by closed reduction and fixation, the posterior ring was all fixated by screws except in the 2 cases receiving no fixation, and the anterior ring was fixated by either screws, an external fixator or a minimally invasive anterior pelvic ring internal fixator except in the 10 cases receiving no fixation. The major peri-operative complications, quality of fracture reduction, mortality, and functional recovery of the pelvis at the last follow-up were recorded.Results:There were no major peri-operative complications. According to the Matta criteria, the quality of fracture reduction was excellent in 31, good in 14, fair in 8 and poor in 2 cases, giving a good to excellent rate of 81.8% (45/55). Fifty-four cases were followed up for (15.4±9.1) months and 1 case was lost to the follow-up. Four cases died within 1 year after operation, giving a mortality of 7.4% (4/54). The modified Majeed score at the last follow-up was 69 (54, 76) for the 46 cases who had been followed up for longer than 4 months; the functional recovery of the pelvis at the last follow-up was rated as excellent in 27 cases, as good in 8 cases and as fair in 11 cases, giving a good to excellent rate of 76.1% (35/46) according to the criteria proposed by the faculty of the writers. The internal fixation failed in 3 cases during the follow-up.Conclusion:For the geriatric pelvic fractures, operative treatment can lead to satisfactory clinical outcomes.
10.Effect of Tongdu Huoxue Decoction on Lumbar Spine Function and Serum Inflammatory Factor Levels in Patients with Acute Lumbar Disc Herniation

ObjectiveTo observe the clinical efficacy of Tongdu Huoxue decoction in the treatment of acute lumbar disc herniation (LDH). MethodA total of 316 patients with acute LDH admitted to the orthopedic outpatient department of Hubei Provincial Hospital of Traditional Chinese Medicine and Honghu City Hospital of Traditional Chinese Medicine from January 2020 to June 2023 were randomly divided into two groups. 156 cases in the control group (two cases with stopped follow-up) were treated with meloxicam tablets, while 153 cases in the observation group (five cases with stopped follow-up) were treated with Tongdu Huoxue decoction. Both groups were treated for three months. The clinical efficacy, McGill Pain Score Scale (SF-MPQ), Oswestry Dysfunction Index (ODI) score, and the Japanese Orthopaedic Association (JOA) scores of the two groups before and after treatment were compared. The serum levels of interleukin-1β (IL-1β), interleukin-6 (IL-6), and tumor necrosis factor-α (TNF-α) of the patients before and after treatment were determined by using enzyme-linked immunosorbent assay (ELISA). The NDI-092 type electromyography-evoked potential instrument was adopted to measure the motor conduction velocity and clinical efficacy of the tibial and common peroneal nerves in patients of the two groups before and after treatment, and the clinical safety of the two groups of patients was compared. ResultAfter treatment, the total effective rate in the observation group was 95.4% (146/153), significantly higher than that in the control group of 76.3% (119/156) (χ2 =23.18, P<0.05). After treatment, both groups showed significant reductions in SF-MPQ and ODI scores, as well as the levels of IL-1β, IL-6, and TNF-α (P<0.05), with the observation group showing a more significant reduction (P<0.05). Both groups showed a significant increase in JOA scores and motor conduction velocities of the tibial and common peroneal nerves after treatment (P<0.05), with the observation group showing a more significant increase (P<0.05). ConclusionTongdu Huoxue decoction can alleviate lumbar and leg pain in acute LDH, improve lumbar spine function, and suppress inflammatory reactions. It is highly safe and is worthy of clinical promotion.
