1.Effect of tolerogenic dendritic cells on autophagy of synovial cells in collagen-induced arthritis rats

Objective:To investigate the effect of tolerogenic dendritic cells(tolDC)on autophagy of synovial cells in collagen-induced arthritis(CIA)rats.Methods:Bone marrow mononuclear cells of rats were extracted and induced into tolDC using IL-4,GM-CSF and NF-κB oligonucleotide decoy,and loaded with BⅡC to become BⅡC-tolDC.SD rats were randomly divided into normal control group,CIA model group and BⅡC-tolDC intervention group,with 3 rats in each group.Normal female SD rats were immunized with bovine type Ⅱ collagen solution to construct CIA model.Rats in BⅡC-tolDC intervention group were infused with BⅡC-tolDC via tail vein on the 21st day after initial immunization for two weeks,arthritis indexes were recorded weekly.On the 35th day,the rats were sacrificed,and synovial histopathology of ankle joint of rats in each group were observed by HE staining;the number of osteo-clasts in cartilage of rats in each group were observed by TRAP staining.The number of autophagic of ankle synovial cells of rats in each group were observed by transmission electron microscopy.Levels of serum TNF-α and IL-1β of rats in each group were detected by ELISA.LC3,Beclin-1 and ATG5 proteins of synovial cells of ankle joints of rats in each group were detected by Immunohistochemical staining.Results:CIA rats were constructed successfully by immunization with bovine type Ⅱ collagen.BⅡC-tolDC intervention re-duced the arthritis index of CIA rats,inhibited synovial inflammation and abnormal proliferation of synovial tissue,improved joint bone and cartilage injury,and reduced the number of osteoclasts in cartilage tissue and the number of autophagosomes in synovial cells.At the same time,reduced levels of serum TNF-α,IL-1β,and protein expressions of LC3,Beclin-1 and ATG5 of synovial cell of CIA rats.Conclusion:BⅡC-tolDC may alleviate arthritis lesions of CIA rats by inhibiting synovial cell autophagy of CIA rats.
2.Changes in Ang-2 Levels Before and after Microwave Ablation Treatment in High-Risk Pulmonary Nodule Patients and Its Impact on Postoperative Recurrence

Objective To analyze the changes in angiopoietin-2(Ang-2)levels before and after microwave ablation(MWA)in patients with high-risk pulmonary nodule and its impact on postoperative recurrence.Method A total of 94 patients with high-risk pulmonary nodules admitted to Guizhou Aerospace Hospital from December 2019 to December 2021 were included and categorized into a recurrence group(n=30)and a non-recurrence group(n=64).Clinical data was compared between the two groups.Pearson correlation analysis was used to analyze the correlation between pre-treatment angiopoietin-2(Ang-2)levels and CT parameters.Logistic regression model,restricted cubic splines,threshold effect analysis,and receiver operating characteristic(ROC)curve were used to analyze the relationship between pre-treatment Ang-2 levels and postoperative recurrence.Kaplan-Meier survival curves were used to analyze the differences in overall survival among patients with different Ang-2 levels.Cox regression model was used to analyze the factors affecting patients'survival.Result Compared to pre-treatment levels within the same group,Ang-2 levels in both groups decreased significantly,with statistically significant differences(P<0.05).Pearson correlation analysis showed that pre-treatment Ang-2 levels were positively correlated with BF,BV,PS,and MTT.Ang-2 level was independently associated with postoperative recurrence.Results of ROC analysis indicated that pre-treatment Ang-2 had certain predictive value for postoperative recurrence(area under the curve=0.789).Restricted cubic spline analysis revealed a nonlinear dose-response relationship between pre-treatment Ang-2 and postoperative recurrence(P<0.05).Threshold effect analysis identified that the inflection point of Ang-2 affecting recurrence as 1905.41 pg/mL.Survival analysis demonstrated that the median overall survival of patients in the Ang-2<1905.41 pg/mL group was longer than that in the Ang-2≥1905.41 pg/mL group(P=0.039).Furthermore,Ang-2≥1905.41 pg/mL was an independent factor affecting patients'survival time.Conclusion The levels of Ang-2 decreased significantly in patients with high-risk pulmonary nodules after MWA,and pre-treatment Ang-2 level has certain predictive value for postoperative recurrence.

Non-coding RNAs (ncRNAs) are a class of functional RNAs that play critical roles in different diseases. NcRNAs include microRNAs, long ncRNAs, and circular RNAs. They are highly expressed in the brain and are involved in the regulation of physiological and pathophysiological processes of central nervous system (CNS) diseases. Mounting evidence indicates that ncRNAs play key roles in CNS diseases. Further elucidating the mechanisms of ncRNA underlying the process of regulating glial function that may lead to the identification of novel therapeutic targets for CNS diseases.

6.Brain edema after oocyte retrieval: a case report and literature review

Objective:To investigate the mechanism, treatment and prevention of brain edema after controlled ovarian hyperstimulation (COH) and transvaginal ovarian puncture and oocyte retrieval.Methods:A retrospective clinical study and literature review were performed to analyze one patient who was diagnosed as having brain edema after COH and transvaginal ovarian puncture and oocyte retrieval.Results:After long acting gonadotropin-releasing hormone analogue (GnRH-a) COH protocol, 30 oocytes were obtained. Hydroxyethyl starch 500 mL was given to treat ovarian hyperstimulation syndrome (OHSS) after oocyte retrieval. The patient had sudden irritability, blurred consciousness and vomiting at the 8th hour after oocyte retrieval. The examinations showed hyponatremia and brain edema. The patient relived after mannitol and hypertonic saline treatment. On the 5th day after oocyte retrieval, the patient performed paracentesis guided by ultrasound due to seroperitoneum. Low molecular weight heparin was applied to prevent thrombosis after the flare up of serum D-Dimer on the 7th day. The patient recovered and discharged after 2 weeks.Conclusion:The incidence of brain edema after COH and transvaginal ovarian puncture and oocyte retrieval was very low. However, the symptoms may be severe and may be life-threatening.

8.Identification of latent class of sleep quality among breast cancer patients during chemotherapy and differences on quality of life based on latent profile analysis

Objective:To identify the classification characteristics and quality of life (QOL) of breast cancer (BC) patients during chemotherapy, so as to provide basis for improving the sleep and QOL of this group.Methods:A cross-sectional investigation was completed among 421 BC patients in 5 tertiary hospitals in Shanghai, Wuhan, Tangshan and Nanning in 1-12 months of 2016 using validated instruments including self-made general information questionnaire, Pittsburgh Sleep Quality Index (PSQI) and Functional Assessment of Cancer Therapy-Breast (FACT-B).Results:Four latent class of patients were identified through latent profile analysis (LPA), named by badly worse sleep quality(SQ) (C1, n=23), medium-SQ with difficulty to fall asleep (C2, n=127), medium-SQ with worse sleeping process (C3, n=30), none sleep disorders (C4, n=241). Total points of SQ among C1-C4 had significant difference ( χ2 value was 309.28, P<0.05). Age, BMI, job status, whether had surgery and course of chemotherapy between classes had statistically significant differences ( χ2 values were 9.57-25.28, all P<0.05). It had significant difference between C2 and C3, C2 and C4, C3 and C1, C3 and C4 on QOL ( χ2 values were 5.96-52.73, all P<0.05). Conclusion:SQ of BC patients during chemotherapy has heterogeneity among population. Different features of SQ of BC patients have different performance on QOL. Health professionals should keep an eye on patients with features of older age, high BMI, in job status, already received surgery and during early-stage chemotherapy, provide personal nursing intervention to improve SQ and QOL.
9.Prevalence and risk factors of low back and pelvic pain in women with rectus abdominis diastasis: a multicenter retrospective cohort study

Background:
To explore the association between low back pain (LBP) and pelvic pain (PP) and rectus abdominis diastasis (RAD) in postpartum women and identify the characteristics and risk factors.
Methods:
Women diagnosed with RAD and a history of labor and delivery, between 2009 and 2018, were identified from six hospitals within the Partners Healthcare System. Univariate and multivariable binary logistic regression analyses were used to identify the risk factors associated with pain.
Results:
Age at onset of RAD in the non-cesarean delivery group was earlier than those in cesarean delivery (CD) group (P = 0.017). Women who underwent CD demonstrated 4.5 times greater risk of RAD than those who had no CD exposure. The cumulative composition ratio of LBP at every age stage of the period from 8 years pre-first delivery to 8 years post-first delivery was significantly higher than the other five conditions (RAD, umbilical hernia, PP, depressive disorder [DD], and strain of muscle, fascia, and tendon [SMFT]) (P for trend < 0.001). Women with DD, SMFT, and PP were more likely to have LBP (odds ratio [OR] = 1.91, 95% confidence interval [CI] 1.06 to 3.47, P = 0.032; OR = 4.50, 95% CI 1.64 to 12.36, P = 0.003; OR = 2.14, 95% CI 1.17 to 3.89, P = 0.013; respectively).
Conclusions
In postpartum women with RAD, DD, SMFT, and PP were found to be risk factors contributing to the development of LBP. Race and LBP also played roles in the development of PP.
10.Practice and exploration of high-quality development of residential training bases

In order to promote the connotation construction of the resident standardized training bases and improve the training quality, a tertiary hospital made key breakthroughs against the existing problems in the base under the guidance of national key training base standards in September 2020. Optimization and innovation were explored in terms of residential teaching supervision, performance appraisal system, regular teaching meeting, educational fund management, pilot project of full-time residential teaching secretaries, clinical skill center construction, cultivation of faculty development, and so on. These experiments have paved the way to high quality and sustainable development of residents standardized training. By the end of December 2021, the hospital had achieved significant progress in such aspects as residential teaching atmosphere, resident learning initiative, and training base branding. These achievements improved the quality of resident standardized training of the hospital in general, and provided a reference for promoting the high-quality development of residential training in China.
