OBJECTIVE To systematically evaluate the influential factors for all-cause mortality in patients with carbapenem-resistant Gram-negative bacteria （CR-GNB） treated with polymyxin B. METHODS PubMed， Embase， the Cochrane Library， Web of Science， Wanfang Data， VIP， CBM and CNKI were searched to collect clinical studies on all-cause death within 30 days or 28 days after treatment with polymyxin B in patients with CR-GNB infection from database establishment to July 2025. After literature screening， data extraction and evaluation of literature quality， meta-analysis was performed using RevMan 5.4 software. RESULTS A total of 12 studies were included， involving 1 326 patients， among whom 529 patients died， with a mortality rate of 39.89%. Meta-analysis results showed that combined with cardiovascular disease [OR＝2.06， 95%CI （1.37， 3.09）， P ＝0.005 ] ， increased Sequential Organ Failure Assessment （SOFA） score [OR＝1.20， 95%CI （1.07， 1.35）， P ＝0.003 ] ， mechanical ventilation [OR＝2.35， 95%CI （1.65， 3.34）， P ＜0.001 ] ， continuous renal replacement therapy （CRRT） [OR＝2.58， 95%CI （1.67， 3.97）， P ＜0.001 ] ， bloodstream infection [OR＝3.24， 95%CI （2.19， 4.78）， P ＜0.001 ] ， multiple-site infection [OR＝1.51， 95%CI （1.03， 2.20）， P ＝0.03 ] ， septic shock [OR＝3.19， 95%CI （1.94， 5.24）， P ＜0.001 ] ， use of vasoactive drugs [OR＝2.90， 95%CI （1.97， 4.27）， P ＜0.001 ] ， and the occurrence of acute kidney injury （AKI） [OR＝2.17， 95%CI （1.41， 3.36）， P ＜0.001 ] were risk factors for all-cause mortality in patients with CR-GNB infection treated with polymyxin B. Conversely， an extended duration of polymy xin B treatment [OR＝0.92， 95%CI （0.86， 0.99）， P ＝0.03 ] and early administration after CR-GNB infection [OR＝0.47， 95%CI （0.25， 0.85）， P ＝0.01 ] were protective factors. CONCLUSIONS Patients with cardiovascular disease， receiving mechanical ventilation or CRRT， having bloodstream infection， multiple-site infection or septic shock， combining with vasoactive drugs， with AKI and increased SOFA scores have a higher risk of all-cause mortality. Conversely， extending the duration of polymyxin B treatment （beyond 7 days） and early administration within 48 hours after confirmed CR-GNB infection can significantly reduce the risk of all-cause mortality.

Objective To track and evaluate the implementation and application of the occupational health standard Radiation shielding requirements for radiotherapy room—Part 4: Radiotherapy room of ²⁵²Cf neutron afterloading (GBZ/T 201.4-2015) by radiation health technical service agencies, medical institutions, health supervision agencies, and radiotherapy facility design units, and to provide a scientific basis for the further revision and implementation of this standard. Methods Following the Guideline for health standards tracking evaluation (WS/T 536-2017) and the project implementation plan, relevant practitioners were randomly selected for a questionnaire survey. The survey primarily focused on their awareness, standard training, application, and revision suggestions of GBZ/T 201.4-2015. The results were summarized and analyzed. Results A total of 168 evaluation questionnaires were collected from relevant practitioners in 28 provinces. Only 31.6% of the respondents reported being “well familiar” or “ familiar” with the standard, 27.4% of the respondents believed that the standard was widely used, and 45.2% of the respondents believed that the standard could meet the needs of their work. Only 14.9% of the respondents had received relevant training on the standard, more than half of the respondents had not applied the standard within the past 10 years, and 45.2% of the respondents believed that the standard "needs to be revised". Conclusion Due to the small number of californium-252 neutron afterloading radiotherapy devices in operation on the market, the overall awareness of the standard is low, suggesting that relevant authorities need to strengthen training and publicity of the standard, and that certain sections of the standard need to be revised or merged.

Background ¹³⁷Cs in atmospheric aerosol is the product of past nuclear weapon tests and nuclear accidents. When ¹³⁷Cs is released into the atmosphere, it will deposit in dry land and marine environment, causing pollution of soil surface, water, agricultural products, and animal byproducts, and affecting public health. Objective To identify the variation pattern of ¹³⁷Cs activity concentration in aerosol and its correlation with dust concentration in Beijing area from 2017 to 2020. Methods A total of 958 aerosol samples were collected from November 1, 2017 to June 30, 2020 in Beijing with a high volume air sampler at a sampling flow rate about 600 m³·h⁻¹ and a collection time for each sample about 24 h. The activity concentration of ¹³⁷Cs in the aerosol samples was determined with a low-background high-purity germanium γ spectrometer. The dust concentration was calculated using the difference in the mass of the aerosol filter before and after sampling. The detection rate of ¹³⁷Cs and dust concentration in different seasons were compared. Spearman rank correlation test was used to analyze the correlation between ¹³⁷Cs activity concentration and dust concentration. Results From 2017 to 2020, the ¹³⁷Cs activity concentrations of 33 from 958 aerosol samples in Beijing were above the minimum detectable activityconcentration, the overall detection rate of ¹³⁷Cs was 3.4%, and the activity concentration ranged from 1.86 to 45.53 μBq·m⁻³, with a median value of 4.85 μBq·m⁻³. The detection rate of ¹³⁷Cs was highest in spring, followed by autumn, and lowest in winter and summer (8.4%, 3.0%, 1.1%, and 0.5%, respectively). The dust concentration ranged from 0.03 to 1.55 mg·m⁻³, with an average value of 0.18 mg·m⁻³. There was a statistically significant difference in the dust concentrations in spring, summer, autumn, and winter (F=45.51, P<0.05), and the highest value was 0.24 mg·m⁻³ in spring (P<0.05). The ¹³⁷Cs activity concentration was positively correlated with the dust concentration (P<0.05). Conclusion The ¹³⁷Cs activity concentration in aerosol in Beijing from 2017 to 2020 fluctuates within the range of background level, and its activity concentration is highest in spring, followed autumn, and lowest in summer and winter.

Post-stroke gait disturbance is one of the core issues leading to impaired motor function and decreased quality of life. Repetitive transcranial magnetic stimulation (rTMS), as a non-invasive brain regulation technique, has shown unique potential in the field of stroke rehabilitation. rTMS can improve gait and balance function by enhancing neural plasticity, regulating neurotransmitter metabolism, regulating cerebral blood flow, and improving cortical excitability and ion balance. Future research should focus on the development of individualized treatment plan, treatment plan optimization, and the synergistic effect of rTMS combined with other rehabilitation methods, in order to provide more effective treatment strategies for patients with post-stroke gait disturbance.

Objective:To explore the present state of radiation protection in an operational large-sized irradiation facility and the distribution of incurred radiation doses in the controlled area under abnormal conditions, and to analyze the acompanied radiation risk.Methods:With an irradiation facility in operation as the research object, the radiation doses were measured using AT1121 X and gamma dose rate meters for the soruce both in working and storage. Thermoluminescent dosimeters (TLDs) were used to measure the dose distribution in an irradiation field under abnormal conditions. The radiation safety features were checked against the several relevant national standards, with the radiation risks evaluated.Results:The radiation dose rates were in the range of 0.09-0.11 μSv/h, lower than the environmental background level whether in storage or working. Under abnormal working conditions, the radiation dose values from high to low were 1.0-101.3 Sv, 32.7-514.0 mSv and 8.7-183.2 μSv in the irradiation field, respectively. At the outside maze, the doses were close to the background level.Conclusions:The protection features of the irradiation facility meet the requirements of the relevant national standards. Under abnormal conditions, radiation could cause serious damages to the persons staying in the irradiation field. These persons were suggested to access to maze as soon as possible to reduce the exposure time, and activate the emergency protection equipments to deescalate the 60Co source onto the well.

Objective To investigate the awareness of the Radiation Shielding Requirements for Radiotherapy Room–Part 2: Radiotherapy Room of Electron Linear Accelerators (GBZ/T 201.2—2011) among relevant practitioners in medical institutions as well as its implementation and application situation and collect relevant problems and suggestions for an evaluation of the scientificalness, standardization, and timeliness of the standard, and to provide a scientific basis for the further revision and implementation of the standard. Methods An online questionnaire survey was conducted among relevant employees in medical institutions providing medical linear accelerator radiotherapy across 22 provinces of China, which investigated the awareness, training, application, and revision suggestions related to GBZ/T 201.2—2011. The questionnaires were collected and analyzed. Results A total of 340 relevant practitioners filled out the questionnaire. Of the participants, 66.80% were physicists; 79.11% had an awareness of the standard; 56.18% ever participated in the standard-related training; but the survey results showed that the practitioners did not have a good knowledge of the standard’s content, and the training and promotion were not enough; 83.24% thought that the standard had been widely used; 17.60% thought that the standard needed to be revised; 76.76% thought that there was a need to add calculation examples; 88.82% thought that neutron shielding needed to be considered for the 10 MV X-ray accelerator room. Conclusion The standard has been widely known in the field of radiotherapy protection. With the development of radiotherapy technology, the standard should be revised to add calculation examples and consider neutron shielding in the 10 MV X-ray accelerator room. The standard is highly technical and difficult to grasp, so the promotion and implementation goals should be appropriate for different personnel groups, the training for employees at key posts should be strengthened, and the methods recommended in the standard should be uniformly used throughout the country.

Objective To analyze the clinical features of children with EV71 positive hand,foot,and mouth disease(HFMD)and EV71 vaccination,and to explore the relationship between the occurrence of severe disease and the preventive effect of EV71 vaccine.Methods From January 1,2020 to December 31,2022,the clinical data of 131 children with HFMD diagnosed with EV71 infection in Kunming Children's Hospital were retrospectively analyzed.The stool samples of patients with clinically confirmed HFMD were selected for enterovirus nucleic acid detection.The clinical data and EV71 vaccination status of children with universal enterovirus positive and EV71 positive HFMD were analyzed.Results Among the 131 positive cases detected,there were 116 mild cases and 15 severe cases.Among the 80 children who received phone consultations about their EV71 vaccine status,17 were vaccinated,and 63 were not vaccinated.The vaccinated children were all mild cases,while among the unvaccinated children,6 were severe cases.From 2020 to 2022,the period from April to September each year is the peak period for detecting EV71-positive hand,foot,and mouth disease(χ2 = 125.705,P = 0.000).The positive detection rate for children under 1 year old and over 5 years old was higher than that for children aged 1 to 5 years(χ2 = 8.765,P = 0.033),and there was no significant difference in the positive detection rate between boys and girls(χ2 = 1.221,P = 0.269).Conclusion EV71 vaccine is of great significance in reducing the occurrence of severe cases.Combined with the current low vaccination rate in Kunming,Yunnan Province,it is suggested that relevant institutions should continue to increase the publicity of EV71 vaccination.

OBJECTIVE To analyze the influential factors of cardioto xicity in patients with positive breast cancer of human epidermal growth factor receptor 2（HER-2）treated by trastuzumab combined with chemotherapy. METHODS From April 2017 to January 2021，200 HER-2 positive breast cancer patients receiving pirarubicin + cyclophosphamide combined with sequential paclitaxel+trastuzumab were collected from our hospital. According to the presence or absence of cardiotoxicity ，the patients were divided into cardiotoxicity group and non-cardiotoxicity group. The clinical data and echocardiographic results of the patients were collected，and the influential factors of cardiotoxicity were analyzed. RESULTS Among 200 patients，43 patients suffered from cardiotoxicity with the incidence of 21.5％. The proportion of patients with cardiotoxicity during pirarubicin+cyclophosphamide therapy accounted for 5.5％（11/200），and the proportion of patients with cardiotoxicity during sequential paclitaxel+trastuzumab therapy accounted for 20.5％（41/200）；the latter was significantly higher than the former （P＜0.01）. At the same time ，the decrease of left ventricular ejection fraction during sequential therapy of paclitaxel and trastuzumab was significantly higher than that during pirarubicin+cyclophosphamide therapy [ 14％（12％，17％）vs. 7％（3％，10％），P＜0.001]. Compared with patients without cardiotoxicity ，the proportion of patients with cardiotoxicity with a history of hyperlipidemia was significantly higher （P＜ 0.01），while the proportion of patients receiving dexrazoxane was significantly lower （P＜0.01）. Results of binary Logistic regression analysis showed that the history of hyperlipidemia [OR ＝3.672，95％ CI（1.499，8.992），P＝0.004] and the use of dextrazoxane [OR ＝0.154，95％ CI（0.072，0.330）， P＜0.001] were associated with the occurrence of cardiotoxicity. CONCLUSIONS Hyperlipidemia is an independent risk factor for cardiotoxicity induced by pirarubicin + cyclophosphamide combined with sequential paclitaxel+trastuzumab in HER 2 positive breast cancer patients ，while the use of dextrazoxane is a protective factor.

At present, domestic scholars in China have conducted research on the implementation and process of doctor-patient joint decision-making, but they are facing difficulties in localization of decision-making theory, human resources of decision-making and transformation of decision-making results. Social work involved in doctor-patient joint decision-making can unlock channels of communication between doctors and patients, make full use of existing resources, and promote the physical and mental health of patients. From the perspective of social work, the involvement of doctor-patient joint decision-making will face the challenges of ambiguous decision-making authority, "non counterpart" social work talents, and the reluctance of doctors and patients to take responsibility for decision-making, makes it difficult for social workers to build a bridge of communication, cooperation, and trust in the intervention process. Therefore, this paper proposed to explore the platform of standardization, diversification and symmetry by establishing an "embedded" intervention process, a "patient-centered" multidisciplinary team, and a "Gong" communication model.

Objective: To study the output dose of external beam radiotherapy in Beijing and analyze the cause of output dose  deviation.  Guidance  is  given  on  the  selection  of  measurement  methods,  calculation  formulas  and  parameters. Methods: During 2020, the output doses of medical electron accelerators, tomotherapy, cyberknife, and γ-knife were meas- ured based on the water absorbed dose calibration factor combined with the requirements in the IAEA TRS398 and TRS483 reports, and low-energy X-rays were measured based on the air kerma calibration factor combined with the requirements in the AAPM protocal. The output dose was measured and compared with the preset irradiation value. Results　: A total of 59 external beam radiation therapy equipments’s output dose were investigated.2 of the 49 accelerators’s absolute value of relative deviation between the system indication value and the measurement exceeded 3%; the deviation of other external beam radiotherapy equipments was less than 3%. Conclusion　 A unified method and multi-party quality control can ensure the accuracy of the output dose. In the routine operation of the equipment, the hospital should strictly implement periodic inspections to ensure accurate and reliable radiotherapy.
