1.Effect of stage Ⅰ comprehensive cardiac rehabilitation in patients with acute ST elevation myocardial infarctionafter emergency percutaneous coronary intervention

Objective This study aimed to investigate the effect of stage Ⅰ comprehensive cardiac rehabili-tation in patients with acute ST elevation myocardial infarction(STEMI)after emergency percutaneous coronary intervention(PCI).Methods A total of 72 patients with acute ST-segment elevation myocardial infarction combined with PCI admitted to the Department of Cardiovascular Medicine of Beijing Electric Power Hospital of State Grid Corporation from June 2021 to June 2022,which were selected as the research objectsand divided into control group and observation group randomly(36 cases in each group).The control group was treated with routine nursing and health education,and the observation group with stage Ⅰ comprehensive cardiac rehabilitation,including initial assessment(cardiovascular comprehensive assessment),exercise training(exercise training and breathing train-ing),daily activity suggestions and health education,discharge assessment(six-minute walking test and Barthel index assessment).The score of Barthel index(BI)at discharge,the 6-minute walking test distance(6MWD)at discharge,the incidence of major adverse cardiovascular event(MACE)during hospitalization and within one month of discharge,and the length of stay were compared between the two groups.Results After intervention,the six-minute walking test distance(6MWD)and Barthel index(BI)score in the observation group were better than those in the control group,the difference was statistically significant(P<0.05).The incidence of major adverse cardiovascular events(MACE)during hospitalization and one month after discharge was lower in the observation group than in the control group,and the difference was statistically significant(P<0.05).The length of hospital-ization in observation group was lower than that in control groupbut there was no statistical difference(P>0.05).Conclusion The application of phase Ⅰ comprehensive cardiac rehabilitation training in patients with acute ST-segment elevation myocardial infarction combined with emergency PCI could improve the patients'exercise ability,improve their ability of daily activity,reduce the incidence of major adverse cardiovascular events(MACE)in the early stage of the disease,facilitate the patients to return to their families and society as soon as possible,and improve their quality of life.It has high clinical application value.

4.Accuracy of endoscopy-based artificial intelligence-assisted diagnostic system in the diagnosis of early esophageal cancer: A systematic review and meta-analysis

Objective To systematically evaluate the accuracy of endoscopy-based artificial intelligence (AI)-assisted diagnostic systems in the diagnosis of early-stage esophageal cancer and provide a scientific basis for its diagnostic value. Methods PubMed, EMbase, The Cochrane Library, Web of Science, Wanfang database, VIP database and CNKI database were searched by computer to search for the relevant literature about endoscopy-based AI-assisted diagnostic systems for the diagnosis of early esophageal cancer from inception to March 2022. The QUADAS-2 was used for quality evaluation of included studies. Meta-analysis of the literature was carried out using Stata 16, Meta-Disc 1.4 and RevMan 5.4 softwares. A bivariate mixed effects regression model was utilized to calculate the combined diagnostic efficacy of the AI-assisted system and meta-regression analysis was conducted to explore the sources of heterogeneity. Results A total of 17 articles were included, which consisted of 13 retrospective cohort studies and 4 prospective cohort studies. The results of the quality evaluation using QUADAS-2 showed that all included literature was of high quality. The obtained meta-analysis results revealed that the AI-assisted system in the diagnosis of esophageal cancer presented a combined sensitivity of 0.94 (95%CI 0.91 to 0.96), a specificity of 0.85 (95%CI 0.74 to 0.92), a positive likelihood ratio of 6.28 (95%CI 3.48 to 11.33), a negative likelihood ratio of 0.07 (95%CI 0.05 to 0.11), a diagnostic odds ratio of 89 (95%CI 38 to 208) and an area under the curve of 0.96 (95%CI 0.94 to 0.98). Conclusion The AI-assisted diagnostic system has a high diagnostic value for early stage esophageal cancer. However, most of the included studies were retrospective. Therefore, further high-quality prospective studies are needed for validation.
5.Clinical features of primary bilateral macronodular adrenal hyperplasia

Objective:To investigate the clinical profile of primary bilateral macronodular adrenal hyperplasia(PBMAH) and sex difference.Methods:One hundred and forty cases of PBMAH were recruited in our center from 2014, and all patients were evaluated for hormone secretion, adrenal imaging, and metabolic parameters.Results:Overt Cushing′s syndrome accounted for 76.4% of PBMAH cohort and 47.9% were female. The overt group had higher serum cortisol and 24 h urinary free cortisol levels, lower adrenocorticotropic hormone, higher serum cortisol after low-dose dexamethasone suppression tests, larger total adrenal size, and a higher percentage of obesity, hypertension, diabetes mellitus, and hypokalemia than the subclinical group(all P<0.05). When compared with the male group, the female group had smaller adrenal size( P<0.001), lower HbA 1C( P=0.003), higher total cholesterol( P=0.005), and lower density lipoprotein-cholesterol levels( P=0.035). Further, 24 h urinary free cortisol in the male group was found to be positively correlated with diastolic blood pressure, fasting glucose, 2 h postprandial glucose after oral glucose tolerance test(OGTT), and HbA 1C after adjusted for age, body mass index, and onset duration, and was negatively correlated with body mass index and potassium levels. While 24 h urinary free cortisol in the female group was positively correlated only with diastolic blood pressure, fasting glucose, and 2 h postprandial glucose after OGTT(all P<0.05). During follow-up, 80.0% of patients achieved remission after unilateral adrenalectomy, with a recurrence rate of 17.9%. Conclusion:PBMAH related metabolic disorder is more pronounced in overt Cushing′s syndrome and males. Unilateral adrenalectomy as an effective treatment can benefit the majority of patients.
6.Analysis on Technical Problems and Countermeasures of Standard Formulation of Traditional Chinese Medicine Formula Granules

After the completion of the pilot work of traditional Chinese medicine formula granules (TCMFGs), the national and provincial medical products administrations have published and implemented about 440 varieties of TCMFGs standards. Based on the previous work, this paper analyzed technical problems encountered in the review and evaluation of Shandong TCMFGs standards, mainly involving the executive standards and distinguishing technologies of raw materials, the adding process of excipients in the procedure item, the rationality of quality control methods, the information content and reproducibility of characteristic chromatograms, the nomenclature and accessibility of reference materials, etc. The common problems such as the coverage of standards, specification differences, and the integrity of quality control items of current TCMFGs standards were discussed deeply. It is proposed to promote the upgrading of provincial standards to national standards, accelerate the research and development of reference materials, advocate the use of high-quality raw materials, explore the evaluation methods of high-quality products, and strengthen the quality supervision of the whole process. Suggestions of this paper is hoped to provide references for the formulation of national and provincial TCMFGs standards, promote the continuous improvement of TCMFGs standard system, and ensure the healthy and orderly development of the TCMFGs industry.
7.Efficacy and Safety of Duloxetine in the Improvement of Pain Symptoms of Knee Osteoarthritis :A Meta-analysis

OBJECTIVE:To systematically e valuate the efficacy and safety of duloxetine in the improvement of pain symptoms of knee osteoarthritis (KOA),and to provide evidence-based reference for clinical treatment of KOA. METHODS :Retrieved from PubMed,Embase,Medline,Cochrane Library ,CNKI,VIP and Wanfang database ,during the establishment of the database to Sept. 2019,RCTs about duloxetine (trial group )vs. placebo (control group )in the improvement of pain symptoms of KOA were collected. After data extraction and quality evaluation using Cochrane system evaluator ’s manual 5.1.0,Meta-analysis was performed by using Stata 14.0 software for WOMAC total score ,WOMAC pain score ,WOMAC stiffness score ,WOMAC function score and BPI-S score ,as well as the incidence of adverse reaction such as dry mouth ,somnolence and nausea. RESULTS:A total of 6 RCTs were included ,involving 2 059 patients. Meta-analysis results showed that WOMAC total score [MD=-0.34,95%CI(-0.48,-0.20),P<0.05],WOMAC pain score [MD =-0.41,95%CI(-0.54,-0.29),P<0.05], WOMAC stiffness score [MD =-0.24,95% CI(-0.37,-0.12),P<0.05],WOMAC function score [MD =-0.43,95%CI (-0.55,-0.31),P<0.05],BPI-S score [MD =-0.38,95%CI(-0.48,-0.28),P<0.05] of trial group were significantly lower than those of control group ;the incidence of dry mouth [RR =3.55,95%CI(2.00,6.29),P<0.05],somnolence [RR =3.23, 95%CI(1.88,5.54),P<0.05] and nausea [RR =6.95,95%CI(2.99,16.15),P<0.05] in trial group were significantly higher than control group ,with significant difference. CONCLUSIONS :Duloxetine can relieve the pain and improve knee function in patients with KOA ,but it is necessary to pay attention to its adverse reactions.

Toxoplasmosis is a worldwide zoonosis caused by an intracellular protozoan parasite, Toxoplasma
gondii. We report here a diabetic patient who was diagnosed as toxoplasmosis with multiple cranial
nerve palsies and cavernous sinusitis. A 37-year-old male presented with an 11-day history of gingival
pain, one day history of ptosis and diplopia. He has been having diabetes mellitus for 6 years, and has
a history of contact with cats. After admission, his symptoms worsened with right 3rd to 7th cranial
nerve palsies. The brain magnetic resonance imaging (MRI) showed cavernous sinusitis in the right
sellar region. Serology for toxoplasma was positive for IgM and negative IgG. The patient was treated
with oral clindamycin (900 mg/day) and dexamethasone (15 mg/day). The right visual acuity and
lid-conjunctival swelling improved after 3 days. At follow-up after a month, the movement of the
right eye significantly improved. This case demonstrate the rare occurrence of multiple cranial nerve
(3rd to 7th) palsies from toxoplasmosis cavernous sinusitis, which is a potentially treatable condition.
9.The technique of guide-wire loop in interventional therapy of patients with Budd-Chiari syndrome with hepatic vein obstruction

Objective To investigate the value of guide-wire loop in interventional therapy of patients with Budd-Chiari syndrome with hepatic vein obstruction.Methods A retrospective study was conducted on 25 patients with Budd-Chiari syndrome (BCS) with hepatic vein obstruction treated from May 2011 to August 2014.The technique of guide-wire loop was used in these patients.The pressure of the hepatic vein was measured before and after treatment.The difference in the pressure was analyzed by the t test.Results All the patients were treated successfully using guide-wire loop angioplasty.No complications of bleeding,pericardial tamponade and liver capsule hemorrhage were observed.After treatment,the pressure of the hepatic vein reduced from (48.3± 8.0) cmH2O to (20.9 ± 3.8) cmH2O (t =26.82,P < 0.05);The symptoms and physical signs of the patients were relieved or disappeared.BCS-related symptoms reappeared on follow-up in 4 patients,2 were treated by balloon dilation successfully.1 patient was treated with transjugular intrahepatic portosystemic shunt (TIPS) because of decompensated liver cirrhosis.1 patient with ankylosing spondylitis had treatment failure and the symptoms relapsed for the third time.Conclusion The technique of guide-wire loop in interventional therapy of patients with Budd-Chiari syndrome with hepatic vein obstruction is safe and effective,and it can improve the overall success rate of treatment.
10.Effect of comprehensive psychological intervention on anxiety and depression in patients with chronic heart failure

Objective To investigate the effect of group cognitive behavioral therapy on anxiety and depression in patients with chronic heart failure ( CHF ) .Methods One hundred patients with CHF were recruited from January 2012 to December 2013 in cardiovascular department , and were divided into the intervention group and the control group , with 50 patients each .Comprehensive intervention including routine heart failure care and group cognitive behavioral therapy combined with other therapies was adopted on the patients in the intervention group .The Self-rating Anxiety Scale ( SAS ) and the Self-rating Depression Scale ( SDS) were used to compare the difference of anxiety and depression between groups .Results At the baseline, the incidence of anxiety was 25%, within which 11 patients had mild anxiety , and 10 patients had moderate anxiety, and 4 patients had severe anxiety.The incidence of depression was 39%, within which 19 patients had mild depression , and 14 patients had moderate depression , and 6 patients had severe depression .The score of SAS and SDS in the intervention group after intervention were (36.90 ±8.465) and (27.53 ±7.162), respectively, which were significantly lower than (40.91 ±8.019) and (36.78 ±10.562) in the control group (t=9.244,10.335, respectively;P<0.01).Conclusions Group cognitive behavioral therapy can effectively relieve the anxiety and depression in patients with chronic heart failure .
