Acute kidney injury (AKI) is a clinical syndrome characterized by severe impairment of renal function. The occurrence of AKI often leads to the accumulation of metabolites, electrolyte imbalance, changes in drug concentration gradients, and may even induce systemic inflammatory responses and indirectly affect distal organs, which is one of the reasons for the high mortality rate of AKI. In traditional Chinese medicine, cordyceps sinensis is a widely used natural herb, which is believed to improve kidney injury and promote the recovery of renal tubular function. It contains abundant chemical components, has diverse species, and the distribution, content and efficacy of its active substances also vary. This article reviews the research progress of cordyceps sinensis in the treatment of AKI.

Background and Aims:Elderly patients undergoing laparoscopic radical resection of colon cancer often experience decreased sleep quality, which may hinder postoperative recovery.Although pharmacological interventions are commonly used in clinical practice to improve postoperative sleep, conventional medications may lead to adverse effects such as delirium and dependence. This study aimed to evaluate the effect of a non-pharmacological intervention—buccal acupuncture combined with ultrasound-guided stellate ganglion block(SGB)—on postoperative sleep quality in elderly patients. Methods:A total of 60 elderly patients who underwent laparoscopic radical resection of colon cancer at the Forth Hospital of Changsha from February to August 2024 were enrolled.Using a random number table, the patients were divided into two groups:30 in the control group(SGB alone)and 30 in the study group(SGB combined with buccal acupuncture).Perioperative mean arterial pressure(MAP),heart rate(HR),postoperative visual analogue scale(VAS)scores,Pittsburgh Sleep Quality Index(PSQI)scores, and the incidence of adverse events were recorded and analyzed. Results:There were no statistically significant differences in baseline characteristics between the two groups(all P>0.05).The study group showed significantly lower intraoperative and postoperative HR and MAP compared to the control group(all P<0.05).VAS scores at 6,24,and 48 h postoperatively,as well as PSQI scores on postoperative days 1,3,and 5,were significantly lower in the study group(all P<0.05).Additionally,the incidence of drowsiness was significantly reduced(P<0.05).No significant differences were found between the two groups in terms of nausea, vomiting,or agitation(all P>0.05).Conclusion:The combination of buccal acupuncture and SGB during the perioperative period can effectively improve postoperative sleep quality, alleviate pain, and reduce adverse reactions in elderly patients undergoing laparoscopic radical resection of colon cancer. This safe and effective non-pharmacological intervention holds promising clinical application value.

Objective To explore the influencing factors of serum uric acid (UA) level and its relationship with SLC2A9 gene polymorphism. Methods A total of 2000people in the health examination center of Yangpu District Central Hospital were selected to examine their blood pressure, blood sugar, blood lipid and other biochemical indicators. The single nucleotide polymorphism (SNP) locus rs2241480 of SLC2A9 gene was detected and analyzed. According to UA level, UA was divided into high UA group (n=217), middle UA group (n=1705) and low UA group (n=78). The biochemical indexes and SLC2A9 genotype of each group were compared, and the relationship between UA and SLC2A9 gene polymorphism was analyzed. Results The prevalence of hyperuricemia (HUA) was 10.85% in physical examination population, 12.92% in males, which was significantly higher than 8.48% in females (P﹤0.05). With the increase of UA level, body mass index (BMI), systolic blood pressure (SBP), diastolic blood pressure (DBP), fasting blood glucose (FBG), total cholesterol (TC), triglyceride (TG), low density lipoprotein cholesterol (LDL-C), blood urea nitrogen (BUN), creatinine (Cr) increased significantly, and high density lipoprotein (HDL-C-c) increased significantly.) there was a significant decrease, with statistical significance (P﹤0.05). The genotyping of rs2241480 locus in different UA levels showed significant difference (P﹤0.05). Male (OR=1.99), BMI (OR=3.01), SBP (OR=3.77) were independent risk factors for HUA, while HDL-C (OR=0.27) and rs2241480 locus genotype (CC, OR=0.41) were protective factors (P﹤0.05). Conclusion Traditional cardiovascular risk factors such as blood pressure and lipid are independent risk factors for UA level. SLC2A9 gene polymorphism may be associated with the occurrence of HUA.

Objective To evaluate the effect of preconditioning with penehyclidine hydrochloride (PHC)on mitochondria-dependent pathway of apoptosis during myocardial ischemia-reperfusion(I∕R)in rats. Methods Twenty-four healthy adult male Wistar rats were allocated into 3 groups(n=8 each)using a random number table: sham operation group(group S), I∕R group and PHC preconditioning group (group P). Myocardial I∕R was produced by occlusion of left anterior descending branch of coronary artery for 30 min followed by 3 h reperfusion. PHC 10 mg∕kg was injected via the tail vein at 30 min before oc-clusion in group P. The rats were sacrificed at the end of reperfusion, hearts were removed and left ventric-ular specimens were obtained for determination of apoptosis in cardiomyocytes(by TUNEL), and apoptosis index(AI)was calculated. Mitochondria and cytoplasm were isolated for detection of mitochondrial mem-brane potential(MMP, by JC-1 method)and expression of Bcl-2, cytochrome c(Cyt c)in mitochondri-a and cytoplasm and activated caspase-3 in myocardial tissues(by Western blot). Results Compared with group S, AI was significantly increased, the expression of Bcl-2 and mitochondrial Cyt c was down-regula-ted, MMP was decreased, and the expression of Cyt c in cytoplasm and activated caspase-3 was up-regula-ted in I∕R and P groups(P<0. 05). Compared with group I∕R, AI was significantly decreased, the ex-pression of Bcl-2 and mitochondrial Cyt c was up-regulated, MMP was increased, and the expression of Cyt c in cytoplasm and activated caspase-3 was down-regulated in group P(P<0. 05). Conclusion The mechanism by which PHC preconditioning attenuates myocardial I∕R injury is related to inhibiting mitochon-dria-dependent pathway of apoptosis in rats.
