1.Noninvasive Screening for Chronic Atrophic Gastritis Using Photoplethysmography-derived Meridian-labelled Harmonic Parameters

ObjectiveChronic atrophic gastritis (CAG) is usually diagnosed by gastroscopy and histopathological biopsy. These procedures remain the reference standard, but their invasive nature and resource requirements may limit their use in large-scale population screening and repeated follow-up. A convenient and reproducible method for noninvasive auxiliary screening may help identify individuals who require further endoscopic assessment. Fingertip photoplethysmography (PPG) provides a noninvasive recording of peripheral pulse waves and allows harmonic features to be extracted from the signal. In this study, the so-called meridian-related variables were defined as PPG-derived harmonic parameters labelled according to meridian nomenclature, rather than as direct measurements of meridian physiology. This study aimed to compare these harmonic parameters between patients with CAG and non-CAG controls, identify parameters that remained different after age adjustment, and develop a multivariable model for noninvasive auxiliary screening and pre-endoscopic risk stratification of CAG. MethodsA total of 343 participants were included, comprising 171 patients with CAG and 172 non-CAG controls. CAG diagnosis was established using gastroscopy and histopathology as the reference standard. Fingertip PPG signals were collected using a PPG-based pulse acquisition device. Eight PPG-derived harmonic parameters labelled according to meridian nomenclature were extracted for analysis. Between-group differences were first assessed using nonparametric tests. Age-adjusted analyses were then performed to reduce potential confounding by age. The false discovery rate (FDR) method was applied for multiple-comparison correction. A multivariable logistic regression model integrating age and multiple harmonic parameters was constructed. Model performance was evaluated using receiver operating characteristic (ROC) analysis and the area under the curve (AUC). Internal validation performance was assessed using stratified five-fold cross-validation and bootstrap optimism correction. Threshold performance was examined using both a high-specificity strategy and a Youden index-based cutoff. Decision curve analysis was used to evaluate the model’s net clinical benefit across a range of threshold probabilities. ResultsAll eight harmonic parameters were non-normally distributed. In the univariate analysis, the stomach-labelled harmonic parameter (ST), bladder-labelled harmonic parameter (BL), and liver-labelled harmonic parameter (LR) differed between the CAG and non-CAG groups. After age adjustment and FDR correction, only ST and BL remained statistically significant. Compared with non-CAG controls, patients with CAG showed higher ST values and lower BL values. This finding indicates an associated differential harmonic pattern that was not fully explained by age distribution. However, the discriminative ability of a single harmonic parameter was limited. The best-performing single indicator was ST, with an AUC of 0.652 (95% CI: 0.595-0.707). The multivariable model integrating age and multiple harmonic parameters achieved an AUC of 0.791 (95% CI: 0.743-0.835), representing an improvement of 0.139 over ST alone. In internal validation, stratified five-fold cross-validation yielded a mean AUC of 0.753 (95% CI: 0.715-0.781), and the bootstrap optimism-corrected AUC was 0.748. These results suggest that the model retained moderate discriminative performance after internal validation.At a specificity of at least 95%, the model achieved a sensitivity of only 40.4% (95% CI: 25.7%-49.7%). This high-specificity cutoff may be suboptimal as the preferred threshold for an initial screening setting because of the potential risk of missed CAG cases. The Youden index-based optimal cutoff was 0.419, corresponding to a sensitivity of 80.7% and a specificity of 62.8%. This threshold may better match the practical aim of noninvasive auxiliary screening, where sensitivity is usually prioritized to reduce missed cases. Decision curve analysis showed that, within a threshold probability range of 10%-55%, the model provided higher net clinical benefit than the reference strategies of recommending gastroscopy for all participants or for none. ConclusionPatients with CAG showed associated harmonic differences in fingertip PPG-derived features, mainly characterized by higher ST and lower BL values after age adjustment and FDR correction. Compared with a single harmonic parameter, the multivariable model showed better overall discrimination and retained moderate internal validation performance. These findings suggest that PPG-derived harmonic parameters labelled according to meridian nomenclature may provide auxiliary information for noninvasive auxiliary screening and front-line triage before gastroscopic confirmation in CAG. The present results support further validation rather than immediate clinical implementation. External validation in independent, multicenter, and preferably prospective screening cohorts is needed to assess the model’s generalizability, screening performance, and potential clinical utility.

3.Retrospecitve study on clinical treatment of 214 children with high altitude pulmonary edema

Objective:To analyse the clinical characteristics of 214 cases of paediatric high altitude pulmonary edema(HAPE)and the efficacy of dexamethasone in adjunctive therapy.Methods:This retrospective study analyzed 214 pediatric cases of HAPE admitted to the Department of Paediatrics of the General Hospital of Tibetan Military between June 2015 to June 2017 and June 2019 to June 2021.Patients were divided into dexamethasone-treated group and dexamethasone-untreated group.Baseline data,clinical characteristics were collected to evaluate the treatment efficacy and drug side effects.Results:There were 107 children in each of the two groups with a median age of 8(5,11)years. The median age of the dexamethasone-treated group was 9(6,12)years and the mean age of the dexamethasone-untreated group was 7(3,10)years. The proportion of male children was 69.60%(149/214);the onset of illness was mostly concentrated within 72 hours,accounting for 97.20%(208/214)of the cases;83.18%(178/214)of the cases had symptoms of combined upper respiratory tract infection before entering the plateau. The most important clinical symptoms of the children were cough(86.92%,186/214),cyanosis(70.09%,150/214),and shortness of breath(66.36%,142/214). The proportion of auscultatory rhonchi was 83.18%(178/214),and all cases showed positive findings on chest radiography. After the dexamethasone regimen,the overall cure rate of the children was 94.39%,the average disappearance time of the symptoms and signs was(40.52±7.85)h,and the average hospital stay was(3.60±1.90)d. After treatment with the dexamethasone-free regimen,the overall cure rate was 92.52%,the mean time to disappearance of symptoms and signs was(42.10±7.62)h,and the mean length of stay in the hospital was(3.84±2.08)d. There was no significant difference in the cure rate,the disappearance time of symptoms and signs,and the average hospitalisation days between the two groups( P>0.05),but a total of 11 children in the dexamethasone-treated group experienced adverse drug reactions,and no children in the dexamethasone-untreated group experienced adverse drug reactions. Conclusion:Han Chinese male children,particularly those with upper respiratory infections,should be closely monitored for HAPE risk within three days of ascending to high altitudes. This study does not recommend the use of dexamethasone for pediatric HAPE due to the lack of therapeutic benefits and potential adverse effects.
4.Study on the mechanism of different concentrations of simvastatin on regeneration of sciatic nerve injury in rats

Objective To explore the effects of different concentrations of simvastatin on nerve regeneration after sciatic nerve injury.Methods Rats were randomly divided into the normal group,the control group,the low-dose group and the high-dose group,with 3 rats in each group.Except for the normal group,adult rat sciatic nerve crush injury models were established in the other groups.Rats in the normal group and the control group were orally administered with water,while those in the low-dose group and high-dose group were orally administered with 98%simvastatin at dosages of 4 mg/mL and 40 mg/mL,respectively.The sciatic nerve regeneration in rats was evaluated by sciatic function index(SFI),HE staining,luxol fast blue(LFB)staining and immunofluorescence staining,etc.Results The SFI of rats in the high-dose group 7 days and 14 days after surgery were higher than those in the control group(P<0.05);there was no significant difference in SFI of rats between the low-dose group and the control group 7 days and 14 days after surgery(P>0.05).HE staining and LFB staining results showed that compared with the control group,the number of neurons of rats in the high-dose group increased,the nerve fibers and myelin were clearer and denser,and the nerve function was significantly restored;while no significant improvement was observed in the sciatic nerve of rats in the low-dose group.The immunofluorescence staining results showed that compared with the control group,the immunofluorescence intensity in the high-dose group increased,while that in the low-dose group decreased,the differences were statistically significant(P<0.05).Conclusion High-dose simvastatin can promote peripheral nerve regeneration by regulating the expression of M2 macrophages.
5.Quality consistency evaluation of Tongmai preparations

AIM To evaluate the quality consistency of Tongmai Granules,Tongmai Tablets,Tongmai Capsules and Tongmai Oral Liquid.METHODS The HPLC fingerprints were established,after which the contents of danshensu,protocatechuic aldehyde,3'-hydroxy puerarin,puerarin,puerarin apioside,daidzin,ferulic acid,salvianolic acid B and salvianolic acid A were determined,and cluster analysis and principal component analysis were adopted in the quality analysis from the perspective of daily intake.RESULTS There were 21 common peaks in the fingerprints for 39 batches of samples with the similarities of 0.765-0.997.Various batches of samples were clustered into 5 categories,2 principal components demonstrated the accumulative variance contribution rate of 83.53% .The daily intakes of various constituents in different dosage forms exhibited obvious differences,especially for that of salvianolic acid B,which were low in tablets and capsules,and their heterogeneities existed among the same dosage forms.CONCLUSION This simple and accurate method can provide a reference for the quality evaluation of Tongmai preparations from different manufacturers.

8.The Mechanobiological Response of Immature Dendritic Cells to Nanomorphologies with Different Crystalline Phases on Titanium Surfaces

Objective To elucidate the regulatory effects of titanium surface modification on the immune function of immature dendritic cells(imDCs),different crystalline nanomorphologies were constructed on titanium surface to investigate the mechanobiological response of imDCs to nanomorphologies with different crystalline phases.Methods Nanomorphologies with different crystalline phases were constructed on the titanium surface by anodic oxidation and calcination.The changes of the cytoskeleton F-actin,cell adhesion and morphology of imDCs cultured on nanomorphologies with different crystalline phases were observed by fluorescence staining.The relative gene expression of adhesion molecules was detected by quantitative real-time PCR.The migration behaviors of imDCs were observed using real-time live-cell imaging,and the membrane fluidity was detected by fluorescence polarization.Results Nanomorphologies with different crystalline phases,namely amorphous phase,anatase and rutile,were obtained on the titanium surface by anodic oxidation and calcination.The cytoskeleton of imDCs on nanomorphologies with different crystalline phases was remodeled.The spreading area of cells on anatase crystalline phase was relatively small,which was(353.3±148.5)μm2.The number of adherent cells was the largest,which was 587±132.The expression of adhesion molecules such as CD11a,integrin β2,ICAM1,and VCAM1 were also increased in cells which cultured on anatase crystalline phase.The imDCs cultured on anatase crystalline phase were equipped with strong migration ability.The accumulative migration distance was(383.6±177.7)μm,and the Euclidean migration distance was(51.82±50.13)μm.The membrane fluidity was relatively weak,and the fluorescence polarization was 0.348 5±0.041 8.Conclusions imDCs can respond to nanomorphologies with different crystalline phases on the titanium surface and exhibit different biomechanical behaviors.The results might provide a theoretical basis for the design of titanium biomaterials with immunomodulatory functions.
9.Establishment and evaluation of a predictive model for spontaneous peritonitis in HBV-related primary liver cancer

Objective To establish and evaluate a nomogram prediction model for spontaneous peritonitis in HBV-related primary liver cancer.Methods A retrospective study was conducted on 1298 patients with HBV-related primary liver cancer hospitalized in the Kunming Third People's Hospital from January 2012 to December 2022.General data and serological indicators were collected,and patients were divided into infection group(n=262)and control group(n=1036)based on the occurrence of spontaneous peritonitis.Univariate and LASSO regression analyses were used to screen variables,followed by binary logistic regression to analyze the influencing factors of spontaneous peritonitis in HBV-related primary liver cancer patients,leading to the establishment of a nomogram prediction model.Finally,the Hosmer-lemeshow(H-L)goodness of fit test,receiver operating characteristic(ROC)curve,calibration curve,decision curve analysis(DCA)and clinical impact curve(CIC)were utilized to evaluate the fit degree,accuracy,calibration,and clinical practicability of the nomogram prediction model.Results Single factor analysis revealed significant differences between infection group and control group in portal vein cancer thrombus(PVTT),Child-Pugh grade,China Liver Cancer Staging(CNLC)stage,alcohol consumption history,smoking history,white blood cell count(WBC),neutrophil count(NE),hemoglobin(Hb),fibrinogen(FIB),abnormal prothrombin(PIVKA-Ⅱ),aspartate aminotransferase(AST),alanine aminotransferase(ALT),total protein(TP),prealbumin(PA),γ-glutamyltransferase(GGT),alkaline phosphatase(ALP),cholinesterase(CHE),total bile acid(TBA),total cholesterol(TC),low density lipoprotein(LDL),creatinine(Cr),HBV DNA,CD3+T cells count,CD4+T cells count,CD8+T cells count,CD4+T cells/CD8+T cells ratio,procalcitonin(PCT),serum amyloid A(SAA),interleukin-6(IL-6),high-sensitivity C-reactive protein(hs-CRP),alpha-fetoprotein(AFP),and IL-4(P<0.05).LASSO regression analysis identified 5 variables:Child-Pugh grade,PVTT,WBC,CHE and hs-CRP.Binary logistic regression analysis indicated that Child-Pugh grade(Grade B:OR=5.780,95%CI 3.271-10.213,P<0.001;Grade C:OR=14.818,95%CI 7.697-28.526,P<0.001),PVTT(OR=2.893,95%CI 2.037-4.108,P<0.001),WBC(OR=1.088,95%CI 1.031-1.148,P=0.002),and hs-CRP(OR=1.005,95%CI 1.001-1.010,P=0.026)were the independent risk factors of spontaneous peritonitis in HBV-related primary liver cancer patients.Using these 4 variables,a nomogram prediction model was constructed and evaluated.The P-value of the H-L goodness of fit test was 0.760.Moreover,the area under ROC curve(AUC)was 0.866,with a sensitivity of 0.870 and a specificity of 0.716.The average absolute error of the calibration curve is 0.022.DCA and CIC analyses demonstrated that the nomogram prediction model possessed some clinical utility.Conclusion The nomogram prediction model for spontaneous peritonitis in HBV-related primary liver cancer patients,constructed using Child-Pugh grade,PVTT,WBC and hs-CRP,exhibits a high fitting degree and accuracy,with the prediction probability highly consistent with the actual occurrence probability,and possesses certain clinical practicability.

Objective To elucidate the regulatory effects of titanium surface modification on the immune function of immature dendritic cells(imDCs),different crystalline nanomorphologies were constructed on titanium surface to investigate the mechanobiological response of imDCs to nanomorphologies with different crystalline phases.Methods Nanomorphologies with different crystalline phases were constructed on the titanium surface by anodic oxidation and calcination.The changes of the cytoskeleton F-actin,cell adhesion and morphology of imDCs cultured on nanomorphologies with different crystalline phases were observed by fluorescence staining.The relative gene expression of adhesion molecules was detected by quantitative real-time PCR.The migration behaviors of imDCs were observed using real-time live-cell imaging,and the membrane fluidity was detected by fluorescence polarization.Results Nanomorphologies with different crystalline phases,namely amorphous phase,anatase and rutile,were obtained on the titanium surface by anodic oxidation and calcination.The cytoskeleton of imDCs on nanomorphologies with different crystalline phases was remodeled.The spreading area of cells on anatase crystalline phase was relatively small,which was(353.3±148.5)μm2.The number of adherent cells was the largest,which was 587±132.The expression of adhesion molecules such as CD11a,integrin β2,ICAM1,and VCAM1 were also increased in cells which cultured on anatase crystalline phase.The imDCs cultured on anatase crystalline phase were equipped with strong migration ability.The accumulative migration distance was(383.6±177.7)μm,and the Euclidean migration distance was(51.82±50.13)μm.The membrane fluidity was relatively weak,and the fluorescence polarization was 0.348 5±0.041 8.Conclusions imDCs can respond to nanomorphologies with different crystalline phases on the titanium surface and exhibit different biomechanical behaviors.The results might provide a theoretical basis for the design of titanium biomaterials with immunomodulatory functions.
