1.Clinical characteristics and long-term follow-up study of basal ganglia infarction after minor head trauma in infants and young children.

OBJECTIVES:
To investigate the clinical characteristics and prognosis of infants and young children with basal ganglia infarction after minor head trauma (BGIMHT).
METHODS:
A retrospective analysis was conducted on the clinical data and follow-up results of children aged 28 days to 3 years with BGIMHT who were hospitalized at Children's Hospital of Soochow University from January 2011 to January 2022.
RESULTS:
A total of 45 cases of BGIMHT were included, with the most common symptom being limb movement disorders (96%, 43/45), followed by facioplegia (56%, 25/45). Cerebral imaging showed that 72% (31/43) had infarction accompanied by basal ganglia calcification. After conservative treatment, 42 children (93%) showed significant symptom improvement, while 3 children (7%) experienced recurrent strokes. The median follow-up time was 82 months (range: 17-141 months). At the last follow-up, 97% (29/30) had residual basal ganglia softening lesions. Among 29 cases participating in questionnaire follow-up, 66% (19/29) recovered normally, 17% (5/29) showed significant improvement in symptoms, and 17% (5/29) had poor improvement. According to the grading of the Global Burden of Disease Control Projects, only 1 child (3%) had severe sequelae. There were no significant differences in age at onset, gender, or presence of concomitant basal ganglia calcification between children with and without neurological sequelae (P>0.05).
CONCLUSIONS
The most common initial symptom of BGIMHT is limb movement disorder, and imaging results indicate that most children have concurrent intracranial calcifications. Most infarct lesions later transform into softening lesions, resulting in a generally good prognosis.

OBJECTIVES:
To evaluate the effectiveness of single-balloon and double-balloon enteroscopy in diagnosing pediatric small bowel diseases and assess the diagnostic efficacy of computed tomography enterography (CTE) for small bowel diseases using enteroscopy as the reference standard.
METHODS:
Clinical data from 576 children who underwent enteroscopy at Hunan Children's Hospital between January 2017 and December 2023 were retrospectively collected. The children were categorized based on enteroscopy type into the single-balloon enteroscopy (SBE) group (n=457) and double-balloon enteroscopy (DBE) group (n=119), and the clinical data were compared between the two groups. The sensitivity and specificity of CTE for diagnosing small bowel diseases were evaluated using enteroscopy results as the standard.
RESULTS:
Among the 576 children, small bowel lesions were detected by enteroscopy in 274 children (47.6%).There was no significant difference in lesion detection rates or complication rates between the SBE and DBE groups (P>0.05), but the DBE group had deeper insertion, longer procedure time, and higher complete small bowel examination rate (P<0.05). The complication rate during enteroscopy was 4.3% (25/576), with 18 cases (3.1%) of mild complications and 7 cases (1.2%) of severe complications, which improved with symptomatic treatment, surgical, or endoscopic intervention. Among the 412 children who underwent CTE, the sensitivity and specificity for diagnosing small bowel diseases were 44.4% and 71.3%, respectively.
CONCLUSIONS
SBE and DBE have similar diagnostic efficacy for pediatric small bowel diseases, but DBE is preferred for suspected deep small bowel lesions and comprehensive small bowel examination. Enteroscopy in children demonstrates relatively good overall safety. CTE demonstrates relatively low sensitivity but comparatively high specificity for diagnosing small bowel diseases.

3.The Pathogenesis and Treatment Progress of Extramedullary Multiple Myeloma --Review.

Extramedullary disease (EMD) is an independent prognostic factor for multiple myeloma (MM). Compared with MM without EMD, MM with EMD has different genetic characteristics, with a higher incidence of high-risk chromosomal abnormalities, more complex genomic profile, and immunophenotypic features related to adhesion molecule and chemokine expression. The mutual regulation between myeloma cells and tumor microenvironment, including changes in immune environment, deposition of extracellular matrix, abnormal expression of adhesion molecules, and autocrine secretion of myeloma cells, is involved in the extramedullary migration of myeloma cells. Various immune-targeted therapies have improved the prognosis of extramedullary MM (EMM). This article reviews the genetic characteristics of EMM, important role of tumor microenvironment, and progress of treatment.

4.Panax notoginseng saponins influence the interaction between endometrial cancer cells and macrophages via the EGFR/HSP27 axis

Aim To investigate the effect of Panax no-toginseng saponins(PNS)on the interaction between endometrial cancer cells and macrophages by regulating the epidermal growth factor receptor(EGFR)/heat shock protein 27(HSP27)axis.Methods Ishikawa cells were divided into Control,DDP,PNS-treated,M2-CM,M2-CM+DDP,M2-CM+PNS,M2-CM+PNS+oe-NC,M2-CM+PNS+oe-EGFR,PNS(200 mg·L-1)+oe-NC,PNS(200 mg·L-1)+oe-EG-FR,oe-NC,oe-EGFR,oe-EGFR+si-NC,and oe-EG-FR+si-HSP27 groups.MTT assay was used to detect cell proliferation,Transwell assay for cell invasion,TUNEL assay for cell apoptosis,qRT-PCR for macro-phage polarization markers CD86 and CD163 mRNA levels,ELISA for iNOS and IL-12 levels,and West-ern-blot for EGFR and HSP27 protein expression.A nude mouse xenograft tumor model was established and treated with PNS to evaluate the effect of PNS in vivo.Results Compared with the Control group,PNS and DDP significantly inhibited the proliferation and inva-sion of Ishikawa cells and induced apoptosis.M2-CM treatment inhibited M1 macrophage markers,promoted M2 macrophage markers,and induced Ishikawa cell growth,but this effect was reversed by PNS treatment.EGFR was confirmed as a target of PNS,and compared with the M2-CM+PNS+oe-NC group,EGFR overex-pression promoted M2 macrophage marker levels,in-duced Ishikawa cell proliferation and invasion,and in-hibited apoptosis.Knockdown of HSP27 reversed the effect of EGFR overexpression on the biological behav-ior of Ishikawa cells.Animal experiments showed that PNS could inhibit tumor growth and reduce the positive expression of CD163,EGFR,and HSP27 in tumor tis-sues.Conclusion PNS affects the interaction be-tween macrophages and EC cells by regulating the EG-FR/HSP27 axis,thereby participating in EC progres-sion.
5.Effect and Safety of a New Conditioning Regimen with Chidamide and BEAM for Autologous Hematopoietic Stem Cell Transplantation in Lymphoma

Objective:To assess the efficacy and safety of a new conditioning regimen with chidamide and BEAM for autologous hematopoietic stem cell transplantation(AHSCT)in patients with lymphoma.Methods:Medical records and further follow-up data from 85 patients with lymphoma from May 2015 to September 2020 in our hospital were retrospectively collected and analyzed.Results:Among 85 patients,52 cases accepted BEAM regimen and 33 cases accepted CBEAM followed by AHSCT.In CBEAM group,18 patients(54.5%)received AHSCT as salvage therapy,while only 26.9%(14 cases)for salvage in BEAM group(P<0.01).CBEAM conditioning resulted in shorter neutrophil engraftment of 2 days,while no significant difference was found in platelet engraftment.Although the incidence of liver impairment was higher in CBEAM group(12.1%),the grade of impairment was only Ⅰ to Ⅱ.The two conditioning regimens both achieved good complete remission rate of over 90%,and no transplant-related death occurred.The median follow-up time in the CBEAM group was 18(12,22)months,and 39(20,59)months in the BEAM group.There were no significantly differences in 2-year progression-free survival(PFS)and overall survival(OS)rate between the two groups(P>0.05).In patients with refractory or relapsed non-Hodgkin lymphoma,the 2-year PFS rate after transplantation in BEAM group and CBEAM group was 74.1%and 92.9%,respectively(P>0.05),indicating that chidamide may have certain advantages in prolonging PFS.Conclusion:CBEAM conditioning regimen has a good efficacy and safety in lymphoma patients before AHSCT,especially in refractory and relapsed non-Hodgkin lymphoma patients,suggesting that it could serve as an alternative conditioning regimen prior to AHSCT for lymphoma.

Objective To design a set of field medical equipment maintenance kit to meet the requirements of medical equipment and devices for rapid repair at wartime and peace time.Methods The field medical equipment maintenance kit was composed of a box body,an electric chassis,maintenance devices and spare parts and an electrical system.The box body was made of aluminum-magnesium alloy with hard foam protection layer,which was divided into an upper maintenance tool area and a lower accessory area.The maintenance devices included basic tools,measuring tools and electric tools.The accessores were used to support radiology,clnical laboratory,comprehensive and electrical instruments.A 650 W high-power brushless motor was involved in the electric chassis designed with the mechanical disc brake.The electrical system consisted of a power supply system and a control system,and the power supply system included a charging system for the maintenance kit and a power supply system for the external devices;the control system consisted of a microcontroller minimum control system,a Bluetooth control circuit,a key circuit and a digital tube display circuit.Results The maintenance kit gained advantages in internal area segmentation,rational use of space and transportability by electrically or manually propelled mode,which met all the requirements for medical equipment and device maintenance and could be put into operation immediately after deployment.Conclusion The maintenance kit behaves well in easy transport,flexibility and mobility,and facilitates the maintenance technician for carrying accessories and tools and repairing medical equipment and devices rapidly.[Chinese Medical Equipment Journal,2025,46(5):112-115]

Numerous studies have shown that depression is main-ly associated with the abnormal expression of connexin 43(Cx43)in astrocytes(Astro)and its mediated dysfunction of gap junction(GJ).However,the molecular mechanism of post-translational modifications targeting Cx43 to regulate neuroin-flammation-associated depression is still unclear.Post-transla-tional modifications of Cx43 mainly include phosphorylation of specific amino acid sites by PKC,PKA,PKG,MAPK and PTK,and protein degradation of Cx43 through the K48/K63 polyubiq-uitylation and deubiquitination pathways,which ultimately lead to protein degradation through K48/K63 polyubiquitination and deubiquitination.These modifications are ultimately involved in the regulation of neuroinflammatory responses through the associ-ation of GJ function.In this paper,we systematically review the role of Cx43 post-translational modifications in neuroinflamma-tion,with the aim of further exploring the potential application of targeting these modifications to modulate the inflammatory re-sponse mechanism in improving depressive symptoms.
