1.Perioperative immune dynamics and clinical outcomes in patients undergoing on-pump cardiac surgery

Objective: To characterize perioperative dynamic changes in immune-cell phenotypes and inflammatory cytokines in patients undergoing CPB (cardiopulmonary bypass) cardiac surgery, and to explore their associations with postoperative outcomes. Methods: In this prospective cohort study, 120 adult patients who underwent elective cardiac surgery under CPB at West China Hospital from May 2022 to March 2023 were enrolled. Perioperative immune-cell phenotypes and concentrations of 40 inflammation-related cytokines were measured. The primary outcomes were the sequential organ failure assessment (SOFA) score at 24 h after surgery and ΔSOFA (the peak SOFA score within 48 h after surgery minus the preoperative SOFA score). Secondary outcomes included major adverse cardiovascular events (MACE), acute kidney injury (AKI), respiratory failure, severe liver injury, and infection. Results: The mean age of enrolled patients was 57±10 years. Of these, 52% (62/120) were male and 90% (108/120) underwent valve surgery. Plasma analysis showed that multiple pro-inflammatory cytokines increased during CPB and remained elevated up to 24 h after surgery; five chemokines and the anti-inflammatory cytokine IL-10 peaked at the end of CPB. The SOFA score increased from 1 (1, 2) preoperatively to 7 (5, 10) at 24 h after surgery, with a ΔSOFA of 6 (4, 8). Within 30 days after surgery, 48 patients (40.0%) developed AKI, 17 (14.2%) developed infection, 4 (3.3%) developed severe liver injury, 3 (2.5%) developed respiratory failure, and 3 (2.5%) experienced MACE. During the 2-year follow-up, 8 patients (6.7%) experienced MACE and 5 (4.2%) died. Conclusion: Multi-organ dysfunction is common after cardiac surgery under CPB (median ΔSOFA, 6), accompanied by perioperative activation of multiple immune-cell subsets and upregulation of pro-inflammatory, anti-inflammatory, and chemotactic mediators. This study provides data-driven evidence and research clues for further investigation of the associations between CPB-related immune perturbations and postoperative organ dysfunction and clinical outcomes.
2.Research progress of urea-containing PET tracers targeting prostate specific membrane antigen

Prostate cancer is one of the most common malignant tumors of male genitourinary system. Prostate cancer has the following characteristics: insidious onset, early asymptomatic or not obvious symptoms, complex etiology and pathogenesis, long incubation period and so on. Therefore, the realization of its early diagnosis and treatment is of great significance to the prognosis of patients. Prostate-specific membrane antigen (PSMA) is a type 2 transmembrane glycoprotein that is highly expressed on the membrane of almost all primary and metastatic prostate cancer cells, and is an ideal target for prostate cancer imaging and treatment. In recent years, with the approval of urea-containing small molecule PET (positron emission computed tomography) radiopharmaceutical based on PSMA (68Ga-PSMA-11, 18F-PSMA-1007), PET-CT (positron emission computed tomography/computed tomography) has shown new potential for early diagnosis and accurate staging of prostate cancer patients. This review mainly summarizes the research progress of urea-containing PSMA PET imaging agents and finds that they have defects such as uptake in non-target tissues like the kidneys, lacrimal glands, and salivary glands. Thus, further optimizing their structure to reduce the uptake in non-target tissues, providing provide convenience for the labeling of therapeutic radiopharmaceuticals, thereby achieving the goal of integrated diagnosis and treatment, is an important development direction in this field.
3.Qualitative and quantitative analysis of chemical components of different processed products of Corni Fructus by UPLC-Q-TOF-MS and UPLC-QqQ-MS/MS.

Qualitative and quantitative analysis methods for chemical components of different processed products of Corni Fructus were established to systematically characterize and identify these components, and the content of the main differential components was determined. The chemical components of different processed products of Corni Fructus were collected using ultra-high performance liquid chromatography-quadrupole time-of-flight tandem mass spectrometry(UPLC-Q-TOF-MS). Through analysis of self-built databases, literature, and reference standards, a total of 93 components were obtained, including 19 iridoids, 15 flavonoids, 16 organic acids, eight triterpenoids, eight tannins, four amino acids, two polysaccharides, five olefins, and 16 other compounds. Additionally, by using multivariate statistical methods, the differential components between different processed products of Corni Fructus were screened under the conditions of VIP>1.0 and FC<0.5 or FC>2.0 and P<0.05. The PCA and OPLS-DA results showed differences in the chemical components between different processed products of Corni Fructus. A total of 21 differential components were screened, including tartaric acid, morroniside, and rutin. On this basis, ultra-high performance liquid chromatography-triple quadrupole tandem mass spectrometry(UPLC-QqQ-MS/MS) was used to determine the content of 10 main common differential components, including gallic acid, morroniside, ursolic acid, loganin, swertiamarin, rutin, 5-hydroxymethylfurfural, cornuside Ⅰ, quercetin, and oleanolic acid. The above 10 components showed a good linear relationship within the determined concentration range, with the precision, stability, repeatability, and sample recovery rate all meeting the requirements. Compared with that in Corni Fructus, the content of iridoid glycosides in wine-prepared Corni Fructus and wine-and honey-prepared Corni Fructus decreased, while the content of gallic acid, rutin, quercetin, 5-hydroxymethylfurfural, ursolic acid, and oleanolic acid increased. Compared with wine-prepared Corni Fructus, wine-and honey-prepared Corni Fructus showed varying degrees of increase in all other components, except for a slight decrease in gallic acid content. In summary, this study clarified the influence of different processing methods on the chemical components of Corni Fructus, providing a theoretical basis for the scientific connotation, overall quality evaluation, and clinically rational application of Corni Fructus processing in the future.

4.Ultra-early administration of eculizumab in a child with atypical hemolytic uremic syndrome: a case report.

A 10-year-old girl was admitted with a 38-hour history of widespread subcutaneous petechiae and hematuria and a 6-hour history of jaundice and oliguria. Physical examination revealed widespread subcutaneous petechiae and jaundice of the skin and sclera. Laboratory tests showed anemia, thrombocytopenia, acute kidney injury, and markedly elevated lactate dehydrogenase. Thrombotic microangiopathy was initially diagnosed, with a high suspicion of atypical hemolytic uremic syndrome (aHUS). Eculizumab was initiated within 9 hours of admission (within 48 hours of onset). After the first infusion, hemolysis rapidly ceased, and the platelet count and renal function gradually returned to normal. Whole-exome sequencing identified homozygous deletions of CFHR1 exon 2 and CFHR4 exon 1. aHUS typically has abrupt onset and rapid progression. Clinicians should maintain high suspicion for aHUS when the triad of thrombocytopenia, microangiopathic hemolytic anemia, and acute kidney injury is present. Ultra-early eculizumab (within 48 hours of onset) rapidly blocks complement-mediated thrombotic microangiopathy, reverses organ injury, and improves long-term prognosis. Additionally, complement-related genetic testing is important for etiological clarification and individualized determination of eculizumab treatment duration.

5.Effect of TBL1XR1 Mutation on Cell Biological Characteristics of Diffuse Large B-Cell Lymphoma.

OBJECTIVE:
To investigate the effect of TBL1XR1 mutation on cell biological characteristics of diffuse large B-cell lymphoma (DLBCL).
METHODS:
The TBL1XR1 overexpression vector was constructed and DNA sequencing was performed to determine the mutation status. The effect of TBL1XR1 mutation on apoptosis of DLBCL cell line was detected by flow cytometry and TUNEL fluorescence assay; CCK-8 assay was used to detect the effect of TBL1XR1 mutation on cell proliferation; Transwell assay was used to detect the effect of TBL1XR1 mutation on cell migration and invasion; Western blot was used to detect the effect of TBL1XR1 mutation on the expression level of epithelial-mesenchymal transition (EMT) related proteins.
RESULTS:
The TBL1XR1 overexpression plasmid was successfully constructed. The in vitro experimental results showed that TBL1XR1 mutation had no significant effect on apoptosis of DLBCL cells. Compared with the control group, TBL1XR1 mutation enhanced cell proliferation, migration and invasion of DLBCL cells. TBL1XR1 gene mutation significantly increased the expression of N-cadherin protein, while the expression of E-cadherin protein decreased.
CONCLUSION
TBL1XR1 mutation plays a role in promoting tumor cell proliferation, migration and invasion in DLBCL. TBL1XR1 could be considered as a potential target for DLBCL therapy in future research.

7.Exploration and practice of constructing national first-class major of preventive medicine in local medical universities

In response to existing issues in preventive medicine education in China, Chongqing Medical University has implemented a series of reform measures in the major of preventive medicine since 2017. The construction of national first-class major of preventive medicine adopted the concept of "student-centered and market-oriented". This major scientifically formulated the talent cultivation goal of "cultivating high-quality, innovative, and application-oriented talents", constructed a curriculum system centered around professional competency training, emphasized a teaching mode that integrated "digital intelligence empowerment" and "combination of theory and practice". Moreover, we aimed to advance the ideological and political education reform of the courses, refine the diversified evaluation system, and bolster the development of teaching staff and the assurance of teaching quality. Following these reforms, the major has made significant achievements in talent cultivation effectiveness, curriculum system development, and teaching mode innovation. In the future, the major will persist in deepening the digital intelligence transformation, fostering interdisciplinary integration, enhancing international cooperation, and continuously advancing the construction and reform of preventive medicine as a national first-class major.
8.Construction of prediction models for hypertensive nephropathy based on machine learning

Objective To explore the construction of a prediction model for hypertensive nephropathy(HN)based on machine learning.Methods A total of 318 hypertensive patients who visited Taizhou First People's Hospital from April 2023 to March 2024 were included and divided into a training set and a validation set at a ratio of 7:3.Least absolute shrinkage and selection operator(LASSO)algorithm was used to select clinical features from the training set,and 12 clinically significant variables were obtained from 18 clinical variables.Based on the Python 3.10 programming language,the training set was used to train the model.Taking the 12 clinically significant indicators were used as input variables,and whether the occurrence of HN was used as the outcome variable.Three machine learning algorithms,namely logistic regression,support vector machine,and artificial neural network,were used to construct prediction models.The test set was used for internal validation of three models.The performance of the models was compared through accuracy,area under the receiver operating characteristic curve,recall rate,precision,and F1.Results Among 12 clinically significant variables screened by the LASSO algorithm,cystatin C and urine protein qualitative were found to be the most predictive.The accuracy,area under the receiver operating characteristic curve,recall rate,precision,and F1 values of the Logistic regression,support vector machine,and artificial neural network prediction models constructed by machine learning was 0.94,0.96,0.95,0.87,0.91;0.94,0.97,0.96,0.86,0.91;0.91,0.94,0.93,0.80,0.86,respectively.Conclusion Logistic regression,support vector machine,and artificial neural network based on machine learning all have good predictive effects on the progression of hypertensive patients to HN.Among them,the predictive effects of Logistic regression and support vector machine are similar and better than artificial neural network prediction model.
9.Determination of alcohol and phenolic antibacterial agents in eye drops by gas chromatography

Objective:A gas chromatographic method was established for the determination of alcohol and phenolic bacteriostatic agents(chlorobutanol,phenol,benzyl alcohol,phenethyl alcohol,phenoxyethanol,4-chloro-o-cresol and thymol)in eye drops for related quality control.Methods:A capillary column with(5%phenyl)methylsilox-ane as the fixing solution.The initial temperature was 60℃and keep for 10 min.The temperature was rised to 150℃at a rate of 20℃·min-1 and keep for 3 min,Next,the temperature was rised to 230℃at a rate of 50℃·min-1 and keep for 2 min.The detector temperature was 250℃,the inlet temperature was 200℃,and the detector was FID.Direct injection of 1 μL was used for injection.Results:The were a good linear relationship between peak area and concentration in the concentration range of 0.05-1.0 mg·mL-1(chlorobutanol),0.05-1.0 mg·mL-1(phenol),0.1-2.0 mg·mL-1(benzyl alcohol),0.05-1.0 mg·mL-1(phenethyl alco-hol),0.05-1.0 mg·mL-1(phenoxyethanol),0.03-0.6 mg·mL-1(4-chloro-o-cresol)and 0.01-0.20 mg·mL-1(thymol)(r values were 0.999 7,0.999 6,0.999 5,0.999 5,0.999 5,0.999 5,0.999 5 and 0.999 5 respectively).The average recoveries of seven component were 103.4%,95.2%,98.4%,97.2%,97.9%,100.5%and 93.5%respectively.All meet the requirements of methodological research.According to the method,79 batches of samples were tested for alcohol and phenolic bacteriostatic agents.Chlorobutanol was detected in 8 batches of samples,and no other bacteriostatic agents were detected.Conclusion:This method meets the require-ments for specificity and sensitivity,and is suitable for the confirmation and determination of alcohol and phenolic antibacterial agents in eye drops.It can be used for related quality control.
10.Exploring the Effects of San Ren Decoction on the Improvement of Symptoms and Macrophage Polarization of Hp-Associated Gastritis with Spleen and Stomach Damp Heat Syndrome Based on Tim-3/aletin-9 Signaling Pathway

Objective To explore the effect of San Ren decoction on macrophage polarization in Helicobacter pylori(Hp)-associated gastritis(HAG)with spleen and stomach damp heat syndrome through Tim-3/Galectin-9 signaling pathway.Methods Seventy-two Wistar rats were randomly divided into control group,model group,quadruple therapy group(omeprazole 2 mg/kg+amoxicillin 100 mg/kg+clarithromycin 50 mg/kg+colloidal pectin secret capsule 35 mg/kg),San Ren decoction low dose group(3 g/kg),San Ren decoction medium dose group(7.5 g/kg),San Ren decoction high dose group(15 g/kg),12 animals in each group.The rat model of HAG spleen and stomach damp heat syndrome was constructed by composite factors(fatigue+diet+bitter cold medicine+environment+biological factors(Hp bacterial solution)),and the drugs were continuously administered for 21 days according to the groups.The basic conditions of rats were observed in the experiment.Hp colonization was detected by rapid urease test.And gastric mucosal inflammation was observed using HE staining.A transmission electron microscope was used to observe the ultrastructure of the gastric mucosa.The levels of IFN-γ,TNF-α,IL-4,IL-10 in serum were measured by ELISA.Gastric mucosa expressed CD68 by immunohistochemistry.Subtypes of macrophages F4/80+CD86+and F4/80+CD206+were detected by flow cytometry.RT-qPCR was performed to detect the mRNA expression of MCP-1,CD80,CD86,IL-4,CD206,and CD163 in gastric mucosa.Western blot was used to detect the expression of Tim-3 and Galectin-9 proteins in gastric mucosal tissues.Results The histopathology of gastric mucosa in the model group was significantly damaged compared with that of the control group,and the rate of Hp colonization,serum IFN-γ and TNF-α,protein expression of CD68,Tim-3 and Galectin-9 in gastric mucosa tissues,and mRNA expression of MCP-1,CD80,CD86,and CD163 were significantly higher compared with that of the control group(P<0.01).The high-dose intervention of San Ren decoction significantly improved the gastric mucosal tissue injury in model rats,significantly reduced the rate of Hp colonization,and inhibited serum IFN-γ and TNF-α,gastric mucosal tissue CD68,Tim-3,and Galectin-9 protein expression,gastric mucosal tissue MCP-1,CD80,and CD86,CD163 mRNA expression(P<0.05).Conclusion San Ren decoction can improve the symptoms,inhibit the inflammatory response and mediate the macrophages polarization of gastric mucosal in rats with HAG spleen and stomach damp heat syndrome,and its mechanism of action might be related to the regulation of Tim-3/Galectin-9 pathway.
