1.The Relationship between Ig Class Switch Recombination and MMR Protein,Microsatellite Phenotype in Extranodal Marginal Zone Lymphoma of Mucosa-associated Lymphoid Tissue

Objective:To investigate the relationship between Ig class switch recombination(CSR)and mismatch repair(MMR)protein,microsatellite phenotype in extranodal marginal zone lymphoma of mucosa-associated lymphoid tissue(MALT lymphoma).Methods:Forty cases of MALT lymphoma archived in the Department of Pathology,Jiading District Central Hospital,Shanghai University of Medicine & Health Sciences were selected as the observation group,and twenty cases of benign lymphoid tissue hyperplasia were as the control group.The expressions of IgG,IgM,IgD,and IgA in both groups were detected by immunohistochemical double staining,and MMR proteins including MLH1,MSH2,MSH6,and PMS2 in both groups were detected by immunohistochemistry.Multiplex fluorescence PCR capillary electrophoresis was used to detect microsatellite phenotype in tumor and adjacent tissues of the experimental group.Results:In the observation group,the proportions of single Ig heavy chain expression(mode Ⅰ),negative expression(mode Ⅱ),and multiple expression(mode Ⅲ)were 65%(26/40),27.5%(11/40),and 7.5%(3/40),respectively,while in the control group were 0(0/20),5%(1/20),and 95%(19/20).The proportion of Ig heavy chain expression mode Ⅰ+Ⅱ in the observation group was 92.5%,which was significantly higher than 5%in the control group(P<0.0 1).In the observation group,partial deletion of MMR protein was observed in 3 cases(7.5%),including 2 cases of MSH6 deletion and 1 case of both MSH6 and PMS2 deletion.In the control group,there was 1 case(5%)with PMS2 deletion.There was no significant difference in the deletion rate of MMR protein between the two groups(P>0.05).A total of 5 cases of microsatellite instability(MSI)were detected in the observation group,including 1 case of low-frequency MSI(MSI-L),4 cases of high-frequency MSI(MSI-H),and 2 cases of MSI-H with MSH6 deletion.When the loss expression of MSI-H or MMR protein was counted as a positive result,the MSI-H rate detected by PCR capillary electrophoresis was 10%(4/40),which was slightly higher than the MMR protein deletion rate detected by immunohistochemistry(7.5%,3/40),but there was no statistically significant difference between the two groups(P>0.05).The MMR protein deletion rates among the Ig heavy chain protein expression mode Ⅰ,mode Ⅱ,and mode Ⅲ groups were 0(0/26),18.2%(2/11),and 33.3%(1/3),respectively.There was a statistically significant difference in the constituent ratios among the three groups(P<0.05).The MMR protein deletion rates among the MSS,MSI-L,and MSI-H groups were 2.9%(1/35),0(0/1),and 50%(2/4),respectively.There was a statistically significant difference in the constituent ratios among the three groups(P<0.05).MMR protein deficiency was positively correlated with Ig heavy chain expression pattern and MSI(r=0.41,P<0.05;r=0.48,P<0.05),but Ig heavy chain expression pattern was not correlated with MSI(r=0.02,P>0.05).Conclusion:Ig heavy chain CSR detection is helpful for the differential diagnosis of MALT lymphoma.Low frequency MMR protein deletion and MSI-H phenotype exist in MALT lymphoma,which may be of certain value for the study of its occurrence,development and clinical treatment.
2.Salidroside alleviates progression of Parkinson's disease by modulating inflammatory responses

Aim To explore the neuroprotective effects of salidroside on Parkinson's disease(PD)through modulation of inflammatory responses and the underly-ing mechanisms.Methods Mice were divided into five groups:healthy control group,1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine(MPTP)disease group,low-dose Rhodioloside intervention group,medium-dose salidroside intervention group,and high-dose salidro-side intervention group.MPTP-induced PD mouse model was established,and salidroside intervention was administered.Behavioral changes,inflammatory cyto-kine levels,autophagy-related protein expression,and neurons were observed through histological analysis and immunohistochemical staining.Results After MPTP treatment,mice exhibited significant behavioral chan-ges,increased pro-inflammatory cytokines,decreased anti-inflammatory cytokines,reduced autophagy-related proteins,and evident pyroptosis.Salidroside interven-tion alleviated these changes in a dose-dependent man-ner.Conclusions Salidroside exerts neuroprotective effects on PD by alleviating inflammatory responses and promoting autophagy,thereby protecting neurons.
3.Comparison of the diagnostic value of serum IL-6,IL-8,TREM1,uPAR and presepsin in patients with septic shock

Objective To observe the dynamic changes in serum levels of IL-6,IL-8,TREM-1,uPAR,and presepsin in patients with septic shock and to analyze the diagnostic significance of these biomarkers.Methods A total of 150 sepsis patients admitted to the hospital between February 2023 and February 2025 were prospectively enrolled as study subjects.According to their clinical conditions,the participants were categorized into a sepsis group(non-shock,n=44)and a septic shock group(n=106).Additionally,30 healthy individuals with normal clinical indicators during the same period were selected as the control group.The serum levels of IL-6,IL-8,TREM-1,uPAR,and presepsin were measured and compared across the different groups.Spearman correlation analysis and logistic regression analysis were conducted to assess the association between these biomarkers and the severity of sepsis.The diagnostic performance of these biomarkers for septic shock was evaluated using receiver operating characteristic(ROC)curve analysis.Results There were statistically significant differences in serum levels of IL-6,IL-8,TREM1,uPAR,and presepsin among the different groups(P<0.05),with the highest values observed in the septic shock group,followed by the sepsis group and the control group(P<0.05).The APACHE Ⅱ and SOFA scores of patients with septic shock were significantly higher than those of patients with sepsis(P<0.05).Spearman correlation analysis revealed that serum concentrations of IL-6,IL-8,TREM1,uPAR,and presepsin were positively correlated with both APACHE Ⅱ and SOFA scores,with correlation coeffi-cients(rs)of 0.758,0.880,0.837,0.832,and 0.846 for APACHE Ⅱ score,and 0.487,0.549,0.557,0.626,and 0.664 for SOFA score,respectively(P<0.05).Logistic regression analysis indicated that serum levels of IL-6(OR=1.055),IL-8(OR=1.054),TREM1(OR=1.038),uPAR(OR=1.010),and presepsin(OR=2.103)were significantly associated with the development of septic shock(P<0.05).ROC curve analysis demonstrated that the AUC values for serum IL-6,IL-8,TREM1,uPAR,and presepsin in diagnosing septic shock were 0.608,0.724,0.887,0.848,and 0.885,with corresponding sensitivities of 0.432,0.909,0.795,0.909,and 0.591,and specificities of 0.880,0.481,0.915,0.736,and 0.977,respectively.When these biomarkers were combined,the AUC increased to 0.973,with a sensitivity of 0.943 and a specificity of 0.953.Furthermore,the AUC for prese-psin alone was significantly higher than that for IL-6 and IL-8 alone(P<0.05).Additionally,the AUC for the combined biomarkers was significantly greater than that for each biomarker individually(P<0.05).Conclusions Serum levels of IL-6,IL-8,TREM1,uPAR,and presepsin are significantly elevated in patients with septic shock,which may aid in the clinical diagnosis of the condition.The combined use of these five biomarkers en-hances the accuracy of diagnosing septic shock.
4.The application of user profiles in the health management of elderly patients with chronic diseases:a scoping review

Objective To review the application of user profiles in the health management of elderly patients with chronic diseases,and to provide references for nursing staff to apply this technology to manage chronic diseases in the elderly.Methods According to the scoping review methodology,a systematic search was conducted across 9 databases including Wanfang Database,CNKI,VIP,Chinese Biomedical Abstracted Database,PubMed,Embase,Web of Science,CINAHL and Cochrane Library.The search time was from the establishment of the database to January 2024.The literature was screened according to inclusion and exclusion criteria,and the information in the literature was independently extracted and summarized by 2 researchers.Results A total of 10 pieces of the literature were included in the study.The results showed that the process of constructing user profiles mainly includes data collection,user feature extraction,and profiles generation and visualization.User profiles are widely used in the health management of chronic diseases in the elderly,commonly used in the management of diabetes,heart failure and other chronic diseases in the elderly,including the formulation of personalised intervention plans,the provision of personalised health education and information,and the adjustment of health management strategies.User profiles significantly enhanced the accuracy of health information delivery and health services,improved user experience,enhanced patients'self-management ability and quality of life.Conclusion As an emerging health information integration tool,user profiles can merge and analyze multidimensional health data to provide personalized and precise health management for elderly patients with chronic diseases.This approach enhances remote monitoring,prevents and manages complications,improves treatment outcomes,and reduces medical costs,thereby demonstrating significant potential in the health management of elderly patients with chronic conditions.
5.Study on the preference for"Internet+TCM Nursing Services"among colorectal cancer patients

Objective To explore the preferences and needs of colorectal cancer patients for"Internet+TCM nursing services",aiming to provide a basis for the improvement of service quality and the formulation of relevant policies,so as to better meet the needs of patients.Methods A total of 189 patients with colorectal cancer who were admitted to a tertiary A Chinese Medicine hospital in Qingdao from September 2023 to July 2024 were selected as the research subjects.A general data questionnaire and Questionnaire on"Internet+Traditional Chinese Medicine Nursing Service"Preference of Colorectal Cancer Patients were used with 6 attributes,including average out-of-pocket cost,nurse qualification,service waiting time,service mode,comprehensiveness of service content,and platform service evaluation,each attribute with 2 to 3 levels.The preference,payment intention of"Internet+TCM nursing services"were analyzed by constructing a mixed Logit model,and the influencing factors of choice preference of different colorectal cancer patients was explored through subgroup analysis.Results Totally 189 questionnaires were sent out,and 156 valid questionnaires were collected,with an effective recovery rate of 82.54%.Among the respondents,112 patients(71.79%)actively accepted"Internet+TCM nursing services",and the attributes of"Internet+TCM nursing services"such as payment fee,service evaluation,service content and nurse qualification had an impact on nursing service preference of colorectal cancer patients(P<0.05).Based on willingness to pay,the ranking of attributes was service evaluation,service content and nurse qualification.Factors such as education level and the presence of ostomy also affected the preference of"Internet+TCM nursing services"in patients with colorectal cancer(P<0.05).Conclusion Patients with colorectal cancer prefer"Internet+TCM nursing services",which has an average service cost of 50 yuan per time,excellent platform service evaluation,comprehensive service content,specialized TCM nurses.The preferences and specific characteristics of patients with colorectal cancer can be considered to optimize the allocation of nursing resources and provide services in line with their preferences.

Objective To analyze the research status and hotspots in the field of astragalus polysaccharides;To provide references for further research.Methods Research literature about astragalus polysaccharides was retrieved from CNKI,Wanfang Data,VIP,PubMed,and Web of Science databases from 1st,Jan.2013 to 1st,July 2023.NoteExpress 3.7 software was used to manage the literature and ultimately establish a database.Excel 2019,CiteSpace 6.2.2R and VOSviewer 1.6.18 were used to visually analyze the publication volume,authors,institutions,and keywords of the included literature.Results A total of 2 462 articles were included,with 1 284 Chinese articles and 1 178 English articles.The main research institutions were Gansu University of Chinese Medicine,Shandong University of Traditional Chinese Medicine,and Beijing University of Chinese Medicine.The core authors of Chinese literature were Liu Yongqi,Wang Hongxin,Lu Meili,etc.The core authors of English literature included Zhang Wei,Li Ke,Yang Xiaojun,etc.High-frequency keywords of Chinese literature included Astragali Radix,rats,polysaccharides,cell apoptosis,and oxidative stress,etc.High frequency keywords in English literature included expression,in vitro,oxidative stress,apoptosis,etc.Conclusion The research on astragalus polysaccharides focuses on their pharmacological effects and mechanisms.Intestinal flora,immune regulation,autophagy and apoptosis are the hot action mechanisms in this field.The focus of disease research involves tumor and diabetes,and antiviral,anti infection and other pharmacological effects are the research trend.
9.Effect of vorinostat on P-gp expression and pharmacokinetic parameters of its substrate phenytoin sodium in rats under hypoxic environments

Aim To investigate the effects of SAHA on the expression of P-gp and the pharmacokinetic pa-rameters of its substrate phenytoin sodium in rats under hypoxic environments.Methods Wistar rats were randomly divided into the normioxic group,the hypoxic model group,and the low-,medium-and high-dose vorinostat(SAHA)groups.Liver tissues were col-lected,and the expression levels of P-gp and HDAC5 were detected by Real-time PCR and Western blot.The morphological changes of liver tissues were ob-served by HE staining.Following intragastric adminis-tration of 50 mg·kg-1 phenytoin sodium to each group,blood samples were collected,and the plasma concentration of phenytoin sodium was determined u-sing UFLC-MS/MS to calculate pharmacokinetic pa-rameters.Results Compared with the normoxic group,the expression of HDAC5 in the liver tissues of hypoxia model rats increased,while the expression of P-gp decreased.After SAHA treatment,HDAC5 expression decreased,and P-gp expression increased.Among the SAHA groups,the medium-dose group showed the most significant effect,and HE staining re-sults indicated that this concentration did not cause damage to rat liver tissues.Compared with the normox-ic group,the AUC,Cmax,and T1/2 of phenytoin sodium in hypoxia model rats were significantly raised.After administration of the medium dose of SAHA,the AUC,Cmax,MRT,and T1/2 were significantly reduced,while CLZ/r was significantly increased.Conclusions Un-der hypoxic environments,the expression of P-gp in rat liver tissue is significantly downregulated,leading to increased systemic exposure of phenytoin,reduced clearance,and consequently elevated blood concentra-tions,raising the risk of central nervous system toxici-ty.In contrast,SAHA suppresses HDAC5 expression,thereby activating P-gp transcription and enhancing its efflux function.This results in decreased systemic ex-posure and improved clearance of phenytoin,signifi-cantly reducing drug accumulation in body and ulti-mately lowering the risk of adverse effects.
