1.Effect of electroacupuncture on the expression of TRPV4 in the dorsal root ganglion of diabetic neuropathic pain model rats

Objective To explore the function of electroacupuncture(EA)on body mass,fasting blood glucose,heat pain threshold,and transient receptor potential vanilloid 4(TRPV4)in the dorsal root ganglia(DRG)of rats with diabetic neuropathic pain(DNP).Methods A DNP rat model was formed by intraperitoneally injecting the animals with STZ.From days 15 to 21,bilateral Zusanli and Kunlun points of the DNP rat model were treated with electroacupuncture once daily for 30 min.We then measured their body mass,fasting blood glucose,and heat pain threshold.The co-expression of TRPV4 and NeuN in the rat L4~L6 DRG was detected by immunofluorescence.The effects of the TRPV4 agonist GSK1016790A on body mass,fasting blood glucose,and the heat pain threshold of DNP rats treated with electroacupuncture were detected.Results After the 7th day,body mass was significantly decreased(P<0.01)and fasting glucose was significantly increased(P<0.01)in the model group compared with the normal group.After the 21st day,compared with the model group,heat pain threshold of the model+electroacupuncture group was significantly higher(P<0.01);the results of co-expression of TRPV4 and NeuN immunofluorescence on rat L4~L6 DRG showed that:the expression of positive cells in the model group was significantly higher(P<0.01)than that in the normal group,the co-expression of TRPV4 and NeuN positive cells in L4~L6 DRG of rats in the model+electroacupuncture group was significantly lower(P<0.01)than that in the model group.The TRPV4 agonist GSK1016790A can reverse the downregulation of thermal pain threshold induced by electroacupuncture in DNP rats(P<0.01).Conclusion Electroacupuncture alleviated the DNP induced by STZ,and its mechanism may involve the inhibition of TRPV4 protein expression in the DRG.

Mid-to-late stage ankle arthritis is a chronic degenerative disease that is extremely common in clinical practice.It is characterized by significant cartilage degeneration and subchondral bone sclerosis,accompanied by the formation of osteophytes around the joint,often leading to joint deformity.This condition causes severe pain in the patients during walking,severely restricts their activities,and affects their qualities of life.In recent years,with the continuous improvement of medical standards,the treatment methods for mid-to-late stage ankle arthritis have shown a diversified development trend.Non-surgical treatments primarily include activity restriction,orthotic devices,oral non-steroidal anti-inflammatory drugs(NSAIDs),and intra-articular injections of the talocrural joint.The surgical treatments primarily include joint distraction arthroplasty,periacetabular osteotomy,total ankle arthroplasty,and ankle arthrodesis.Tissue engineering therapy,as an emerging method,has also received considerable attention.This article systematically reviewed the selection principles and research progress of various treatment options for mid-to-late stage ankle arthritis,including traditional treatments,non-surgical treatments,surgical treatments,and tissue engineering treatments.By deeply analyzing the basic principles and advantages and disadvantages of each treatment method,and combining the latest research findings on clinical outcomes,a scientific and comprehensive clinical decision-making reference system was constructed to provide clearer and more comprehensive treatment choices for both doctors and patients,thereby effectively improving treatment outcomes and enhancing the quality of life for the patients.
3.Correlation between biopsy rate and positive rate of biopsy in assessment of diagnostic competence in endoscopists

Objective To explore whether the biopsy rate and the positive rate of biopsy during colonoscopy can serve as effective indicators for evaluating the diagnostic proficiency of endoscopists.Methods Total fifteen endoscopists from the First Affiliated Hospital of Army Military Medical University,Guiqian International General Hospital and No.958 Hospital of PLA Army were enrolled and served as the study subjects.According to their years of experience,gender and working hours,they were divided into a senior group(n=8)and a junior group(n=7),a male group(n=9)and a female group(n=6),and a morning period(08:00-12:00)and an afternoon period(13:00-18:00).The data of all patients examined by the aforementioned endoscopists between January 2023 and December 2023 were collected,including the basic demographics,colonoscopic results,biopsy outcomes and pathological diagnoses of the patients.ANOVA was used to analyze the differences in the biopsy rate and positive rate of biopsy existed among the endoscopists of different years of experience,gender and working at different periods(morning and afternoon).Additionally,multivariate logistic regression analysis was employed to identify the independent risk factors associated with positive rates of biopsy.Results The senior group had significantly lower biopsy rate[(5.73±0.81)%vs(6.89±0.97)%],but obviously higher positive rate of biopsy[(53.52±3.87)%vs(33.70±7.85)%]than the junior group(both P<0.01).The biopsy rate was notably higher during the morning working time period than the afternoon working time period in all the endoscopits[(6.76±1.11)%vs(5.53±1.57)%,P<0.05].Logistic regression analysis showed that the years of experience[(53.52±3.87)%vs(33.70±7.85)%,OR=92.187,95%CI:3.118~2 725.366,P<0.01)and working time[(48.35±10.50)%vs(37.80±10.65)%,OR=20.885,95%CI:1.266~344.602,P<0.05)were influencing factors for positive rate of biopsy.Conclusion The biopsy rate is low and positive rate of biopsy is high among the senior experienced endoscopists,while,opposite results are seen in the junior experienced clinicians,indicating correlation of biopsy rate and positive rate of biopsy with diagnostic proficiency of endoscopists.

Speech prosody is a supra segmental feature of voice,which plays an important role in daily com-munication.Cochlear implantation(CI)can help the hearing-impaired children to normalize their speech,but CI children still have varying degrees of speech prosody obstacles.This paper summarizes the perception,production characteristics and intervention strategies of CI children's speech prosody at home and abroad,in order to attract the attention of education,medical,rehabilitation and academic fields to CI children's speech prosody research,and to provide reference for future intervention of CI children's speech prosody.
5.Legal challenges and responses in the application of deep brain stimulation technology in mental disorders

Deep brain stimulation (DBS) has been widely applied in treating neurological disorders such as Parkinson’s disease, with corresponding evidence of safety and efficacy. However, when DBS is experimentally applied in the field of mental disorders, the uncertainty of its therapeutic mechanisms and targets becomes prominent, and there are peculiarities such as the lack of substantive informed consent, partial deprivation of autonomy, and the impact on the homogeneity of personality for research participants or patients. In response to the above situations, the current normative documents are still insufficient, the ethical review re-examination procedure has not yet formed a perfect system, the substantive informed consent of research participants or patients has not yet been clearly guaranteed, as well as the technical scope and regulatory boundaries are not clear. For the new application of DBS in the field of mental disorders, the principle of prudent supervision should be upheld, and appropriate regulation should be carried out to prevent the dilemma of “Collingridge”. Specifically, the ethical review system should be innovated and refined, whole-process informed consent of research participants or patients and guardian assistance in decision-making should be ensured, the legal nature of “experimental treatment” should be clarified, as well as the relevant provisions in the Mental Health Law of the People’s Republic of China should be improved, with a view to providing legal theoretical support and practical operational guidelines for the legalization and standardization of DBS’s application in mental disorder research.
6.Free inferior gluteal perforator flap for immediate breast reconstruction: a case report and literature review

Objective:To explore the possibility of using a inferior gluteal artery perforator flap (IGAPF) for breast reconstruction in the patient who did not have suitable donor site in back and abdomen.Methods:In November 2024, a 25-year-old unmarried and childless woman with right breast cancer received immediate right breast reconstruction by a right free IGAPF after modified right mastectomy in the Department of Breast and Thyroid Surgery, Second Affiliated Hospital of Hainan Medical University. The locations of perforators were confirmed by both Multi-detector computed tomography angiography (MDCTA) and portable Doppler blood flow detector before surgery. The IGAPF was designed to take the inferior gluteal wrinkle as the lower edge, the axis of the flap was parallel to the inferior gluteal wrinkle, and the width of the flap was estimated where the incision could be directly closed. The size of right IGAPF was 6.0 cm×19.0 cm. Sharp dissection was performed between the sarcolemma and muscle fibres of gluteus, then the perforators were dissected along the direction of muscle fibres of gluteus. The vascular pedicle was kept at about 8.0 cm in length. The diameter of artery was about 2.0 mm and that for the veins was about 1.5 mm. End-to-end anastomoses with the right thoracodorsal artery and vein were successfully carried out. The donor site was directly closed, and it was hidden in the inferior gluteal wrinkle. Postoperative outpatient clinical review was made.Results:Pathological examination reported: an invasive carcinoma of right breast, axillary lymph node metastasis (2/10). The patient recovered well and the flap survived without any complication, i.e. ischemic necrosis, infection and haematoma. The patient was off-bed at 3 days and discharged at 13 days after surgery. At the 40 days of postoperative follow-up, the patient achieved a good recovery and the lower limb activity was not affected by the surgery. The patient was satisfied with the reconstructed breast and donor site recovery. The patient followed with scheduled chemotherapy and subsequent radiotherapy. The volume of reconstructed breast was smaller than the other breast, of which the patient was fully informed before the surgery.Conclusion:A free IGAPF provides an alternative donor sites for achieving a breast reconstruction due to the reliable pedicle vessels and invisible donor scars.

8.Advances in the regulation of orexigenic/anorexigenic neuropeptides and signaling pathways by metabolic bariatric surgery

Metabolic bariatric surgery is an effective intervention for obesity and related metabolic disorders.Beyond substantial weight reduction,it modulates orexigenic and anorexigenic neuropeptides and their signaling pathways to improve energy homeostasis and metabolic function.This review summarizes the regulatory effects of various bariatric procedures(such as Roux-en-Y gastric bypass and sleeve gastrectomy)on orexigenic neuropeptides and anorexigenic factors,highlighting the involvement of key signaling pathways including mTOR and AMPK.These mechanisms contribute to weight control,enhanced insulin sensitivity,and remission of metabolic diseases.By integrating central and peripheral regulatory processes,metabolic surgery provides new biological insights into individualized obesity management.Future research should focus on interindividual variability and long-term neuro-metabolic adaptations to optimize surgical outcomes and improve metabolic health.
