1.Effect of 3D rotational digital subtraction angiography-guided drug balloon predilation bare-metal stent in patients with vertebral artery origin stenosis

Objective To investigate the effect of 3D rotational digital subtraction angiography(DSA)-guided drug balloon predilation bare-metal stents on vertebral artery origin stenosis(VAOS).Methods A total of 63 VAOS patients who were admitted to Hai'an People's Hospital from May 2020 to December 2022 were selected and divided into two groups according to different treatment methods.There were 16 males and 14 females with a mean age of(52.10±3.62)years in the observation group.There were 19 males and 14 females with a mean age of(52.47±3.85)years in the control group.There was no statistical difference in the basic data between the two groups(P>0.05).The observation group was treated with 3D rotational DSA-guided drug balloon predilation bare-metal stent,while the control group was treated with conventional drug balloon predilation bare-metal stent.The efficacy of the two groups was compared.Results The success rate of operation was 100%(30/30)in the observation group,which was higher than that in the control group(96.97%[32/33],χ2=0.924,P=0.336).There was no significant difference in the stenosis rate between the two groups either before surgery or 3 months after surgery(P>0.05).The hemodynamic level of the initial segment of vertebral artery in the observation group was lower than that in the control group 3 months after surgery(P<0.05).The scores of neurological function and quality of life at 3 months after surgery and the incidence of recurrent stroke at 6 months after surgery in the observation group were lower than those in the control group(P<0.05).Restenosis occurred in 2 cases(6.67%)in the observation group and 9 cases(27.27%)in the control group,and there was significant difference in the incidence of restenosis between the two groups(χ2=4.630,P=0.031).Conclusion 3D rotational DSA-guided drug balloon predilation bare-metal stent can effectively reduce the stenosis rate and the incidence of restenosis,and improve the nerve function and the quality of life of VAOS patients.
2.Relationship of TyG index and atherogenic index of plasma with coronary artery remodeling in elderly patients with CHD and T2DM

Objective To investigate the relationship of triglyceride-glucose(TyG)index and atherogenic index of plasma(AIP)with positive remodeling of coronary arteries in elderly coro-nary heart disease(CHD)patients with concomitant type 2 diabetes mellitus(T2DM).Methods A total of 120 elderly CHD patients with concomitant T2DM admitted to our department from January 2022 to June 2023 were continuously recruited,and according to the remodeling index,they were divided into positive remodeling group(47 cases)and non-positive remodeling group(73 cases).The clinical data were compared between the two groups.Multivariate logistic analysis was used to identify the risk factors for positive remodeling of coronary artery.Spearman correla-tion analysis was employed to analyze the correlation of TyG and AIP with positive remodeling.ROC curve was plotted to analyze the predictive value of TyG and AIP for the remodeling.Results The positive remodeling group had larger ratio of smoking,higher TG and HbA1c levels,and in-creased TyG index and AIP,but lower HDL-C and blood calcium levels than the non-positive re-modeling group(P<0.05,P<0.01).Univariate logistic regression analysis showed that smoking,TG,HDL-C,HbA1c,blood calcium,TyG and AIP were risk factors for positive coronary artery remodeling in elderly patients with CHD and T2DM(P<0.05,P<0.01).Multivariate logistic re-gression analysis showed that TyG index(OR=7.253,95%CI:2.458-13.364,P=0.035)and AIP(OR=6.017,95%CI:2.205-12.025,P=0.030)were independent risk factors for positive coronary artery remodeling in elderly patients with CHD and T2DM(P<0.05).The AUC value of TyG index and AIP in predicting positive coronary artery remodeling in elderly CHD patients with T2DM was 0.783 and 0.766,respectively,and the value of their combination in the prediction was 0.868,which was significantly better than the indicator alone(P<0.05).Conclusion TyG in-dex and AIP are closely associated with positive coronary artery remodeling in elderly CHD pa-tients with T2DM,and can be used as effective indicators in the prediction of positive remodeling,which is of great significance for early clinical identification of high-risk patients and formulation of individual intervention strategies.

4.Analysis of temporal trends of the incidence rate of tuberculosis in Shaanxi Province

Objective:To analyze the temporal trends of the incidence rate of tuberculosis (TB) in Shaanxi Province and provide a reference for WHO to control the prevalence of TB effectively.Methods:Joinpoint regression was used to analyze the trend of the incidence rate of TB in Shaanxi Province from 2004 to 2022, and the seasonal autoregressive moving average model was used to forecast the incidence rate of TB in Shaanxi Province to 2030.Results:The incidence rate of TB in Shaanxi Province decreased from 90.896/100 000 in 2004 to 35.364/100 000 in 2022, showing a general downward trend (AAPC=-7.72%, P<0.001). From 2014 to 2019, the reduction trend slowed down (APC=-0.69%, P=0.814), of which the largest decline occurred from 2019 to 2022 (APC=-13.26%, P=0.010). The predicted incidence rate of TB in Shaanxi Province from 2020 to 2022 was higher than the reported incidence rate, with the expected incidence rate of 51.342/100 000 in 2022 and 43.468/100 000 in 2030. Conclusion:The incidence rate of TB in Shaanxi Province shows a downward trend from 2004 to 2022, but the decline has shrunk in recent years. It is predicted that the downward trend will continue to slow down by 2030.

ObjectiveTo observe the efficacy and safety of Uyghur medicine Yangxin Dawayimixike Honey Paste (养心达瓦依米西克蜜膏, YDMHP) in the treatment of stable angina pectoris (SAP) of qi stagnation and blood stasis syndrome. MethodsA randomized , double-blind, positive-controlled,multi-center clinical trial was conducted, in which 370 patients with SAP of qi stagnation and blood stasis syndrome were randomly divided into treatment group(279 cases)and control group(91cases)at a ratio of 3∶1. The treatment group was orally administered with YDMHP, 3 g each time, and placebo of Xuefu Zhuyu Capsule (血府逐瘀胶囊), 2.4 g each time, while the control group was treated with Xuefu Zhuyu Capsule, 2.4 g each time, and placebo of YDMHP, 3 g each time, both twice a day for a course of 12 weeks. The primary outcome was the effect of angina pectoris symptom. The secondary outcomes include single angina symptom scores such as number of attacks, duration of attacks, pain intensity and usae of nitroglycerin scores, the total angina symptom score before and after the treatment, the usage of nitroglycerin, the exercise duration in treadmill exercise test (TET) and the Duck treadmill score among patients,the scores of Seattle Angina Questionnaire (SAQ) on five dimensions including physical limitations, anginal stability, anginal frequency, treatment satisfaction, and disease perception, and efficacy of TCM syndrome and of each single TCM symptom after treatment. The safety were evaluated by examine blood routine, urine routine, liver and kidney function, fasting blood sugar, electrocardiogram, adverse events. ResultsThe total effective rate of angina symptom in the treatment group was 71.69% (200/279), significantly higher than 51.64% (47/91) in the control group (P<0.01). The curative and markedly effective rate of TCM syndrome in the treatment group was 53.05% (148/279), which was significantly higher than 25.27% (23/91) in the control group (P<0.01). After treatment, scores of the number as well as duration of angina attacks and pain severity, the total score of angina symptoms, and the usage of nitroglycerin significantly decreased in both groups, and more changes were seen in the treatment group than in the control group; the scores of physical limitations, anginal stability, anginal frequency, treatment satisfaction, and disease perception in both groups significantly increased, and more improvement were shown in the experimental group regarding the anginal stability, anginal frequency and treatment satisfaction (P<0.05 or P<0.01). The effects of chest pain, chest tightness, palpitation, shortness of breath and fatigue in experimental group were significantly higher than those in control group (P<0.05 or P<0.01). There was no significant difference in the exercise duration of treadmill test and Duke score among patients between the two groups either before or after treatment (P>0.05). Adverse events occurred in 66 cases (23.66%) of the experimental group and 16 cases (17.58%) of the control group, with no statistical significance between the two groups (P>0.05). ConclusionThe Uyghur medicine YDMHP can effectively improve symptoms of angina pectoris, reduce the number, duration, and intensity of attacks, decrease the dosage of nitrogly-cerin and improve the individual TCM symptoms and has good safety in the treatment of SAP patients of qi stagnation and blood stasis.

Genotype 1b patients were centered between 30-59 years old, with a higher detection rate in females than in males, while genotypes 3a and 3b were mainly between 30-49 years old, with a higher detection rate in males than in females.The patients in Pishan County, Moyu County, and Hotan County in Hotan region were mainly of genotype 1b, while the proportion of genotype 3 was relatively high in Hotan City, Luopu County, and Yutian County.Conclusions:The distribution of HCV genotypes in the Hotan region is diverse, with genotype 1b being the main one, and genotype 3 accounting for a high proportion. Therefore, it is recommended to actively conduct HCV genotype testing in key regions and populations to formulate optimized antiviral treatment plans and improve treatment efficacy.

9.Computed tomography findings, clinicopathological features, genetic characteristics and prognosis of and minimally invasive lung adenocarcinomas.

OBJECTIVE:
To investigate the computed tomography findings, clinicopathological features, genetic characteristics and prognosis of in situ adenocarcinoma (AIS) and minimally invasive adenocarcinoma (MIA) of the lung.
METHODS:
We retrospectively analyzed the data including computed tomography (CT) images, histopathological findings, Ki-67 immunostaining, and genetic mutations in patients with lung adenocarcinoma undergoing surgery at our hospital between 2014 and 2019.
RESULTS:
Of the total of 480 patients with lung adenocarcinoma we reviewed, 73 (15.2%) had AIS (=28) or MIA (=45) tumors. The age of the patients with MIA was significantly younger than that of patients with AIS ( < 0.02). CT scans identified pure ground-glass nodules in 46.4% of AIS cases and in 44.4% of MIA cases. Multiple GGOs were more common in MIA than in AIS cases ( < 0.05), and bluured tumor margins was less frequent in AIS cases ( < 0.05). No significant difference was found in EGFR mutations between MIA and AIS cases. A Ki-67 labeling index (LI) value ≥2.8% did not differentiate MIA from AIS. The follow-up time in MIA group was significantly shorter than that in AIS group, but no recurrence or death occurred.
CONCLUSIONS
Despite similar surgical outcomes and favorable survival outcomes, the patients with AIS and MIA show differences in terms of age, CT findings, EGFR mutations and Ki-67 LI.

10.Coronary artery lesions in different types of heart failure with ischemic heart disease

Objective To compare the degree of coronary lesions in different types of heart failure with ischemic heart disease. Methods This retrospective analysis include 282 cases diagnosed as heart failure with ischemic heart disease in Zhengzhou People′s Hospital from January 2015 to May 2016. Based on LVEF and the level of BNP and NT-proBNP examination results ,282 cases were divided into HFmrEF(89 cases),HFrEF(109 cases) and HFpEF (84 cases). The clinical basic data ,cardiac color Doppler ultrasound and the difference of coronary artery lesions were compared. Results (1) HFpEF,HFmrEF and HFrEF groups gensini score were 30.45 ± 33.18,62.12 ± 41.25,86.72 ± 38.80. The differences are significant(P < 0.05). Pairwise comparison showed significant difference.(2)HFpEF,HFmrEF and HFrEF groups SYNTAX score were 11.77 ± 8.39,19.08 ± 8.39,26.41 ± 10.31. Compared with HFpEF group,the SYNTAX score of other groups are higher(P < 0.05). (3)The duration of disease ,LVEDD ,LVESD ,LVMI ,the number of vascular lesions was significantly higher in HFrEF group than those in other groups. (4) Multivariate correlation analysis suggested that SYNTAX scores were related to cardiac function classification,BNP,NT-proBNP and LVEF. Gensini integral is related to cardiac function classification ,BNP and NT-proBNP. Conclusions With the severity and complexity of coronary artery disease,the degree of heart failure is more serious. The complexity and severity of coronary artery disease is an important factor in heart failure in patients with ischemic heart disease.
