1.Application of domestic high-flow percutaneous left ventricular assist device in patients with low cardiac output syndrome after cardiac surgery: a preclinical trial report

Objective:To report the preclinical trial results of the application of a domestic high-flow percutaneous left ventricular assist device (pLVAD) in patients with low cardiac output syndrome (LCOS) following cardiac surgery.Methods:Six patients who developed LCOS after direct cardiac surgery were implanted with a domestic high-flow pLVAD. Clinical outcomes, including hemodynamic changes, complications, and survival rates were observed post-implantation.Results:Four patients underwent pLVAD implantation under digital subtraction angiography (DSA) guidance, while two patients had the procedure performed under ultrasound guidance. The implantation process was straightforward, rapid, and uneventful, with no instances of bleeding or arrhythmias. The flow rate at the initiation of pLVAD support was 3.8-5.0 (4.22±0.44)L/min, and the flow rate during pump removal was 1.0-1.3(1.18±0.15)L/min. The duration of pLVAD support was 16.5-165.0(101.3±60.65)h. Hemodynamic parameters showed immediate improvement following pLVAD support: mean arterial pressure increased from (62.67±4.46)mmHg to (80.50±18.96)mmHg (1 mmHg=0.133 kPa, P=0.049), cardiac output increased from (2.45±0.66)L/min to (4.35±1.32)L/min( P=0.01), cardiac index improved from (1.95±0.21)L·min -1·m -2 to (2.77±0.33)L·min -1·m -2( P<0.001), pulmonary artery diastolic pressure decreased from (27.50±1.87) mmHg to(18.33±4.18)mmHg( P=0.001), and left ventricular ejection fraction improved from 0.27±0.04 to 0.37±0.06 ( P=0.004). No visible hemoglobinuria was noted during the support period. No malignant arrhythmias or cerebrovascular complications occurred. One patient required transition to surgical LVAD implantation, while the other five patients had the pLVAD successfully removed and were discharged. Three months later, all six patients were alive, with functional status classified as New York Heart Association (NYHA) Class Ⅰ-Ⅱ. Conclusion:The implantation of a domestic high-flow pLVAD provides a safe and effective therapeutic option for patients with LCOS following cardiac surgery.

In 2040, neurodegenerative diseases (NDD) will overtake cancer as the second leading cause of death after cardiovascular and cerebrovascular diseases. Therefore, the search for effective intervention measures has become the top priority to deal with this difficult burden. Hyperbaric oxygen therapy (HBOT) has been used for the past 50 years to treat conditions such as decompression sickness, carbon monoxide poisoning and radiation damage. In recent years, studies have confirmed that HBOT has good effects in improving cognitive impairment after brain injury and stroke, and alleviating neurodegeneration and dysfunction related to NDD. Here we reviewed the pathogenesis and treatment state of NDD, introduced the application of HBOT in animal models and clinical studies of NDD, and expounded the application potential of HBOT in the treatment of NDD from the perspective of mitochondrial function, neuroinflammation, neurogenesis and angiogenesis, oxidative stress, apoptosis, microcirculation and epigenetics.

3.Feasibility study on the construction of predictive models of knee joint cartilage thickness

Objective To determine the knee joint cartilage thickness using different methods and explore the feasibility of mathematical statistical models of dataset for the prediction of cartilage thickness.Methods A total of 304 patients diagnosed as knee osteoarthritis(OA)combined with varus deformity and undergoing unilateral total knee arthroplasty at the Second Affiliated Hospital of Army Medical University from March 2023 to March 2024 were selected for the study.All patients had complete preoperative and postoperative clinical data.The healthy cartilage at four anatomical sites of patients,including the distal femur lateral condyle,lateral tibial plateau,posterior medial femoral condyle,and posterior lateral femoral condyle were selected,and the knee joint cartilage thickness was determined based on preoperative MRI analysis,robotic navigation system tracing,tissue section of surgical specimen and digital vernier caliper.The baseline indicators of demographics,disease and imaging ffor patients were collected to construct a dataset,and four models of linear regression analysis,principal component analysis,Least Absolute Shrinkage and Selection Operator(LASSO)regression analysis,and K-nearest neighbors(KNN)analysis were established for predicting the accuracy,determination coefficient(R2)and root mean square error(RMSE),and the regression equation for predicting cartilage thickness was established.Results The knee joint cartilage thicknesses determined by preoperative MRI analysis,robotic navigation system tracing,tissue section of surgical specimen had no statistically significant difference with that by digital vernier caliper(P>0.05).The predictive efficiencies of models of linear regression analysis,principal component analysis,and LASSO regression analysis for the knee joint cartilage thickness all failed to meet the expectations(R2<0.3,RMSE>0.03).The predictive effect of KNN model on the cartilage thickness of the distal femur lateral condyle and lateral tibial plateau was not ideal(R2=0.23,RMSE=0.29),while it had potential predictive value(accuracy=0.21,accuracy=0.15).Conclusion The prediction model of knee joint cartilage thickness based on individual parameters has certain scientificity,and the feasibility of KNN model is relatively high.However,due to insufficient sample size and unclear individual parameter weight,the efficiencies of the four established prediction models are not ideal,which fails to provide definite prediction equations.Therefore,the construction scheme of the prediction model still needs to be further optimized.

Traumatic supraorbital fissure syndrome (TSOFS) is a symptom complex caused by nerve entrapment in the supraorbital fissure after skull base trauma. If the compressed cranial nerve in the supraorbital fissure is not decompressed surgically, ptosis, diplopia and eye movement disorder may exist for a long time and seriously affect the patients′ quality of life. Since its overall incidence is not high, it is not familiarized with the majority of neurosurgeons and some TSOFS may be complicated with skull base vascular injury. If the supraorbital fissure surgery is performed without treatment of vascular injury, it may cause massive hemorrhage, and disability and even life-threatening in severe cases. At present, there is no consensus or guideline on the diagnosis and treatment of TSOFS that can be referred to both domestically and internationally. To improve the understanding of TSOFS among clinical physicians and establish standardized diagnosis and treatment plans, the Skull Base Trauma Group of the Neurorepair Professional Committee of the Chinese Medical Doctor Association, Neurotrauma Group of the Neurosurgery Branch of the Chinese Medical Association, Neurotrauma Group of the Traumatology Branch of the Chinese Medical Association, and Editorial Committee of Chinese Journal of Trauma organized relevant experts to formulate Chinese expert consensus on the diagnosis and treatment of traumatic supraorbital fissure syndrome ( version 2024) based on evidence of evidence-based medicine and clinical experience of diagnosis and treatment. This consensus puts forward 12 recommendations on the diagnosis, classification, treatment, efficacy evaluation and follow-up of TSOFS, aiming to provide references for neurosurgeons from hospitals of all levels to standardize the diagnosis and treatment of TSOFS.
8.Progress in gene-modified stem cell therapy for penile erectile dysfunction

Erectile dysfunction(ED)is a common disorder in men and lacks effective treatment.Stem cell(SC)possess the potential in self-renewal and multi-directional differentiation,and secrete a variety of active substances.Stem cell therapy,as a promis-ing option for the treatment of ED,is a focus in regenerative medicine research.However,the effect of stem cell therapy alone on ED is limited due to the complexity of the condition and the limitations of SC.Gene modification can enhance the performance of SC in multiple aspects and play the role of targeted genes,and therefore can better improve ED and its related pathological changes than SC therapy alone.In recent years,gene-modified stem cell therapy has become a hotspot in andrological research.This review summarizes the advances in the studies of gene-modified SC in the treatment of ED,aiming to provide some ideas for further research.
9.Analysis of metastasis and survival after different treatment in patients with T1 stage colonic neuroendocrine tumors

Objective To explore the metastasis rate and related risk factors of T1 stage colonic neuroendocrine tumor(C-NET),and to compare the long-term survival outcomes of patients with non-metastatic(T1N0M0 stage)C-NET after local excision(LE)or radical surgery(RS).Methods Clinical information of 433 patients diagnosed with C-NET in the SEER database from January 1,2004 to December 31,2015 were analyzed.Cox regression was used to analyze the influencing factors of metastasis of C-NET.The patients without metastasis were divided into LE group and RS group,and assigned in a 1∶1 ratio using propensity score matching(PSM)according to gender,age,tumor largest diameter,and infiltration depth,with a caliper value set to 0.02.Kaplan-Meier survival curve was used to analyze 5-year cancer-specific survival(CSS)and overall survival(OS)of patients.Cox regression analysis was used to evaluate the influence of metastasis on survival.Results Among 419 C-NET patients,19(4.52%)had distant metastases.Cox regression analysis showed that 11-20 mm of tumor large diameter(HR=9.264,95%CI 3.322-25.835,P<0.001),right colon location(HR=0.116,95%CI 0.042-0.321,P<0.001),and submucosal invasion(HR=5.842,95%CI 1.858-18.371,P=0.003)were independent risk factors for distant metastasis of T1 stage C-NET.The 5-year OS rates of non-metastatic and metastatic patients were 94.5%and 47.4%,respectively(χ2=79.762,P<0.001),and their 5-year CSS rates were 99.5%and 55.7%,respectively(χ2=164.604,P<0.001).Before PSM,the 5-year OS rates of non-metastatic C-NET patients after LE and RS were 95.8%and 90.1%(χ2=2.679,P=0.063),and the 5-year CSS rates were 100.0%and 97.2%(χ2=0.579,P=0.038);after PSM,the 5-year OS rates of non-metastatic patients after LE and RS were 96.8%and 92.1%(χ2=3.606,P=0.058),and the 5-year CSS rates were 100.0%and 98.5%(χ2=1.015,P=0.314).After PSM,there was no significant difference in the 5-year OS and CSS of patients with defferent tumor location,tumor large diameter,or submucosal invasion between the LE and RS groups.Conclusions 11-20 mm of tumor diameter,right colon location,and submucosal invasion might be independent risk factors for distant metastasis of T1 stage C-NET,and LE could be an appropriate treatment option for non-metastatic C-NET.
10.Correlations of Expression Levels of microRNA-206 and microRNA-186 in the Serum With Disease Severity and Prognosis of Elderly Patients With Vascular Dementia.

Objective To investigate the correlations of the expression levels of microRNA (miR)-206 and miR-186 in the serum with the disease severity and prognosis of elderly patients with vascular dementia (VD). Methods A total of 104 elderly patients with VD treated in Dalian Municipal Central Hospital from October 2020 to May 2023 were selected into the VD group and 104 healthy volunteers who underwent physical examinations were enrolled in the control group.Baseline data of both groups were collected,and fluorescence quantitative PCR was used to measure the expression levels of miR-206 and miR-186 in the serum.According to the severity of VD,the patients were assigned into mild,moderate,and severe VD groups.The elderly patients with VD were followed up for 6 months after discharge.According to the prognosis,the patients were assigned into good/moderate and poor prognosis groups.The baseline data and the serum levels of miR-206 and miR-186 were compared between the control and VD groups as well as between the good/moderate and poor prognosis groups.The serum levels of miR-206 and miR-186 were compared between the mild,moderate,and severe VD groups.The correlations of serum miR-206 and miR-186 levels with mini-mental state examination (MMSE) score in elderly patients with VD were analyzed,on the basis of which the factors affecting the poor prognosis of elderly patients with VD were predicated.Furthermore,the predictive values of serum levels of miR-206 and miR-186 on the poor prognosis of elderly patients with VD were evaluated. Results The VD group had higher proportions of diabetes history (P=0.021),hypertension history (P<0.001),and hyperlipidemia history (P<0.001),higher serum levels of miR-206 (P<0.001) and miR-186 (P<0.001),and lower score of activity of daily living (P<0.001) than the control group.The serum miR-206 and miR-186 levels varied between the mild,moderate,and severe VD groups (all P<0.001).The trend test results showed that serum miR-206 and miR-186 levels presented a rising trend in mild,moderate,and severe VD groups (all P<0.001).The serum miR-206 and miR-186 levels in elderly VD patients were negatively correlated with the MMSE score (all P<0.001).The poor prognosis group had lower MMSE score and higher serum levels of miR-206 and miR-186 than the good/moderate prognosis group (all P<0.001).The MMSE score was a protective factor and miR-206 and miR-186 were risk factors for poor prognosis in elderly patients with VD (P=0.005,P=0.001,P=0.007).The area under curve of combined serum miR-206 and miR-186 levels in predicting poor prognosis in the elderly patients with VD was 0.963,which was higher than those (0.866 and 0.878) of serum miR-206 and miR-186 levels separately (P=0.010,P=0.020). Conclusions The serum levels of miR-206 and miR-186 in elderly patients of VD are high and closely related to the severity and prognosis of VD.The combination of the two demonstrates high performance in predicting the prognosis of elderly patients with VD.
