Objective:To analyze the clinical features and prognosis of patients with lymphoplasmacytic lymphoma/Waldenstr?m macroglobulinemia (LPL/WM) transformed into diffuse large B-cell lymphoma (DLBCL) .Methods:This study retrospectively analyzed the clinical data of five patients with LPL/WM transformed to DLBCL diagnosed and treated at a multicenter hospital in Hunan Province from December 2020 to April 2023. Clinical manifestations, treatment regimens, and therapeutic efficacy before and after the transformation were compared. Results:Of the five patients, four were male and one was female, with a median age of 64.0 (57.0–80.0) years, all of whom had abnormally increased β 2-microglobulin levels at diagnosis, and two were combined with increased lactate dehydrogenase levels. The MYD88 L265P mutation was detected in 4 patients, whereas 1 carried the FAT1 and NOTCH1 mutations, and none demonstrated CXCR4 mutations. Three patients were negative for the TP53 mutation, and two were not tested. Before transformation, three patients were treated with Bruton tyrosine kinase inhibitor therapy, and one patient was treated with the bendamustine plus rituximab regimen. All patients eventually transformed into non-growth center-derived DLBCL, with a median time to conversion of 11.8 (4.0–19.0) months, and most of them presented with weight loss, lymph node enlargement, splenomegaly, and extranodal involvement. Posttransformation, the patients were mainly treated with the rituximab, cyclophosphamide, hydroxydaunorubicin, oncovin, and prednisone (R-CHOP) regimen, with an optimal outcome of partial remission. Disease progression occurred in 4 of the patients, with a median overall survival of 16.8 (10.0–26.0) months. Conclusion:Transformation from LPL/WM to DLBCL is rare. Patients should remain highly vigilant for transformation if they develop rapidly enlarging lymph nodes and/or newly involved lymph nodes, worsening systemic symptoms, and declining body mass. R-CHOP regimen may induce a partial response in some cases; however, the overall prognosis remains poor.

Objective To study the consonant articulation performance and speech intelligibility of patients with submucous cleft palate(SMCP)and to provide a reference for clinical speech evaluation and subsequent speech rehabilitation. Methods A total of 333 preoperative SMCP patients aged 4.5 years and older participated in this study. The accuracy, type of error, and error rates were assessed across participant genders and their varying levels of velopharyngeal closure function. Results Among the 333 patients,196 had complete velopharyngeal closure, while 137 had incomplete closure.A total of 145 patients(43.54%)demonstrated normal articulation of all conso-nants,while 188 patients(56.46%)displayed various degrees of articulation disorders.Compensatory articulation behaviors were observed in 66 patients(19.82%).No significant differences in articulation errors were found be-tween male and female patients. The accuracy ranking for consonants was from high to low as follows:nasal sounds, lateral sounds, fricatives, plosives, and affricates.Substitution was the most common error type with an incidence of 35.93%,followed by omission at 34.62%and compensatory errors at 25.51%.The average accuracy rates for plosives,fricatives,affricates,lateral/nasal sounds were 73.27%,78.20%,69.29%,and 93.39%,re-spectively.Substitution was the most common error for plosives and fricatives, while omission was most frequent for affricates.Compensatory errors occurred most often with affricates, and no compensatory errors were found in nasal or lateral sounds. Conclusion Substitution, omission, and compensatory errors are the most common articula-tion errors in SMCP patients, occurring across plosives, fricatives, and affricates. The severity of articulation disor-ders is related to velopharyngeal closure function but is independent of gender.
5.Research Progress on the Correlation Between Microecological Flora and Endometriosis
Medical Journal of Peking Union Medical College Hospital 2025;16(1):184-191
Microecological flora,an important part of the human microenvironment, changes with the changes in the internal and external environments of the human body, and plays an important role in maintaining human body's normal function.Endometriosis(EMs)is a common estrogen-dependent female disease chara-cter-ized by the growth of endometrial glands and mesenchyme outside the uterine cavity.Existing studies have shown that imbalance of intestinal and reproductive tract flora can trigger local immune-inflammatory responses and cause systemic immune-inflammatory responses through cytokines and metabolic pathways, which in turn participate in the progression of EMs.Thus, the diagnosis and treatment of EMs through microecological flora interventions is one of the hotspots research in this field. This article aims to review the research progress on the correlation be-tween microbial dysbiosis and EMs,with the hope of providing more references for the treatment of EMs.
6.Research Progress on the Relationship Between Reproductive Tract Microbes and Tubal Pregnancy
Medical Journal of Peking Union Medical College Hospital 2025;16(3):749-755
Recent studies have demonstrated a close association between human health, disease suscep-tibility,and microbial communities. In women of reproductive age, alterations in the reproductive tract microbi-ota have been linked to various pregnancy outcomes.Traditionally, tubal pregnancy has been attributed to fallo-pian tube inflammation, developmental or functional abnormalities, and surgical interventions.However,emer-ging research on microbiota has revealed that dysbiosis of the reproductive tract microbiome, such as vaginal mi-crobial imbalance, may impair endometrial receptivity and consequently increase the risk of tubal pregnancy.Through a comprehensive literature review, this article summarizes current research progress on the correlation between reproductive tract microbiota and tubal pregnancy, aiming to explore potential pathogenic mechanisms and provide valuable insights for further clinical investigations.

Objective:
To explore the feasibility of synthetic magnetic resonance imaging (syMRI) combined with clinicopathological characteristics for the pre-treatment prediction of chemoradiotherapy (CRT) response in advanced nasopharyngeal carcinoma (ANPC).
Materials and Methods:
Patients with ANPC treated with CRT between September 2020 and June 2022 were retrospectively enrolled and categorized into response group (RG, n = 95) and non RGs (NRG, n = 32) based on the Response Evaluation Criteria in Solid Tumors (RECIST) 1.1. The quantitative parameters from pre-treatment syMRI (longitudinal [T1] and transverse [T2] relaxation times and proton density [PD]), diffusion-weighted imaging (apparent diffusion coefficient [ADC]), and clinicopathological characteristics were compared between RG and NRG. Logistic regression analysis was applied to identify parameters independently associated with CRT response and to construct a multivariable model. The areas under the receiveroperating characteristic curve (AUC) for various diagnostic approaches were compared using the DeLong test.
Results:
The T1, T2, and PD values in the NRG were significantly lower than those in the RG (all P < 0.05), whereas no significant difference was observed in the ADC values between these two groups. Clinicopathological characteristics (Epstein–Barr virus [EBV]-DNA level, lymph node extranodal extension, clinical stage, and Ki-67 expression) exhibited significant differences between the two groups. Logistic regression analysis showed that T1, PD, EBV-DNA level, clinical stage, and Ki-67 expression had significant independent relationships with CRT response (all P < 0.05). The multivariable model incorporating these five variables yielded AUC, sensitivity, and specificity values of 0.974, 93.8% (30/32), and 91.6% (87/95), respectively.
Conclusion
SyMRI may be used for the pretreatment prediction of CRT response in ANPC. The multivariable model incorporating syMRI quantitative parameters and clinicopathological characteristics, which were independently associated with CRT response, may be a new tool for the pretreatment prediction of CRT response.
