Objective To investigate the impact of exosomal miRNAs derived from endoplasmic reticulum-stressed(ERS)head and neck squamous cell carcinoma(HNSCC)cells on macrophages. Methods This study was reviewed and approved by the Ethics Committee. The expression levels of ERS-associated proteins, including protein kinase R-like endoplasmic reticulum kinase(PERK)and glucose-regulated protein 78(GRP78),in HNSCC tissues and para-tumor tissues were detected by Western blot(WB)and quantitative real-time PCR(RT-qPCR).HN4 human laryngeal squamous cell carcinoma cells were treated with 500 U/mL interferon-γ(IFN-γ)for 48 h to induce ER stress, and exo-somes secreted by ER-stressed HN4 cells were collected and identified. The types of miRNAs in exosomes were identi-fied through bioinformatics analysis, and the target genes of miRNAs were predicted.Macrophages were transfected with miRNA,co cultured with collected exosomes, and the expression of PTEN in macrophages was knocked down. The downstream signaling pathway regulated by exosomal miRNAs was studied by WB and RT-qPCR.Results Compared with that in para-tumor tissues, the expression level of ER stress-associated proteins in HNSCC tissues was increased(P<0.05).RNA-seq analysis revealed that miR-26a-5p was highly upregulated in ER-stressed HN4 cell-derived exo-somes(P<0.05).PTEN is the target gene for miR-26a-5p.miR-26a-5p increased the expression level of PD-L1 in mac-rophages and downregulated the expression of PTEN(P<0.05).Macrophages co cultured with ERS extracellular vesi-cles showed an increase in miR-26a-5p and PD-L1 expression,a decrease in PTEN expression, and an increase in p-AKT expression(P<0.05).Knock down the expression of PTEN in macrophages and increase the expression of PD-L1(P<0.01).Conclusion ERS HNSCC cell-derived exosomal miR-26a-5p promotes the expression of PD-L1 in macro-phages through the PTEN/AKT signaling pathway.

Objective To explore the lateral lymph node metastasis in patients with rectal cancer below peritoneal reflection and construct predictive models. Methods A total of 102 patients with rectal cancer be-low peritoneal reflection admitted in the hospital from January 2020 to April 2023 were selected as the re-search objects.According to the existence of lateral lymph node metastasis(LLNM),the patients were divided into metastatic group(n=31)and non-metastatic group(n=71).The general clinical data, tumor pathologi-cal features, laboratory indexes[carcinoembryonic antigen(CEA).carbohydrate antigen 199(CA199),neutro-phil/lymphocyte ratio(NLR),platelet/lymphocyte ratio(PLR),transforming growth factor(TGF)-α and TGF-β1]between the two groups were compared. The risk factors of LLNM in patients with rectal cancer be-low peritoneal reflection were screened by Spearman correlation analysis and Logistic regression analysis, and the predictive model was constructed and the efficacy of the model was evaluated. Results The mean maxi-mum tumor diameter, the short diameter of lateral lymph nodes, the proportion of poorly differentiated pa-tients and the proportion of patients with T3-T4 stage in the metastasis group were significantly higher than those in the non-metastasis group(P<0.05).The average levels of TGF-α,TGF-β1 and NLR in the metasta-sis group were significantly higher than those in the non-metastasis group(P<0.05).Spearman correlation a-nalysis,univariate and multivariate Logistic regression analysis showed that poor differentiation,T3-T4 stage, maximum tumor diameter, longer short diameter of lateral lymph nodes, higher levels of TGF-α,TGF-β1 and NLR were independent risk factors for LLNM in patients with rectal cancer below peritoneal reflec-tion.Receiver operating characteristic(ROC)curve analysis showed that the area under the curve(AUC)of the predictive model based on the above risk factors was 0.915(95%CI:0.847-0.984),which had high pre-dictive performance. Conclusion Patients with LLNM and rectal cancer below peritoneal reflection often have longer tumor diameter and lateral lymph node short diameter, and higher levels of TGF-α,TGF-β1 and NLR.Lateral lymph node dissection should be actively carried out for patients with above characteristics, and timely monitoring should be carried out to see if there is still lymph node metastasis after surgery,so as to provide certain clinical basis for improving the judgment of LLNM in rectal cancer below peritoneal reflection.

Objective:To evaluate the efficacy and safety of immunotherapy with or without radiotherapy in the treatment of recurrent or metastatic esophageal squamous cell carcinoma (R/M ESCC).Methods:A retrospective analysis was conducted on the data of 75 patients with R/M ESCC treated with sintilimab at Anyang Tumor Hospital from January 2020 to October 2021. The patients were divided into the radiotherapy (RT) group ( n=37) and non-radiotherapy (NRT) group ( n=38) based on whether they received radiotherapy. The objective response rate (ORR), disease control rate (DCR), progression-free survival (PFS), overall survival (OS), and adverse effects were compared between two groups. Count data were expressed as composition ratios and analyzed using Chi-square test or Fisher's exact test. Survival analysis was performed using Kaplan-Meier method and log-rank test. Results:There was no statistically significant difference in ORR and DCR between the RT and NRT groups (70% vs. 61%, P=0.375; 95% vs. 89%, P=0.414). However, the complete response (CR) rate in the RT group was higher compared to that in the NRT group (19% vs. 3%, P=0.022). The median follow-up duration was 25.4 months. There was no statistically significant difference in the median PFS and OS between the RT and NRT groups (13.8 months vs. 9.9 months, P=0.221; 20.2 months vs. 18.9 months, P=0.214). Subgroup analysis demonstrated that among patients with recurrence or metastasis confined to local and / or ≤3 distant lymph nodes, there was no statistically significant difference in the median PFS between the RT and NRT groups (15.1 months vs. 8.4 months, P=0.115), but the median OS in the RT group was better than that in the NRT group (not reached vs. 12.3 months, P=0.036). Compared to the NRT group, besides an increase in grade 1-2 pneumonitis (41% vs. 18%, P=0.035), no significant increase in treatment-related toxicity was observed in the RT group. Conclusion:Immunotherapy combined with radiotherapy is safe in patients with R/M ESCC, and shows survival benefit in patients with recurrence or metastasis confined to local and / or ≤3 distant lymph nodes.

Chinese Journal of Pharmacology and Toxicology 2023;37(7):481-482
OBJECTIVE Epileptic networks are char-acterized as two states, seizures or more prolonged inter-ictal periods. However, cellular mechanisms underlying the contribution of interictal periods to ictal events remain unclear.METHODS Here,we present the procedure for labeling seizure-activated and interictal-activated neuro-nal ensembles in mouse hippocampal kindling model using an enhanced-synaptic-activity-responsive element. This technique is combined with genetically encoded effectors to characterize and manipulate neuronal ensembles recruited by focal seizures(FS-Ens)and interictal periods(IP-Ens)in piriform cortex,a region that plays a key role in seizure generation.RESULTS Ca2+ activities and histo-logical evidence reveal a disjointed correlation between the two ensembles during FS dynamics.Optogenetic acti-vation of FS-Ens promotes further seizure development, while IP-Ens protects against it.Interestingly, both ensem-bles are functionally involved in generalized seizures(GS)due to circuit rearrangement.IP-Ens bidirectionally modulates FS but not GS by controlling coherence with hippocampus.CONCLUSION This study indicates that the interictal state may represent a seizure-preventing environment, and the interictal-activated ensemble may serve as a potential therapeutic target for epilepsy.

Peptide‒drug conjugates (PDCs) are drug delivery systems consisting of a drug covalently coupled to a multifunctional peptide via a cleavable linker. As an emerging prodrug strategy, PDCs not only preserve the function and bioactivity of the peptides but also release the drugs responsively with the cleavable property of the linkers. Given the ability to significantly improve the circulation stability and targeting of drugs in vivo and reduce the toxic side effects of drugs, PDCs have already been extensively applied in drug delivery. Herein, we review the types and mechanisms of peptides, linkers and drugs used to construct PDCs, and summarize the clinical applications and challenges of PDC drugs.
