1.Study on the Expression and Clinical Value of Serum LRG1 and Thiol Levels in Patients with Systemic Lupus Erythematosus Nephritis

Objective To investigate the correlation of serum leucine-rich alpha-2 glycoprotein-1(LRG1)and Thiol levels with autoimmune antibodies and renal injury in patients with systemic lupus erythematosus nephritis(SLEN).Methods 80 SLEN patients(SLEN group)and 80 simple SLE patients(non-SLEN group)admitted to the Department of Nephrology at Qingdao Eighth People's Hospital and the Department of Rheumatology and Immunology at Affiliated Hospital of Qingdao University from January 2021 to December 2023 were selected.An enzyme-linked immunosorbent assay(ELISA)was used to detect serum LRG1 levels,while a colorimetric assay was used to detect serum Thiol levels.Pearson/Spearman correlation analysis was used to investigate the correlation between serum LRG1 and Thiol levels,autoimmune antibodies,and renal function indicators in SLEN patients.The diagnostic value of serum LRG1 and Thiol levels in SLEN was analyzed using receiver characteristic curves.Results The serum LRG1(26.00±8.76 μg/ml)and Scr(98.03±49.71μmol/L)in the SLEN group were higher than those in the non-SLEN group(16.71±5.51μg/ml,73.85±33.51μmol/L),and Thiol(80.69±36.90μmol/L)was lower than those in the non-SLEN group(131.49±45.34μmol/L),the differences were statistically significant(t=8.026,3.608,7.772,all P<0.001).Pearson/Spearman correlation analysis showed that serum LRG1 in SLEN patients was positively correlated with anti-double-stranded deoxyribonucleic acid(dsDNA)antibody,anti-complement 1q(C1q)antibody,anti-Smith antibody,serum creatinine(Scr),24-hour urine protein(r=0.226~0.637,all P<0.05),and negatively correlated with estimated glomerular filtration rate(eGFR)(r=-0.633,P<0.05).Serum Thiol was negatively correlated with anti-dsDNA antibodies,anti C1q antibodies,anti-Smith antibodies,Scr and 24-hour urine protein(r=-0.612~-0.256,all P<0.05),and positively correlated with eGFR(r=0.614,P<0.05).The AUC(95%CI)of serum LRG1 combined with Thiol levels for diagnosing of SLEN was 0.907(0.851~0.947),which was higher than that of serum LRG1 and Thiol alone at 0.802(0.731~0.860)and 0.808(0.738~0.866),and the differences were statistically significant(Z=3.450,3.749,all P<0.05).Conclusion Elevated serum LRG1 levels and decreased Thiol levels in SLEN patients were closely related to positive expression of autoimmune antibodies and renal dysfunction.Combined detection of serum LRG1 and Thiols can provide reference for the diagnosis of SLEN.

Notoginseng Radix et Rhizoma as a traditional Chinese herbal medicine has now been recognized and paid attention to by the pharmaceutical community.Modern phytochemical studies have shown that Panax notoginseng saponin is the main chemical compo-nent of Notoginseng Radix et Rhizoma.Modern pharmacological studies and clinical applications have revealed that it has anti-cancer,antioxidant and cardiovascular disease effects.In this study,we reviewed the research progress of the main chemical components and pharmacological effects of Notoginseng Radix et Rhizoma,with the aim of providing assistance for the clinical application and later stud-ies of Notoginseng Radix et Rhizoma.
4.Clinical significance of telomerase reverse transcriptase promoter mutation in radioactive iodine refractory papillary thyroid cancer

Objective:To evaluate the influence of telomerase reverse transcriptase (TERT) promoter mutation on radioiodine uptake status of radioactive iodine refractory papillary thyroid cancer (RAIR-PTC) and radioiodine therapy response by analyzing the mutation frequency of TERT promoter in RAIR-PTC.Methods:A total of 37 patients with RAIR-PTC (15 males, 22 females, age (49.8±16.1) years) and 40 PTC patients with effective radioiodine therapy (13 males, 27 females, age (39.8±10.9) years) between January 2005 and June 2020 in JiangYuan Hospital Affiliated to Jiangsu Institute of Nuclear Medicine were retrospectively analyzed. TERT promoter mutation and B-Raf proto-oncogene, serine/threonine kinase (BRAF) V600E mutation of patients were observed. The differences across genotype patterns on radioiodine uptake status and therapy response were compared. The Fisher′s exact test and independent-sample t test were used for data analysis. Results:The incidence rate of TERT promoter mutation in the RAIR-PTC group was 40.54% (15/37, all C228T), which was significantly higher than that in the effective radioiodine therapy group (0, 0/40; P<0.001). No statistically significant difference was found for the mutation rate of BRAF V600E between the RAIR group (64.86%, 24/37) and the effective radioiodine therapy group (72.50%, 29/40; P=0.858). Patients with TERT promoter mutation were older ( t=3.76, P=0.001) and the non-intake rate of radioiodine in distant metastases of those patients was higher ( P=0.037). Furthermore, 2/3 of patients who received targeted therapies and 3/4 deaths had TERT promoter mutation. Among 35 patients with negative thyroglobulin antibody (TgAb), 11/14 of patients with TERT mutation had a rising stimulated thyroglobulin (sTg), while the percentage of the non-TERT mutation group was 57.1% (12/21; P=0.357). Conclusion:The TERT promoter mutation rate is significantly increased in RAIR-PTC patients and can serve as a prognostic predictor in RAIR.
5.Minimally invasive mitral valve surgery via right minithoracotomy.

OBJECTIVE:
To summarize the successful experience of 45 patients with minimally invasive mitral valve surgery via right minithoracotomy.
METHODS:
Forty-five patients with mitral valve disease were enrolled. A main surgical wound (4-6 cm)was made over the lateral border of the right breast. Cardiopulmonary bypass was established via femoral cannulation. The Chitwood clamp was inserted thlution the 2nd or 3rd intercostal space. After the cross-clamp was placed, cold blood cardioplegic solution was delivered via right minithoracotomy. The procedures of mitral valve replacement were performed via left atrialotomy.
RESULTS:
All the operations were successful. There was no death. The duration of operation and extracorporeal circulation was 80-291(139±35) min, and 25-110(49±21) min, respectively. The cross-clamped time was 18-77 (33±12) min, the ventilation time was 4.5-45(14.8±10) h, the total chest tube drainage was 50-1 050(262±110) mL, and the postoperative hospital stay was 5-14(7.5±1.8) d.
CONCLUSION
Minimally invasive mitral valve surgery via right minithoracotomy is feasible, safe, and less invasive with rapid recovery,and has superior cosmetic results for patients.
