1.Research progress on the mechanism of action and clinical application of Shenqi dihuang decoction in the treatment of diabetic nephropathy

Diabetic nephropathy(DN) is a common and severe microvascular complication of diabetes. In recent years, the classical herbal formula Shenqi dihuang decoction has demonstrated unique advantages in the clinical treatment of DN. This article conducts a systematic review of the mechanisms of action and clinical applications of Shenqi dihuang decoction in the treatment of DN. It reveals that the mechanism by which this formula improves DN involves multi-target synergistic regulation. For instance, Shenqi dihuang decoction exerts multiple pharmacological effects by regulating signaling pathways including phosphatidy linostiol 3-kinase/protein kinase B, AMP-activated protein kinase/silent information regulator 1/forkhead box O1, and nuclear factor erythroid 2-related factor 2/heme oxygenase-1 pathways.These effects include regulating glucose and lipid metabolism, inhibiting oxidative stress, reducing inflammation, improving insulin resistance, modulating cell death (apoptosis/autophagy/ferroptosis/pyroptosis), and preventing renal fibrosis. Existing clinical studies indicate that Shenqi dihuang decoction and its modified formulas, alone or in combination with other therapeutic methods, can significantly improve glucose and lipid metabolism, reduce proteinuria, and delay renal function decline in patients with DN. These effects are superior to those of Western medicines such as irbesartan, valsartan, and empagliflozin, and the treatment demonstrates good safety. Future research should leverage systems biology and artificial intelligence technologies to further elucidate the integrated mechanisms in the treatment of DN by Shenqi dihuang decoction, thereby advancing the precision and standardization of its clinical application.
2.Effects of Qingre Lishi Prescription on ATP/P2X7R/IL-1β signaling pathway in rats with acute gouty arthritis

Objective:To study the effects of Qingre Lishi Prescription on ATP-P2X7R-IL-1β signaling pathway in rats with acute gouty arthritis.Methods:Otally 60 SPF-grade SD rats were divided into normal group, model group, colchicine group, and Qingre Lishi Prescription low-, medium- and high-dosage groups according to random number table method; the normal group and model group were given distilled water at a dosage of 10 ml/kg by gavage, the colchicine group was given colchicine solution at a dosage of 0.6 mg/kg by gavage, and Qingre Lishi Prescription low-, medium- and high-dosage groups were given Qingre Lishi Prescription extract suspension at dosages of 11.76, 23.52, and 47.04 g/kg by gavage, once a day for 7 consecutive days. Except for the normal group, AGA models were prepared by gavage on the 4th day in all other groups, and the swelling index of the right posterior ankle joint of rats was measured at 6, 12, 24, 48, and 72 hours after modeling; after 7 days of administration, samples were taken and HE staining was used to observe the pathological changes in the synovial tissue of the right posterior ankle joint. ELISA was used to detect the levels of serum ATP, IL-1 β, IL-8, and IL-37, and immunohistochemistry was used to detect the positive rates of P2X7R and IL-1 β proteins in the synovial tissue of the right posterior ankle joint.Results:At 6, 12, 24, 48, and 72 hours after modeling, compared with the model group, the swelling index of the colchicine group and Qingre Lishi Prescription low-dosage group decreased ( P<0.01); the levels of serum ATP, IL - β, IL-8, and IL-37 decreased in the colchicine group and Qingre Lishi Prescription low-, medium-, and high-dosage groups ( P<0.01); the expressions of P2X7R and IL-1 β proteins in the synovial tissue of the right posterior ankle was reduced in the colchicine group and Qingre Lishi Prescription high-dosage group ( P<0.05). Conclusion:Qingre Lishi Prescription may improve joint swelling and alleviate synovial tissue inflammation in AGA model rats through ATP/P2X7R/IL-1β-mediated inflammatory factor pathway.

This article introduced TCM master Zhang Zhen's experience of using theory of"one body with two wings,regulating qi movement"in the treatment of thyroid nodules.The onset of thyroid nodules is due to liver depression and qi stagnation,and the booster of the onset is spleen and kidney dysfunction.The main pathogenesis is liver depression and qi stagnation,spleen deficiency and phlegm coagulation,kidney deficiency and blood stasis.The disease is located in the liver,spleen,and kidney.The general principle for treating thyroid nodules is to regulate the liver to regulate"the body",while the key points for treating thyroid nodules are to supplement the spleen and kidneys to supplement"the two wings".The combination of promoting blood circulation and dispersing nodules is the target for treating thyroid nodules.The clinical treatment of thyroid nodules with Shutiao Xiaohe Decoction has a significant therapeutic effect.
