1.A Qualitative Study on the Caregiving Experiences of Parents of Young Adults with Mental Disorders

Purpose:
The incidence of mental disorders in young adults has been increasing since the COVID-19 pandemic. The specific characteristics of young adults with mental disorders can lead to significant caregiving challenges. Understanding these challenges can help healthcare professionals provide timely interventions and support. Thus, this study aimed to understand the caregiving experiences of parents of young adult children with mental disorders and to grasp their essential meaning.
Methods:
A qualitative phenomenological design was adopted. Semi-structured individual interviews were conducted with nine parents and the data were analyzed using the phenomenological method. Data collection was conducted by nine parents of young adult children with mental disorders from July to October 2023 at S General Hospital.
Results:
The analysis revealed the overarching theme of “A journey of finding hope in a dark tunnel,” supported by three main themes: 1) life feels like walking on eggshells with the child; 2) being aware of the changing family lifestyle and seeking balance; and 3) lifelong tasks for parents and society.
Conclusion
Parents experienced various difficulties intertwined with their own and their child’s developmental tasks. However, both the parents and children grew, discovering positive elements such as happiness and hope.

5.Validity and Reliability of Korean Version of Assertiveness Scale for Nurses (K-ASN)

Purpose:
: Assertiveness is a crucial factor in patient safety, workplace violence, and work stress, and is associated with better interpersonal relationships in clinical settings. Despite its significance, a standardized assertiveness scale reflecting Korean healthcare culture is currently lacking. Thus, this study aimed to investigate the validity and reliability of the Korean version of the assertiveness scale for nurses (K-ASN).
Methods:
: The survey was conducted with 250 nurses working in secondary and tertiary hospitals in Korea. The study examined content, criterion, and construct validity using exploratory factor analysis and also assessed internal consistency reliability.
Results:
: The K-ASN consists of 22 items and 6 factors, namely self-denial, confrontation, dealing with criticism, positive assertion, spontaneity, and active assertion. The K-ASN demonstrated good content validity and a high criterion validity with the Rathus Assertiveness Schedule (RAS) at .75. The internal consistency reliability was also satisfactory, with a Cronbach’s alpha of .80, and subscale reliability ranging from .52 to .87.
Conclusion
: The K-ASN is a reliable and valid assertiveness scale for nurses in Korea. The K-ASN can be a valuable tool to be used to assess the assertiveness of healthcare professionals which is associated with improvements in interpersonal relationships, patient safety, and health-related outcomes.
6.Effects of Using Mobile Apps for Mental Health Care in Korea: A Systematic Review

Purpose:
This study aimed to systematically evaluate the content and effect of using Korean mobile apps for mental health care as reported in previous studies.
Methods:
We conducted a systematic review according to PRISMA guidelines. Randomized controlled trials published up to May 2021 were searched in the PubMed, Cochrane Library, EMBASE, CINAHL, PsycINFO, RISS, KISS, and KoreaMed databases. We used the Cochrane risk-of-bias tool version 2 for randomized trials to evaluate the selected studies.
Results:
Out of 3,374 articles retrieved, 12 were selected. These studies included people with mental disorders (mood disorder, panic disorder, or obsessive-compulsive disorder), psychiatric symptoms (depression, anxiety, attention deficit, etc.), and people in specific groups (college students or nurses). The apps were based on cognitive-behavioral therapy, cognitive training, well-being cognition technique, or relaxation techniques. The apps were effective in reducing depression, anxiety, panic attacks, attention deficit, negative emotions, and dysfunctional attitudes, and boosting memory and positive emotions.
Conclusion
Mobile apps with well-established, theory-based content can serve as an effective intervention for managing several mental health symptoms. However, the scope of the research subjects and symptoms in Korea remains limited. Further studies with more varied populations and symptoms are required.
7.Current Status of Outsourced Food Service Operations According to the Type of Long-Term Care Institution and Plans for Improvement

This study aimed to explore the status of food service outsourcing behavior of long-term care institutions (LTCIs) through a cross-sectional survey using a questionnaire administered between July 16th and August 7th, 2020. The survey respondents were either dietitians or facility managers, who worked at 731 nursing homes, 477 group homes, and 673 day-care centers. Approximately 25.9% of nursing homes, 11.7% of group homes, and 33.1% of day-care centers used a managed-services company to operate their food service units.The main reason for outsourcing food service by nursing homes was related to the staffing of dietitians and cooks, whereas group homes and day-care centers outsourced food services due to factors relating to meal costs and the cooking process. Almost all the LTCIs entered into private contracts for outsourced food services. Only a few food service contracts included the types of meals, nutrition standards such as protein and calories per meal, and the parameter or ratio of food cost. Of the respondents, 84.5% from nursing homes, 87.5% from group homes, and 87.1% from day-care centers agreed that the quality of outsourced food services of the LTCIs should be regulated. Meals are essential for maintaining the health and functional status of LTCI users. As more LTCIs outsource their food services, we suggest the following: (1) Increasing the minimum dietitian staffing standards for LTCIs as per the Welfare of Senior Citizens Act and requiring at least one dietitian for every nursing home, (2) Making it mandatory to use a standard food service contract template when drafting food service contract, and (3) Developing realistic standards for food service operations considering the size and operation type of the LTCIs.
8.Temporary blindness caused by corneal edema after a local anesthetic injection in the eyebrow region: a case report

Plastic surgery around the eyes is usually performed under local anesthesia, using a mixture of lidocaine and epinephrine. Blindness is a rare but devastating complication after the injection of local anesthesia in this region. Most cases reported to date have been caused by occlusion of the ophthalmic artery or central retinal artery. In this case report, however, we present a highly unusual case of blindness caused by corneal edema after a local anesthetic injection. A patient visited the emergency room with a laceration on the eyebrow, and local anesthesia was injected before suturing. Immediately after the injection, severe corneal edema developed, making it impossible to observe the structures in the anterior chamber in detail or check the light reflex and visual acuity of the naked eye. An antibiotic (moxifloxacin hydrochloride) and high-concentration steroid eyedrops were promptly applied. High-concentration steroids were also administered orally. On day 13 post-injury, the visual acuity of the naked eye improved to 1.0, and no recurrence of corneal lesions was observed. Although the cause of corneal edema after the local injection could not be conclusively identified, we hope that this report will help raise clinicians’ awareness of this complication and appropriate treatment methods.
9.Anesthetic Management during Electroconvulsive Therapy

Some patients with depression require electroconvulsive therapy performed undergeneral anesthesia. 10.Effects of various methods of dexmedetomidine administration for sedation in elderly patients undergoing spinal anesthesia: a randomized controlled study

Background:
The purpose of this study was to investigate the degree of sedation and the incidence of adverse effects resulting from various methods of administering the initial dose followed by continuous infusion of dexmedetomidine (DEX) for sedation in elderly patients undergoing spinal anesthesia.
Methods:
In total, 72 patients aged over 65 years who were to be administered spinal anesthesia were randomly allocated into three groups. The initial doses were injected to the groups as follows: group DD, DEX 0.5 μg/kg for 10 min; group MD, midazolam 0.02 mg/kg; and group D, no initial dose. This was followed immediately by infusing a maintenance dose of DEX 0.5 μg/kg/h to all groups.
Results:
The Bispectral index (BIS) in the D group was significantly higher than in the other two groups. There were no significant differences in the Ramsay sedation scale (RSS) among the groups. The RSS 3 level was reached in 10 min from the start of sedation in MD and DD groups and in 20 min from the start of sedation in D group. Neither bradycardia nor hypotension was observed in any of the groups.
Conclusions
Patients in all three groups reached the RSS 3 sedating-effect level. However, the group that received continuous infusion only without the initial dose showed higher BIS than the other two groups and reached the RSS 3 later. No adverse events were observed in any of the groups.
