1.The application study of bronchial arterial embolization with blank microspheres combined with microcoils in the treatment of hemoptysis

Objective To investigate the efficacy and safety of blank microspheres combined with microcoils in treating hemopty-sis due to arteriae bronchiales(BA)malformation during bronchial arterial embolization(BAE).Methods A retrospective analysis was conducted on the preoperative data,embolization materials,technical success rate,clinical success rate,postoperative recurrence rate,and complications of 87 patients who underwent BAE using blank microspheres for hemoptysis due to BA malformation.Results There were 87 patients in this study,and BA computed tomography angiography(CTA)classification was as follows:32 cases(36.78%)of type Ⅰ,23 cases(26.44%)of type Ⅱ,19 cases(21.84%)of type Ⅲ,and 13 cases(14.94%)of other types(Ⅳ-Ⅸ).A total of 171 blood vessels were embolized in 87 patients,including 16 ectopic BA,and 130 embolized microspheres,including 104 microspheres of 500-700 μm and 26 of 700-900 μm.There were 265 microcoils,with a technical success rate of 100%,a 24 h clinical success rate of 100%,and a 30 d clinical success rate of 98.85%.The recurrence rate was 3.45% at 6 months,5.75% at 12 months,and 8.05% at 24 months.Postopera-tive complications(Clavien-Dindo classification):complications of grade Ⅰ,embolic syndrome recurrence rate was 8.05%,chest pain incidence was 3.45%,dysphagia incidence was 2.30%,BA injury incidence was 3.45%;complications of grade Ⅳ,paraplegia inci-dence was 1.15%.Conclusion The use of blank microspheres combined with microcoils in the treatment of hemoptysis caused by BA malformation has high safety and clear efficacy.
2.A blood supply model for the emergency care of severe trauma

Objective: To establish and validate a whole blood (WB) supply model, thereby providing practical experience for the clinical application of WB in domestic trauma emergency care and informing the development of a wartime blood supply system for the military. Methods: A “10×24” WB supply model was established by formulating blood collection protocols, storage standards, and transfusion criteria. Multiple WB samples were tested under specific storage conditions to assess key indicators at different time points, including red blood cell (RBC), white blood cell (WBC), and platelet counts, hemoglobin concentration, coagulation parameters (PT, APTT, TT, FIB), coagulation factor activity, thromboelastography (TEG) parameters, and electrolyte levels. Additionally, clinical data from hemorrhagic patients who met the criteria for WB transfusion and were admitted between March and July 2024 were analyzed to evaluate WB transfusion volume. Results: RBC counts and hemoglobin levels remained stable in WB stored at 4℃ for up to 10 days. However, platelet counts and coagulation function (PT, APTT) significantly declined with prolonged storage, while potassium levels increased. From March to July 2024, the model was successfully applied to 23 patients with acute hemorrhage, with a median WB transfusion volume of 543 mL. A detailed case study of a severe traumatic hemorrhagic shock patient was reported, who was successfully treated with 5.5 units of refrigerated WB combined with component blood. Conclusion: The “10×24” WB supply model demonstrated acceptable changes in critical quality parameters under strict management and a 10-day rotation cycle. This model effectively supports the treatment of acute hemorrhage and holds promise for integration into the future wartime blood supply system of the military.
3.Value of Detection of Serum Exosome hsa_circ_0084443 in Diabetic Foot Ulcer Patients as Early Clinical Diagnosis

Objective To analyze the feasibility of serum exosome hsa_circ_0084443 as a biomarker for early diagnosis of diabetic foot ulcer.Methods GSE114236 chip data from the Gene Expression Omnibus(GEO)database was downloaded to screen the differentially expressed circular DNA(circRNA).From December 2019 to August 2021,44 T2DM patients with diabetic foot ulcer(DFU)who received treatment at Baoji Central Hospital were included as the study subjects,all patients had Wagner grades 1~2.In addition,185 newly diagnosed T2DM patients without comorbidities(DM group)were included as controls during the same period.Serum samples from all participants were collected for extracting purified serum exosomes.The expression of serum exosomal hsa_circ_0084443 was detected by real-time fluorescence quantitative polymerase chain reaction(qRT-PCR),and the differences among different groups was compared.Spearman rank correlation analysis was used to investigate the relationship between serum exosomal hsa_circ_0084443 and Ankle-Brachial Index(ABI).Receiver operating characteristic(ROC)curves was used to analyze the efficacy of serum exosomal hsa_circ_0084443 in diagnosing early DFU.Results By analyzing the GSE114236 dataset,it was found that hsa_circ_0084443 was the circRNA with the highest differential expression.In the whole cohort analysis,the expression of serum exosome hsa_circ_0084443 in DFU group was significantly higher than that in DM group[1.18(0.84,2.06)vs 0.73(0.51,1.16)],and the differences was statistically significant(Z=-4.219,P<0.001).The expression level of serum exosome hsa_circ_0084443 in DFU group was negatively correlated with ABI(r=-0.420,P=0.005).After adjusting for age,smoking history and creatinine,the increased expression level of serum exosome hsa_circ_0084443 was still an independent risk factor for DFU(P<0.001).Finally,the area under ROC curve(AUC)of serum exosome hsa_circ_0084443 for the diagnosis of early DFU was 0.705.Conclusion Serum exosome hsa_circ_0084443 is highly expressed in DFU patients and can be used as an important marker for early diagnosis of DFU.

6.Spatial transcriptome analysis of long non-coding RNAs reveals tissue specificity and functional roles in cancer.

Long non-coding RNAs (lncRNAs) play a significant role in maintaining tissue morphology and functions, and their precise regulatory effectiveness is closely related to expression patterns. However, the spatial expression patterns of lncRNAs in humans are poorly characterized. Here, we constructed five comprehensive transcriptomic atlases of human lncRNAs covering thousands of major tissue samples in normal and disease states. The lncRNA transcriptomes exhibited high consistency within the same tissues across resources, and even higher complexity in specialized tissues. Tissue-elevated (TE) lncRNAs were identified in each resource and robust TE lncRNAs were refined by integrative analysis. We detected 1 to 4684 robust TE lncRNAs across tissues; the highest number was in testis tissue, followed by brain tissue. Functional analyses of TE lncRNAs indicated important roles in corresponding tissue-related pathways. Moreover, we found that the expression features of robust TE lncRNAs made them be effective biomarkers to distinguish tissues; TE lncRNAs also tended to be associated with cancer, and exhibited differential expression or were correlated with patient survival. In summary, spatial classification of lncRNAs is the starting point for elucidating the function of lncRNAs in both maintenance of tissue morphology and progress of tissue-constricted diseases.

7.Fasting Blood Glucose, Cholesterol, and Risk of Primary Liver Cancer: The Kailuan Study

Purpose:
The influence of fasting blood glucose (FBG) and cholesterolemia primary liver cancer (PLC) in china was analyzed via a large prospective cohort study based on a community population, and the combined effects between them were investigated.
Materials and Methods:
Overall, 98,936 staff from the Kailuan Group who participated in and finished physical examinations between 2006 and 2007 were included in the cohort study. Their medical information was collected and they were followed up after examination. The correlations of serum FBG or TC with PLC were analyzed. Then, we categorized all staff into four groups: normal FBG/ non-hypocholesterolemia, normal FBG/hypocholesterolemia, elevated FBGon-hypocholesterolemia, elevated FBG/hypocholesterolemia and normal FBG/ non-hypocholesterolemia was used as a control group. The combined effects of elevated FBG and hypocholesterolemia with PLC were analyzed using the Age-scale Cox proportional hazard regression model.
Results:
During 1,134,843.68 person*years follow up, a total of 388 PLC cases occured. We found the elevated FBG and hypocholesterolemia increases the risk for PLC, respectively. Compared with the non-hypocholesterolemiaormal FBG group, the risk of PLC was significantly increased in the non-hypocholesterolemia/elevated FBG group (HR=1.19,95%CI 0.88–1.62) and hypocholesterolemiaormal FBG group (HR=1.53,95%CI 1.19–1.97), and in the hypocholesterolemia/elevated FBG group (HR=3.16 95%CI2.13-4.69). And, a significant interaction effect was found of FBG and TC on PLC. All results were independent from the influence of liver disease.
Conclusion
Elevated serum FBG and hypocholesterolemia are risk factors for PLC, especially when combined. Thus, for the prevention and treatment of PLC, serum FBG and TC levels should be investigated.
