Objective To study the impact of tympanic membrane opening on respiratory-driven middle ear pressure in patients with patulous eustachian tube(PET),using a simplified in vitro model. Methods CT imaging data from a PET patient(with full-length eustachian tube opening observed during a Valsalva maneuver followed by breath-holding)were used to design a simplified eustachian tube model. Two simplified in vitro models of the eusta-chian tube were constructed using silicone-based 3D printing technology and connected to a pressure controller and pressure sensors. The pressure controller was activated to introduce negative-pressure airflow into the nasopharyn-geal model to simulate respiratory-induced middle ear pressure fluctuations.A hemostat was used to alternately open and close the external interface of the middle ear chamber, simulating conditions of an open and intact tympanic membrane, while middle ear pressure was continuously monitored using pressure sensors. Results In the first mod-el,with-800 mbar negative pressure applied at the nasopharynx, the middle ear pressure stabilized between-3.9 mbar and-4.3 mbar with tympanic membrane opening, and between-7.9 mbar and-8.2 mbar with intact tym-panic membrane. In the second model, under the same pressure setting, middle ear pressure stabilized between-2.7 mbar and-3.1 mbar with tympanic membrane opening, and between-5.0 mbar and-7.7 mbar with intact tympanic membrane. Conclusion This study, based on a simplified in vitro model, demonstrates that tympanic membrane opening can effectively reduce respiratory-driven pressure in the middle ear. This phenomenon may partly explain the clinical efficacy of tympanostomy tube insertion in certain PET patients.

Objective:To establish a method for the rapid detection of varicella-zoster virus (VZV) by recombinase-aid amplification (RAA) combined with Clustered regularly interspaced short palindromic repeats (CRISPR)/Cas12a system. Methods:Clinical samples of suspected herpes zoster in Shandong province and Shanghai from 2023 to 2024 were collected, nucleic acids of positive samples were extracted, RAA-specific primers and crRNA (CRISPR RNA, crRNA) were designed for the conserved region of VZV, and the fluorescence intensity generated by Cas12a non-specific cleavage of single-stranded fluorescent probes was used to screen highly sensitive crRNAs and optimize the concentrations of crRNA, Cas12a and ssDNA probes. The sensitivity and reproducibility of the RAA-CRISPR/Cas12a detection method were evaluated by using synthesized plasmids and clinical samples, and the specificity of the method was evaluated by using other viral nucleic acids. The method was used to detect clinical samples by using the method and quantitative real-time PCR (qPCR) method, and the detection rate and consistency of the two method were compared. Results:The highly sensitive crRNA-4 was screened from the four crRNAs designed, and a VZV detection method for RAA-CRISPR/CAS12a based on fluorescence intensity measurement was established, which could be detected at 37℃ in 45 min, and the sensitivity of the detection could reach 10 copies/μL, a minimum clinical sample with a Ct value of 38.980 can be detected. It has high specificity and no cross-reactivity with Adenovirus 7, Herpes simplex virus type I, Herpes simplex virus type II, Coxsackieviruses A16, Cytomegalovirus, Epstein-Barr virus, Measles virus, Mumps virus, Enterovirus 71, Japanese encephalitis virus genotype 5. It has good stability, and can be successfully detected in low, medium and high concentrations of viral positive plasmids with good consistency. The detection rate of the clinically positive samples was 100%, which was completely consistent with the qPCR test result. Conclusions:RAA isothermal amplification technology combined with CRISPR-CAS12a technology was used to establish an accurate method for the detection of VZV virus, which was highly sensitive, specific, and had low requirements for experimental conditions, and could be completed within 45 min, which could provide strong technical support for the early detection of VZV.

Objective·To construct a probiotic(Escherichia coli Nissle1917,EcN)system(EcN@PVA-ALG)loaded on polyvinyl alcohol(PVA)/alginate(ALG)hydrogel(PVA-ALG)rich in negative hydroxyl groups, and to explore its colonization in the inflammatory site of colon and its therapeutic effect on dextran sulfate sodium salt(DSS)-induced inflammatory bowel disease(IBD).Methods·EcN suspension was added to the PVA-ALG hydrogel, and then EcN@PVA-ALG hydrogel probiotic complex was obtained after screening and centrifugation. The synthesis of PVA-ALG hydrogel was verified by rheometer. The surface charge of EcN@PVA-ALG was detected by potentiometer and the load of EcN on PVA-ALG was observed by fluorescence microscope. The absorbance of EcN@PVA-ALG at 600 nm was detected by enzyme labeling instrument.Meanwhile, the bacterial plate count of EcN@PVA-ALG complex suspension was taken to study the growth activity of EcN in EcN@PVA-ALG.The CCK-8 kit was used to assess the inhibitory ability of EcN@PVA-ALG on HEK cell proliferation. In vivo imaging system(IVIS)was used to firstly analyze the enrichment of PVA-ALG on inflammatory colon to study its inflammatory targeting property;then EcN was loaded on PVA-ALG,and IVIS was used to observe the enrichment of EcN@PVA-ALG on inflammatory colon to study its ability to colonize the inflammatory site.To establish the model of IBD mice induced by DSS,EcN@PVA-ALG group(n=5)was given 1×108CFU EcN@PVA-ALG every day for 5 d,and PVA-ALG group,EcN group,PBS group and healthy control group with 5 mice were set up.During the treatment, the body mass of the mice was recorded every day. After treatment, the colonic tissue was taken, and the length of colon was measured. The disease activity index(DAI)score was graded. The levels of inflammatory cytokines, including tumor necrosis factor-α(TNF-α),interleukin(IL)-6,IL-10 and transforming growth factor-β(TGF-β)were detected, and the pathological evaluation of colonic tissue was made by H-E staining. Results·Both PVA-ALG and EcN@PVA-ALG were negatively charged.EcN was successfully loaded onto PVA-ALG and PVA-ALG did not affect the growth viability of EcN,which contributed to the subsequent colonization of inflammatory colons.PVA-ALG had a favorable safety profile on normal cells.Compared with healthy controls,PVA-ALG had more than 2-fold enrichment effect on inflammatory colon tissue. In vitro and in vivo experiments revealed that EcN@PVA-ALG complex loaded with EcN had 8 times higher enrichment effect on inflammatory tissue than EcN without any modification. After EcN@PVA-ALG treatment, the body weight of mice recovered rapidly. The increase of DAI was significantly inhibited. The length of colon was similar to that of healthy mice. The levels of TNF-α and IL-6 decreased, while the levels of IL-10 and TGF-β increased. The crypt structure of colon tissue recovered. Conclusion·Compared to unmodified EcN,EcN@PVA-ALG promotes the colonization of EcN at inflammatory sites of colon and allows it to exert better efficacy on treating DSS-induced IBD.
