1.Varicella vaccination status in Changsha City: a study of birth cohort

Objective To analyze the varicella vaccination rate in the 2012-2023 birth cohort in Changsha. Methods The varicella vaccination information of all children born in Changsha from 2012 to 2023 was collected through the Hunan Provincial Immunization Planning Information System. The varicella epidemic information from 2012 to 2023 was collected through the China Information System for Disease Control and Prevention. The varicella vaccination rate and its characteristics of children in the birth cohort were descriptively analyzed. Results The first dose vaccination rate of varicella vaccine for children born from 2012 to 2023 was 64.82 %, and the full vaccination rate was 40.87 %. The full vaccination rate of the birth cohort in each year ranged from 18.11 % (2020 birth cohort) -73.07 % (2017 birth cohort), showing an overall increasing trend year by year. Conclusion The varicella vaccination rate has increased among children born between 2012 and 2023 in Changsha, but overall coverage is low. It is necessary to strengthen the publicity of varicella vaccine knowledge and promote timely vaccination of school-age children.
2.Reliability and validity analysis of quantitative diagnostic criteria for hysterosalpingography in clinical trials

Objective To conduct a reliability and validity analysis of the quantitative diagnostic criteria for hysterosalpingography(HSG)in clinical trials.Methods Questionnaire survey method was used for this study.A test questionnaire scale was constructed based on the imaging data of 10 patients who had received HSG.Fifteen medical workers engaged in HSG-related clinical and scientific research work were invited to score the test questionnaire according to quantitative diagnostic criteria.Internal consistency testing,correlation analysis,and exploratory factor analysis were used to evaluate the reliability and validity of the quantified diagnostic criteria.Results The overall Cronbach's α coefficient for this quantitative diagnostic criteria was 0.834.The Cronbach's α coefficients for the three evaluation functions,including tubal patency,tubal adhesion and pelvic adhesion,were 0.722,0.627 and 0.724 respectively.The Spearman-Brown split-half reliability coefficient was 0.859,and the Guttman split-half coefficient was 0.830.The inter-rater reliability measured by the Kendall W coefficient was 0.806(P<0.05).The analysis indicated that the evaluation function for pelvic adhesion was suitable for factor analysis(KMO value=0.573,Bartlett Spherical test P=0.003).Two common factors were extracted(characteristic root being set at 1),and the cumulative variance interpretation rate was up to 74.74%.These factors were named as"items with low ambiguity"and"items with potential ambiguity"respectively.Conclusion The overall reliability and validity of this quantitative diagnostic criteria are satisfactory,it can meet the requirements of HSG-related clinical trials and has the potential for clinical promotion.
3.Discussion on Coronary Microcirculation Disorder after Myocardial Ischemia Reperfusion Based on “Collaterals-Sweat Pore Qi and Fluid” Theory

Coronary microcirculation disorder after myocardial ischemia reperfusion (MIR) is a prominent problem in the treatment of coronary heart disease. According to the physiological commonality between “collaterals-sweat pore qi and fluid” and coronary microcirculation, and the evolution of the course of MIR, it is believed that “heart collateral stasis obstruction, sweat pore constraint and block” is the cause of coronary microcirculation disorder. The evolution of the pathogenesis can be divided into three periods. During the myocardial ischemia period, the pathogenesis is heart collaterals obstruction and sweat pores empty, while during the ischemia reperfusion period, it is internal formulation of deficiency wind, spasms of collaterals or slight heart collaterals obstruction; in the coronary microcirculation disorder period, sweat pores constraint and block, constraint transforming into heat, qi and fluid failing to diffuse are the pathogenesis. The corresponding treatment principle is assisting dredge with supplementation, and supplementing deficiency to dispel stasis; treating wind and blood simultaneously, and extinguishing wind to arrest convulsion; clearing heat and cooling blood, and diffusing qi and unblocking qi and fluid. Moreover, it is recommended to treat the heart and lungs simultaneously, and regulate the heart and liver at the same time.
4.Effect and Mechanism of Exercise for Patients with Cardiovascular Diseases (review)

Exercise is one of the important techniques of cardiovascular rehabilitation. Exercise can reduce inflammatory response to improve endothelial function, and improve mitochondrial function to increase myocardial cell activity. For cardiovascular risk factors, exer-cise can promote the activity of lipoprotein, increase the level of high-density lipoprotein;improve the function of insulin receptor to reduce insulin resistance, reduce platelet aggregation and improve endothelial function to reduce blood pressure. For the respiratory system, aerobic exercise can improve the function of respiratory muscle, thus relieve the dyspnea. Exercise can promote the activation of immune factor and increase metabolism, to increase immune function and anti-aging. Resistance exercise can improve mitochondrial function and promote fi-ber type conversion, to improve the function of skeletal muscle system.
5.Construction of cardiac atrioventricular electrical conduction pathway by rabbit bone marrow mesenchymal stem cells

BACKGROUND:Autologous stem celltransplantation to the heart is a research direction of heart failure treatment, but there are relatively few of studies about autologous bone marrow mesenchymal stem celltransplantation targeting the cardiac conduction system. OBJECTIVE:To evaluate the potential of rabbit bone marrow mesenchymal stem cells for treatment of heart block. METHODS:Rabbit bone marrow mesenchymal stem cells were induced by 5-azacytidine to differentiate into cardiomyocyte-like cells. After thoracotomy, the left atrium-left ventricular anterior wal was sutured in 14 New Zealand white rabbits (8 in the experimental group and 6 in the control group). One month after the surgery, in the experimental group, autologous bone marrow mesenchymal stem cells induced by 5-azacytidine for 4 weeks were labeled with 4',6-diamidino-2-phenylindole and then injected into the suture area when opening the thoracic again. In the control group, cells cultured in medium were used. One month after celltransplantation, the third thoracotomy was done to insert electrodes into the left atrium and left ventricular anterior wal , for cardiac electrophysiological detection. Whether atrioventricular pathway formed in the suture area was observed. RESULTS AND CONCLUSION:After cells were transplanted into the sutured area, two rabbits in the experimental group were discovered to form the atrioventricular pathway in the sutured area through cardiac electrophysiological examination. After transplantation, transplanted cells were visible on the heart frozen sections under fluorescence microscope in the left ventricle and sutured area, but there was no cellin the control group. In the experimental group, bone marrow mesenchymal stem cells expressed Cx43 and formed gap junction intercellular communication with cardiomyocytes, which was presented as formation of the atrioventricular pathway on cardiac electrophysiology examination. These findings indicate that bone marrow mesenchymal stem cells can be used to treat cardiac conduction system block diseases.
