1.Construction and efficacy verification of an intelligent pharmaceutical Q&A platform based on AI hallucination-suppression

OBJECTIVE To construct an intelligent pharmaceutical Q&A platform for precision medication with low “artificial intelligence (AI) hallucination”, aiming to enhance the accuracy, consistency, and traceability of medication consultations. METHODS Medication package inserts were batch-processed and converted into structured data through Python programming to build a local pharmaceutical knowledge base. The retrieval and question-answering processes were designed based on large language models, and system integration and localized deployment were completed on Dify platform. By designing typical clinical medication questions and comparing the output of the intelligent pharmaceutical Q&A platform with the online version of DeepSeek across dimensions such as peak time retrieval, half-life, and dosage adjustment reasoning for patients with renal impairment, the accuracy and reliability of its retrieval and reasoning results were evaluated. RESULTS The intelligent pharmaceutical Q&A platform, constructed based on local drug package inserts, achieved 100% accuracy in retrieval and reasoning for peak time, half-life, and dosage adjustment schemes. In comparison, the online version of DeepSeek demonstrated accuracies of 30%(6/20), 50%(10/20), and 38%(23/60) across these three dimensions, respectively. CONCLUSIONS The constructed intelligent pharmaceutical Q&A platform is capable of accurately retrieving and extracting information from the local knowledge base based on clinical inquiries, thereby avoiding the occurrence of AI hallucinations and providing reliable medication decision support for healthcare professionals.
2.Comparison of multilevel anterior cervical discectomy and fusion utilizing zero-profile self-locking cages versus the cage-with-plate system: a 5-year-minimum follow-up study

Methods:
Sixty-two patients (2014–2019) were divided into the SC (n=32) and PC (n=30) groups. Clinical outcomes were evaluated using the Visual Analog Scale (VAS), modified Japanese Orthopedic Association (mJOA) score, and Neck Disability Index (NDI). Radiological parameters, including cervical lordosis (CL), fused segment angle (FSA), and disc height (DH), were assessed. Cage subsidence, adjacent segment degeneration (ASD), and other complications were also documented. The relationship between postoperative radiological changes and symptoms was also analyzed.
Results:
Both groups showed significant improvement in symptoms, with fusion achieved in all surgical segments. The SC group had a shorter operative time and reduced intraoperative blood loss. However, the SC group had a higher subsidence rate (16.7% vs. 5.6%, p=0.017). The PC group maintained better cervical alignment in terms of CL, FSA, and DH (p<0.05), with an increased incidence of dysphagia and ASD (p<0.05). Spearman correlation analyses showed that neither CL nor FSA loss at the 5-year follow-up was significantly associated with improvements in the mJOA, VAS, and NDI scores (p>0.05) in either group. Regression analyses indicated that subsidence or loss of lordosis did not correlate with the degree of postoperative symptoms (p<0.05).
Conclusions
Both SC and PC achieved sustained pain relief, neurological improvement, and solid fusion in three-level ACDF at the 5-year follow-up. SC demonstrated significant advantages in reducing dysphagia and ASD, while PC better maintained cervical alignment and segmental stability. Patient-specific factors should be used to guide implant selection. Further multicenter studies are required to validate these findings.
3.The impact of DIP2B on the proliferation, migration and invasion abilities of gastric cancer cells

ObjectiveTo investigate the role of disco interacting protein 2 homolog B (DIP2B) in the proliferation, migration, and invasion processes of gastric cancer cells. MethodsBioinformatic analysis was performed using public datasets, including TCGA, GEPIA and TIMER2.0, to assess DIP2B expression and its clinical prognostic value in gastric cancer. DIP2B expression levels in gastric cancer cell lines were validated by Western blot and RT-qPCR. A DIP2B knockdown cell model was established via siRNA mediated gene silencing, and the knockdown efficiency was confirmed. Cell proliferation was assessed using real-time cellular analysis (RTCA) and colony formation assays. Cell migration and invasion were evaluated by Transwell assays, while flow cytometry was employed to detect changes in apoptosis and cell cycle distribution. ResultsDIP2B was significantly upregulated in gastric cancer cells and correlated with poor prognosis in patients (P<0.05). The experimental results revealed that DIP2B knockdown markedly suppressed the proliferation, migration, and invasion of gastric cancer cells, while promoting apoptosis and inducing cell cycle arrest (P<0.05). ConclusionDIP2B is highly expressed in gastric cancer and associated with unfavorable prognosis. Knockdown of DIP2B inhibits the proliferation, migration, and invasion of gastric cancer cells and promotes apoptosis, suggesting that DIP2B may serve as a potential prognostic biomarker and therapeutic target for gastric cancer.
4.Clinical Study on Treatment of Melasma Using Angelica and Safflower Spot Removal Essence Based on State-target Medicine

ObjectiveTo investigate the clinical efficacy and safety of state-target formula Angelica and Safflower spot removal essence in the treatment of melasma. MethodsA randomized,double-blind,placebo-controlled parallel-group trial was conducted,selecting patients with Qi stagnation and blood stasis type of melasma who visited the dermatology outpatient department of Beijing Hospital of Integrated Traditional Chinese and Western Medicine from September 2022 to February 2023. In accordance with the double-blind principle,a random number table was generated to randomly divide the patients into an experimental group (50 cases) and a control group (50 cases). The experimental group applied the Angelica and Safflower spot removal essence topically,while the control group applied a placebo topically,with continuous treatment for 8 weeks and a follow-up of 1 month. Melasma Area and Severity Index (MASI),Physician Global Assessment (PGA),patient self-assessment scoring,VISIA detection and scoring were performed before medication and on the first day after stopping medication,and all adverse events were recorded. ResultsIn a study involving 100 patients,87 cases were included in the analysis. The experimental group consisted of 45 cases,with 5 dropouts,and the control group had 42 cases,with 8 dropouts. The baseline data of the patients in both groups were consistent,with no statistically significant differences. After treatment,the total effective rate of the experimental group was 75.56% (34/45),significantly higher than the control group's 2.38% (1/42),and the difference was statistically significant (χ2=48.38,P<0.01). Compared with before treatment,after treatment,the MASI score,VISIA image spot,brown spot,and red area score of the experimental group patients were significantly reduced (P<0.05,P<0.01),while there was no statistically significant difference in the control group patients. Compared with the control group after treatment,the experimental group showed more significant improvement (P<0.05,P<0.01). VISIA image analysis showed that after 8 weeks of treatment,the experimental group showed significant improvement in surface spots,deep spots,and red areas,while the control group remained unchanged or showed an increasing trend. 42.22% (19/45) of the experimental group had a PGA score of 3 or below,while 88.89% (40/45) had a PGA score of 4 or below. 80.95% (34/42) of the control group had a PGA score of 5 or above,and the difference between the two groups was statistically significant (χ2=38.26,P<0.01). There was no statistically significant difference in the recurrence rate and adverse reaction rate within one month between the experimental group and the control group. ConclusionThe state-target formula Angelica and Safflower spot removal essence is clinically effective and safe in the treatment of melasma.
5.Exploration on the Syndrome Differentiation and Treatment Strategies for Inflammation-Cancer Transformation in Inflammatory Bowel Disease Based on the Theory of Cold Qi-Induced Accumulation

It is proposed that cold qi-induced accumulation encapsulates the core pathogenesis of the inflammation-cancer transformation in inflammatory bowel disease (IBD). Cold pathogens may serve as the initiating factor. When first invading the intestines, cold pathogens obstruct the flow of qi; over time, the lingering cold impairs the middle jiao (焦), eventually leading to the accumulation of cold-phlegm and blood stasis. Based on the progressive nature of this transformation, the process can be divided into three stages, active stage, remission stage, and carcinogenic stage. In the active stage, the main pathogenesis involves stagnation of cold qi and accumulation of damp-heat in the intestines; in the remission stage, cold qi impairs the spleen, disrupting its transport and transformation functions; and in the carcinogenic stage, the mechanisms include cold-induced accumulation, phlegm accumulation from cold, and stagnation of cold and blood stasis. Accordingly, the treatment strategies are proposed.In the active stage, regulating qi, relieving stagnation, and harmonizing cold and heat; in the remission stage, warming yang, dispersing cold, tonifying qi, and strengthening the spleen; and in the carcinogenic stage, promoting qi circulation, dispersing cold, resolving phlegm, activating yang, and eliminating stasis to remove accumulation. These approaches aim to interrupt the transformation of IBD into colorectal cancer.
6.Effect and mechanism of tetramethylpyrazine on atherosclerotic plaques in ApoE-/-mice

Aim To investigate the effect and potential mechanisms of tetramethylpyrazine(TMP)on athero-sclerotic plaques.Methods 43 ApoE-/-mice were used to establish the animal model of atherosclerosis(As)by high-fat diet for 8 weeks,3 of which were used for model outcome verification,and another 40 model mice were randomly divided into model group,TMP low dose(25 mg/kg)group,TMP medium dose(50 mg/kg)group,TMP high dose(100 mg/kg)group and atorvastatin(AT,2.6 mg/kg)group,with 8 mice in each group;another 8 C57BL/6J mice were set as control group.After gavaging administration for 8 weeks,the levels of total cholesterol(TC),triglyceride(TG),low density lipoprotein(LDL),high density lipoprotein(HDL)in the serum were detected by biochemical methods,and the As index was calculated.The levels of oxidized low density lipoprotein(ox-LDL),tumor necrosis factor-α(TNF-α),interleukin-1β(IL-1β),monocyte chemoattractant protein-1(MCP-1),intercellular adhesion molecule-1(ICAM-1)in serum were detected by ELISA method.The pathological changes of aorta was evaluated by HE staining.The aortic plaque formation was evaluated by oil red O staining,and the plaque area percentage was calculated.The aortic fibrosis was evaluated by Masson staining,and the collagen area percentage was calculated.The expression of monocyte macro-phage antibody-2(MOMA-2)and α-smooth muscle actin(α-SMA)in the aorta was detected by immunohistochemistry(IHC)method,and the plaque vulnerability index was calculated.The mRNA and protein expression of peroxisome pro-liferator-activated receptor γ(PPARγ),nuclear factor-κB p65(NF-κB p65)in aorta were detected by RT-qPCR or West-ern blot method.Results Compared with control group,the levels of TC,TG,LDL,ox-LDL,TNF-α,IL-1β,MCP-1,ICAM-1 in serum and As index of the mice were significantly increased in model group,while the level of HDL was sig-nificantly decreased(P<0.05).The aorta showed pathological changes such as uneven thickening of the intima,accu-mulation of foam cells and fat cells,formation of a large number of plaques,lumen stenosis and infiltration of inflammatory cells;the percentage of aortic plaque area,percentage of collagen area,MOMA-2 and α-SMA positive area,plaque vulner-ability index were all significantly increased(P<0.05).The mRNA and protein expression of PPARγ in aorta were sig-nificantly decreased,and the mRNA and protein expression of NF-κB p65 were significantly increased(P<0.05).Com-pared with model group,the levels of TC,TG,LDL,ox-LDL,TNF-α,IL-1β,MCP-1,ICAM-1 in serum and As index of the mice were significantly decreased in the TMP medium,high dose group and AT group,the level of HDL was signifi-cantly increased(P<0.05).The pathological changes of aorta were significantly improved.The plaque area percentage,collagen area percentage,MOMA-2 positive area percentage and plaque vulnerability index were significantly decreased,and the α-SMA positive area percentage was significantly increased(P<0.05).The mRNA and protein ex-pression of PPARγ in aorta were significantly increased,the mRNA and protein expression of NF-κB p65 were significantly decreased(P<0.05).Moreover,TMP exhibited a certain dose-dependent effect on various detection indicators(except MCP-1)in As mice.The regulatory effect of TMP high dose group on various detection indicators(except LDL and As index)in As mice was comparable or superior to those of AT group.Conclusion TMP can reduce the area of As plaque and improve the stability of vulnerable plaque in As mice,its mechanism may be related to regulating PPARγ/NF-κB signaling pathway,improving lipid metabolism and inhibiting inflammatory response.
7.Radiofrequency Ablation for T1N0M0 Papillary Thyroid Carcinoma with Rich Blood Supply Versus Poor Blood Supply

Purpose To evaluate the long-term efficacy of radiofrequency ablation(RFA)for T1N0M0 papillary thyroid carcinoma(PTC)with rich blood supply and poor blood supply.Materials and Methods Clinical data,ultrasound and contrast-enhanced ultrasound(CEUS)images,RFA parameters,and postoperative follow-up indicators of 375 T1N0M0 PTC patients who received RFA from June 2014 to December 2018 were retrospectively collected.The propensity score matching method was used to match the baseline data of the two groups in a 1:1 ratio,finally a total of 212 patients were included in the study.The RFA parameters,volume,tumor disappearance,and local tumor progression(LTP)were compared between the groups.Besides,the correlation between rich blood supply and LTP was analyzed using multivariate COX regression.Results According to the peak intensity displayed by preoperative CEUS,all lesions were divided into a group with rich blood supply(n=129)and another with poor blood supply(n=246).After a mean follow-up time of(84.48±14.44)months,the tumor disappearance in the rich blood supply group was 87.74%,while in the poor blood supply group it was 88.68%,and there was no statistically significant difference(Z=0.05,P=0.831).There were 10 cases of LTP in the rich blood supply group(6 cases of residual cancer and 4 cases of new cancer),and 6 cases of LTP in the poor blood supply group(2 cases of residual cancer and 4 cases of new cancer).The LTP rates of the two groups were 9.43%and 5.67%,with no statistically significant difference(x2=1.08,P=0.298).The Kaplan-Meier survival curve showed no statistically significant difference in progression free survival between both groups(P=0.265).The adjusted multivariate COX proportional risk model analysis showed that rich blood supply was no association with LTP(HR=1.54,P=0.409).Conclusion RFA for T1N0M0 PTC with rich blood supply can achieve effective ablation,and its long-term efficacy is similar to that of poor blood supply.RFA can serve as an effective alternative treatment for T1N0M0 PTC patients with different blood supply.
8.Clinical,CT and MRI manifestations of CIC-rearranged sarcoma

Objective To observe the clinical,CT and MRI manifestations of CIC-rear ranged sarcoma(CRS).Methods Eight patients with single CRS lesion ranged from 3.79 to 98.81 cm and the median of 21.37 cm were retrospectively enrolled,6 lesions located in deep soft tissue and 2 located in the femur.The clinical and imaging data were observed.Results All 8 CRS lesions presented as rapidly growing local masses,including 6 solid cystic soft tissue lesions and 2 non enlarged bone lesions with osteolytic bone destruction,which were all lobulated lesions with uneven equal-low density/slightly high T1 and slightly high T2 signals,with multiple focal map like necrosis.Diffusion weighted imaging showed lesions with limited diffusion and invasion of adjacent tissue,and"ice melting sign"was observed in 4 cases.After administration of contrast agents,the solid components of lesions enhanced unevenly and moderately or significantly and sustainedly,with slightly more pronounced enhancement at the edges,abundant blood vessels were visible inside and at the edges."Vascular floating sign"was noticed in 2 cases.Conclusion The clinical,CT and MRI manifestations of CRS had certain characteristics.

10.Development and validation of the Online Fraud Susceptibility Scale

Objective:To develop and validate the Online Fraud Susceptibility Scale(OFSS).Methods:The construct of online fraud susceptibility was defined through qualitative interviews(n=14)and expert evaluations(n=6).Based on prior literature,an initial item pool was generated.A sample of 805 adults completed the preliminary scale for item analysis and exploratory factor analysis(EFA).Another sample of 491 adults completed the formal scale for confirmatory factor analysis(CFA),criterion validity,and internal consistency analysis.Of these,233 par-ticipants were retested after four months.The Vulnerability to Fraud Questionnaire(VFQ)and the Susceptibility to Persuasion-Ⅱ Scale(StP-Ⅱ)were used as criterion measures.Results:The 25-item OFSS consisted of 5 factors,na-rely monetary motivation,heuristic processing,financial risk-seeking,anti-fraud knowledge,and susceptibility to so-cial influence,explaining 51.94%of the total variance.The factor loadings ranged from 0.52 to 0.80.The 5-factor model showed an acceptable structural fit(x2/df=2.39,CFI=0.86,TLI=0.84,CFI=0.86,RMSEA=0.05).The OFSS scores were positively correlated with the scores of VFQ and StP-Ⅱ(r=0.51,0.47,Ps<0.001).The inter-nal consistency reliabilities of the total score and 5 factors ranged from 0.64 to 0.80,and the retest reliabilities(ICC)of the total score and 5 factors ranged from 0.53 to 0.78.Conclusion:The Online Fraud Susceptibility Scale demonstrates satisfactory psychometric properties providing a practical tool to assess the vulnerability to online fraud.
