1.Optimization of drug dispensing and pickup process in traditional Chinese medicine pharmacy based on data-intelligence-driven

OBJECTIVE To explore the transformation of the dispensing and drug pickup process in traditional Chinese medicine pharmacy (TCM Pharmacy) in our hospital based on data-intelligence-driven, aiming to improve pharmacists’ work efficiency and patients’ drug pickup experience. METHODS Value stream mapping and journey mapping were used to systematically identify non-value-added links in pharmacists’ dispensing process and key pain points in patients’ drug pickup under the traditional process. An intelligent dispensing and drug pickup system for the TCM Pharmacy was developed based on the C# and Android television platforms, and a machine-learning model was adopted to predict patients’ drug pickup waiting time. A comprehensive evaluation was performed from three perspectives: system performance, prediction accuracy, and satisfaction of pharmacists and patients. RESULTS The system successfully streamlined non-value-added links such as “waiting for writing on the board” and “searching for drugs”, and realized multimodal dynamic prompts of dispensing status through auditory (number calling) and visual (television terminal) channels. The constructed model for predicting drug pickup waiting time exhibited good fitting degree and generalization ability (mean absolute error=4.28 min, R 2 =0.882). The comprehensive satisfaction scores of pharmacists and patients in the traditional mode were significantly increased from (70.99±1.74) and (73.58±1.98) to (90.02±1.30) and (88.61±2.08) in the new system, respectively ( P <0.01). CONCLUSIONS The transformation of the intelligent drug dispensing and pickup system for TCM pharmacy based on data-intelligence-driven effectively improves the efficiency of pharmacists’ dispensing work, realizes process transparency and waiting time predictability, and significantly enhances patients’ drug pickup experience.
2.Comparative study on medical operation efficiency of the Beijing-Tianjin-Hebei region in China

Objective:To analyze the medical operation efficiency of the Beijing-Tianjin-Hebei region, so as to provide references for optimizing medical resource allocation in the area.Methods:Based on the China Statistical Yearbook (2012 to 2023), data regarding healthcare operational inputs (3 indicators including number of healthcare institutions, number of healthcare personnel, and number of beds in healthcare institutions) and outputs (6 indicators including number of patients treated and discharged, etc.) in the Beijing-Tianjin-Hebei region were included, and the DEA-Malmquist index model was employed to analyze healthcare operational efficiency in the region. The grey prediction model was used to predict the number of healthcare institutions, healthcare personnel, and beds in healthcare institutions from 2023 to 2027. Results:From 2011 to 2022, the overall efficiency of medical operations in Beijing and Tianjin showed an increasing trend, with total factor productivity of 1.213 1 and 1.246 0, respectively. However, the overall efficiency of medical operations in Hebei was showing a downward trend, with a total factor production of 0.933 1 and a scale efficiency of 0.909 5. The number of healthcare institutions, healthcare personnel, and beds in healthcare institutions showed an upward trend in the Beijing-Tianjin-Hebei region from 2023 to 2027.Conclusions:The medical operational efficiency in Beijing and Tianjin was consistently improving. The utilization rate of medical resources in Hebei was relatively low, accompanied by a persistent decline in medical operational efficiency. The Beijing-Tianjin-Hebei region should further enhance the rational allocation of medical resources, so as to promote the coordinated development of the regional healthcare system.
3.Impact of palliative care on medication use and medical utilization in patients with advanced cancer.

OBJECTIVE:
To evaluate the effect of palliative care on drug use, medical service utilization and medical expenditure of patients with advanced cancer.
METHODS:
A cohort of patients including pal-liative care and standard care was constructed using the medical records of the patients in Peking University Cancer Hospital from 2018 to 2020, and coarsened exact matching was used to match the two groups of patients. The average monthly opioid consumption, hospitalization rate, intensive care unit (ICU) rate and operation rate, and the average monthly total cost were selected to evaluate drug use, medical service utilization and medical expenditure. Chi-square test and Wilcoxon signed rank test were used to compare the differences between the two groups before and after exposure and the change in the palliative care group. The net impact of palliative care on the patients was calculated using the difference-in-differences analysis.
RESULTS:
In this study, 180 patients in the palliative care group and 3 101 patients in the stan-dard care group were finally included in the matching, and the matching effect of the two groups was good (L1 < 0.1). Before and after exposure, the average monthly opioid consumption in the palliative care group was significantly higher than that in the standard care group (Before exposure: 0.3 DDD/person-month vs. 0.1 DDD/person-month, P < 0.01; After exposure: 0.7 DDD/person-month vs. 0.1 DDD/person-month, P < 0.01; DDD refers to defined daily dose), palliative care significantly increased the average monthly opioid consumption in the patients (0.3 DDD/person-month, P < 0.01). The hospitalization rate (48.9% vs. 74.3%, P < 0.01) and operation rate (3.9% vs. 8.8%, P < 0.01) of the patients in palliative care group were significantly lower than those in standard care group, and the ICU rate became similar between the two groups (1.1% vs. 1.6%, P=0.634). Palliative care significantly reduced the patients ' hospitalization rate (-25.6%, P < 0.01), ICU rate (-4.9%, P < 0.01) and operation rate (-14.5%, P < 0.01). Before and after exposure, the average monthly total costs of pal-liative care group were slightly higher than those of standard care group (Before exposure: 20 092.3 yuan vs. 19 132.8 yuan, P=0.725; After exposure: 9 719.8 yuan vs. 8 818.8 yuan, P=0.165). Palliative care increased the average monthly total cost by 2 208.8 yuan, but it was not statistically significant (P=0.316).
CONCLUSION
Palliative care can increase the opioid consumption in advanced cancer patients, reduce the rates of hospitalization, ICU and surgery, but has no significant effect on medical expenditure.

4.Role and value of bile diversion in the treatment of acute pancreatitis

In recent years, the individualized comprehensive treatment mode led by mini-mally invasive techniques in the early stage of acute pancreatitis (AP) has significantly reduced its mortality rate. However, the overall complication rate remains high. This predicament has been prompting us to continuously explore and improve the early diagnosis and treatment strategies for AP. Changes in the functional and morphological aspects of the gallbladder are no longer confined to biliary pancreatitis alone. Cholestasis, gallbladder enlargement, increased tension, and significant alterations in the composition of bile are commonly observed in patients with AP caused by various factors, and these changes are closely related to the prognosis of the patients. Against this backdrop, the authors propose the new concept of bile diversion treatment, that is, percutaneous transhepatic gallbladder drainage guided by ultrasound combined with bile reinfusion. In practice, it has been observed that the application of bile diversion can effectively improve the short-term efficacy of AP patients with gallbladder enlargement and accelerate their recovery. The authors elaborate on the role and value of bile diversion in the treatment of AP. It is believed that with the continuous in-depth development of basic research and the continuous optimization of clinical practice pathways and protocols, bile diversion is expected to become an important part of the comprehensive treatment of AP, thus bringing more survival benefits to AP patients.

Objective To understand the dilemma and coping experience of pediatric nurses participating in medication safety management,so as to provide a reference for improving the quality of medication safety management.Methods This study was a descriptive phenomenological study.The puiposive sampling method was used to conduct semi-structured interviews with 21 pediatric nurses from in a tertiary A hospital in Nanjing from January to February 2024.The interview data were extracted,coded,cluster screened,interpreted and verified by the Colaizzi seven-step analysis method.Results In terms of the dilemmas of pediatric nurses involved in medication safety management,there were 3 themes,including knowledge and skills,environment and resources,understanding and cooperation,and 7 sub-themes.In terms of coping experience of pediatric nurses involved in medication safety management dilemma,there were 3 themes,including responsibility and emotion,communication and collaboration,system and supervision,and 4 sub-themes.Conclusion Pediatric nurses will encounter various difficulties and actively adopt various coping strategies when participating in medication safety management.It is recommended that nursing managers strengthen education and training,quality management,continuously improve nurses'safety responsibility awareness and empathy ability,encourage parents of children to participate in medication safety management,and actively improve the pediatric working environment,continuously strengthen organizational support,and promote communication and collaboration to ensure medication safety.

9.Chain mediating effect of rumination and avoidance coping between information anxiety and suicidal ideation in college students

Objective:To explore the mediating roles of rumination and avoidance coping between information anxiety and suicidal ideation.Methods:In March 2022, a total of 896 college students were surveyed by the information anxiety scale(IAS), ruminative responses scale(RRS), coping style questionnaire(CSQ)and self-rating idea of suicidal ideation scale(SIOSS). The ANOVA analysis, Pearson correlation analysis and mediating effect analysis were performed by the SPSS 22.0 software.Results:The detection rate of suicidal ideation among college students was 9.15%(82/896). The information anxiety score was(73.84±17.29), the rumination score was(47.73±12.16), the avoidance coping score was(3.76±2.52), and the suicidal ideation score was(5.41±4.09). Information anxiety was significantly positively correlated with rumination, avoidance coping and suicidal ideation( r=0.49, 0.36, 0.37, all P<0.05). Rumination was significantly positively correlated with avoidance coping and suicidal ideation( r=0.42, 0.59, both P<0.05). There was a significantly positive correlation between avoidance coping and suicidal ideation( r=0.45, P<0.05). Information anxiety affected suicidal ideation of college students through four paths.The direct effect value of information anxiety on suicidal ideation was 0.06, accounting for 16.67% of the total effect. The effect values of the separate mediating effect of rumination and avoid coping were 0.22 and 0.04, and accounting for 61.11% and 11.11% of the total effect respectively. The chain mediating effect value of rumination and avoid coping was 0.04, accounting for 11.11% of the total effect. Conclusion:Information anxiety can directly affect suicidal ideation of college students and indirectly affect suicidal ideation through rumination and avoidance coping.
10.Preparation of 4-sulfonylcalix6arene-modified cotton for uranium contamination removal

Objective To prepare 4-sulfonylcalix[6]arene-modified cotton fibers for adsorption and removal of uranium based on the specific complexation of calix[6]arene with uranium (VI). Methods Chemical grafting was used for the modification of cotton, which reacted with α-bromoisobutyryl bromide, glycidyl methacrylate, and 4-sulfonylcalix[6]arene. Scanning electron microscopy (SEM), X-ray photoelectron spectroscopy (XPS), and infrared spectroscopy (FTIR) were used to characterize the structure of 4-sulfonylcalix[6]arene-modified cotton (Cotton S-C[6]a). A Franz diffusion cell was used to simulate uranium-contaminated skin. Laser fluorimetry was used to determine the uranium content. Results SEM, XPS, and FTIR showed that cotton fibers were successfully grafted with 4-sulfonylcalix[6]arene. The optimal conditions of Cotton S-C[6]a for the adsorption of uranium (VI) was pH 4.0, duration of 20 min, and 20 mg of adsorbent. The adsorption process fitted well with pseudo-secondary-order kinetics. The uranium removal efficiency of Cotton S-C[6]a was up to 78.46% in aqueous solution and 81.72% on skin. Conclusion The synthesized Cotton S-C[6]a is highly efficient in the removal of uranium (VI) in solution and on contaminated skin.
