1.Regulation of Oxidative Stress by Traditional Chinese Medicine in Prevention and Treatment of Myocardial Ischemia-reperfusion Injury: A Review

Myocardial ischemia-reperfusion injury (MIRI) is a major complication following coronary revascularization. Studies indicate that its pathophysiological mechanisms of MIRI are closely associated with oxidative stress, iron overload, inflammatory responses, and lipid peroxidation. Oxidative stress refers to an imbalance in redox homeostasis under pathological conditions, characterized by the abnormal accumulation of reactive oxygen species (ROS), which disrupts the dynamic balance between pro-oxidant systems and antioxidant defense networks. In recent years, traditional Chinese medicine (TCM) has demonstrated unique advantages in the prevention and treatment of MIRI due to its multi-target and multi-pathway antioxidant properties. Research reveals that TCM primarily exerts protective effects against oxidative stress-induced MIRI by regulating signaling pathways such as nuclear factor erythroid 2-related factor 2 (Nrf2), adenosine monophosphate-activated protein kinase (AMPK), phosphatidylinositol 3-kinase/protein kinase B (PI3K/Akt), nuclear factor kappa-B (NF-κB), Janus kinase 2/signal transducer and activator of transcription 3 (JAK2/STAT3), and protein kinase C beta Ⅱ/nicotinamide adenine dinucleotide phosphate oxidase 2/reactive oxygen species (PKCβⅡ/NOX2/ROS). This article reviews recent literature on TCM monomers, compound formulas, and their active components, which alleviate oxidative stress to prevent and treat MIRI by modulating the aforementioned signaling pathways. It summarizes a concise overview of the molecular mechanisms by which oxidative stress-related signaling pathways lead to MIRI, discusses how TCM regulates these pathways to reduce oxidative stress-induced MIRI, and explores clinical application prospects and research challenges, aiming to provide a theoretical reference for the research and clinical management of MIRI.
2.Risk factors for peritoneal dialysis-related peritonitis in type 2 diabetes mellitus patients complicated with renal anemia and establishment of prediction model

OBJECTIVE To explore the risk factors for peritoneal dialysis-associated peritonitis(PDAP)in the type 2 diabetes mellitus patients complicated with renal anemia and construct the risk prediction model so as to provide references for clinical diagnosis and treatment of PDAP.METHODS A total of 142 type 2 diabetes mellitus patients who were complicated with renal anemia and underwent peritoneal dialysis in nephrology department of the First Teaching Hospital of Tianjin University of Traditional Chinese Medicine from Jan.2017 to Dec.2023 were recrui-ted as the research subjects and were divided into the peritonitis group with 68 cases and the non-peritonitis group with 74 cases according to the status of complication with PDAP.The related data were collected,the univariate a-nalysis and binary logistic regression analysis were performed,the risk prediction model was established,and the obtained model was visualized for further analysis.RESULTS The univariate analysis showed that the age and lev-els of glycosylated hemoglobin,triglyceride and fibrinogen were higher in the peritonitis group than in the non-per-itonitis group;the levels of serum potassium,serum iron and total iron binding capacity of the peritonitis group were lower than those of the non-peritonitis group,and there were significant differences(all P<0.05).The bina-ry logistic regression analysis indicated that glycosylated hemoglobin no less than 7.00%(OR=4.047,95%CI:1.663 to 9.847,P=0.002),no less than 60 years of age(OR=2.181,95%CI:1.039 to 4.578,P=0.039),triglyc-eride greater than 1.47 mmol/L(OR=2.393,95%CI:1.140 to 5.026,P=0.021)and serum iron less than 7.90μmol/L(OR=2.582,95%CI:1.188 to 5.608,P=0.017)were the risk factors for the PDAP in the type 2 diabe-tes mellitus patients complicated with renal anemia.CONCLUSIONS The glycosylated hemoglobin no less than 7.00%,no less than 60 years of age,triglyceride greater than 1.47 mmol/L and serum iron less than 7.90 μmol/L are the risk factors for PDAP in the type 2 diabetes mellitus patients complicated with renal anemia.The risk pre-diction model can be established based on the above factors,and the intervention measures should be actively taken so as to reduce the risk of PDAP and improve the quality of life.
3.Prokaryotic expression, purification and immunogenicity of SARS-CoV-2 omicron variant nucleocapsid protein.

Objective The study aims to investigate the immunological functions of the nucleocapsid (N) protein of the novel coronavirus Omicron (BA.1, BA.2) and evaluate the differences among different N proteins of mutant strains in immunogenicity. Methods By aligning sequences, the mutation sites of the Omicron (BA.1, BA.2) N protein relative to prototype strain of the novel coronavirus (Wuhan-Hu-1) were determined. The pET-28a-N-Wuhan-Hu-1 plasmid was used as template to construct pET-28a-BA.1/BA.2-N through single point mutation or homologous recombination. The three kinds of N protein were expressed in prokaryotic system, purified through Ni-NTA affinity chromatography, and then immunized into mice. The titer and reactivity of the polyclonal antibody, as well as the expression level of IL-1β and IFN-γ in mouse spleen cells, were detected using indirect ELISA and Western blot assay. Results The constructed prokaryotic expression plasmids were successfully used to express the Wuhan-Hu-1 N, BA.1 N, and BA.2 N proteins in E.coli BL21(DE3) at 37 DegreesCelsius for 4 hours. The indirect ELISA test showed that the titers of polyclonal antibody prepared by three N proteins were all 1:51 200. All three N proteins can increase the expression of IFN-γ and IL-1β cytokines, but the effect of Omicron N protein in activing two cytokines was more obvious than that of Wuhan-Hu-1 N protein. Conclusion The study obtained three new coronavirus N proteins and polyclonal antibodies, and confirmed that mutations in the amino acid sites of the N protein can affect its immunogenicity. This provides a basis for developing rapid diagnostic methods targeting N protein of different novel coronavirus variants.

4.Analysis on Pathogenesis and Syndrome Differentiation in Post-PCI Coronary Heart Disease Based on the Theory of"Fire and Original Qi Cannot Coexist"

Percutaneous coronary intervention(PCI),as one of the primary approaches for revascularization,still faces complications such as restenosis,myocardial ischemia-reperfusion injury and no-reflow/slow-flow phenomena,with no currently effective interventions ensuring long-term efficacy.Based on Li Dongyuan's theory that"fire and original qi cannot coexist",this article inherited Academician Chen Keji's academic perspective on"toxin-stasis pathogenesis"and the hemodynamic characteristics of coronary arteries to propose a"four-stage pathological progression"in post-PCI patients,namely spleen-stomach impairment-original qi deficiency-endogenous yin-fire-toxin-stasis accumulation.It emphasized that the heart vessels rely on qi and blood for nourishment and patency for function,elucidated the therapeutic rationale of Danggui Buxue Decoction,and presented the self-formulated Yixin Hemai Prescription,modified through syndrome differentiation,and performed simultaneous reinforcement and dredging,in order to provide diagnosis and treatment ideas for coronary heart disease after PCI treated with TCM.

7.Using Instrumental Approach to Estimate the Effect of PM2.5 Concentrations on Daily Deaths

Objective Many environmental epidemiological studies have shown the associations between short-term exposure of air pollution and daily deaths.However,the generally available population data only contain a small number of measured confounding factors,which is faced with the problem that a large number of unobserved confounding factors are not included in the model,resulting in biased estimates.The instrumental variable method can solve the problem of estimating the effects caused by unobserved confounders.In this paper,We used the instrumental variable method to estimate the effects of PM2.5 on daily mortality.Methods We collected daily PM2.5 concentrations,meteorological data,and nonaccidental daily deaths in a Chinese city from 2016 to 2019.We used boundary layer height and wind speed as instrumental variables to estimate the effects of PM2.5 on nonaccidental daily mortality.Negative exposure control was used to test the hypothesis of instrumental variables.Meanwhile time series bootstrap method was used to estimate confidence interval.We compared the results of the generalized additive model and instrumental variable method.Results The instrumental variable method showed that PM2.5 was significantly related to daily deaths.For every 10 μg/m3 increase of PM2.5 concentrations,the nonaccidental daily deaths increased by 0.94%(95%CI:0.39%~1.55%).Negative exposure control results showed no correlation between negative exposure and nonaccidental deaths(P=0.19),indicated that the aforementioned instrumental variable model was not affected by unmeasured and uncontrolled confounders.The traditional generalized additive model estimated that for every 10 μg/m3 increase in PM2.5 concentrations,the nonaccidental deaths would increase by 0.24%(95%CI:0.01%~0.47%).Conclusion The instrumental variable method estimated that PM2.5 concentrations were significantly correlated with the nonaccidental daily deaths.Boundary layer height and wind speed can be used as instrumental variables to estimate the effects of PM2.5 concentrations on nonaccidental deaths.
8.Diagnostic and prognostic valuation of CD64, heparin binding protein and procalcitonin in children with sepsis shock

Objective:To investigate the value of serum CD64, heparin binding protein (HBP) and procalcitonin (PCT) in early diagnosis of septic shock and its prognostic value.Methods:The clinical data of 40 children with sepsis (sepsis group) and 40 children with septic shock (septic shock group) in Children′s Hospital of Xuzhou Medical University from January 2018 to November 2021 were retrospectively analyzed. PCT was detected by chemiluminescence, HBP was detected by enzyme-linked immunosorbent assay, and CD 64 was detected by flow cytometry. All children were followed up within 1 month after discharge, and the occurrence of poor prognosis (including multiple organ failure and death) was recorded. The receiver operating characteristic (ROC) curve was drawn to evaluate the predictive value of serum CD 64, HBP and PCT for septic shock. Multivariate Logistic regression was used to analyze the independent risk factors for poor prognosis in children with septic shock. Results:The serum CD 64, HBP and PCT in septic shock group were significantly higher than those in sepsis group: 0.667 ± 0.120 vs. 0.501 ± 0.115, (116.46 ± 11.41) μg/L vs. (87.34 ± 23.49) μg/L and (11.41 ± 1.25) μg/L vs. (9.29 ± 1.31) μg/L respectively, and there were statistical differences ( P<0.05). ROC curve analysis result showed that the area under curve (AUC) of serum CD 64, HBP and PCT for predicting septic shock were 0.837, 0.894 and 0.880 respectively, and the optimal cut-off values were 0.586, 106.2 μg/L and 11.28 μg/L respectively; the AUC of serum CD 64, HBP and PCT combined detection for predicting septic shock was 0.914, with a sensitivity of 75.7%, specificity of 96.0%, and accuracy of 71.7%. According to the optimal cut-off values of serum CD 64, HBP and PCT, 40 children with septic shock were divided into high expression group and low expression group. The prognosis was good in 25 cases and bad in 15 cases. The incidence of poor prognosis in CD 64 high expression group, HBP high expression group and PCT high expression group was significantly higher than that in corresponding CD 64 low expression group, HBP low expression group and PCT low expression group: 56.52% (13/23) vs. 2/17, 10/17 vs. 21.74% (5/23) and 11/18 vs. 18.18% (4/22), and there was statistical difference ( P<0.01 or <0.05). Multivariate Logistics regression analysis result showed that serum CD64, HBP and PCT were independent risk factors for poor prognosis in children with septic shock ( OR = 0.818, 1.204 and 3.633; 95% CI 0.674 to 0.994, 1.022 to 1.419 and 1.090 to 12.108; P = 0.043, 0.026 and 0.036). Conclusions:The serum levels of CD 64, HBP and PCT in children with septic shock are significantly increased, which play an important role in the occurrence and development of septic shock, and which have predictive value for septic shock. The combined detection of the 3 indexes could be used to evaluate the prognosis, with a higher predictive value.
9.ROBIS Evaluation of Quality Assessed by Iconographic Diagnostic Test System

Purpose To evaluate the quality assessed by iconographic diagnostic test system using ROBIS. Materials and Methods"Diagnostic tests, system evaluation, Meta analysis, diagnoses*test, diagnoses*trial, systematic review, meta-analysis" were used as search term to retrieve relevant literatures recorded in CBM, CNKI, Wanfang Data, PubMed, Embase, and Cochrane Library from January. 1, 2014 to December. 31, 2016. After two independent researchers screened the literature and extracted the information, the quality of included literatures was evaluated using ROBIS. Results Totally 219 articles were included in the study, including 93 Chinese literatures and 126 English literatures. The results of subgroup analysis showed that the quality difference between Chinese and English documents was statistically significant (P=0.018); the quality difference between the Chinese literatures and those of other regions was statistically significant (P<0.001); there was no statistical significance in quality difference regarding the year of publication (P=0.34). The ROBIS evaluation results showed that there were only 15 literatures (6.85%) with low-risk, including 2 Chinese literatures and 13 English literatures. Conclusion The quality evaluated by Chinese and English diagnostic test system, which is publicly published in iconographic diagnostic test from 2014 to 2016, is generally low, and the quality of literatures published by domestic scholars and in Chinese is still behind international levels. The maker of system evaluation in this field should prepare their research plans in advance and fully report them in future research, adequately obtain evidence, minimize bias in respects of document screening, information extraction, and evaluation of original research bias, and take into account of results stability, based on which the quality of system evaluation in this field should be further improved.
