Objective To explore the predictive value and efficacy evaluation of plasma microRNA(miR)-132,miR-134 combined with miR-124 in patients with depression. Methods A total of 75 patients diagnosed with depression in the psychiatric department of the hospital from June 2021 to July 2023 were selected as the study group, and 75 healthy subjects who underwent physical examination in the hospital during the same pe-riod were selected as the control group. The relative expression levels of miR-132,miR-134 and miR-124 in plasma of the two groups were detected. The levels of miR-132,miR-124,miR-134,brain-derived neurotrophic factor(BDNF).inflammatory factors[interleukin(IL)-6,IL-18,tumor necrosis factor-a(TNF-a)]were com-pared between the two groups before and after 8 weeks of treatment.Multiple linear regression analysis was used to construct a joint prediction model. The application value of plasma miR-132,miR-134,miR-124 and combined prediction in the diagnosis of depression was evaluated by receiver operating characteristic(ROC)curve. Results The relative expression levels of miR-132 and miR-124 in plasma of the study group were sig-nificantly higher than those of the control group, with statistical significance(P<0.05).The relative expres-sion level of miR-134 in the study group was significantly lower than that in the control group, and the differ-ence was statistically significant(P<0.05).ROC curve analysis showed that the area under the curve of plas-ma miR-132,miR-134,miR-124 and combined prediction of depression were 0.858 8,0.851 9,0.763 1 and 0.971 4,respectively.Compared with 8 weeks before treatment, plasma miR-132,miR-124,IL-6,IL-18 and TNF-a levels were significantly down-regulated,while plasma miR-134 and BDNF levels were significantly up-regulated after 8 weeks of treatment. Conclusion miR-132,miR-134 and miR-124 are closely related to the oc-currence of depression, and the combination of the three can be used to predict and early diagnose depression patients and evaluate the drug efficacy of depression patients.

Objective To predict the intervention targets of empagliflozin(EMPA),a specific inhibitor of sodium-glucose cotransporter 2(SGLT2),in gastric adenocarcinoma through comprehensive network pharmacology, and to validate the effects and the molecular mechanisms of EMPA through cellular and molecular biology experiments. Methods Bioinformatics analysis of gastric adenocarcinoma was conducted to assess the correlation between gastric adenocarcinoma prognosis and SGLT2 expression.Network pharmacology was utilized to identify shared targets of EMPA and gastric adenocarcinoma.AGS cells,a human gastric adenocarcinoma cells line, were incubated with EMPA at different concentrations for 24 h and, then, cell proliferation was assessed using the CCK8 assay. After AGS cells were incubated with EMPA at the doses of 0,3,and 6 mmol/L,real-time cell analysis(RTCA)and 5-ethynyl-2-deoxyuridine(EdU)incorporation were used to evaluate EMPA's inhibitory effects on the proliferation of the AGS cells. In addition, wound healing and Transwell assays were performed to assess the inhibitory effect of EMPA on the migration and invasion of the APC cells and Western blot analysis was conducted to examine the expression of mammalian target of rapamycin(mTOR)and phosphorylated mTOR(p-mTOR).BALB/c(nu/nu)nude mice were implanted with 5x106 AGS cells in the axilla. The mice were divided into three groups,a control group,a low-dose group, and a high-dose group, each consisting of 7 mice. After one week, the control group received daily intraperitoneal injections of normal saline, while the low-dose group and high-dose group received daily intraperitoneal injections of EMPA at the doses of 3 mg/kg and 5 mg/kg,respectively.The tumor volume was measured one week after the drug intervention started. Results Gastric adenocarcinoma patients with low expression of SGLT2 exhibited longer survival time and higher survival rate than those with high expression of SGLT2 did.A total of 104 EMPA-related potential targets and 2028 targets associated with gastric adenocarcinoma were identified.Among these,45 targets associated with gastric adenocarcinoma overlapped with potential targets of EMPA.Further analysis revealed 10 relevant pathways and 4 core genes. The core genes were cyclin-dependent kinase 4(CDK4),glyceraldehyde-3-phosphate dehydrogenase(GAPDH),mTOR,and cyclin El(CCNE1).CCK-8 assay revealed that EMPA at concentrations ranging from 0.39 to 50 mmol/L effectively inhibited the proliferation of AGS cells.RTCA results indicated a downward shift in the cell growth curve. In comparison to the findings for the control group,EdU assay demonstrated that EMPA at the concentrations of 3 mmol/L and 6 mmol/L significantly inhibited AGS cell proliferation(P<0.05).Results from wound healing and Transwell assays indicated a decrease in the levels of cell migration and invasion(P<0.05)and,notably,there was a significant difference between the high and low-dose EMPA groups(P<0.05).Western blot showed no statistically significant difference in the expression of total mTOR protein between the groups. However, the expression of p-mTOR in the 3 mmol/L and 6 mmol/L EMPA groups decreased compared to that of the control group(P<0.05),with the 6 mmol/L EMPA group exhibiting a more pronounced reduction(P<0.05).Nude mice xenograft tumor experiment demonstrated that, compared to that of the control group, the tumor volumes in the EMPA-treatment groups were significantly reduced(P<0.05),with the high-dose group showing a more pronounced reduction(P<0.05).Conclusion EMPA inhibits the abnormal proliferation and migration of gastric adenocarcinoma cells, potentially through the modulation of mTOR protein activation. This study provides new potential medication and intervention targets for gastric adenocarcinoma treatment.

Objective:
To investigate the value of bedside echocardiography in diagnosis and risk assessment of in-hospital death of patients with Stanford type A aortic dissection.
Methods:
The clinical data of 229 patients with Stanford type A aortic dissection diagnosed by CT angiography in Zhongshan Hospital affiliated to Fudan University between January 2009 and January 2016 were retrospectively analyzed. The patients were divided into survival group(191 cases)and non-survival group(38 cases)according to presence or absence of in-hospital death. The bedside echocardiography features were analyzed, and influence factors of in-hospital death were determined by multivariate logistic regression analysis.
Results:
(1) Compared with the survival group, the non-survival group had lower surgery rate (60.52%(23/38) vs. 85.34%(163/191),

Objective To understand the current situation and existing problems in the training of healthcare-asso-ciated infection(HAI)management, and provide scientific basis for strengthening the management of HAI preven-tion and control system. Methods A questionnaire survey was adopted to investigate situation of training on HAI in 15 provincial-level HAI training agencies in China during the past 30 years, and basic condition of training on HAI management in recent 5 years. Results Among 15 provincial-level training agencies,66.67%(n=10)were respon-sible by HAI management quality control centers,80.00% have already conducted training in each city,53.33%carried out training for 10 to 20 times,33.34% performed training for ≤2 times per year.Of 33 728 trainees in 2011-2015,41.30% were 41-50 years old,61.82% were nursing staff,50.56% had bachelor degree,43.96%were with the intermediate professional title. Most trainers were HAI prevention and control experts in their respec-tive province, accounting for 68.07%,the curriculums were mainly designed on professional course, and only 26.78% were involved in management. Conclusion Professional structure of HAI management personnel is not reasonable, faculty is imbalance, knowledge update is lacking, and HAI training and education system need to be improved further.
