Myofascial trigger points, as hyperirritable spots within taut bands of skeletal muscle, can induce local or referred pain, and show a high degree of similarity to acupoints in traditional Chinese medicine (TCM), particularly the so-called sinew knot lesion point. From the perspective of channel sinew theory, and by examining the correlations of myofascial trigger points with acupoints and channel sinew disorders, this study aims to compare the similarities and differences between MTrPs and sinew knot lesion points in terms of pathological mechanisms, needling analgesic mechanisms, and therapeutic approaches. The goal is to deepen the understanding of MTrPs and dry needling, and provide a modern scientific perspective on channel sinew theory and the sinew knot lesion point.

Acute kidney injury(AKI)is characterized by high morbidity,high mortality,high disability rate and high treatment cost,its pathological mechanism has not been fully elucidated and there is a lack of ef-fective treatment methods.Klotho,a kidney-specific protective protein,is mainly expressed in renal tubular ep-ithelial cells,regulates the AKI progression and mitigates the renal injury through multiple pathways,inclu-ding the regulation of the renin-angiotensin-aldosterone system(RAAS),antioxidant stress,anti-inflamma-tion,modulation of cell death and anti-fibrotic effects.At present,the Klotho-based strategies for AKI preven-tion and treatment remain in the preclinical stage,requiring further investigation.This article reviews the mo-lecular regulatory mechanisms of Klotho in AKI and its diagnostic and therapeutic potential,aiming to provide new idea for the pathological mechanisms and clinical translation of AKI.
4.Molecular mechanism of active ingredients of Ligustri Lucidi Fructus against osteoporosis

BACKGROUND:Traditional Chinese medicine has been proved to have a significant role in anti-osteoporosis,and the effectiveness and mechanism of Ligustri Lucidi Fructus and its the active ingredients against osteoporosis have gradually gained the attention of scholars.OBJECTIVE:To analyze and summarize the research progress of Ligustri Lucidi Fructus and its active ingredients against osteoporosis in vitro and in vivo.METHODS:We searched the relevant literature included in CNKI and PubMed databases using the search terms of"Osteoporosis,Bone marrow mesenchymal stem cells,Osteoblast,Osteoclast,Ligustri Lucidi Fructus,Signal path"in Chinese and English,respectively.According to the needs of the research,we established the corresponding criteria and screened the literature.A total of 82 papers were included in the final review.RESULTS AND CONCLUSION:(1)The active ingredients of Ligustri Lucidi Fructus that exert anti-osteoporotic effects in vitro and in vivo mainly involve the following:Salidroside activates the Wnt/β-catenin signaling pathway by inhibiting the expression of Sclerostin and Dickkopf-related protein 1.This activation process enhances the expression of phosphorylated low-density lipoprotein receptor-related protein 6 in ovariectomized rats and primary osteoblasts,while decreasing the expression of glycogen synthase kinase 3β.Further,it promotes the expression of β-catenin,runt-related transcription factor 2 and cellular myelocytomatosis oncogene in the nucleus,thereby promoting the bone formation capacity of osteoblasts.The advantage is that it acts directly on osteoblasts to promote bone formation,which provides a new strategy for the treatment of osteoporosis.(2)Olive bittersweet significantly increases bone mineral density and regulates bone metabolism by decreasing terminal interleukin-6 and alkaline phosphatase concentrations in Sprague-Dawley rats.In vitro experiments showed that olive bittersweet promotes the proliferation of osteoblasts and up-regulates the protein and mRNA expression of osteoprotegerin,while inhibiting the protein and mRNA expression of receptor activator of nuclear factor-κB ligand.This mechanism of action is closely related to the regulation of the balance of the osteoprotegerin/receptor activator of nuclear factor-κB ligand system,demonstrating the advantage of increasing bone mineral density and maintaining bone health by regulating factors related to bone metabolism,but there is no significant effect on Ca2+concentration,which may limit its use in some specific types of osteoporosis.(3)By decreasing the expression of phosphatidylinositol 3 kinase,reducing the phosphorylation of protein kinase B and the expression of osteoclast-specific marker protein c-Fos,pineconiferin effectively inhibits the activation of phosphatidylinositol 3 kinase/protein kinase B/c-Fos pathway in osteoclasts.This inhibition reduces the proliferation and maturation of osteoclasts,which can help to reduce bone resorption.The advantage of this inhibition is that it can directly target osteoclasts,which provides a new target for osteoporosis treatment.However,the specific regulatory mechanism of osteoclasts needs to be studied in depth,and its long-term effect and safety need to be further evaluated.(4)The active ingredients of Ligustri Lucidi Fructus have shown good therapeutic effects on osteoporosis,but their mechanism of action is complex,involving the interaction of multiple genes,proteins and signaling pathways.In the future,large-scale clinical trials need to be carried out to verify its effectiveness and safety,and the strategy of combining the active ingredients of Ligustri Lucidi Fructus with other drugs needs to be further explored in order to obtain better therapeutic effects.
5.Risk factors and management measures for perioperative puncture site complications of radiofrequency ablation due to tachyarrhythmias

Objective To explore the risk factors for puncture site complications after radiofrequen-cy catheter ablation(RFCA)in elderly patients with tachyarrhythmias.Methods A retrospective study was conducted on 290 elderly tachyarrhythmic patients undergoing RFCA in our depart-ment from January 2017 to June 2023.According to the occurrence of postoperative complications during hospitalization,they were divided into a complication group(13 cases)and a non-complica-tion group(277 cases).The basic clinical data were compared between the two groups.Multivari-ate logistic regression analysis was used to identify the independent influencing factors for periop-erative puncture site complications in RFCA,and ROC curves were plotted to determine the pre-dictive value of BMI on puncture site complications.Results Puncture site complications were more common in atrial arrhythmia radiofrequency ablation,followed by supraventricular arrhyth-mia,and but rare in ventricular arrhythmia,with statistical difference in the incidence(P=0.014).The complication group had significantly higher BMI and larger proportion of atrial ar-rhythmias when compared with the non-complication group(P<0.05).There were no statistical differences between the two groups in terms of age,proportions of ventricular arrhythmia,su-praventricular arrhythmia,hypertension and type 2 diabetes(P>0.05).Multivariate logistic re-gression analysis showed that higher BMI was a risk factor for puncture site complications in eld-erly tachyarrhythmic patients after RFCA(OR=1.256,95%CI:1.032-1.528,P=0.023).ROC curve analysis indicated that the AUC value of BMI in predicting puncture site complications was 0.702(95%CI:0.586-0.817,P=0.018),with a sensitivity of 91.7%,a specificity of 49.6%,and a Yoden index of 0.413.Conclusion RFCA for atrial arrhythmias is closely associated with punc-ture site complications,and BMI has a predictive value for the complications of puncture site.

8.Establishment and Validation of a Platinum Resistance Recurrence Prediction Model for Advanced Epithelial Ovarian Cancer

Objective:To analyze the influencing factors of platinum resistant recurrence in advanced epithelial ovarian cancer(AEOC),establish a nomogram model to predict platinum-resistant recurrence of AEOC,and inter-nally validate it.Methods:The clinicopathological data of 577 AEOC patients who achieved complete remission af-ter initial treatment in the Department of Gynecology,Yunnan Cancer Hospital from June 1,2013 to December 31,2021 were collected.According to whether the platinum free interval(PFI)was less than 6 months,the patients were divided into platinum-resistant recurrence group(130 cases)and non-platinum-resistant group(447 cases,including patients with platinum-sensitive recurrence and no recurrence after 6 months of follow-up).Multivariate Logistic regression analysis was used to screen for the independent risk factors affecting the recurrence of plati-num-resistant patients.Based on the independent risk factors,a nomogram prediction model was established,and Bootstrap method was used for internal verification.The area under the ROC curve(AUC),calibration curve and decision curve(DCA)were used to evaluate the performance of the model.Results:①There were statistically sig-nificant differences in age,bilateral ovarian invasion,FIGO staging,menopause,neoadjuvant chemotherapy(NACT),chemotherapy interval(TTC),platelet count(PLT),platelet count/lymphocyte count ratio(PLR),fibrino-gen/lymphocyte count ratio(FLR),prognostic nutritional index(PNI),albumin(ALB),CA125 level,ascites volume,residual lesions,perioperative chemotherapy frequency,and CA125 half-life between the two groups(P<0.05).②Multivariate Logistic regression analysis showed that bilateral ovarian invasion,FIGO stage Ⅳ,TTC>16 days,ini-tial ascites volume>1000ml,perioperative chemotherapy frequency>9 times,surgery with R2 resection,CA125 half-life>52 days were independent risk factors for recurrence of platinum-resistant AEOC patients(OR>1,P<0.05).③The AUC of the nomogram model constructed based on the above 7 indicators was 0.791(95%Cl 0.747-0.835),and the calibration curve and ideal curve fitted well.DCA showed that the net benefit interval of the model was 0.037-0.800.Conclusions:The nomogram prediction model based on independent risk factors for the recurrence of platinum-resistance of AEOC patients has good discrimination,calibration and clinical appli-cability,which can better predict the recurrence risk of platinum-resistance in AEOC patients after the initial treat-ment.
