1.Investigation and analysis of the review and comment on inpatient medical orders in Beijing municipal hospitals

OBJECTIVE To investigate the current situation of the review and comment on inpatient medical orders in Beijing municipal hospitals, and to put forward countermeasures and suggestions for further improving related work. METHODS A questionnaire survey was conducted to investigate the current situation of the review and comment on inpatient medical orders in 22 Beijing municipal hospitals. The statistical analysis was conducted for the survey results. RESULTS A total of 22 questionnaires were distributed, with recovery effective rate of 100%. The 22 hospitals carried out inpatient medical order comment, but their proportion varied among hospitals (0.88%-98.54%); medical order comment mainly focused on antibiotics, proton pump inhibitors, anesthetic drugs/class Ⅰ psychotropic drugs, auxiliary drugs and other categories; 205 pharmacists participated in the comment of inpatient medical orders, most of whom hold intermediate or higher professional titles (89.27%); 21 hospitals conducted inpatient medical order comment and feedback the results to relevant departments/responsible persons, but the intervention situation was not the same. Eighteen hospitals had carried out the review of inpatient medical orders; reviewed drug category was roughly the same as the category involved in the medical order comment; review content involved the suitability of administration routes, dosage, etc. The review was conducted mainly through the cooperation of audit software and pharmacists. CONCLUSIONS The comment and review of inpatient medical orders in Beijing municipal hospitals carried out in an orderly manner, and preliminary results have been achieved; at the same time, it is necessary to further increase the ability of participating pharmacists, improve audit standards, optimize pre-audit (No.ZYLX201805) software, and promote rational drug use among hospitalized patients through doctor-pharmacist collaboration.
2.Correlation between PRX1,CACNA2D1 expression and the efficacy of intensity-modulated radiotherapy in patients with non-small cell lung cancer

Objective To analyze the correlation between Peroxiredoxin 1(PRX1)and Calcium channelα2δ1 subunit(CACNA2D1)expression and the efficacy of intensity-modulated radiotherapy in patients with non-small cell lung cancer(NSCLC).Methods The cancer tissues in totally 112 NSCLC patients who re-ceived intensity-modulated radiotherapy in Shaanxi Cancer Hospital from January 2020 to January 2022 were selected as the NSCLC group,and according to the efficacy of radiotherapy,112 NSCLC patients were divided into an effective group(74 cases)and an ineffective group(38 cases).Normal lung tissues adjacent to cancer in 60 NSCLC patients undergoing surgical treatment were selected as the control group.Immunohistochemis-try and Western blot were used to detect the expression of PRX1 and CACNA2D1 in tissues.Spearman rank correlation analysis was used to investigate the correlation between PRX1 and CACNA2D1 expression.Multi-variate Logistic regression was used to analyze the influencing factors of efficacy of intensity-modulated radio-therapy in NSCLC patients.Results The positive rates of PRX1 and CACNA2D1 in NSCLC group were high-er than those in control group(x2=46.615,46.456,P<0.05).The relative grayscale values of PRX1 and CACNA2D1 in NSCLC group were higher than those in control group(t=9.357,9.509,P<0.05).The ex-pression of PRX1 and CACNA2D1 in NSCLC cancer tissues was positively correlated(r=0.724,P<0.001).The positivity rates of PRX1 and CACNA2D1 in cancer tissues of patients with TNM stage Ⅲ-Ⅳ,low differ-entiation,and lymph node metastasis were higher than those of patients with TNM stage Ⅰ-Ⅱ,medium and high differentiation,and no lymph node metastasis(P<0.05).The proportion of patients with TNM stageⅢ-Ⅳ,the proportion of patients with lymph node metastasis,positivity rate of PRX1 and positivity rate of CACNA2D1 in the ineffective group were higher than those in the effective group(P<0.05).TNM staging stages Ⅲ-Ⅳ,lymph node metastasis,PRX1 positive,and CACNA2D1 positive were risk factors affecting the efficacy of intensity modulated radiotherapy in NSCLC patients(P<0.05).Conclusion The positivity rates of PRX1 and CACNA2D1 in NSCLC patients are increased,which is related to TNM staging,differentiation de-gree and lymph node metastasis,and they are risk factors affecting the efficacy of intensity-modulated radio-therapy in NSCLC patients.
3.Clinical study on the stability of L5S1 fusion for degenerative lumbar scoliosis after long-segment decompression and fusion

Objective To explore the influence factor of the stability of L5S1 fusion and the relationship between the stability of sacrum fusion and the clinical outcome for degenerative lumber scoliosis with long-segment fusion.Metheds Fifty patients with degenerative lumber scoliosis who underwent the long-segment fusion from June 2010 to January 2015 were included in this retrospective study.Based on post-operative L5S1 fusion,patients were divided into two groups.There were 15 patients(30%,9 male and 6 female patients) with a failing sacrum fusion whose average age 60±7.6 years (range from 58 to 78 years).6 patients had the situation at T12-S1,7 at L1-S1 and 2 at L2-S1.There were 35 patients (70%,26 male and 9 female patients) with a satisfying sacrum fusion whose average age 58.4±4.8 years (range from 50 to 80 years).Among them,16 were at T12-S1,12 at L1-S1 and 7 at L2-S1 levels.The interbody fusion rate and accuracy of pedicle screws were evaluated by anteroposterior and lateral spine radiographs in standing and CT scanning during the postoperative follow-up.The age,body mass index (BMI),bone mineral density,Cobb's angel,lumbar lordosis (LL),sagittal vertical axis (SVA),thoracic kyphosis (TLK),sacral slope (SS),pelvic inclination (PI) and pelvic tilt (PT) were compared between the 2 groups to analysis the relevant factor of the failing L5S1 fusion.Oswestry disability index (ODI) and visual analogue scale (VAS) were compared between the 2 groups in the postoperative follow-up.Results There were no statistically significant difference between the 2 groups preoperative in the age,BMI,lumbar Cobb angel,LL,SVA,TLK,SS,PI and PT.But there was a significant difference between the bone mineral density of 2 groups.LL were-40.5°± 8.7° vs.-41.2°±7.9°;TLK were 1.7°±7.4° vs.1.8°±6.7°;SS were 32.1°±5.6° vs.32.4°±5.5°;PT were 18.7°±10.5° vs.19.5°± 10.1°;PI were 42.3°±4.4° vs.40.1°±5.2°;SVA were 9.2±3.5 cm vs.9.5±3.1 cm;T value were-2.7±1.1 vs.-1.2±1.4.There were no statistically significant difference between the 2 groups in the ODI and VAS in the postoperative follow-up.Oswestry disability index were 8.2%±5.2% vs.7.8%±4.5%;VAS value were 3.4±2.6 vs.3.1±2.1.Conclusion The smaller the bone mineral density of the patients with DS,the higher the incidence of the fusion failure L5S1 fusion.And when the failing L5S1 of fusion happened,the clinical outcome will not always be poor.
4.Observation on analgesic effect of oxycodone acetaminophen and celecoxib for military training injury

Objective To investigate the analgesic effect and safety of oxycodong acetaminophen and celecoxib on the military training injury,and provide a reliable evidence for the usage of drugs. Methods 90 patients were randomly divided into 3 groups(n=30),which were celecoxib group,oxycodone acetaminophen group and placebos group. The efficacy and safety were evaluatedv by the visual analogue score (VAS) before using the drug and the 2nd day,4th day,6th day,8th day after giving drugs. Results The VAS of celecoxib group at each time point were less than that of oxycodong acetaminophen group. And there were nearly no adverse reactions of celecoxib group. The differences were statistically significant (P<0. 05). Conclusion The celecoxib has a good analgesic effect on military training injury and less adverse reactions. Which should be the first choice for paitents suffering from the pain of military training injury.
