OBJECTIVE:
To investigate the Wnt signaling pathway and miRNAs mechanism of extracts of Plastrum Testudinis (PT) in the treatment of osteoporosis (OP).
METHODS:
Thirty female Sprague Dawley rats were randomly divided into 5 groups by random number table method, including sham group, ovariectomized group (OVX), ovariectomized groups treated with high-, medium-, and low-dose PT (160, 80, 40 mg/kg per day, respectively), with 6 rats in each group. Except for the sham group, the other rats underwent bilateral ovariectomy to simulate OP and received PT by oral gavage for 10 consecutive weeks. After treatment, bone mineral density was measured by dual-energy X-ray absorptiometry; bone microstructure was analyzed by micro-computed tomography and hematoxylin and eosin staining; and the expressions of osteogenic differentiation-related factors were detected by immunochemistry, Western blot, and quantitative polymerase chain reaction. In addition, Dickkopf-1 (Dkk-1) was used to inhibit the Wnt signaling pathway in bone marrow mesenchymal stem cells (BMSCs) and miRNA overexpression was used to evaluate the effect of miR-214 on the osteogenic differentiation of BMSCs. Subsequently, PT extract was used to rescue the effects of Dkk-1 and miR-214, and its impacts on the osteogenic differentiation-related factors of BMSCs were evaluated.
RESULTS:
PT-M and PT-L significantly reduced the weight gain in OVX rats (P<0.05). PT also regulated the bone mass and bone microarchitecture of the femur in OVX rats, and increased the expressions of bone formation-related factors including alkaline phosphatase, bone morphogenetic protein type 2, collagen type I alpha 1, and runt-related transcription factor 2 when compared with the OVX group (P<0.05 or P<0.01). Meanwhile, different doses of PT significantly rescued the inhibition of Wnt signaling pathway-related factors in OVX rats, and increased the mRNA or protein expressions of Wnt3a, β-catenin, glycogen synthase kinase-3β, and low-density lipoprotein receptor-related protein 5 (P<0.05 or P<0.01). PT stimulated the osteogenic differentiation of BMSCs inhibited by Dkk-1 and activated the Wnt signaling pathway. In addition, the expression of miR-214 was decreased in OVX rats (P<0.01), and it was negatively correlated with the osteogenic differentiation of BMSCs (P<0.01). MiR-214 mimic inhibited Wnt signaling pathway in BMSCs (P<0.05 or P<0.01). Conversely, PT effectively counteracted the effect of miR-214 mimic, thereby activating the Wnt signaling pathway and stimulating osteogenic differentiation in BMSCs (P<0.05 or P<0.01).
CONCLUSION
PT stimulates bone formation in OVX rats through β-catenin-mediated Wnt signaling pathway, which may be related to inhibiting miR-214 in BMSCs.

NAFLD is the most prevalent chronic liver disease, which has become a world public health issue and the incidence rate is also showing an increasing trend.A series of liver disea-ses,such as simple fatty liver disease,NASH,liver cirrhosis,liv-er failure and liver cancer, can be collectively referred to as NAFLD.Through the study of numerous factors that influence the production of NAFLD,it has been found that the main patho-logical mechanism is excessive synthesis of fat, which is difficult to be transported into the blood, causing massive lipid accumula-tion.Alcohol has a direct damaging effect on liver and will in-hibit the breakdown of liver fat, eventually forming AFLD.How-ever,it is still controversial whether alcohol has a synergistic effect on NAFLD onset. This article provides a review on the effect of alcohol intake on NAFLD and its potential mechanisms of action.

Objective:To investigate the additional prognostic value of coronary CT angiography (CCTA)-based flow reserve fraction (CT-FFR) over semi-quantitative CCTA risk scores in predicting the occurrence of major adverse cardiovascular events (MACE) in type 2 diabetic patients. Methods:A total of 231 patients with type 2 diabetes mellitus who underwent CCTA at Lanzhou University from May 2020 to April 2021 were retrospectively enrolled. Clinical baseline data were collected, and patients were divided into a MACE-positive group (20 cases) and a MACE-negative group (211 cases) based on follow-up results. The CCTA images of all patients were analyzed by semi-quantitative CCTA risk score, which included coronary artery disease reporting and data system classification, segment involvement score, segmental stenosis score, Leaman score, and Leiden score. CT-FFR measurements of CCTA data of all patients were performed using Coronary Analysis software. t-test, U-test, and χ2 test were used to compare baseline parameters between MACE-positive and MACE-negative groups. The Cox proportional hazards regression model was used to analyze the relationship between semi-quantitative CCTA risk score and CT-FFR with the occurrence of MACE, and the area under the curve (AUC) of the receiver operating characteristic (ROC) was used to calculate the efficacy of the prediction model established by the semi-quantitative CCTA risk score combined with CT-FFR. Results:There was no statistically significant difference in baseline data between patients in the MACE-positive and MACE-negative groups ( P>0.05), and there were significant differences in semi-quantitative CCTA risk scores and CT-FFR ( P<0.05). Multivariate Cox proportional risk regression analysis of CT-FFR≤0.80 ( HR=3.860, 95% CI 1.477-10.087, P=0.006) and Leaman score≥5 ( HR=5.210, 95% CI 1.136-23.908, P=0.029) were the best and independent predictors for the occurrence of MACE events. The combined CT-FFR and Leaman score prediction model (AUC=0.791, 95% CI 0.733-0.842, P<0.001) was a better predictor of MACE than CT-FFR alone (AUC=0.718, 95% CI 0.656-0.775, P<0.001) and Leaman score alone (AUC=0.711, 95% CI 0.648-0.768, P<0.001) both had better predictive efficacy ( Z=2.62, 1.98, P=0.009, 0.047). Conclusion:CT-FFR independently predict the occurrence of MACE in patients with type 2 diabetes mellitus and significantly improve the predictive capacity of semi-quantitative CCTA risk score for MACE.

Objective:To evaluate the safety and feasibility of double-port laparoscopic cholecystectomy (LC) combined with transcystic common bile duct (CBD) exploration using ultrafine choledochoscopy on patients with gallbladder stones and common bile duct stones. Methods:Clinical data of 35 patients undergoing double-port LC combined with transcystic CBD exploration using ultrafine choledochoscopy in Anhui No.2 Provincial People’s Hospital from December 2021 to June 2024 were retrospectively analyzed, including 8 males and 27 females, aged (45.8±18.1) years. In all patients, the diameter of the gallbladder duct was greater than 3 mm, the maximum diameter of the stones was less than 10 mm, and the number of stones was less than 5, and the gallbladder ducts were normal. Magnetic resonance cholangiopancreatography (MRCP) was used to measure the diameter of CBD, the number and the maximum diameter of stones. The operative time, intraoperative blood loss, postoperative anal exhaust time, postoperative hospital stay and complications (including abdominal infection, biliary tract infection, bile leakage, bleeding, etc.) of all patients were analyzed. The incidence of bile duct stenosis, residual stone or stone recurrence were followed up by telephone or outpatient review. Results:MRCP measurement indicated that the common bile duct diameter of patients was (8.1±1.3) mm. Single CBD stone occurred in 27 cases (77.1%, 27/35), and the mean maximum diameter of CBD stones was (3.9±1.3) mm. All patients successfully underwent the procedure. The operative time was (80.1±10.9) min, the intraoperative blood loss was (25.5±10.2) ml, the recovery time of postoperative anal exhaust was (17.3±4.7) h, and the postoperative hospital stay was (2.5±0.6) d. There were no complications such as abdominal and biliary tract infection, bile leakage and bleeding. All patients were followed up for 1-30 months, with a median follow-up time of 12 months. No biliary stricture, residual stones or recurrence occured during the follow-ups.Conclusion:In selected cases, double-port LC combined with transcystic CBD exploration using ultrafine choledochoscopy could be safe and feasible, with less trauma, quick recovery and short operative time.

In recent decades, the incidence of human papillomavirus(HPV)-associated oropharyngeal squamous cell carcinoma(OPSCC)has shown a marked increase.Significant changes have also occurred in the OPSCC diagnosis and treatment paradigm.Deter-mining HPV status prior to treatment is now essential, and radiotherapy/chemotherapy,immunotherapy,and minimally invasive surgical techniques have progressively emerged as key modalities for managing OPSCC.However, alongside these paradigm shifts,a comprehen-sive technical consensus guiding the entire diagnostic and therapeutic process for OPSCC patients is currently lacking.Given China's large population base and the rising incidence of OPSCC,an expert panel convened to develop a clinical technical consensus on OPSCC diagno-sis and management tailored to China's specific context. This consensus aims to further enhance and standardize understanding of OPSCC management techniques among relevant healthcare professionals.
