1.WANG Xiuxia's Clinical Experience in Treating Hyperprolactinemia with Liver Soothing Therapy

This paper summarizes Professor WANG Xiuxia's clinical experience in treating hyperprolactinemia using the liver soothing therapy. Professor WANG identifies liver qi stagnation and rebellious chong qi (冲气) as the core pathomechanisms of hyperprolactinemia. Furthermore, liver qi stagnation may transform into fire or lead to pathological changes such as spleen deficiency with phlegm obstruction or kidney deficiency with essence depletion. The treatment strategy centers on soothing the liver, with a modified version of Qinggan Jieyu Decoction (清肝解郁汤) as the base formula. Depending on different syndrome patterns such as liver stagnation transforming into fire, liver stagnation with spleen deficiency, or liver stagnation with kidney deficiency, heat clearing, spleen strengthening, or kidney tonifying herbs are added accordingly. In addition, three paired herb combinations are commonly used for symptom specific treatment, Danggui (Angelica sinensis) with Chuanxiong (Ligusticum chuanxiong), Zelan (Lycopus lucidus) with Yimucao (Leonurus japonicus) , and Jiegeng (Platycodon grandiflorus) with Zisu (Perilla frutescens).
2.Scapular motion and shoulder function in patients suffering from rotator cuff tears with typeⅢscapular dyskinesis

Objective To explore the differences in scapular motion and shoulder function between patients suffering from rotator cuff tears(RCT)with and without type Ⅲ scapular dyskinesis(SD).Meth-ods Between September 2021 and March 2023,sixteen patients suffering from rotator cuff tears with SD(SD group)and 17 counterparts without SD(non-SD group)were recruited from the Sports Hospital of the General Administration of Sport of China.Their scapular motion was assessed by measuring three parameters in the X-rays,including scapular spine line(LSS),scapular upward rotation angle(SU-RA),and coracoid upward shift distance(CUSD).Moreover,their shoulder range of motion in flexion,abduction and external rotation were recorded,and further evaluated using the Pain Visual Analog Scale(VAS)and American Shoulder and Elbow Surgeons Score(ASES).Results No significant differenc-es were found between the two groups in the average score of SURA,CUSD and LSS at 0°~30° shoul-der abduction,or in that of CUSD and LSS at 60°~90°shoulder abduction.However,the average SU-RA score of the SD group at 60°~90°shoulder abduction was significantly greater than the other group(P<0.05).The shoulder ranges of motion during active flexion,abduction and external rotation were significantly smaller in the SD group than in the non-SD group(P<0.05).Moreover,the average VAS score in the SD group was significantly higher than the non-SD group(P<0.05),while the average ASES score was significantly lower than the latter group(P<0.05).Conclusions RCT patients type III SD exhibits greater scapular upward rotation during shoulder abduction compared to those without SD.Moreover,the former patients suffer from more severe pain and have worse shoulder range of motion and functional performance than the latter.

Allyl isothiocyanate (AITC) is a natural product commonly used in food preservation and pharmaceutical applications. Toxoplasmosis, caused by the protozoan pathogen Toxoplasma gondii, is prevalent globally while the impact of AITC on toxoplasmosis is unclear. We explored the effect of AITC on acute toxoplasmosis. We infected C57BL/6 mice with T. gondii type I RH strain following AITC administration. On the 4th day after infection, which corresponds to the initial stage of infection, we collected serum for the determination of inflammatory cytokine levels. The mice serum of the AITC-administered group contained significantly lower levels of granulocyte colony-stimulating factor, interferon-gamma, interleukin (IL)-23 subunit p19, IL-4, IL-6, and monocyte chemoattractant protein-1. The lifespan of the mice in the AITC-administered group was significantly reduced. In vitro experiments showed that AITC promoted the proliferation of intracellular T. gondii accompanied by the inhibition of IL-4, IL-1β, and IL-6 production in RAW264.7 macrophages. Our results showed that AITC facilitated T. gondii infection in the early stage by inhibiting the production of several inflammatory cytokines.
4.Modified Superior Mesenteric Artery Approach Totally Laparoscopic Radical Resection for Right Colon Cancer

Objective To explore the safety and feasibility of a modified superior mesenteric artery(SMA)approach in totally laparoscopic complete mesocolic excision(CME)and D3 lymphadenectomy for right colon cancer.Methods A retrospective analysis was performed on clinical data of 77 cases of totally laparoscopic radical surgery for right colon cancer from April 2021 to April 2023.Before August 2022,42 cases underwent traditional SMA approach(control group,only marking with the ileocolic vascular pedicle as the tail of SMA),while after August 2022,35 cases underwent modified SMA approach(modified group,marking with the Treitz's ligament and ileocolic vascular pedicle as the head and tail of SMA,respectively).There was no statistically significant difference in general information between the two groups(P>0.05).The intraoperative conditions,postoperative recovery,and postoperative complications were compared between the two groups.Results Compared with the control group,the modified group had a shorter surgical time[(147.3±35.8)min vs.(173.4±29.9)min,t =-3.428,P =0.001].There were no statistically significant differences in the number of lymph node dissection,number of positive lymph nodes,drainage volume,exhaust time,postoperative hospital stay,and incidence of complications between the two groups(P>0.05).Conclusion The modified SMA approach for totally laparoscopic radical resection of right colon cancer shortens the surgical time,reduces the difficulty and risk of surgery,and has high safety and feasibility.

7.Genetic Characteristics,Diagnosis and Treatment of Short Stature in 14 children with Neurofibromatosis Type 1

Objective This study aims to analyze the clinical and genetic characteristics of 14 children with neurofibromatosis type 1(NF1)presenting with short stature,with the goal of enhancing clinical awareness and promoting early diagnosis and intervention.Methods Clinical data were collected from 14 children diagnosed with NF1 and short stature at Tongji Hospital,Tongji Medical College,Huazhong University of Science and Technology,between May 2017 and June 2024.The clinical characteristics,treatment approaches,and follow-up outcomes were analyzed.Results Among the 14 patients,there were 7 boys and 7 girls.The average age at diagnosis was 6.61±3.53 years old,with a height standard deviation score(SDS)of-2.60±0.66.All patients had scattered café-au-lait spots on their skin,with an averaging number of 9.64±4.86,and the largest diameter was 6 cm.Genetic testing identified five types of NF1 gene mutations:6 nonsense mutations,4 missense mutations,2 frameshift mutations,1 splice mutation,and 1 whole-gene mutation.During the follow-up,three patients developed cutaneous neurofibromas during puberty,four experienced an increase in the number of café-au-lait spots,and one developed tibial pseudarthrosis.Growth hormone(GH)stimulation tests were conducted on 10 patients.Two patients with GH deficiency received recombinant human growth hormone(rhGH)treatment,which resulted in a slightly increase in their growth rate.The remaining 12 children,who did not receive rhGH treatment,showed no significant change in growth rate during follow-up.Conclusion The clinical manifestations of NF1 are diverse and age-related.In children with short stature and multiple café-au-lait spots,NF1 should be considered as a potential diagnosis.Genetic testing should be conducted if necessary,and regular monitoring is essential to assess the disease's evolution and to enable prompt interventional strategeies.
8.Risk factors for pancreatitis after endoscopic retrograde cholangiopancreatography in children

Objective To investigate the application of endoscopic retrograde cholangiopancreatography(ERCP)in children and the risk factors for post-ERCP pancreatitis(PEP).Methods A retrospective analysis was conducted on the clinical data of 66 children,aged≤16 years,who underwent ERCP for pancreaticobiliary diseases at the Gastrointestinal Endoscopy Center of the Second Affiliated Hospital of Kunming Medical University from September 2013 to September 2023.The incidence rate of PEP and the risk factors for the development of PEP were analyzed.Results A total of 78 ERCP procedures were performed on 66 children,with 5 diagnostic ERCPs,69 therapeutic ERCPs,and 4 failed procedures.The success rate of ERCP operations was 95%(74/78).There were 17 cases of PEP in total,with an incidence rate of 22%.In the PEP group,the proportion of children with normal preoperative bilirubin and the proportion of guidewire insertion into the pancreatic duct during surgery were higher than in the non-PEP group(P<0.05).The multivariate logistic regression analysis showed that guidewire insertion into the pancreatic duct was an independent risk factor for PEP(P<0.05).Conclusions With the increasing application of ERCP in children with pancreaticobiliary diseases,it is important to select an appropriate intubation technique during surgery to avoid blindly entering the guidewire into the pancreatic duct and reduce the occurrence of PEP.
9.Recommendations and reflections on natural ventilation in healthcare buildings from World Health Organization

Infection prevention and control in healthcare buildings is important in blocking the spread of infectious diseases.As the most common form of ventilation,natural ventilation can reduce the infection risk in healthcare workers,patients and visitors in healthcare buildings.This study summarizes the recommendations from the World Health Organization on natural ventilation in healthcare buildings during the epidemic period from severe acute re-spiratory syndrome(SARS)to coronavirus disease 2019(COVID-19)(2003-2022),the current situation of such issues in China,and the enlightenment from the recommendations.The recommendations proposed by the World Health Organization are relatively clear and specific,and worthy for reference learning for relevant management de-partments and research institutions in China.Some of these experiences can provide useful references for the future revision of Chinese standards or codes such as Technique standard for isolation in hospitals,Management specifi-cation of air cleaning technique in hospitals,Code for design of infectious diseases hospital and Code for design of general hospital.
10.Wash-hand basin and hand-rinse basin in UK healthcare buildings:an analysis based on infection prevention and control concepts

Effective infection prevention and control measures can reduce the risk of infection in patients,health care workers,and visitors in healthcare buildings.Hand hygiene facilities,such as wash-hand basin and hand-rinse basin,play important roles in this.Based on a number of medical building guidelines issued by the UK Department of Health and Social Security,this paper analyzed the wash-hand basin and hand-rinse basin in medical buildings in the UK,and summarized similar problems in China.This paper clarified the differences between wash-hand basin and hand-rinse basin in the UK medical buildings,as well as classification of users,size and space requirements,classification and design requirements,and configuration requirements of wash-hand basin and hand-rinse basin.It also discussed the status of such problems in China as well as relevant experience and enlightenment from the UK.Some of the UK experience can provide reference for the future revision of Chinese standards such as Code for de-sign of infectious diseases hospital,Code for design of general hospital,Standard for hand hygiene for health-care workers in healthcare settings.
