The"capitation payment with retained surplus and shared accountability for reasonable overruns"mechanism constitutes a pivotal institutional framework for advancing the high-quality development of County Medical Communities(CMCs).This study addresses two critical operational challenges:identifying the sources of medical insurance fund surplus and optimizing the governance of fund retention processes.Grounded in transaction cost theory,we develop an analytical framework examining the formation of medical insurance fund surplus through the dual lenses of intra-organizational dynamics within CMCs and external medical insurance payment mechanism design.Utilizing Deqing County,Zhejiang Province as an empirical case, this research proposes a five-pronged strategy:Clarifying generation channels of insurance fund surplus, scientifically determining regional medical insurance budgets, implementing bundled payment mechanisms for CMCs,adopting hybrid payment models integrating unified and differentiated approaches, and establishing performance-based incentive systems. These findings elucidate the formative pathways of medical insurance fund surplus while offering theoretical and practical insights for enhancing payment system reforms to support CMC development.

OBJECTIVE To explore the significance of pharmaceutical care through the diagnosis and treatment of a patient with exogenous insulin autoimmune syndrome(EIAS),combined with the analysis of literature reports.METHODS Clinical pharmacist participated in the diagnosis and treatment process of one case of EIAS.Based on the characteristics of the patient's condition, the pharmacist provided medication suggestions and formulated pharmaceutical monitoring measures. At the same time, the pharmacist searched for relevant literature on insulin autoimmune syndrome(IAS)and EIAS,extracted data(gender, age, occurrence time, laboratory tests, clinical symptoms, intervention and outcome),and conducted analysis.RESULTS Based on the patient's medication information in the past 3 years, clinical pharmacist determined that the EIAS was likely caused by insulin aspartate 30.The clinician adopted the clinical pharmacist's suggestion to discontinue insulin and switch to oral hypoglycemic drugs. The patient improved after treatment. The literature analysis showed that among the 257 patients with IAS reported,212 cases were caused by drugs;among them,23 cases were caused by lipoic acid, and 56 cases were caused by exogenous insulin. There were no significant differences in age, glycosylated hemoglobin, and body mass index between the two groups. The lowest blood glucose level in the lipoic acid group was significantly lower than that in the exogenous insulin group(P<0.05).The proportion of females and the proportion of fasting insulin≥1 000 μU/mL were significantly higher in the lipoic acid group than in the exogenous insulin group(P<0.05).CONCLUSIONS Compared with EIAS,lipoic acid-induced IAS usually causes more severe hypoglycemia, and the fasting insulin level is usually higher than 1 000 μU/mL,which is more common in female patients. The participation of clinical pharmacists in the diagnosis and treatment of EIAS can help improve the diagnosis and treatment level of similar rare diseases and ensure the safety of patients'medication.

Aim To investigate the effects of Ixerin Z,a sesquiterpene lactone from Ixeris sonchifolia,on bone-lipid metabolic imbalance in ApoE-/-mice and to elu-cidate its molecular mechanisms. Methods A mouse model of ApoE-/-was induced using a high-fat diet, followed by eight weeks of Ixerin Z administration at doses of 1 and 10 mg·kg-1.Serum markers related to bone-lipid metabolism and inflammatory cytokines were quantified.Bone mineral density, biomechanical prop-erties,bone tissue morphology, and bone microstructure changes were analyzed.Computational molecular doc-king was performed to identify potential target proteins of Ixerin Z,and its regulatory effects on bone-lipid me-tabolism were investigated. Results Treatment with Ixerin Z markedly decreased the serum levels of total cholesterol,triglycerides,TNF-α,and IL-1β in ApoE-/-mice.It significantly improved bone mineral density, enhanced biomechanical strength, restored tra-becular structure, and reduced fat accumulation in bone tissue.Investigations revealed that Ixerin Z activated PPARα,thereby promoting fatty acid β-oxidation in bone tissue, and stimulating the Wnt/β-Catenin signa-ling pathway to facilitate bone formation.Furthermore,Ixerin Z suppressed the OPG/RANKL/NF-κB signaling pathway, leading to reduced bone resorption,independ-ent of PPARα activation. Conclusions Ixerin Z dem-onstrates potent therapeutic effects on bone-lipid meta-bolic imbalance in ApoE-/-mice.The mechanism in-volves activating PPARα to promote fatty acid β-oxida-tion in bone tissue, activating PPARα/Wnt/β-Catenin signaling pathway to promote bone formation, and in-hibiting OPG/RANKL/NF-κB signaling pathway to re-duce bone resorption.

Neuropathic pain, a debilitating condition caused by dysfunction of the somatosensory nervous system, remains difficult to treat due to limited understanding of its molecular mechanisms. Bioinformatics analysis identified cerebellin 2 (CBLN2) as highly enriched in human and murine proprioceptive and nociceptive neurons. We found that CBLN2 expression is persistently upregulated in dorsal root ganglia (DRG) following spinal nerve ligation (SNL) in mice. In addition, transcription factor SOX11 binds to 12 cis-regulatory elements within the Cbln2 promoter to enhance its transcription. SNL also induced SOX11 upregulation, with SOX11 and CBLN2 co-localized in nociceptive neurons. The siRNA-mediated knockdown of Sox11 or Cbln2 attenuated SNL-induced mechanical allodynia and thermal hyperalgesia. High-throughput sequencing of DRG following intrathecal injection of CBLN2 revealed widespread gene expression changes, including upregulation of numerous NF-κB downstream targets. Consistently, CBLN2 activated NF-κB signaling, and inhibition with pyrrolidine dithiocarbamate reduced CBLN2-induced pain hypersensitivity, proinflammatory cytokines and chemokines production, and neuronal hyperexcitability. Together, these findings identified the SOX11/CBLN2/NF-κB axis as a critical mediator of neuropathic pain and a promising target for therapeutic intervention.

Chinese Medical Equipment Journal 2025;46(5):73-77
Objective To propose a ventilator failure probability prediction method based on kernel ridge regression(KRR).Methods Firstly, the failure interval data of ventilators was collected and preprocessed to remove outliers.Secondly, the median rank method was used to estimate the failure probability. Finally, using the time data as the feature variable and the failure probability value as the target variable,a KRR model was established and trained by selecting the optimal kernel function and hyperparameter combination from radial basis kernel function, linear kernel function, polynomial kernel function, and S-type kernel function through grid search and cross-validation methods to predict ventilator failures. To verify the performance of the KRR model in predicting ventilator failure probability,it was compared with Weibull and its extended models. Results KRR achieved a coefficient of determination of 0.993 5,a mean squared error of 5.399 5×10-4,a root mean squared error of 0.023 2 and a mean absolute error of 0.018 3,outperforming Weibull and its extended models in prediction accuracy and error control. Conclusion The failure probability prediction method for ventilators based on KRR demonstrates exceptional performance in prediction accuracy and error control, and thus holds great potential for application.[Chinese Medical Equipment Journal,2025,46(5):73-77]
