BACKGROUND:Radiotherapy significantly improves survival rates in patients with various malignant tumors. However, with prolonged post-treatment survival, many patients face the risk of radiation-related cardiac toxicity. This is especially true after chest radiotherapy, where the risk of radiation-induced heart disease significantly increases, becoming one of the most severe complications affecting prognosis survival.OBJECTIVE:To identify diagnostic markers of endothelial cellular senescence in radiation-induced heart disease through systematic transcriptomic analysis.METHODS:Firstly, genes associated with cellular senescence were screened from the CellAge database and intersected with the transcriptomic training dataset of a mouse model of radiation-induced heart disease to identify differentially expressed senescence-related genes.Secondly, weighted gene co-expression network analysis and machine learning were used to identify key hub genes that play critical roles in radiation-induced heart disease. The expression of these genes was validated using a dataset of radiation-induced endothelial injury.Additionally, the quanTlseq method was employed to assess the immune infiltration status related to radiation-induced heart disease. The expression levels of key genes and their association with survival in esophageal squamous cell carcinoma patients receiving chest radiotherapy were explored through the analysis of The Cancer Genome Atlas database.RESULTS AND CONCLUSION:(1)Systematic transcriptomic analysis identified CCND1 as the core gene of endothelial cellular senescence in radiation-induced heart disease, and this finding was validated in the mouse model of radiation-induced heart disease.(2)The diagnostic model constructed from these data indicated that CCND1 had high specificity and sensitivity for diagnosing radiation-induced heart disease.(3)Immune infiltration analysis revealed significant immune response dysregulation in the mouse model of radiation-induced heart disease, and CCND1 was closely related to various immune cells.(4)Kaplan-Meier survival analysis showed that CCND1 was associated with poorer disease-specific survival in esophageal squamous cell carcinoma patients receiving chest radiotherapy. This study systematically uncovers, for the first time, the pivotal role of CCND1 in endothelial cell senescence associated with radiation-induced heart disease.CCND1,a gene integral to cell cycle regulation, can induce cellular senescence when abnormally expressed.Furthermore, the findings highlight its potential as an early diagnostic marker.

Objective:To explore the maintenance of sinus rhythm and the changes in functional tricuspid regurgitation(FTR) in patients with atrial fibrillation(AF) combined with FTR following the Mei Mini Maze Procedure, and to analyze the correlation between these two factors. Methods:We retrospectively included 180 patients with AF and FTR who underwent left-sided thoracoscopic AF ablation(the Mei Mini Maze Procedure) at Xinhua Hospital in Shanghai from January 2019 to December 2021. After propensity score matching to eliminate confounding factors, the patients were divided into two groups based on the severity of FTR before surgery: the non-significant FTR group(+ -+ +, 68 cases) and the significant FTR group(≥+ + +, 68 cases). Outpatient follow-up with 24-72 hour dynamic electrocardiogram and transthoracic echocardiography(TTE) was conducted at 3, 6, 12, 24, and 36 months post-discharge. The maintenance of sinus rhythm and changes in FTR were compared between the two groups. Results:All patients successfully completed the Mei Mini Maze Procedure. Compared to preoperative values, the severity of FTR improved in both groups. Non-significant FTR group: 59% of patients(40 cases) had no regurgitation, 27%(18 cases) had mild FTR, 11%(8 cases) had moderate FTR, and 3%(2 cases) had severe FTR; significant FTR group: 19%(n=13) had no regurgitation, 25%(17 cases) had mild FTR, 25%(17 cases) had moderate FTR, and 31%(21 cases) had severe FTR. The Kaplan-Meier(KM) curve revealed that the sinus rhythm maintenance rate at 3 years post-surgery was significantly higher in the non-significant FTR group(72.06%) than in the significant FTR group (57.35%), with a statistically significant difference( P=0.011). Multivariate analysis showed that postoperative FTR ≥ moderate( HR=11.469), preoperative significant FTR( HR=1.206), age over 65 years( HR=3.734), non-paroxysmal AF( HR=2.346), and longer AF duration( HR=1.151) were risk factors for AF recurrence in this cohort. Conclusion:The recovery of sinus rhythm in AF patients is closely related to the severity of FTR. Patients with preoperative significant FTR had a lower rate of sinus rhythm maintenance after surgery. Both preoperative and postoperative significant FTR are risk factors for AF recurrence in AF patients with FTR. Therefore, for patients with significant FTR, simultaneous treatment of FTR during the surgery, including a standard Cox-Maze Ⅳ procedure, may be more beneficial for maintaining postoperative sinus rhythm.

Objective:To observe the effect of mirror threshold load training on respiration of persons with respiratory muscle fatigue after a cerebral hemorrhage. Methods:Fifty cerebral hemorrhage patients with respiratory muscle fatigue were randomly divided into an observation group and a control group, each of 25. In addition to routine rehabilitation training, the control group was given threshold load training of the respiratory muscles, while the observation group was provided with mirror threshold load training, twice a day in the morning and afternoon, 5 days a week for 4 weeks. Before the experiment and after 1, 2, 3 and 4 weeks of the treatment, everyone′s maximum inspiratory pressure (MIP) and maximum expiratory pressure (MEP) was recorded. Before and after the 4 weeks forced expiration volume in the first second (FEV1) was measured along with 25% of the forced expiration volume (FEF25), maximum sound time (MPT) and respiration rate (RR).Results:At each time point the MIP and MEP values of both groups were significantly better than those before the treatment. After 4 weeks the average MIP and MEP values of the observation group were significantly better than those of the control group. And after 4 weeks the FEV1, FEF25, MPT and RR values of both groups had also improved significantly, on average. All of the observation group′s averages except MPT were then significantly better than the control group′s averages. Conclusions:Mirror threshold load training of the respiratory muscles can significantly improve the respiration of persons with respiratory muscle fatigue after a cerebral hemorrhage. It is more effective than respiratory muscle threshold load training.

Objective:Dehydrated hereditary stomatocytosis (DHS), a rare condition caused by pathogenic PIEZO1 or KCNN4 mutations, remains under-recognized. This study aimed to improve the diagnosis and management of DHS in China by retrospectively analyzing clinical and genetic characteristics. Methods:Patients diagnosed with DHS at Peking Union Medical College Hospital between 2018 and 2024 were included. All had suspected congenital hemolytic anemia and were confirmed by next-generation sequencing to harbor pathogenic PIEZO1 or KCNN4 mutations. Clinical features, genotypes, iron overload, and treatment outcomes were collected and analyzed.Results:Twenty-three DHS patients were identified, including 21 with PIEZO1 and 2 with KCNN4 mutations. Common manifestations were splenomegaly, jaundice, cholelithiasis, and secondary iron overload. Mild and moderate anemia occurred in 8.7% and 17.4% of patients, respectively, and stomatocytes were present in only 17.4%. The median time from symptom onset to diagnosis was 11 years. The predominant PIEZO1 variants were c.7367G > A and c.6328C > T, with nine novel mutations identified. Both KCNN4 mutations involved c.1055G > A. No clear genotype–phenotype correlation was observed. Secondary hepatic iron overload was found in 21.7% of patients. Conclusion:DHS is often underdiagnosed or misdiagnosed. Genetic testing enables accurate identification, and regular monitoring for secondary hepatic iron overload is crucial for long-term management.

Journal of Sichuan University (Medical Sciences) 2025;56(5):1251-1259
Food safety problems caused by foodborne pathogenic bacteria pose a serious threat to public health, creating an urgent need to develop testing methods and techniques with excellent performance and are simple to use and of affordable cost.Traditional testing methods, such as isolation and culture, morphological observation, biochemical identification, and serological tests, have many limitations, including complex procedures, reliance on specialized technical equipment and personnel, and long turnaround time, rendering them inadequate for meeting current and future testing demands. Therefore,it is particularly important to develop simple, rapid, and highly sensitive methods for analyzing pathogenic bacteria. The fusion of nucleic acid aptamers and nanozymes brings new ideas for the rapid testing of pathogenic bacteria. On one hand, aptamers offer specific recognition capability for target bacteria and can be combined with various nucleic acid signal amplification techniques. On the other hand, the enzyme-like catalytic activity and signal amplification effect of many nanomaterials provide a basis for highly sensitive testing. This review highlights the application potential of nanozyme?aptamer coupling systems in the field of microbial analysis by briefly summarizing the latest research progress in the use of nanozymes combined with aptamers for the detection of foodborne pathogenic bacteria.First of all,two main approaches to conjugating nanozymes with aptamers are introduced. Then, the testing mechanisms and typical applications of nanozyme?aptamer coupling systems for foodborne pathogenic bacteria are discussed. Finally, future development trends and existing challenges are disucssed from four perspectives, including specificity, high sensitivity, high throughput, and intelligent detection. This review aims to provide a useful reference for the fusion of nanozymes and aptamers and for the development of on-site rapid testing techniques for foodborne pathogens, and to encourage broader academic interest to further advance this promising research field.
