1.Influence factors analysis of early postpartum pelvic organ prolapse following transvaginal delivery in advanced maternal age primipara

Objective To explore the influencing factors of early postpartum pelvic organ prolapse(POP)after vaginal delivery in advanced maternal age primipara.Methods Clinical data of 153 postpartum women were collected from Beijing Tongren Hospital Mentougou Campus,Capital Medical University,from June 2015 to May 2024.According to whether pelvic organ prolapse occurred during the 6-week postpartum reexamination of the primiparas,the parturients were divided into two groups:non-POP group(91 cases)and POP group(62 cases).Univariate and multivariate Logistic regression analyses were conducted to identify the independent factors influencing early postpartum POP.Subgroup analysis was also performed to assess the relationship between family history of POP and early postpartum POP after adjusting for confounding factors.Results Multivariate Logistic regression analysis revealed that neonatal birth weight,maternal pre-pregnancy body mass index(BMI),use of delivery balls,and family history of POP were independent factors associated with early postpartum POP occurrence(P<0.05).Among the parturients who did not use of delivery balls,before and after using Logistic regression to adjust for birth weight,pre-pregnancy BMI and perineal laceration,the impact of family history of POP on the occurrence of POP was statistically significant(P<0.05).However,among the parturients who used the delivery balls,regardless of whether the adjustment was made or not,the impact of family history of POP on the occurrence of POP was not statistically significant(P>0.05).Conclusion Neonatal birth weight,maternal pre-pregnancy BMI,and family history of POP are risk factors for early postpartum POP,while the use of delivery balls is a protective factor.Women with a family history of POP are advised to use delivery balls to reduce the risk of early postpartum POP.

3.Construction of a quantitative diagnosis model for predicting the nature of thyroid nodules based on multi-modality ultrasound images

Objective:To construct a quantitatively diagnostic nomogram model by analyzing the clinical information of patients and the features of multi-modality ultrasound images of thyroid lesions, so as to preoperatively predict the malignant probability of suspicious thyroid nodules and provide effective references for clinical decision-making.Methods:A total of 933 patients, 1 121 thyroid nodules of C-TIRADS 3-5 categories, who underwent surgery in the Second Affiliated Hospital of Harbin Medical University from September 1, 2020 to June 10, 2021 were collected. The nodules were randomly divided into training ( n=897) and test groups ( n=224) in 8∶2 ratio. Finally, the diagnostic performance was evaluated by area under the curve (AUC). Results:①After preliminary screening by univariate analysis, multivariate analysis showed that age, echogenicity, orientation, echogenic foci, margin, posterior features, and elastic score were significantly correlated with benign and malignant nodules (all P<0.001), and the difference of halo between benign and malignant nodules was also statistically significant ( P=0.012). ②The AUC of nomogram was up to 0.903(95% CI=0.862-0.944) in the test set, and 0.889(95% CI=0.832-0.946) and 0.960(95% CI=0.925-0.994) in nodules with maximum diameter of ≤10 mm and of >10 mm respectively, which showed high diagnostic performance. Conclusions:The nomogram model could accurately differentiate malignant from benign thyroid nodules preoperatively, with the highest diagnostic performance for the nodules with maximum diameter of >10 mm, and effectively avoid the unnecessary fine-needle biopsy and surgical operation.
4.Total Asymmetric Synthesis of 3'R,4'R-Disubstituted Angular Dihydropyranocoumarins

AIM:To obtain new typical compounds with dual mechanism,antagonizing Ca2+and inhibiting acetylcholinesterase,on Alzheimer's disease.METHODS:Resorcinol was chosen as primitive substrate,and AD-mix-β as asymmetrically dihydroxylated reagent;all products were screened against acetylcholinesterase in vitro.RESULTS:cis-3′R,4′R-disubstituted angular dihydropyranocoumarins were synthesized enantioselectively,however,its inhibitory activity on acetylcholinesterase is distinctively lower than that of cis-3′R,4′R-disubstituted linear dihydropyranocoumarins.
5.Chromones from Angelica morri Hayata

AIM:Bioactive constituents were expected to be obtained from the roots of Angelica morri Hayata. METHOD:They were extracted with 95% alcohol and isolated by using column chromatography and recrystallization methods. The structures were elucidated by means of physico-chemical data and UV,IR,1HNMR, 13CNMR,and EIMS. The inhibitory effect on the constriction of rat aortic rings was induced by K+ or Ca2+. RESULT:3′S-(-)-hamaudol,3′S-(-)-Ο-acetylhamaudol,3′R-(+)-hamaudol,and (±)-hamaudol were isolated from the pieces of Radix Angelica Morri. The inhibitory rate of 3′S-(-)-Ο-acetylhamaudol and (±)-hamaudol on the above pharmacologic model appears the relation of quantity response. CONCLUSION:All the above compounds were found in this species for the first time,and(±)-hamaudol is a new. One of effect mechanisms of 3′S-(-)-Ο-acetylhamaudol and (±)-hamaudol diluted aorta could contribute to be inhibiting Ca2+ influx of vascular smooth muscle.
