1.Autologous hamstring tendon reconstruction with preserved synovial sheath combined with remnant suture for anterior cruciate ligament tears

Objective:To investigate the clinical effectiveness of autologous hamstring tendon reconstruction with preserved synovial sheath combined with remnant suture in the treatment of anterior cruciate ligament (ACL) tears.Methods:A retrospective study was conducted to analyze the clinical data of 38 patients with ACL tear who had been admitted to Department of Sports Medicine, The Fourth Hospital of Wuhan from January 2016 to July 2020. There were 30 males and 8 females, with an age of (25.4±7.2) years. By the typing of MRI Sherman tear locations (MSTL) for anterior cruciate ligament, 7 cases were Type I and 31 cases type II. A total of 18 patients underwent single bundle reconstruction of autologous hamstring tendon combined with remnant suture (group A), and the other 20 patients underwent conventional reconstruction to clean the synovial sheath of ACL (group B). The operation time,Tenger scores, Lysholm scores, International Knee Documentation Committee (IKDC) subjective score and side-to-side differences in relaxation at pre- operation, 1 year and 2 years after operation, as well as ACL healing of the affected knee at 2 years after operation, were observed and compared between the 2 groups. The graft synovial coverage was evaluated by secondary arthroscopy in some cases.Results:There was no significant difference in the preoperative general data between the 2 groups, indicating comparability ( P>0.05). All patients were followed up for (38.5±8.4) months. No surgical site infection or knee stiffness occurred in the 2 groups after operation, and 2 patients in each group developed intermuscular venous thrombosis at a lower limb. There was no significant difference in operation time between the 2 groups ( P>0.05). There were significant differences in Tenger score [(6.3±1.6) points versus (4.7±1.7) points], Lysholm score [(93.4±4.2) points versus (85.9±4.3) points] and IKDC subjective score [(89.1±2.9) points versus (81.2±2.9) points] between group A and group B at 1 year after operation ( P<0.05), but there was no significant difference in Tenger score, Lysholm score or IKDC subjective score between the 2 groups at 2 years after operation ( P>0.05). There was no significant difference in side-to-side difference in relaxation between the 2 groups at 1 year or 2 years after operation ( P>0.05). MRI evaluation at 2 years after operation showed no recurrent rupture or no sign of impingement between the graft and the intercondylar fossa in either group. Conventional secondary arthroscopy for 2 patients in group A and 3 patients in group B revealed better synovial coverage in group A. Conclusion:In the treatment of ACL tears of MSTL types I and II, compared with conventional reconstruction to clean the synovial sheath of ACL, autologous hamstring tendon reconstruction with preserved synovial sheath combined with remnant suture can lead to better functional recovery at 1 year after operation, though there is no significant difference in function at 2 years after operation.

Artificial intelligence (AI) is revolutionizing the diagnosis and treatment of biliary diseases, shifting from experience-driven to data-driven approaches. By integrating multi-source data, AI demonstrates significant advantages in early diagnosis, personalized treatment, and prognosis management of biliary diseases. It not only enhances diagnostic accuracy and treatment precision, but also optimizes medical resource allocation and reduces medical costs. The authors systematically elaborate on the current application status and challenges faced by AI in the diagnosis and treatment of biliary diseases. It is believed that the problems, such as data barriers, algorithm limitations, and ethical issues faced in AI clinical applications, need to be solved by establishing a standardized data governance system, developing explainable deep learning models, and improving the medical AI ethical review mechanism. In the future, AI will become a core enabler in the biliary disease diagnosis and treatment ecosystem, driving the transition to knowledgeable healthcare and leading to a new era of precision medicine.
3.Development and Application of Caregiver Burden Scale for Primary Family Caregivers of Cancer Patients

Objective The study intends to develop a multi-dimensional specific scale for the primary family caregivers of cancer patients,verify its reliability and validity,and reveal the burden level of the primary family caregivers of cancer patients.Methods This study investigated the primary family caregivers of cancer patients by questionnaire survey in Harbin of two medical institutions,finally we recalled 571 valid questionnaires.The study used SPSS 27.0 and AMOS 28.0 software to conduct reliability and validity analysis.The critical value of the burden score was determined by the quartile method to evaluate the burden level.Results The revised scale includes 21 items in 5 dimensions.The Cronbach's α coefficient range of the total scale and each dimension of the scale was 0.798~0.922,the Omega coefficient range was 0.806~0.918,and the split-half reliability range was 0.753~0.862.The average variance extracted(AVE)value and composite reliability(CR)value of the five dimensions of the scale meet the criteria,and the correlation coefficient between each dimension is less than the square root of AVE.Based on the critical value,there are 54.8%of carers suffer from moderate or heavy burdens.Among them,the financial burden and the care burden have the highest scores.Conclusion It shows that the burden scale for primary family caregivers of cancer patients has good reliability and validity.There are more than half of carers suffer from relatively heavy burden.Policymakers should draw up policies to support economic medical expense reimbursement and last comfort service.
4.Construction of an evaluation index system for the effectiveness of ideological and political education in the curriculum of professional master's degree nursing programs

Objective:To construct an evaluation index system for the effectiveness of ideological and political education in the curriculum of professional master's degree nursing programs, providing an evaluation tool for objectively assessing the teaching quality of ideological and political education in these programs.Methods:The initial draft of the index system was designed based on a review of domestic and international literature. From October 2022 to January 2023, 15 experts were selected using convenience sampling for three rounds of Delphi expert consultation, and the analytic hierarchy process was used to determine the weight of the indicators.Results:The positive response rate from the 15 experts in all three rounds of consultation was 100% (15/15), and the expert authority coefficient was 0.813. The Kendall's coefficient of concordance for the overall indicators in the third round was 0.116 ( P<0.01). The constructed evaluation index system includes 5 first-level indicators (goal and concept, faculty, course resources, teaching implementation, teaching effectiveness), 12 second-level indicators, and 49 third-level indicators. Conclusions:The evaluation index system for the effectiveness of ideological and political education in the curriculum of professional master's degree nursing programs demonstrates good scientific validity and reliability. It can provide objective and quantitative evaluation criteria for the development of ideological and political education in these programs.

9.Changing trends of dengue fever in China Meta-analysis of comorbidity rates and clinical manifestations

Abstract: Objective To analyze the changing trends of severe dengue (SD) among adults and children in China and compare the clinical manifestations between the two groups, providing scientific evidence to optimize dengue prevention strategies and reduce severe cases. Methods Literature related to clinical or infection aspects of dengue fever clinical or infection studies published from database inception to December 31, 2023, were retrieved from CNKI, Wanfang, VIP, and Scopus databases using keyword searches. The literature was screened according to predefined inclusion and exclusion criteria, and a meta-analysis was conducted using R (4.2.3) software. Results A total of 7 148 records were retrieved, ultimately including 30 studies on adults and 12 studies on children, involving eight provinces. The severe dengue comorbidity rate in adults decreased from 6.6% during 1978-1995 to 0.9% during 2014-2023, while in children, it significantly dropped from 15.8% during 1980-1995 to 0.5% during 2014-2019. In adult studies, the primary clinical manifestations and their comorbidity rates were: fever (99.2%), myalgia (55.6%), headache (52.0%), rash (45.5%), and fatigue (34.4%). In pediatric studies, the primary clinical manifestations and their comorbidity rates were: fever (97.6%), headache (38.8%), anorexia (31.9%), rash (30.5%), and lymphadenopathy (20.5%). Conclusions In recent years, the severe dengue comorbidity rates have significantly decreased in both adults and children, indicating that China's dengue prevention and control strategies have achieved remarkable results. The prominent clinical manifestations in adults are associated with organ damage, while children predominantly show gastrointestinal symptoms. Moving forward, efforts should be made to strengthen early diagnosis and standardized treatment to further reduce the clinical impact of dengue fever.

Objective:To investigate the normal reference values of spinal bone mineral density measured by quantitative computed tomography (QCT) and the differences of bone mineral density (BMD) in different regions of in Chinese adult males.Methods:Men who underwent low-dose CT lung scan for cancer screening in regions of Northeast, North, East, South, Central and Southwest of China from January 2018 to December 2019 were selected. And the lumbar vertebrae BMD values in the male subjects were measured by the QCT system (Mindways Software, Inc.). The mean BMD values and their decline rates were calculated at an age interval of 10 years, and the prevalence of osteoporosis was calculated according to the American College of Radiology spine QCT osteoporosis diagnostic criteria.Results:A total of 50 682 males with a mean age of (50.22±12.79) years (ranged 20 to 98 years) were included in this study. The peak BMD of (173.11±28.56) mg/cm 3 in the healthy Chinese adult male population appeared in the age group of 20 to 29 years and then declined with age. Before the age of 70 years, the BMD was relatively higher in males in South China, and it was lower in Central China and Southwest China, and it was intermediate in Northeast, North and East of China, with statistically significant differences. There was no significant differences in BMD in the males in the two age groups of 70 to 79 years and 80 and older among the regions in China. The overall decline rate of spinal BMD in Chinese males under QCT was about 46.92% over the lifetime, and it declined obviouslyin the 40-49 age group. The overall prevalence of osteoporosis in Chinese male population aged 50 years and above was approximately 11.42%, with the highest prevalence in Southwest China and Central China (14.72% and 13.87%, respectively) and the lowest in North China and South China (8.53% and 7.71%, respectively). Conclusions:A reference of lumbar spine BMD values for healthy males in China based on QCT is established. BMD values were highest in South China and Lowest in Central China.
